An Epidemiological Study on the Industrial Injuries among Metal Products Manufacturing Workers in Young-Dung-Po, Seoul (일부 금속 및 기계제품 제조업체 근로자들의 산업재해($1980{\sim}1981$)에 관한 조사)

  • The followings are the results of the study on industrial accidents occurred at 12 factories manufacturing metal products during the period of 2 years from January 1980 to December 1981 in the area of Yong-Dung-Po in Seoul. The results of the study are as follows: 1. The incidence rate of industrial injuries was 45.7 per 1,000 workers of the sample group and the rate of male (54.0) was three times higher than that of female (17.5). 2. In age groups, the highest rate was observed in the group of under 19 years old with 83.5, while the lowest in the group of 40s. 3. It was found that those who had short term of work experience produced a higher rate of injuries, particularly, the group of workers with less than 1 year of experience showed the highest rate of it as 48.1%. 4. In working time, the highest incidence rate occurred 3 and 7 hours after the beginning of their working showing the rate of 6.0 and 6.1 per 1,000 workers, respectively. 5. The highest incidence rate was observed on Monday as 8.4 per 1,000 workers, and it was 18.3% in aspect of the days of a week. 6. In aspect of the months of a year, the highest incidence was observed on July 1,000 workers and the next was on March as 4.8. These figures account for 11.8% of total occurrence in respective month. as 5. 4 per and 10.5% 7. In causes of injuries, the accident caused by power driven machinery showed the highest rate with 37.5%, the second was due to handling without machinery with 17.2%, and the third was due to falling objects with 14.2%, and striking against objects with 10.2%, and so on. 8. By parts of the body affected, the most injuries 84.3% of them occurred on both upper and lower extremities with the rate of 58.8% for the former and 25.5% for the latter. Fingers were most frequently injured with a rate of 40.3%. Comparing the sites of extremities affected, rate of injuries on the right side was 55.0% and 45.0% on the left side. 9. In the nature of injury, laceration and open wound were the highest with 34. 0%, the next was fracture and dislocation with 31. 9%, and sprain was the third with 8.1%. 10. On the duration of treatment, it lasted less than one month in 68.9% of the injured cases, of which 14.5% of the cases were recovered within 2 weeks, and 54.4% of them were treated more than 2 weeks. And the duration of the treatment tended to be prolonged in larger industries. 11. The ratio of insured accidents to uninsured accidents was 1 to 4.7.

Treament of Sternal Dehiscence or Infection Using Muscle Flaps (근육편을 이용한 흉골열개 및 감염의 치료)

  • Background: Sternal infection or dehiscence after cardiac surgery through median sternotomy is rare. If suitable treatment is not performed for the complication, however, the mortality is high. For 12 patients with sternal dehiscence or infection, we performed wide excision of the infected and necrotic tissue and covered with muscle flap(s) to obliterate the mediastinal dead space. Material and method: Sternal infection or dehiscence occurred in 13 of patients who underwent cardiac surgery One patient, who died of cerebral infarction before the sternal complication was treated, was excluded in this study. The sternal wound complication occurred in 6 of patients with valve replacement and 6 of patients with coronary bypass surgery, respectively. Since 1991, 9 patients underwent definite surgical debridement and muscle transposition as soon as fever was controlled with closed irrigation and drainage. The necrotic tissue and bone was widely excised and the sternal dead space was eradicated with the single flap or the combined flaps of right pectoralis flap(turnover flap), left pectoralis flap(turnover flap or rotation-advancement flap), and right rectus muscle flap. Result : There was no mortality in 12 patients with coverage of muscle flap(s) for sternal infection or dehiscence The mean interval between the diagnosis of sternal complication and the myoplasty was 6.6$\pm$3.9 days. In 4 patients, one pectoralis muscle flap was used, and in 8 patients both pectoralis muscle flaps were used. For each 1 patient and 2 patients in each group, right rectus muscle flap was added. For the last 3 patients, a single pectoralis flap was used to eradicate the mediastinal dead space and the longer placement of the mediastinal drain catheter was needed. One patient, who had suffered from necrosis of left pectoralis flap(rotation-advancement flap) with subsequent chest wall abscess after coverage of both pectoralis flaps, was managed with reoperation using right rectum flap. Conclusion : Sternal dehiscence or infection after cardiac operation can be readily managed with wide excision of necrotic infected tissue(including bone) and muscle flap coverage after short-term irrigation of sternal wound. The sternal(mediastinal) dead space may be completely eradicated with right pectoralis major muscle flap alone.

Serum ghrelin and leptin concentrations in children with cancer : comparisons with normal children (소아 종양 환아의 혈중 Ghrelin과 Leptin의 농도: 정상 소아와 비교)

  • Purpose : Ghrelin, being secreted from the stomach, stimulates growth hormone secretion and controls energy homeostasis by increasing appetite. Leptin, being secreted from the adipocytes, controls weight and energy homeostasis by decreasing appetite. Leptin concentration is reported to increase after childhood cancer therapy. This study was aimed to compare ghrelin and leptin concentrations in normal children and children who received cancer therapy. Methods : We enrolled forty-three patients who were diagnosed with cancer and received radiotherapy or chemotherapy during Dec. 2004 through Dec. 2005 in St. Marys hospital and Kangnam St. Marys hospital. Forty-five healthy children were selected as a control group whose age, gender, weight and height were similar to those of cancer group. The serum leptin and ghrelin concentrations were also measured by radioimmunoassay. Results : The cancer group showed higher BMI and leptin concentrations. The control group showed higher concentrations of ghrelin. Both control and cancer groups revealed positive correlations between leptin concentrations and BMI. Ghrelin concentrations in the control group showed negative correlations with age, height, weight and BMI but no significant correlation was found in the cancer group. All the parameters in the group treated with chemotherapy only were not different from those in the group treated with chemotherapy and irradiation. But the level of ghrelin in the acute myeloid leukemia group was much higher than those in the acute lymphoblastic leukemia group. Conclusion : Patients with pediatric cancer treatment have presented higher BMI and leptin concentrations but lower ghrelin concentrations than those in healthy children. Because of the relatively short duration and cross sectional method of the study, however, further long term and prospective study will be required in the future.

Effect of Temperature and Gas Permeability of Functional Packing Films on Storability of Fresh-cut Salicornia herbacea Classified by Size (저장온도와 기능성 필름의 기체 투과도가 다양한 크기의 퉁퉁마디 신선편이의 저장성에 미치는 영향)

  • These studies were identified the effect of four types of non-perforated breathable (NPB) packing film and three sizes on storage ability of fresh-cut for ready to eat packaging at $5^{\circ}C$ for 25 days and $10^{\circ}C$ for 15 days storage in Salicornia herbacea. The fresh weight loss was less than 2% in every films at $5^{\circ}C$ condition after 25 days storage, and the $10^{\circ}C$ also had same result on 15 days storage except 100,000 cc NPB film. Compare with storage after 15 days, storage condition at $5^{\circ}C$ had shown better result under the 1% fresh weight loss rate. The 5,000 cc and $5^{\circ}C$ condition had been shown the characteristics of MA packaging in carbon dioxide and oxygen concentrations. The ethylene concentration in vacuum film was higher 10 to 100 times than in NPB film treatments during storage. But ethylene concentration was not statistically significant differences among size treatments. Every conditions had been measured the anti-oxidant activity by DPPH method after storage at $5^{\circ}C$ for 25 days and $10^{\circ}C$ for 15 days. S. herbacea at $5^{\circ}C$ had been more than twice of activity compare with that at $10^{\circ}C$. 100,000 cc NPB film had been higher contents of anti-oxidant activity at $5^{\circ}C$ and $10^{\circ}C$. As the fresh-cut sizes, 3 cm and 5 cm sizes had changed depending on film types but 10 cm were not effected by the film types in the DPPH activity. When panel test had been tried to measure the visual quality and off-flavor after storage, $5^{\circ}C$ with a filme of 5,000 cc treatment had established higher value than other treated conditions. As these results, it's may be suggested that the $5^{\circ}C$ with 5,000 cc non-perforated breathable film for MA storage in Salicornia herbacea at fresh cut distribution system. Fresh cut size 10 cm with 100,000 cc NPB film also had the good quality for 15 dyas storage at $10^{\circ}C$, and this result can be applied for short term distribution system in Korea.

Nitrogen Assimilation and Carbohydrate Concentration as Affected by the N Supply Form and Their Level in Shoot of Perennial ryegrass (Lolium perenne L.) (페레니얼 라이그라스에서 질소공급형태 및 수준에 따른 질소동화와 탄수화물 대사산물의 변화)

  • To investigate the short-term effects of N-supply form ( $NO_3\;^-$ or $NH_4\;^+$ ) and their level (0.2, 1.0 and 6.0 mM) on N assimilation and C metabolism were examined in perennial ryegrass (Lolium perenne L.). The increase in shoot fresh for $NH_4\;^+$-fed plants much less than $NO_3\;^-$ fed ones. Nitrate concentration in $NO_3\;^-$ -fed plants tended to increase with increasing the supply level, while that of $NH_4\;^+$-fed plants was nearly stable. Nitrate reductase activity (NRA) responded much quickly, showing a proportional increase within 24 h of feeding. NRA in $NO_3\;^-$ -fed plants at 72 h increased by 13.7, 40.3 and 84.0% in 0.2, 1.0 and 6.0 mM $NO_3\;^-$ -fed, but it was not changed in$NH_4\;^+$-fed plants regardless of the supply level. After 72 h of treatment the sugar accumulation in the plants supplied with 0.2 and 1.0 mM -$NH_4\;^+$fed was remarked. After 72 h of feeding, fructan hydrolysis was observed in all levels fur $NH_4\;^+$-fed plants, but only in 6.0 mM for $NO_3\;^-$ -fed plants.
