A Study on the Improvement of Patients' Privacy in General Hospitals - The Case of the Seoul Asan Hospital Renovation Project - (종합병원의 환자 프라이버시 개선에 관한 연구 - 서울아산병원 레노베이션 사례를 중심으로 -)

  • Among the countless elements for healing environment in healthcare facility, 'privacy' has been rather neglected in general hospital designs in Korea. The fact may be due either to present situation of excessively crowded patients or to cultural origin which more stresses on the mass than on individuals. However, as patient amenity has increasingly become important, privacy should be viewed as one of the key elements in the general hospital design, in order to improve the quality of patient service as well as to compete with other hospitals. Also it is very important that those methods for privacy improvement should be not only theoretically correct but also practical enough to be compatible with medical efficiency, space availability or construction cost within a particular hospital. The purpose of this case study is to present an actual examples to improve patients' privacy adopted in the schematic design of the Seoul Asan Hospital renovation project, along with comparisons with other hospital cases.

  • Korea is entered to the aging society with 21C. Also it is forecast we will enter in aging society quickly. for 5 years there are some growth in facility at least 2~3 times but our society needs more. But this quantitative growth is worried about less quality. That's why we needs more research about the facility for an geriatrics hospital. The people who suffered from senile disease needs more treatment days in the ward of geriatric hospital. so we have to consider about better condition in ward of geriatrics than in general. Better environment for eldery is included not only the ward space but also the corridor and the dayroom. This study which it performed to improve their habitability is researched on public space. and this reserch deal with general feature against a public space in the ward of geriatric, and investigate 2 facility for divede the the general feature of the space. and this investigate is recorded according to behavior of patients. It can find us which factor of the space is prefered by them.

Comparative Evaluation of Measures against the Spread of Air-borne Infections in a Large National Hospital and Small and Medium-sized Clinics in Korea (국내 대형병원과 중·소규모 의원의 공기감염 확산 방지 대책의 비교 평가)

  • The purpose of this study is to compare and analyze the air infections in middle and small hospitals with the facilities of large national hospitals that have air-borne infection isolation (AII) wards through actual condition investigation and airflow analysis simulation (CFD) and to provide basic data for prevention. The method and scope of the study are as follows. First, through literature review, data related to prevention of infection spread in domestic medical institutions were investigated. Second, we conducted a survey on the status of isolation facilities to prevent the spread of infectious diseases in large hospitals and small and medium - sized clinics in Korea. Third, airflow analysis simulation (CFD) was carried out using the isolation ward of the nationally designated inpatient ward and the data of the plane and facility system of the small clinic. As a result of the study, it is found that regulations applicable to small and medium-sized clinics are insufficient. In addition, the simulation results show that the infectious disease virus is likely to spread to other patients in the hospital.

A Study on the Archtectural Planning Compared with Space for Before and After Remodeling of General Hospitals in Korea (국내 종합병원의 리모델링 전.후 공간비교에 관한 건축계획적 연구)

  • An early stage of general hospital in Korea was the quantitative growth period. Remodeling has been required to meet the increasing medical demands. To accept the change of paradigm and management operation method, remodeling occurred from the 20th century. Remodeling plans are difficult to define the direction of the detailed plan due to various factors in the beginning of the construction, Therefore, it is necessary to analyze the possibility and limitation of a feasible remodeling plan of general hospitals through the comparison of existing hospital before remodeling and after the remodeling takes effect. A comparison of the researched hospital's blue prints before and after the remodeling and recently built hospital's blue print, analyzed characteristic of the space variation by the remodeling. The purpose of research is to confirm a possibility and limitation of remodeling of general hospitals comparing with merits and faults of extension, reconstruction, and construction. In conclusion, the area of remodeling hospitals increase mostly medical department(ward, outpatient department, inpatient department) and subsidiary facilities, but the area of ward and inpatient department are less than the new hospitals. Especially, public area is greatly increased by the diversification of corridor function. Also, remodeling hospitals represent a limitation to plan departments in need of equipment-intensive space. To address this problem, expansion space is used mainly with inpatient department and existing space is placed mainly with low-impact department by equipment ; outpatient department, administration department.

  • Facing the aging society in Korea, the necessity of the facility for the elderly is arising as a substitute to cut down the cost and to provide suitable and skillful medical services for the aged and chronical. Among there, health care facilities for the elderly with dementia are considered a essential facility. Therefore, it is necessary to prepare the guidelines for planning and design of the facility. On the bases of survey and analysis, the basic data for planning and design the hospital for the elderly with dementia has been proposed.

  • Purpose: There have been more researches focused on eco-friendly construction practices in order to save energy consumed and these practices have been extensively applied for constructing buildings. However, researches on energy consumption saving and efficiency for hospital facilities have been still insufficient. This research aims to draw factors that affect energy efficiency of hospital buildings through literature reviews and perform an expert-survey using AHP(Analytical Hierarchy Process) method in order to analyze the importances and priorities of these energy efficient factors. Method: Using the AHP method, this paper suggests the importances and priorities of factors to affect energy efficiency in hospital buildings. The survey of experts at a architectural design and a construction management companies was conducted via e-mail and mail. Result: As a result, factors of window and door, insulation, ventilation, and natural lighting turned out relatively important as respectively 0.104, 0.102, 0.101, and 0.095 n the energy efficiency in hospital buildings, while factors of artificial lighting, geothermal, solar heat, and control did unimportant as 0.027, 0.033, 0.043, and 0.053.

A Study on the Economic Feasibility of 4-bed Rooms in Public Hospitals (공공병원의 4인병실 도입에 따른 경제적 타당성에 관한 연구)

  • Purpose: At the moment, a lot of Public Hospitals in Korea have adopted 5 or 6 bedrooms as a standard multi-bedroom type. However 5-6 bedrooms have many problems related to inpatient satisfaction and nosocomial diseases. Therefore 4 bedroom is under considering for standard multi-bed room in Public Hospitals in Korea. This paper tries to prove that adoption of 4 bedroom in Public Hospitals has nothing to do with economic loss which is now an obstacle in adopting 4 bedrooms. Methods: 3 Methods have been used in this paper. 1) Comparative analysis between medical insurance fee and service cost for hospitalization has been conducted through literature survey. 2) Scenario analysis has been used for the estimation of inpatient number when 4 bedrooms are adopted in Public Hospitals. 3) Relation analysis between profit and proportion of 4 bedroom in Public Hospitals. Results: Adoption of 4 bedroom as a standard multi-bedroom in Public Hospitals has been proved to have nothing to do with the economic loss of hospitals. Implications: It is necessary to introduce and expand 4 bedrooms instead of 5-6 bedrooms in hospitals for the upgrade of hospital environment and easy control of cross infection in inpatient bedrooms.

A Study on the Spatial Characteristics of Public Dental Healthcare Center for the Disabled in Korea (한국 내 장애인 구강진료센터의 공간특성에 관한 연구)

  • Purpose: The characteristics of site plan and space configuration of public dentistry through examining the public dental healthcare centers for the disabled in Korea and comparison between them are necessary for the development of planning of the dental healthcare system. This study has been started to provide basic informations such as nationwide distribution, site relationship, and space configuration for the planning of public dental hospital architecture. Methods: Literature review of publicness and public dental healthcare and investigation on current status of public dental healthcare center for the disabled in Korea have been conducted. The site plan and space configuration of eight public healthcare centers for the disabled have been analyzed. Results: The result of this study can be summarized in three points. The first one is that public dentistry in Korea are distributed public dental hospital for the disabled in Seoul and public dental healthcare centers for the disabled in eight provinces. The second one is that the types of the dental healthcare center for the disabled are divided with remodeling type which is diverted from existed dental or medical out patient clinic space or independent building type which is planned with a new and exclusive usage for the disabled. The third one is that the space configuration of dental healthcare center for the disabled is needed more required programs, larger treatment unit space, and more private clinic space than typical dental treatment plan. Implications: This study is the starting point for the research of public dentistry and it is necessary to analyze the dental prevention and dental public policy to develop the public dental healthcare system.

Preliminary Study on the Enhancement of Seismic Performance of Korea Hospital Buildings (우리나라 병원건축물의 내진성능향상을 위한 기초연구)

  • Secure operation of hospitals during and right after earthquake is essential. Past lessons from earthquake damages have shown that most of the injured and the death occurred within 30 minutes after earthquake and the portion of nonstructural damage has become significant. However, hospital buildings in Korea have not prepared fully to address such rising issues. This paper is to study what type of damage patterns are related to hospital buildings and how to develop a preparedness plan to keep hospitals operational at all earthquakes if possible. This paper first reviews on past earthquake damages reported as critical to hospital buildings while classifying them into four groups: (1) structural element; (2) architectural-nostructural element; (3) medical equipments and contents; and (4) utility facility. Upon such classification, some detailed concerns can be specified under each group explicitly. Then a hierarchy for hospital building is also developed for the classified groups, which enables us to identify required things for the enhancement of seismic performance of hospital building that consists of heterogeneous elements. To upgrade the level of seismic performance for existing hospital buildings, the concept of performance-based approach can be adopted to address the heterogeneous problems in a systematic and stepwise manner. Finally a conceptual framework for the seismic risk assessment for hospital building is proposed toward the seismic enhancement of hospital buildings using performance-based approach.

A Study on the characteristic of Living behaviors of the Elderly with Dementia in Recuperation facilities (치매노인의 요양생활(療養生活) 전개특성(展開特性)에 관(關)한 연구(硏究))

  • The purpose of this study is to clarify the actual condition of the daily living activities of the elderly with dementia in recuperation facilities. The thorough investigation and observation works were made to them from the view points of daily living behaviors and activities of the recuperating elderly. This article discusses about the basic characteristics of the elderly and the actual condition of the daily living activities of the elderly with dementia in 2 nursing homes and 1 geriatric hospital. As a result, we are able to draw out the following conclusion ; There is no wide difference between geriatric hospital and nursing homes on the characteristic of living behaviors of the elderly with dementia.
