부정교합 및 교정치료에 따른 저작능력 조사연구 (A study of chewing ability According to malocclusion and orthodontic treatment)

  • Purpose: The purpose of this study is to compare and analyze the difference in chewing ability according to the malocclusion and orthodontic treatment for the university students who are adults and consider the effect of the malocclusion and orthodontic treatment on chewing ability. Methods: This study conducted the survey for 400 students of the two universities located in Daegu, Gyeongsangbuk-do, and except for the poor or erroneous surveys among 400 survey copies of the participants, total 363(90.75%) survey copies were used for the study analysis. Results: The total result of chewing ability for the food showed that middle-class students had higher chewing ability than high-class students and the students who didn't get orthodontic treatment had higher chewing ability than the students who are getting orthodontic treatment now. In the chewing ability for various kinds foods, in case of young radish kimchi, the result showed that the students who have a little crowding or normal level of teeth or normal teeth has higher chewing ability than the students who have severe crowding level of teeth, and in case of the foods including kkakdugi or galbi, the result showed that the students who have normal teeth location or the 3rd level malocclusion have higher chewing ability statistically significantly than the students who have the 2nd malocclusion. Conclusion: For the chewing ability, the middle-class students in the home economic power showed significantly higher chewing ability than the high-class students in the home economic power, and the students who didn't get any orthodontic treatment showed higher chewing ability statistically significantly than the students who are getting orthodontic treatment now.

II급 치근이개부병변에서 비흡수성막과 흡수성막을 이용한 조직유도재생술의 임상적 비교 (A Clinical Comparision of Nonresorbable and Resorbable Membrane in the Treatment of Human Class II Furcation Defects)

  • The purpose of this study was to compare clinical results of guided tissue regeneration(GTR) using either a nonresorbable ePTFE membrane or a resorbable membrane made from a synthetic copolymer of glycolide and lactide(PLGA) in the treatment of human class Ⅱ furcation defects. The ePTEE membranes were applied to 16 patients with maxillary molar buccal class Ⅱ furcation defects as Group I, PLGA membranes were applied to 15 patients with maxillary molar buccal class Ⅱ furcation defects as Group Ⅱ, ePTFE membranes were applied to 20 patients with mandibular molar buccal class Ⅱ furcation defects as Group Ⅲ and PLGA membranes were applied to 20 patients with mandibular molar buccal class Ⅱ furcation defects as Group Ⅳ and bone graft materials(DFDBA) were applied in all groups. Probing depth, gingival recession, clinical attachment level, tooth mobility and sulcus bleeding index(SBI) were measured at baseline, 3, 6 and 12months postoperatively. In addition, membrane exposure levels were measured at surgery, 1, 2 and 6weeks postoperatively and postoperative complications were evaluated. The results were as follows: In all groups, there were statistically significant differences in probing depth reduction, gain of clinical attachment and mobility reduction at values of 3, 6 and 12months postoperatively compared to values of baseline, whereas no significant differences in SBI except Group I and gingival recession(p<0.05). Membrane exposure levels were increased at 1, 2 and 6weeks postopratively compared to value of baseline in Group I(p<0.05). There were no statistically significant differences between ePTFE and PLGA membrane in probing depth, clinical attachment level and SBI. There were minimal gingival recession and membrane exposure in Group Ⅳ and pain and swelling were the most common postoperative complications in Group Ⅱ, Ⅲ(p<0.05). In conclusion, this study showed that both nonresorbable membrane and resorbable membrane were effective similarly in the treatment of class Ⅱ furcation defects, without statistical differences in clinical measurements.

Frog appliance를 이용한 상악 대구치의 원심 이동: 증례 보고 (The frog appliance for upper molar distalization: a case report)

  • 본고의 목적은 2급 1류 치열안면 구조를 가지는 한 환자에 적용된 새로이 고안된 상악 대구치 원심 이동 장치인 Frog appliance의 효과를 평가하기 위함이다. 11세의 여자 환자가 교정 치료를 위해 본 진료실로 의뢰되었다. 환자는 미약한 2급 골격관계와 2급의 대구치 및 견치 관계를 양측 모두에서 보이고 있었다. 고정성 장치 치료를 통해 양측 상악 제1대구치를 원심 이동시키는 방법을 포함하는 치료 계획을 수립하였으며 상악 대구치를 원심 이동하기 위해 새로이 고안된 Frog appliance를 제작 및 적용하였다. 측모 두부방사선 사진으로 치료 결과를 평가하였으며 상악 제1대구치의 원심 이동이 4개월의 치료 기간 동안 이루어 졌고 1급의 구치 관계가 얻어졌다. 총 치료 기간은 16개월이 소요되었다. 두부방사선 사진을 평가한 결과 약간의 고정원 상실과 함께 대구치의 원심 이동이 치축 이동에 가깝게 일어난 것을 확인하였다. 결론적으로 Frog appliance는 환자의 협조를 요하지 않는 장치로서 간단하고 효과적으로 양측 대구치의 원심이동을 이룰 수 있는 구내 장치이다.

Correction of late adolescent skeletal Class III using the Alt-RAMEC protocol and skeletal anchorage

  • This case report describes skeletal anchorage-supported maxillary protraction performed with the Alternate Rapid Maxillary Expansion and Constriction (AltRAMEC) protocol over a treatment duration of 14 months in a 16-year-old female patient who was in the late growth-development period. Miniplates were applied to the patient's aperture piriformis area to apply force from the protraction appliance. After 9 weeks of following the Alt-RAMEC protocol, miniplates were used to transfer a unilateral 500-g protraction force to a Petit-type face mask. A significant improvement was observed in the soft tissue profile in measurements made both cephalometrically and in three dimensional photographs. Subsequently, the second phase of fixed orthodontic treatment was started and the treatment was completed with the retention phase. Following treatment completion, occlusion, smile esthetics, and soft tissue profile improved significantly in response to orthopedic and orthodontic treatment.

Three-dimensional evaluation of tooth movement in Class II malocclusions treated without extraction by orthodontic mini-implant anchorage

  • Objective: The aim of this study was to analyze tooth movement and arch width changes in maxillary dentition following nonextraction treatment with orthodontic mini-implant (OMI) anchorage in Class II division 1 malocclusions. Methods: Seventeen adult patients diagnosed with Angle's Class II division 1 malocclusion were treated by nonextraction with OMIs as anchorage for distalization of whole maxillary dentition. Three-dimensional virtual maxillary models were superimposed with the best-fit method at the pretreatment and post-treatment stages. Linear, angular, and arch width variables were measured using Rapidform 2006 software, and analyzed by the paired t -test. Results: All maxillary teeth showed statistically significant movement posteriorly (p < 0.05). There were no significant changes in the vertical position of the maxillary teeth, except that the second molars were extruded (0.86 mm, p < 0.01). The maxillary first and second molars were rotated distal-in ($4.5^{\circ}$, p < 0.001; $3.0^{\circ}$, p < 0.05, respectively). The intersecond molar width increased slightly (0.1 mm, p > 0.05) and the intercanine, interfirst premolar, intersecond premolar, and interfirst molar widths increased significantly (2.2 mm, p < 0.01; 2.2 mm, p < 0.05; 1.9 mm, p < 0.01; 2.0 mm, p < 0.01; respectively). Conclusions: Nonextraction treatment with OMI anchorage for Class II division 1 malocclusions could retract the whole maxillary dentition to achieve a Class I canine and molar relationship without a change in the vertical position of the teeth; however, the second molars were significantly extruded. Simultaneously, the maxillary arch was shown to be expanded with distal-in rotation of the molars.

Effectiveness of the Invisalign Mandibular Advancement Appliance in Children with Class II Division 1 Malocclusion

  • Background: This study aimed to determine the skeletal and dental effects in pediatric and adolescent Korean patients with Class II Division 1 malocclusion treated using the Invisalign Mandibular Advancement (MA®) appliance. Methods: The study included patients aged 6 to 18 years who received orthodontic treatment with the MA® appliance for Class II Division 1 malocclusion at the Department of Pediatric Dentistry, Wonkwnag University Daejeon Dental Hospital, between July 1, 2018, and December 31, 2021. The treatment group consisted of 20 patients, 10 boys and 10 girls. The control participants were also 10 boys and 10 girls. Lateral cephalometric radiographs were taken before and after treatment, and 41 measurements of skeletal and dental changes were measured and analyzed using the V-CephTM 8.0 (Osstem Implant). All analyses were performed using SPSS software (IBM SPSS for Windows, ver 26.0; IBM Corp.), and statistical significance was tested using paired and independent samples t-tests for within-group and between-group comparisons, respectively. Results: The patients in the treatment group showed significant decreases in ANB (A point, Nasion, B point), maxillary protrusion, maxillary anterior incisor labial inclination, and maxillary protrusion after treatment. However, when compared with the growth changes observed in the control group, only ANB and maxillary protrusion decreased, with no significant differences in SNA, SNB, and mandibular length. Conclusion: Collectively, the results of this study confirm that the use of MA® appliance in pediatric and adolescent Korean patients with Class II Division 1 malocclusion results in a reduction of anteroposterior skeletal and dental disharmony.

Distalization with a modified C-palatal plate for severe upper crowding and a missing lower incisor

  • This case report presents the orthodontic treatment of a 25-year-old patient with skeletal Class II and severe maxillary arch crowding, moderate mandibular arch crowding, anterior crossbite, and a missing lower incisor. He was treated with molar distalization using a modified C-palatal plate and temporary anchorage devices to create sufficient space for retraction. The total treatment duration was 21 months. After treatment, his occlusion and smile esthetics showed significant improvement. The modified C-palatal plate represents a treatment modality that enhances the prospects of non-extraction treatment and reduces the need for extraction.

발치 및 비발치 치료증례에서의 치료전후 치열궁형태의 변화에 관한 연구 (A STUDY OF THE VARIANCES IN PRE- AND POST-TREATMENT DENTAL ARCH SHAPES IN EXTRACTION AND NON-EXTRACTION CASES)

  • This study was carried out in order to findout the amount of tooth movement, the changes arch size and the changes in arch morphology following orthodontic treatment and to provide a guideline for to predict post-treatment arch morphology. The sample group for this study consists of 15 males and 22 females, totalling in 37 persons, who received orthodontic treatment at Orthodontic Department of Dankook Univ. Dental Hospital. They are classified into Extraction Class I treatment group (E I), Non-extraction Class I treatment group (N I), and Non-extraction Class III treatment group (N III), according to their pre-treatment malocclusion state and methods of treatment. Following conclusions and averaged dental arch form for each group were obtained by cephalometric linear measurements and dental arch measurements using pre- and post-treatment lateral cephalograms and plaster study models. 1. Intercanine width were reduced in max. of both EI and NI during the period of treatment, 2. Intermolar width were reduced in max. of EI and increased in max. of NI. Therefore although there was no difference between these two groups before the treatment, intermolar width of the max, of NI was wider than that of E1 after the treatment. 3. PMV-incisor distance and PMV-canine distance were decreased in both max. and mand. of EI and that of NI, during the period of treatment. PMV-molar distance was decreased in both max. and mand. of NI and in mand. of NIII. 4. Items that showed stability during the treatment were: max. & mand. PMV-molar distance, mand. intercanine and intermolar width in EI; mand. intercanine and intermolar width in NI; mand. & max. PMV-incisor distance, PMV-canine distance, max. PMV-molar distance and max. & mand. intercanine and intermolar width in NIII. 5. The differences in averaged canine and molar variances to post-treatment dental arch form were present only in EI and in NI. There was no variance between maxilla and mandible in each group.

Effects of a Prefabricated Functional Orthodontic Appliance on Children with Class II Division 1 Malocclusion

  • Background: This study aimed to evaluate the effect of a prefabricated functional appliance (Myobrace®) on skeletal, dental, and soft tissue components in children with Class II, division 1 malocclusion. Methods: Thirteen patients with Class II, division 1 malocclusion (9 girls and 4 boys; mean age, 8.2±0.9 years at the start and 9.3±1.0 years at the end of the treatment) were treated with Myobrace® for a mean period of 12.9±4.0 months. Patients were instructed to use the appliance daily for 1 hour and overnight while sleeping. A control group of 10 patients with untreated Class II, division 1 malocclusion (3 girls and 7 boys; mean age, 9.0±1.6 years at the start and 10.4±2.1 years at the end of the observation) was included to eliminate possible growth effects. The mean observation period for this group was 17.7±11.2 months. Lateral cephalograms were taken at the start and end of the treatment, and findings from 41 measurements were analyzed using the V-CephTM program. The mean and standard deviation of cephalometric measurements were analyzed using paired and independent sample t-tests. Results: The treatment group showed significant changes in SNB, ANB, maxillary protrusion, ramus height, proclination of upper anterior teeth, interincisal angle, overjet, and upper lip protrusion compared with the control group. However, only decrease in ANB, maxillary protrusion, overjet, upper lip protrusion, and increase in interincisal angle were significantly higher in the treatment group than in the control group. Conclusion: The prefabricated functional appliance induced skeletal, dentoalveolar, and soft tissue changes, resulting in a significant reduction in anteroposterior discrepancy.

  • 본 증례는 Angle씨(氏) 2급 2류 부정교합 환자로서 Bioprogressive Mechanism을 적용시켜 양호한 결과를 얻었기에 이에 보고하는 바이며 치료결과는 다음과 같다. 1) 주소인 상하악 전치부 crowding을 해소하였다. 2) 구치부 관계는 Angle씨(氏) classII에서 classI으로 개선되었다. 3) Overbite와 overjet를 기능적으로 해소시킴과 동시에 전치부 torque control에 의한 만족스러운 치축경사도로 개선시켰다.(Photo.5) 4) 치료기간은 1년 11개월이 소모되었다.
