Objective : This study was intended to investigate the analgesic effects of electroacupuncture(EA) on mechanical allodynia according to the frequency and intensity of EA. Also to know if mechanical allodynia and the analgesic effects of EA is related to the sympathetci nervous system and/or the purinergic system. Methods : mechanical allodynia-induced rats were produced by resecting S1-S2 nerve. The zusanli(ST36) was used for acupoint and the rats were divided into 4 groups. Each group was given different stimuli[low frequency low intensity-EA(LFLI-EA), low frequency high intensity-EA(LFHI-EA), high frequency low intensity-EA(LFHI-EA), high frequency high intensity-EA(HFHI-EA)]. Futhermore, to make sympathectomy6-OHDA and phentolamine were administered intraperitonially and the concentration of norepinephrine(NE) were measured. As a ATP blocker, suramin was applied for this study. Results : Comparing to control group, each of the 4 groups(LFLI-EA, LFHI-EA, HFLI-EA, HFHI-EA) showed a significant reduction of response frequency of mechanical allodynia. LFHI-EA was more effective than that of LFLI-EA. The LFHI-EA group also had longer lasting effects from the stimulation than the other groups. Sympathectomy didn't show any reduction of response frequency of mechanical allodynia.(Each n=6, n=4). Nor did both sympathectomy and ATP block. The response frequency wasn't reduced by sympathectomy or by sympathectomy and ATP block, but was significantly reduced with LFHI-EA Conclusions : These results suggest that EA has a significant analgesic effect on mechanical allodynia which has no connection with NE and/or ATP.
Anti-nociceptive and anti-inflammatory effects of Clematidis Radix (CR)-distillates were investigated in three different pain animal models. Highly purified distillate of CR was injected to Zusanli (ST36) acupoint, called CR herb-acupuncture in the Korean traditional medicine. In tail flick latency test, the CR herb-acupuncture treatment did not show a significant effect of relieving acute phasic pain. To investigate the anti-inflammatory effect of CR herb-acupuncture, inflammatory pain was induced by subcutaneous injection of formalin to the plantar tissue or intra-articular injection of carrageenan to the tibio-tarsal joint in the rats. And the medicinal effect of CR herb-acupuncture was evaluated by analyzing pain behavior such as licking or biting behavior, or by measuring weight distribution ratio between two foot and ankle circumference. In the rat formalin test, the analgesic effect of CR herb-acupuncture was more pronounced in the late phase (for 20 min after the early phase) than in the early phase (for the first 10 min post formalin injection). It also significantly alleviated the carrageenan-induced monoarthritis, in terms of weight distribution ratio and ankle edema. These results revealed that CR herb-acupuncture was effective to treat the inflammatory pain and could be used as an analgesic treatment with an antiinflammatory effect.
Bee venom (BV) has been traditionally applied to relieve pain and to cure inflammatory diseases such as rheumatoid arthritis (RA) and neuritis. While several investigators have evaluated the anti-inflammatory effect of BV treatment, the anti-nociceptive effect of BV treatment on inflammatory pain is not reported. Therefore, we decided to evaluate the analgesic effect of BV treatment using Freund's adjuvant induced chronic arthritis model. Freund's adjuvant-induced arthritis has been used as an experimental animal model for RA in humans to assess the efficacy of the anti-inflammatory/analgesic drugs. In this study, subcutaneous BV treatment (1mg/kg/day) produced significantly reductions of symptoms related to arthritic pain (i.e. mechanical hyperalgesia and thermal hyperalgesia). The anti-nociceptive effect of BV was observed from at least 12 days after BV treatment. Furthermore, BV treatment significantly suppressed adjuvant induced Fos expression in lumbar spinal cord. We also found that local injection of BV into near the inflammatory site (especially Zusanli-acupoint) showed more potent analgesic effect on arthritic pain rather than distant injection of BV from inflammatory site (arbitrary side of back). The present study demonstrates that BV treatment has anti-nociceptive effect on arthritis induced inflammatory pain. The analgesic effect of BV on RA is probably mediated by the effect of BV itself or possible other mechanism such as counter-irritation. Furthermore, it is possible that BV acupuncture is one of the promising candidates for long-term therapy of RA.
Objectives: The purpose of this study was to show effectiveness of Acupuncture treatment on gastrointestinal dysfunctions after Cesarean section by analyzing randomized controlled clinical trials. Methods: We searched randomized controlled clinical trials related with acupuncture treatment on gastrointestinal dysfunctions after Cesarean section through national and overseas database and analyzed them in detail. Results: 6 articles were included according to our selection criteria and 1,084 women were involved. 3 studies used TEAS (Transcutaneous Electrical Acupoint Stimulation), 2 studies used Elastic band and 1 study used Acupressure by hand. Their results were statistically more effective than control groups. The most frequently used acupoints were Neiguan (PC6), Zusanli (ST36) followed by Sanyinjiao (SP6), Hegu (IL4). Conclusions: There was significant difference in the effectiveness of the intervention including Acupuncture treatment. Based on analysis, it could be an effective way for the treatment of gastrointestinal dysfunctions after Cesarean section in clinical practice.

Objective: There are currently no disease-modifying medications or definite long-term sustainable interventions for patients with Parkinson's disease (PD), indicating an unmet treatment need. Our goal was to create a long-term sustainable intervention for PD patients that can be used in Korean medicine clinics. Methods: The Meridian Activation Remedy System (MARS) was created to stimulate a patient's 12 meridians and sinew channels using a combination of acupoint stimulation and exercise. The acupoints and motions used in MARS were selected through literature studies and expert advice. The methodologies were refined using observational and case studies. With slow and fast movements, the MARS intervention was intended to activate both slow- and fast-twitch muscle fibers. Intradermal acupuncture and motion that shift the center of gravity were employed to enhance the patient's balance and proprioception. In addition, the intervention included alternating movement exercises to address the complex cognitive decline commonly occurring in PD patients. Results: The following acupoints were chosen for the MARS intervention: bilateral Hegu (LI4), Houxi (SI3), Waiguan (TE5), Neiguan (PC6), Zhongchong (PC9), Yuji (LU10), Zusanli (ST36), Yanglingquan (GB34), Taichong (LR3), Kunlun (BL60), and Taixi (KI3). We also developed actions that can stimulate the body's 12 meridians. Conclusion: We developed the MARS intervention, which combines acupuncture and exercise, to address the unmet therapeutic needs of PD patients. We hope that with additional research, the MARS intervention can be set as an effective therapeutic program for PD patients.
Objectives: This study aimed to systematically review thread-embedding treatment studies for depression, anxiety, and dementia and examine the effectiveness and safety of thread-embedding treatment. Methods: Ten domestic and international search databases were used for study selection, including CNKI, PubMed, CENTRAL, EMBASE, CINAHL, AMED, PsycArticles, RISS, OASIS, and KCI. We included studies that presented diagnostic or appropriate criteria for depression, anxiety, and dementia, and randomized control studies using thread-embedding therapy. We searched papers published until October 10, 2023. Results: Twenty-one studies were selected, of which 11 studies were conducted on depression, nine on dementia, and one on anxiety disorders. The most commonly used acupoint for depression was Gansu (BL18), whereas zusanli (ST36) and fenglong (ST40) were used for dementia. The most commonly used type of thread was 1-0 United States pharmacopeia (USP) and 1 cm for depression and 2-0 USP and 1 cm for dementia. The treatment period for most of the studies was once every 2 weeks and for 8 weeks. Among the included studies, 17 showed significant improvements in depression scales, such as Hamilton depression rating scale and Self rating depression scale, activities of daily living scales, and cognitive function scales, such as Hasegawa dementia scale and Mini-mental state examination. Six studies reported adverse events, and no studies reported significant adverse events. Two studies reported follow-ups. Conclusions: This study presents limited evidence for the effectiveness and safety of thread-embedding therapy for depression, anxiety, and dementia. Well-designed studies are needed to review the clinical efficacy and safety of thread-embedding therapy in the future.
Objective : Functional brain mapping study on acupuncture stimulation using the [14C]2-deoxyglucose([14C]2-DG) autoradiography provides quantitative data and visualized pathway in central nervous system(CNS). We aimed to investigate the neural pathway and spatial distribution of metabolic activity elicited in CNS on electroacupuncture stimulation using [14C]2-DG autoradiography. Methods : The study were divided into three groups by stimulation times. 45-mins stimulation group according to Sokoloffs method, 5-mins stimulation group according to Duncun's method, and 15-mins stimulation group. ;A venous catheter was equipped into right jugular vein. The rats (Sprague-Dawley rats, 230-260g) were kept fastened loosely on a holding platform without anesthesia. Electroacupuncture stimulation (5 ms, 2 Hz, 1~3 mA) were applied on the left Zusanli (ST36) acupoint and [14C]2-DG ($25{\mu}Ci/rat$) injection was performed through the catheter. After sacrifice, the brain and the spinal cord were made to sections for film image. The film images were digitalized as the isotope concentration based upon comparison of optical densities with that of the standards and normalized by the optical density of corpus callosum. Results : 1. 15-mins stimulation group was most effective among 3 experiments. 2. On 15-mins stimulation group, medial geniculate nucleus, intetpeduncular nucleus intermedius, ventral periolivary nucleus, caudal periolivary nucleus, medial superior olive, lateral paragigantocellular nucleus, including hypothalamic arcuate nucleus were increased by more than 25% (at least, p<0.05) by electroacupuncture stimulation. 3. Especially, the metabolism in hypothalamic arcuate nucleus was increased by 90% (p<0.05). 4. The fact that arcuate nucleus of hypothalamus might play a role of interconnection area between ascending and descending pathway of acupuncture stimulation was demonstrated visually. Conclusions : Advanced study on electroacupuncture stimulation elicited significant increase of metabolic activity in various nuclei of hypothalamus will provide the important experimental basis in research of the relationship between electroacupuncture stimulation and internal visceral functions.
In order to examine the anti-arthritic properties of ShinEumHur(SEH)-herbal acupuncture, an adjuvant-induced arthritic rat was generated by the intra-articular injection of dried cells of Mycobacterium tuberculosis emulsified in squalene into the right knee joint. Fifty microliter of SEH extract was injected into Zusanli(ST36) acupoint on the ipsilateral hind paw every other day for 2 weeks. The body weight, knee circumference, squeak threshold, and weight distribution ratio were analyzed as the assessment methods addressing arthritic symptoms such as arthritic pain, edema, and tenderness. The weight distribution ratio was measured by a digital-type analgesia instrument using the dual channel scale that separately measures the weight the arthritic rat distributes to each hind paw, and thus quantifies both of swelling and pain severities at once. The therapeutic effects of SEH-herbal acupuncture, assessed by squeaking threshold and weight distribution ratio, were observed on 8th day after the arthritis induction as compared to saline group and control group. On 10th day, SEH-herbal acupuncture therapy significantly started to alleviate the growing pattern of knee circumference of an arthritic rat in the range of 0.2㎝. However, the loss of body weight was not significantly recovered. Taken together, the SEH-herbal acupuncture exhibited the significant therapeutic efficiency to treat adjuvant-induced monoarthritis in rat.
Backgrounds : Recently, commercial indirect moxibustion has been widely used as a substitute for traditional indirect moxibustion by clinicians due to its convenience in using. But, there has been lack of studies about the similarity of thermal stimulation between commercial indirect moxibustion and traditional indirect moxibustion. Objectives : To demonstrate commercial indirect moxibustion can substitute for traditional indirect moxibustion by comparing thermal stimulation between the two. Methods : We measured bottom temperature of commercial indirect moxibustion and traditional indirect moxibustion with various thicknesses of ginger and garlic. A clinical trial was performed on 18 healthy subjects to evaluate thermal stimualtion. The subjects were randomly assigned into two groups, traditional indirect moxibustion with ginger and garlic. The skin temperature and heating time were measured. They received moxibustion therapy with commercial indirect moxibustion and traditional indirect moxibustion according to their groups. Acupoint Chogcoe($LU_6$), Zusanli($ST_{36}$) and Tianshu($ST_{25}$) were used in each group for 3 times. Results : There was no difference in bottom temperature among commercial indirect moxibustion, traditional indirect moxibustion with 2mm ginger and 3mm garlic. No major difference was detected in the subjects' baseline data. There was no difference in skin temperature and heating time among these 3 type of moxibustions. Also, No major difference was detected in VAS score of thermal stimulation and frequency of burns among these 3 type of moxibustions. But, most subjects regarded their sensation weren't identical because of its own distinct characteristics. Conclusions : Commercial indirect moxibustion, traditional indirect moxibustion with 2mm ginger and 3mm garlic have same thermal stimulation. Thus, commercial indirect moxibustion can substitute for traditional indirect moxibustion.
Objectives : The present study was conducted to evaluate the effects of automatically controlled rotating acupuncture (ACRA) on thermal allodynia in neuropathic pain rats, and to examine whether the endogenous opioid system mediates the effects of ACRA. Methods : For the neuropathic surgery, the right superior caudal trunk was resected at the level between S1 and S2 spinal nerves innervating the tail. Two weeks after the nerve injury, ACRA stimulation with 4 different stimulation conditions (i.e., angle and frequency of rotation: 90o+1Hz, 90o+1/4Hz, 360o+/1Hz, and 360o+1/4Hz) was delivered to the Zusanli (ST36) acupoint for 15 min. The behavioral signs of thermal allodynia were evaluated by the tail immersion test (i.e., immersing the tail in cold $(4^{\circ}C)$ or warm $(4^{\circ}C)$ water and measuring the latency to an abrupt tail movement) before and after the stimulation. In an additional set of experiments, we examined the effects of naloxone (opioid Results : ACRA stimulations under all of the conditions above significantly relieved thermal antagonist, 2mg/kg, i.p.) on the action of ACRA stimulation. allodynia. There is no difference in the anti-allodynic effects among the 4 stimulation conditions. In addition, the effect of ACRA on thermal allodynia was reversed by naloxone pretreatment. Conclusion : These results indicate that ACRA stimulations have relieving effects on thermal allodynia in neuropathic pain rats, irrespective of stimulation parameters, and that this is mediated by the endogenous opioid system.
