Association of Cytotoxic T-lymphocyte Antigen-4 Polymorphisms with Malignant Bone Tumors Risk: A Meta-analysis

  • Background: Previous studies have assessed the association between the Cytotoxic T-lymphocyte Antigen-4(CTLA-4) polymorphism with the risk of malignant bone tumor, but the conclusions were inconsistent. We aimed to clarify association of cytotoxic T-lymphocyte antigen-4 polymorphisms with malignant bone tumors risk by performing a meta-analysis. Materials and Methods: The databases including PubMed, EMBase databases and the Cochrane Library were searched to identify the eligible studies prior to January 30 2016. Odds ratio (OR) with 95% confidence interval (95%CI) were used to estimate the strengths of the association between the CTLA-4 polymorphism and the malignant bone tumor risks. The meta-analysis was performed by STATA 12.0. Results: Four individual studies with a total of 1003 cases with malignant bone tumor and 1162 controls were included in our meta-analysis. The results of meta-analysis on those data demonstrated that CTLA-4 +49G>A polymorphism was associated with the risk of Ewing's sarcoma and osteosarcoma strongly (A vs. G: OR=1.36, 95%CI:1.20-1.54, p=0.000; AA+AG vs. GG: OR=1.35, 95%CI:1.14-1.61, p=0.001; AA vs. GG: OR=2.24, 95%CI:1.67-2.99, p=0.000; AA vs. AG+GG: OR=2.00, 95%CI:1.53-2.62, p=0.000), but CTLA-4 -318C/T polymorphism was not associated with the risk of malignant bone tumor (C vs. T: OR=0.76, 95%CI:0.76-1.08, p= 0.262; CC+CT vs. TT: OR=0.70, 95%CI:0.41-1.20, p= 0.198; CC vs. TT: OR=0.69, 95%CI:0.40-1.19, p= 0.183; CC vs. CT+TT: OR=0.92, 95%CI:0.75-1.13, p= 0.419). Subgroup analysis showed that there are significantly positive correlations between CTLA-4 +49G>A polymorphism and increased risks of malignant bone tumors in large size of sample (A vs. G: OR=1.347, 95%CI: 1.172,1.548, p=0.000; AA vs. GG: OR=2.228, 95%CI: 1.608,3.085, p=0.000), Ewing's Sarcoma or Osteosarcoma (A vs. G: OR=1.361, 95%CI: 1.201,1.540, p=0.000; AA vs. GG: OR=2.236, 95%CI: 1.674,2.986, p=0.000), and PCR-RFLP or Sequencing(A vs. G: OR=1.361, 95%CI: 1.201,1.540, p=0.000; AA vs. GG: OR=2.236, 95%CI: 1.674,2.986, p=0.000), but CTLA-4 -318C/T polymorphism was not associated with the risk of malignant bone tumors in diagnosis, genotype method, and sample size (all p>0.05). Conclusions: CTLA-4 +49A/G variant was associated with an increased risk of developing the malignant bone tumors, such as Ewing's sarcoma and osteosarcoma. However, it failed to show the association between CTLA-4 -318C/T polymorphism and the risk of malignant bone tumors. Future large-scale studies remain to be done to confirm our conclusions.

우지 Alcoholysis 반응물을 이용한 Oil-in-Water Emulsion의 포화도에 따른 산화특성 및 안정성 연구 (Stability of Oil-in-Water Emulsions with Different Saturation Degrees from Beef Tallow Alcoholysis Products)

  • 본 연구는 urea fractionation을 통하여 포화도 함량이 5.8%, 28.4%, 39.2%, 50.7%, 72.3%인 우지 alcoholysis 반응물을 제조하고, 이를 유상(oil phase)으로 ultrasonic processor 및 고압균질기를 이용하여 emulsion을 제조한 후 이에 대한 유화 안정성과 산화 안정성을 실험하였다. 유화 안정성(ES)을 관찰한 결과 ES값은 46.0(${\Sigma}$SFA5), 39.5(${\Sigma}$SFA28), 32.7(${\Sigma}$SFA39), 32.6(${\Sigma}$SFA50) 및 27.3(${\Sigma}$SFA72)로써 포화도가 낮은 emulsion일수록 높은 안정성을 보였으며, Turbiscan을 이용하여 시료 emulsion의 creaming 및 clarification 특성을 조사한 결과도 ES 결과와 유사하게 나타났다. 고압균질기로 제조한 emulsion 시료에 대하여 30일간 산화특성 및 안정성 연구를 진행한 결과, 제조한 emulsion의 hydroperoxides 함량이 1.880(${\Sigma}$SFA5), 1.267(${\Sigma}$SFA28), 1.062(${\Sigma}$SFA39), 0.342(${\Sigma}$SFA50) 및 0.153(${\Sigma}$SFA72)mg $H_2O_2/mL$로 서로 유의적 차이(p<0.05)를 나타내면서 불포화도가 높은 emulsion 시료일수록 hydroperoxide의 함량이 높았고, TBARS 값은 저장 30일에 6.229(${\Sigma}$SFA5), 6.801(${\Sigma}$SFA28), 6.246(${\Sigma}$SFA39), 4.419(${\Sigma}$SFA50) 및 4.226(${\Sigma}$SFA72) mg TBA/mL로 ${\Sigma}$SFA50과 ${\Sigma}$SFA72가 다른 emulsion 시료보다 유의적으로(p<0.05) 낮은 값을 보이며 산화 안정성이 우수하였다.

Case-Control Study of Diet in Patients with Cervical Cancer or Precancerosis in Wufeng, a High Incidence Region in China

  • Purpose: To investigate the diet of patients with cervical cancer and precancerosis in the Wufeng area, a high-incidence region in China. Methods: In the case group, 104 patients diagnosed with cervical cancer or cervical intraepithelial neoplasias (CINII/III) were recruited from the Wufeng area. Nine hundred thirty-six healthy women were selected from the same area as the matched controls. A questionnaire, which included questions about general lifestyle conditions, smoking and alcohol status, source of drinking water, green tea intake, and diet in the past year, was presented to all participants. Results: Green tea intake (P=0.022, OR=0.551, 95% CI=0.330-0.919) and vegetable intake (P=0.035, OR=0.896, 95% CI=0.809-0.993) were identified as protective factors against cervical cancer or CINII/III. There was no indication of any associations of other lifestyle factors (smoking status, alcohol status, source of drinking water) or diet (intake of fruit, meat/egg/milk, soybean food, onion/garlic, staple food and pickled food) with cervical cancer. Conclusions: The results suggest that eating more fresh vegetables and drinking more green tea may help to reduce the risk of cervical cancer or CINII/III in people of the Wufeng area.

항전간제를 투여받은 한국인 환자에서의 약리유전학적 영향 (Pharmacogenetic Impact on Korean Patients Receiving Antiepileptic Drugs)

  • 간질은 가장 흔한 만성 신경 장애로 70% 이상의 환자에서 항전간제로 증상의 조절이 가능하다. 약물의 치료효과를 예측하는데 있어 개인이 가진 유전적 다형성이 부분적인 영향을 주는 것으로 알려지면서 항전간제의 효과에 영향을 주는 유전자 연구가 빠르게 진행되고 있다. 본 연구에서는 83명의 간질 환자를 대상으로 간질 환자의 약물대사와 관련된 것으로 알려진 유전자(CYP2C9, CYP2C19, ABCB1, SCN1A)의 다형성과 약물부작용의 관계에 대해 연구하였다. 연구 결과, 약물 이상 반응과 약물 용량의 상관관계는 통계적으로 유의한 수준은 관찰되지 않았다. 한편, carbamazepine 계열에서의 환자군에서는 SCN1A 유전자의 인트론 유전자 유전형 CC와 CT 유전형에 비해 TT 유전형에서 약물용량과 연관을 보였으며, 500 mg 이상의 용량을 투약한 환자에서는 TT 유전형에 비해 CC와 CT를 가진 유전형에서 통계적으로 유의한 상관성을 보였다.

비소세포폐암 세포주에서 pemetrexed의 세포독성과 유전학적 다형성과의 상관성 조사 (Association of Genetic Variations with Pemetrexed-Induced Cytotoxicity in Non-Small Cell Lung Cancer Cells)

  • 페메트렉시드(pemetrexed, $alimta^{(R)}$)는 중피종(mesothelioma)과 비소세포폐암 (non-small cell lung cancer)을 비롯한 다양한 암종에서 엽산(folate) 대사과정에 관여하는 대사물질의 활성을 억제하여 항암효능을 나타낸다. 다중표적 항암제 (multitargeted antifolate)인 pemetrexed는 엽산의 세포내 주요 이동통로인 reduced folate carrier(RFC)를 통해 세포 내로 유입된 후 folylpolyglutamate synthetase (FPGS)에 의해 폴리글루타민산염(polyglutamate) 유도체로 활성되고 thymidylate synthase (TS)와 dihydrofolate reductase (DHFR)를 표적하는 것으로 알려져 있다. 조직형이 서로 다른 비소세포폐암 세포주를 선정하여 pemetrexed의 대사과정에 관여하는 유전자들의 단일염기서열 다형성을 조사하고, mRNA와 단백질의 발현 정도를 비교하여 pemetrexed의 세포독성 효과와의 상관성을 분석하였다. 4개의 비소세포폐암 세포주인 A549, PC14, HCC-1588과 H226에서 RFC, FPGS, TS와 DHFR의 유전형을 조사하였다. Pemetrexed의 약물의 감수성을 알아보기 위해 real-time PCR과 Western blot 방법으로 mRNA 발현과 단백질 발현 정도를 비교하였고, SRB 법으로 약물에 대한 세포독성 효과를 측정했다. PC14 세포주와 H226 세포주에서는 약물처리 전 RFC와 FPGS의 mRNA 발현이 높은 것으로 나타났고, $IC_{50}$값이 각각 $0.08{\pm}0.01\;uM$$0.07{\pm}0.01\;uM$로 pemetrexed에 대한 감수성이 높은 것을 알 수 있었다. A549 세포주에서 TS의 유전형이 2R/2R일 때 mRNA발현이 증가하고 pemetrexed의 약물 저항성과 관련이 있었다. 반면, TS의 유전형이 3R/3R로 나타난 H226에서는 mRNA 발현이 낮은 것을 알 수 있었지만 pemetrexed의 높은 감수성과 관련이 있었다. 세포주 모두에서 pemetrexed 약물처리 후 DHFR의 mRNA 발현은 약물처리 전보다 낮아지는 경향을 보였지만 단백질 발현은 오히려 증가하는 상반된 결과를 보였다. 또한 DHFR 프로모터에 위치한 -1726C>T, -1188A>C SNP는 서로 연쇄 불평형 상태(linkage disequilibrium, LD)에 있었다. 연구결과에서 pemetrexed의 세포독성 효과는 약물 대사과정에 관여하는 여러 분자들의 유전형과 발현 정도에 의해 결정되는 것을 알 수 있었고, 다양한 분석결과를 토대로 항암효능을 평가하는 것이 필요하다고 생각된다.

일본(日本) 의학(醫學)의 '절충파(折衷派)'에 관(關)한 연구(硏究) (A Study on the ' Zhe Zhong Pai'(折衷派) of the Traditional Medicine of Japan)

  • The outline and characteristics of the important doctors of the 'Zhe Zhong Pai'(折衷派) are as follows. Part 1. In the late Edo(江戶) period The 'Zhe Zhong Pai', which tried to take the theory and clinical treatment of the 'Hou Shi Pai (後世派)' and the 'Gu Fang Pai(古方派)' and get their strong points to make treatments perfect, appeared. Their point was 'The main part is the art of the ancients, The latter prescriptions are to be used'(以古法爲主, 後世方爲用) and the "Shang Han Lun(傷寒論)" was revered for its treatments but in actual use it was not kept at that. As mentioned above The 'Zhe Zhong Pai' viewed treatments as the base, which was the view of most doctors in the Edo period. However, the reason the 'Zhe Zhong Pai' is not valued as much as the 'Gu Fang Pai' by medical history books in Japan is because the 'Zhe Zhong Pai' does not have the substantiation or uniqueness of the 'Gu Fang Pai', and also because the view of 'gather as well as store up'(兼收並蓄) was the same as the 'Kao Zheng Pai'. Moreover, the 'compromise'(折衷) point of view was from taking in both Chinese and western medical knowledge systems(漢蘭折衷). Generally the pioneer of the 'Zhe Zhong Pai' is seen as Mochizuki Rokumon(望月鹿門) and after that was Fukui Futei(福井楓亭), Wadato Kaku(和田東郭), Yamada Seichin(山田正珍) and Taki Motohiro(多紀元簡). Part 2. The lives of Wada Tokaku(和田東郭), Nakagame Kinkei(中神琴溪), Nei Teng Xi Zhe(內藤希哲), the important doctors of the 'Zhe Zhong Pai', are as follows. First Wada Tokaku(和田東郭, 1743-1803) was born when the 'Hou Shi Pai' was already declining and the 'Gu Fang Pai' was flourishing and learned medicine from a 'Hou Shi Pai' doctor, Hu Tian Xu Shan(戶田旭山) and a 'Gu Fang Pai' doctor, Yoshimasu Todo(吉益東洞). He was not hindered by 'the old ways(古方)' and did not lean towards 'the new ways(後世方)' and formed a way of compromise that 'looked at hardness and softness as the same'(剛柔相摩) by setting 'the cure of the disease' as the base, and said that to cure diseases 'the old way' must be used, but 'the new way' was necessary to supplement its shortcomings. His works include "Dao Shui Suo Yan(導水瑣言)", "Jiao Chiang Fang Yi Je(蕉窗方意解)" and "Yi Xue Sho(醫學說)". Second. Nakagame Kinkei(中神琴溪, 1744-1833) was famous for leaving Yoshimasu Todo(吉益東洞) and changing to the 'Zhe Zhong Pai', and in his early years used qing fen(輕粉) to cure geisha(妓女) of syphilis. His argument was "the "Shang Han Lun" must be revered but needs to be adapted", "Zhong Jing can be made into a follower but I cannot become his follower", "the later medical texts such as "Ru Men Shi Qin(儒門事親)" should only be used for its prescriptions and not its theories". His works include "Shang Han Lun Yue Yan(傷寒論約言)". Third, Nei Teng Xi Zhe(內藤希哲, 1701-1735) learned medicine from Qing Shui Xian Sheng(淸水先生) and went out to Edo. In his book "Yi Jing Jie Huo Lun(醫經解惑論)" he tells of how he went from 'learning'(學) to 'skepticism'(惑) and how skepticism made him learn in 'the six skepticisms'(六惑). In the latter years Xi Zhe(希哲) combines the "Shen Nong Ben Cao Jing(神農本草經)", the main text for herbal medicine, "Ming Tang Jing(明堂經)" of accupuncture, basic theory texts "Huang Dui Nei Jing(皇帝內經)" and "Nan Jing(難經)" with the "Shang Han Za Bing Lun", a book that the 'Gu Fang Pai' saw as opposing to the rest, and became 'an expert of five scriptures'(五經一貫). Part 3. Asada Showhaku(淺田宗伯, 1815-1894) started medicine at Zhong Cun Zhong Zong(中村中倧) and learned 'the old way'(古方) from Yoshimasu Todo and got experience through Ouan Yue(川越) and Fu Jing(福井) and received teachings in texts, history and Wang Yangmin's principles(陽明學) fmm famous teachers. Showhaku(倧伯) meets a medical official of the makufu(幕府), Ben Kang Zong Yuan(本康宗圓), and receives help from the 3 great doctors of the Edo period, Taki Motokato(多紀元堅), Xiao Dao Xue Gu(小島學古) and Xi Duo Cun Kao(喜多村栲窻) and further develops his arts. At 47 he diagnoses the general Jia Mao(家茂) with 'heart failure from beriberi'(脚氣衡心) and becomes a Zheng Shi(徵土), at 51 he cures a minister from France and received a present from Napoleon, at 65 he becomes the court physician and saves Ming Gong(明宮) Jia Ren Qn Wang(嘉仁親王, later the 大正天皇) from bodily convulsions and becomes 'the vassal of merit who saved the national polity(國體)' At the 7th year of the Meiji(明治) he becomes the 2nd owner of Wen Zhi She(溫知社) and takes part in the 'kampo continuation movement'. In his latter years he saw 14000 patients a year, so we can estimate the qualjty and quantity of his clinical skills. Showhaku(宗伯) wrote over 80 books including the "Ju Chuang Shu Ying(橘窻書影)", "Wu Wu Yao Shi Fang Han(勿誤藥室方函)", "Shang Han Biang Shu(傷寒辨術)", "Jing Qi Shen Lun(精氣神論)", "Hunag Guo Ming Yi Chuan(皇國名醫傳)" and the "Xian Jhe Yi Hua(先哲醫話)". Especially in the "Ju Chuang Shu Ying(橘窻書影) he says "the old theories are the main, and the new prescriptions are to be used"(以古法爲主, 後世方爲用), stating the 'Zhe Zhong Pai' way of thinking, In the first volume of "Shang Han Biang Shu(傷寒辨術)" and "Za Bing Lun Shi(雜病論識)", 'Zong Ping'(總評), He discerns the parts that are not Zhang Zhong Jing's writings and emphasizes his theories and practical uses.
