Acquired Pharyngeal Diverticulum after Anterior Cervical Fusion Operation Misdiagnosed as Typical Zenker Diverticulum

  • A pharyngeal diverticulum is a rare complication of an anterior cervical discectomy and fusion (ACDF). We present a case of a pharyngeal diverticulum after an ACDF, which was misdiagnosed as a typical Zenker diverticulum. A 54-year-old woman presented with dysphagia and a sense of irritation in the neck following C5 through C7 cervical fusion 3 years prior. The patient underwent open surgery to resect the diverticulum with concurrent cricopharyngeal myotomy. An ACDF-related diverticulum is difficult to distinguish from a typical Zenker diverticulum.

  • Submucosal endoscopy or third-space endoscopy utilizes the potential space between the mucosal and muscularis layers of the gastrointestinal tract to execute therapeutic interventions for various diseases. Over the last decade, endoscopic access to the submucosal space has revolutionized the field of therapeutic endoscopy. Submucosal endoscopy was originally used to perform endoscopic myotomy in patients with achalasia cardia, and its use has grown exponentially since. Currently, submucosal endoscopy is widely used to resect subepithelial tumors and to manage refractory gastroparesis and Zenker's diverticulum. While the utility of submucosal endoscopy has stood the test of time in esophageal motility disorders and subepithelial tumors, its durability remains to be established in conditions such as Zenker's diverticulum and refractory gastroparesis. Other emerging indications for submucosal endoscopy include esophageal epiphrenic diverticulum, Hirschsprung's disease, and esophageal strictures not amenable to conventional endoscopic treatment. The potential of submucosal endoscopy to provide easy and safe access to the mediastinum and peritoneal spaces may open doors to novel indications and rejuvenate the interest of endoscopists in natural orifice transluminal endoscopic surgery in the future. This review focuses on the current spectrum, recent updates, and future direction of submucosal endoscopy in the gastrointestinal tract.

  • Killian-Jamieson 게실은 경부 식도에 발생하는 드문 질환이며 Zenker 게실과는 발생 기전과 부위가 다른 것으로 알려져 있다. Killian-Jamieson 게실의 치료원칙에 대해서는 정립된 바가 없다. Killian- Jamieson 게실 환자에서 경부 절개 하에 수술적 치료를 시행하여 이를 보고하고자 한다.

  • Esophageal diverticulum is a rare entity, the true incidence of which is unknown, mainly because there are usually no clinical symptoms. Most esophageal diverticulum are found incidentally during an endoscopic or radiologic examination. Their classification is based on anatomical location; namely, upper third(Zenker or pharyngoesophageal), middle third(thoracic), or lower third(epiphrenic). Here we report a 52-years-old female presenting with dysphagia and regurgitation. Esophagogram showed esophageal diveticulum at lower cervical esophagus. Its positional aspect, it is different from Zenker's diveticulum. Treatment is surgical via an endoscopic or external approach. In view of the patient's age and anatomical location, various surgical approaches were considered as a therapeutic option for the management. This paper presents our experience in the management of esophageal diverticulum which was unusual location and stapled open resection without sternotomy.

  • Tracheal diverticulum is very rare, that is usually an incidental finding at routine chest computed tomography scan. Differential diagnosis of tracheal diverticulum includes pharyngocele, laryngocele, Zenker diverticulum, apical lung hernia, blebs and bulla, and pneumomediastinum. Treatment options can be devided into medical and surgical. The majority of patients is asymptomatic and requires no specific intervention. We experienced one case of tracheal diverticulum in patient with tongue cancer and report it with reviews of literature.

  • Killian-Jamieson diverticulum is a rare diverticular disease. This disease differs from Zenker's diverticulum in its location and mechanism. Various treatment modality have been attempted, but traditional surgical treatment has been recommended for a symptomatic Killian-Jamieson diverticulum due to the concern of possible nerve injury. We performed surgical treatment by cervical incision. We report here on a case of Killian-Jamieson diverticulum and we briefly review the relevant literature.

  • 식도게실은 식도 근육층의 결손 부위로 점막층이 감입됨으로써 발생하는 국한성 낭성 구조물로서 선천성 또는 후천성으로 발생할 수있다. 저자들은 1990넌 4월 17일 식사시 흉골 상부의 불쾌감을 주소로 중앙대학교 부속병원을 내원한 32세 여자 환자에서 식도 조영 촬영상 식도 우후방부, 윤상인두협부 하방 3㎝ 부위에서, 흔히 볼 수 있는 Zenker's diverticulum이 아닌 식도게실을 발견하여 식도게실제거술 및 근절개술 시행으로 특이한 합병증 없이 치유된 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

  • Killian-Jamieson and Zenker diverticula are both rare pharyngoesophageal diverticula. Both are outpouching of the mucosal and submucosal layers of the esophageal wall, which protrude through a mucosal gap at the level of the pharyngoesophageal esophagus. When these diverticula are large enough, they can be in proximity to the thyroid gland and may mimic a thyroid nodule. We report a case in which a diverticulum was filled with dietary residue and thus simulated a thyroid cyst on CT scan. And it was finally diagnosed as a Killian-Jamieson diverticulum by the surgery.
