전이성 고환주위 폐포성 횡문근육종 1예 (Paratesticular Alveolar Rhabdomyosarcoma with Multiple Lymph Nodes Metastasis Successfully Treated with Chemotherapy)

  • Rhabdomyosarcomas are soft tissue sarcomas; while extremely rare in adults, they are one of the most common neoplasms in children and adolescents. Histologically, they can be classified into embryonal (ERMS), alveolar (ARMS), pleomorphic, and undifferentiated types. The ARMS type is very rare, and is associated with a poor prognosis. Common primary sites of ARMS are the trunk and extremities. We report on a case of paraaortic, supraclavicular, and axillary lymph node metastasis from paratesticular ARMS treated with VAC (vincristine, dactinomycin, cyclophosphamide)/ IE(ifosfamide, etoposide) chemotherapy in a young adult. Administration of six cycles of chemotherapy with VAC/IE resulted in complete remission. The patient has maintained complete remission over the past 27 months.

히드록시프로필메칠셀룰로오스 프탈레이트 및 에칠셀룰로오스를 이용한 이부딜라스트 함유 서방성 매트릭스 정제의 개발 (Formulation of Sustained Release Matrix Tablets Containing Ibudilast with Hydroxypropylmethylcellulose Phthalate and Ethylcellulose)

  • To develop a sustained-release tablet which had the similar dissolution to commercial ibudilast-loaded sustained-release capsule, the tablets were prepared using hydroxypropylmethylcellulose phthalate (HPMCP), ethylcellulose (EC) and hydroxypropylcellulose (HPC), and dissolution test were carried out with paddle method in KP. The tablet prepared only with HPMCP and EC showed sno similar dissolution pattern to the commercial product. As the ratio of HPMCP/HPC in tablet decreased, the dissolution rate of drug decreased in pH 1.2 but increased in pH 6.8. Furthermore, an ibudilast-loaded sustained-release tablet composed of [ibudilast/EC/HPMCP/HPC (10/10/170/10 mg/tab)] gave similar dissolution to commercial product in pH 1.2 for 3 h and in pH 6.8 for 10 h. Thus, it could be a potential candidate for the substitute of commercial capsule.

  • Toxic epidermal necrolysis (TEN) is a rare, acute, serious, and potentially fatal skin disease, in which cell death causes the epidermis to separate from the dermis. It is thought to be a hypersensitivity complex that affects the skin and mucous membranes, and is caused by certain medications, infections, genetic factors, underlying immunologic disease, or more rarely, cancers. We report two cases of TEN associated with deflazacort (DFZ), a derivative of prednisolone, used in the first episode of nephrotic syndrome (NS). The skin eruption appeared on the $4^{th}$ and $5^{th}$ weeks after DFZ administration, while NS was in remission. The widespread lesions were managed by intensive supportive treatment, discontinuation of DFZ, and oral administration of cyclosporine. Both patients showed a rapid improvement in symptoms of TEN without any complications or relapse of NS.

  • Quality evaluation of Magnoliae Flos and Anthrici Radix was studied to ensure their safety and efficacy, which was based on physico-chemical determination of the content of total ash, acid insoluble ash and diluted ethanol-soluble extract. In addition, quantitative analysis of magnolin and anthricin by HPLC suggested that the content of magnolin and anthricin in Magnoliae Flos and Anthrici Radix was $31.09{\pm}15.74%$ and $1.08{\pm}0.85%$.
