가상쐐기와 기존쐐기의 물리적 특성 비교 (Physical Characteristics Comparison of Virtual Wedge Device with Physical Wedge)

  • 목적 : 지멘스사의 가상쐐기의 임상적용을 위하여 물리적 특성을 조사하고 기존쐐기의 특성과 비교하였다. 대상 및 방법 : 6 그리고 15MV x-선(Siemens PRIMUS)을 사용하여 각각의 명목상의 쐐기각(15, 30, 45, 그리고 60$^{\circ}$)에 대해서 가상쐐기와 기존쐐기에 대한 측정이 수행되었다. 쐐기인자는 조사면의 크기와 측정 깊이를 변화시키면 서 물속에서 전리함을 이용하여 측정되었으며 가상쐐기의 경우 빔이 조사되는 동안 upper jaw가 움직이기 때문에 쐐기각도는 일정시간 동안 방사선을 조사하여 누적된 값을 기록하였다. 쐐기각도는 조사면의 크기가 15cm${\times}$ 20cm이고 측정깊이가 10cm일 때 전리함을 물 속에 위치시킨 후 빔의 중심 축에 대해서 수직인 방향으로 off-axis 상에서 측정되었다. 쐐기의 사용으로 인한 표면선량의 변화를 조사하기 위하여 쐐기를 사용하지 않은 경우와 가상쐐기와 기존쐐기를 각각 사용하였을 때 팬톰 표면과 특정깊이에 각각 평판형 전리함(Markus chamber, PTW 23343, Freiburg, Germany)과 파머형 전리함(NE2571, Nucleal Enterprise, England)을 빔의 중심축 상에 위치시킨 후 방사선량을 동시에 측정하였다. 이때 조사면의 크기는 15cm${\times}$20cm이었고 폴리스티렌 팬톰을 사용하였다. 결과 : 가상쐐기와 기존쐐기의 조사면의 크기에 따른 쐐기인자의 변화량은 각각 최대 2.1${\times}$와 3.9${\times}$이었으며 깊이에 따른 변화량은 각각 최대 1.9${\times}$와 2.9${\times}$ 였다. 가상쐐기와 기존쐐기의 l0m 깊이에서의 명목상의 쐐기각에 대해 모두 정확하게 일치하였다. 기존쐐기를 사용했을 때 표면선량이 가상쐐기나 쐐기를 사용하지 않은 경우에 대해 최대 20${\times}$ 정도(x-선 에너지 : 6-MV, 명목상의 쐐기각:45$^{\circ}$, SSD:80cm) 감소하였다. 결론 : 지멘스사의 가상쐐기와 기존쐐기의 특성을 측정결과를 근거로 비교하였다. 가상쐐기는 기존쐐기에 비해 쐐기인자의 깊이 의존성이 적었으며 조사면의 크기 의존성에는 별 차이가 없었다. 쐐기각도의 정확성은 가상쐐기와 기존쐐기 모두 명목상의 쐐기각과 잘 일치하였다. 가상쐐기와 쐐기를 사용하지 않은 경우에 비해서 기존쐐기를 사용한 경우가 표면선량을 줄이는데 효과적이었다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The correlation between glucose concentration and parameters was the best at 610nm. 30535 T00401030535 ^x Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made. Comparisons between the valves were made on the transvalvular pressure drop, regurgitation volume and maximum valve opening area. The polymer valve showed smaller regurgitation volume and transvalvular pressure drop compared to the mechanical valve at higher heart rate. Because the point of maximun temperature of laser-irradiated gelatin exists below the surface due to surface cooling, evaporation at the boiling temperature is made explosively from below the surface. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. We obtained a two-dimensional current density distribution within a phantom by processing the real and imaginary MR images from a 4.77 NMR machine. We implemented a resistivity mage reconstruction algorithm using the finite element method and sensitivity matrix. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. Third, best-fit boundary is detected by the cost function which is based on the best-fit elliptic model. We use combination of MLP and fuzzy filter for pattern recognition to estimate the direction of a muscle and Hogan's method for the force estimation. They include the modules for electrocardiograph, respiration, invasive blood pressure, noninvasive blood pressure, temperature, and SpO2 with Plethysmograph.SpO2 with Plethysmograph.

The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. 두경부종양 세포주에서 Ku 단백질 발현 정도에 따른 방사선 민감도 (Expression of Ku Correlates with Radiation Sensitivities in the Head and Neck Cancer Cell Lines)

  • 목적: DNA-dependent protein kinase (DNA-PK)는 serine/threonine kinase로 470 kDa의 catalytic subunit (DNA- PKcs) 와 각각 70 kDa과 86 kDa의 무게를 갖는 Ku 70, Ku 80 단백질로 구성된다. 이 DNA-PK는 방사선에 의해 DNA의 두 가닥이 동시에 절단되는 경우 DNA 손상 복구에 핵심적 역할을 하는 것으로 알려져 있다. 본 연구에서 Ku 발현과 이온화방사선에 민감도와의 상관 관계를 알아보고자 하였다. 대상 및 방법: 예비실험으로 두경두종양 기원의 세포주 9개에 대한 방사선에 대한 민감도실험을 한 결과 AMC-HN3이 방사선에 가장 민감하였고, AMC-HN9이 방사선에 가장 저항성을 보여 2개의 세포주에 대한 Ku70/80의 발현을 Western blot과 면역형광 염색을 시행하여 방사선의 반응도와의 상관관계를 알아보았다. 결과: 방사선에 저항성을 보이는 AMC-HN9에서 Ku80 발현이 높았고 방사선에 민감한 AMC-HN3에서 세포자멸사가 더 많이 일어남을 관찰할 수 있었다. 결론: Ku80 단백질 발현이 높은 세포는 방사선에 대한 DNA손상 복구가 많이 되어 방사선에 대한 내성을 보이는 것으로 생각되었다.

급성전골수성백혈병 HL-60 세포주에서 방사선조사에 의한 세포고사기전 (A Study on Apoptotic Signaling Pathway in HL-60 Cells Induced by Radiation)

  • 목적 : 방사선 조사에 의하여 유발되는 세포고사의 신호전달기전, 특히 caspase계 cysteine protease의 활성화, Bcl2 및 Bax 단백질, cytochrome c의 세포질내로의 방출, Fas 와 Fas-L 단백질의 발현양상 등의 조사를 통하여 방사선 조사에 의하여 유발되는 세포고사기전을 규명하고자 하였다. 대상 및 방법 : HL-60 세포주에 6 MV의 X-선을 조사하고 세포생존율, Caspase의 활성도, $Bcl_2$ 및 Bax 단백질, cytochrome c의 세포질내로의 방출여부, 및 Fas 와 Fas-L 단백질의 발현양상을 조사하였다. 결과 : 방사선조사 후 세포의 생존율은 조사선량과 조사 후 시간경과에 따라 감소되었다. 세포고사의 특징인 사다리형 DNA 분절은 방사선조사 4시간 후부터 시간경과에 따라 증가하였으며, 조사선량이 증가할수록 더욱 현저하였다. 방사선조사 후 caspase계 cysteine proteases 중 caspase-2, 3, 6, 8 및 9의 활성화가 시간경과에 따라 증가하였으며, 16 Gy의 방사선량조사 4시간 후에 poly (ADP-ribosyl) polymerase (PARP)의 분절과 Western blot을 이용한 procaspase-3의 분절을 확인함으로서 caspase-3의 활성을 간접적으로 증명할 수 있었다. $Bcl_2$ 단백질은 방사선조사 후 시간경과에 따라 감소하였으며, Bax 단백질은 시간경과에 따라 발현이 증가하는 양상을 관찰할 수 있었다. 방사선조사 후 cytochrome c의 세포질내로의 방출을 확인하였다. 또한 Fas 및 Fas-L 단백질 모두 방사선조사 후 발현이 증가하는 양상을 관찰할 수 있었다. 결론 : HL-60 세포주에서 방사선 조사에 의해 유발되는 세포사멸이 세포고사기전에 의해서 매개됨을 확인하였으며, 이는 세포내 caspase계 cysteine proteases, $Bcl_2$, Bax, 세포질내로의 cytochrome c 방출 그리고 Fas, Fas-L가 관여하는 신호전달경로의 활성화에 의한 것임을 의미하였다.

  • 목적 : 강내에 발생된 종양치료용 원통형 전자선 조사기구(Electron cone)는 기하학적으로 강내벽에 위치한 종양치료에 부적당하므로 후방 또는 측면방향으로 산란되는 전자선을 이용하여 체강 내벽점막 등에 발생된 종양을 효과적으로 치료할 수 있는 산란전자선 치료방법을 개발하고자 한다. 강내조사기구내에 전자선 입사방향에 수직 또는 일정한 각도의 산란판을 배치하여 측면방향으로 산란전자선을 방출시키는 강내 측면조사기구를 제작하고 산란판의 제원과 전자선 에너지에 따라 산란방출된 산란선의 특성과 조직내 선량분포를 측정 평가하였다. 새상 미 방법 : 외부조사용 전자선조사기구(Electron cone) 대신에 강내 삽입용 전자산란선 조사통(Intracavitary backscatter electron cone)과 이를 콜리메이터와 연결시킬 수 있는 차폐연결기구(Shielded electron device)를 고안하였다. 산란전자선 조사기구는 직경이 $2\~3\;cm$이고 길이가 25 cm인 금속(내식강)원통을 이용하였으며 입구에서 20 cm위치에 산란판을 부착시키고 원통 측면에 직경 $1\~2\;cm$의 산란선 방출구를 제작하였다. 산란판은 $2\~10\;mm$의 연판을 사용하였으며, 오제전자와 특성 엑스선을 제거하기 위하여 주석, 구리, 알루미늄판 등을 부착시켰으며 종양위치를 관찰할 수 있도록 표면을 처리하였다. 고에너지 방사선치료용 선형가속기(Clinac 2100C/D)에서 발생된 $6\~12\;MeV$ 에너지의 전자선을 이용하였으며 선량측정은 평행평판형 전리상(Markus chamber, PTW 23343)을 조직등가 팬텀(Polystyrene)에 삽입하여 측정하였다. 전자산란선의 에너지분포는 Monte Carlo (EGS4) 계산으로 예측하였으며 조직내 선량분포는 필름 흑화도(X-Omat V, Wellhofer 700i)에 의하여 측정하였다. 결과 : 전자선 입사에너지가 6 MeV일 때 전자산란선의 평균 에너지는 약 1.5 MeV 이었으며 산란각이 클수록 에너지는 줄어들었다. 입사 전자선 에너지 6 MeV 에서 산란판의 각도 $30^{\circ},\;45^{\circ}$ 에 따른 최대선량지점은 산란선 방출구의 중심에서 각각 5 mm 및 -10 mm지점의 표면에서 발생되며 입사전자선에 대한 전자산란선의 선량비는 약 $8.5\%$ 내외로 측정되었다. 입사전자선에너지 6 MeV에서 산란판각도 $45^{\circ},\;60^{\circ}$에 의한 $50\%$의 심부선량분포는 각각 6 mm와 7 mm 깊이에 도달하였으며 입사에너지 증가에 비례하였다. 결론 : 전자선 후방산란의 특성을 연구하고 이를 인체 강내 측방 점막부위에 발생한 종양을 효과적으로 치료할 수 있는 강내 전자산란선 조사통을 고안 제작 하였다. 시험용으로 제작한 전자산란선 조사기구를 이용하여 전자선 에너지와 산란판의 각도에 따른 산란선의 선량비율과 심부율을 측정하였다. 구강, 자궁, 직장 등 강내측벽 점막 등에 발생된 악성종양의 모양과 깊이에 가장 적당한 입사 에너지, 산란판의 각도, 산란창구 및 조사각도를 선택함으로서 방사선치료방법을 향상시킬 수 있을 것이라고 기대된다.

폐암의 방사선치료 결과 (Result of Radiation Therapy for the Lung Cancer)

  • An analysis has been made of two hundred seven patients who were treated at the department of Radiation Oncology of Korea University Hospital for lung cancer from January 1981 through December 1986. There were 137 patients of nonsmall cell carcinoma (137/207, 66%), 26 patients of small cell carcinoma (26/207, 12.5%) and 44 patients of unproven histology. By aims of treatment, there were 104 patients (104/207, 50%) treated for cure, 89 patients (89/207, 42.9%) for palliation and 14 patients treated postoperatively. In 22 out of 207 patients, chemotherapy was done with radiotherapy, 12 of which were patients with small cell carcinoma. Stage II patients were 49 (49/207, 23.6%), stage III patients were 157 (157/207, 75.8%) and one patient had an occult cancer The tumor was initial Iy measured by CAT scan and chest X-rays in the 165 (165/207, 79.7%) patients, among which 117 patients had tumor diameter more than 5cm and 48 patients less than 5cm. Radiation therapy was given with Cobalt 60 teletherapy unit and the treatment volume encompassed primary tumor and the mediastinum. For curative aim, daily tumor dose of 180 cGy was given up to the range of 5,400~6,120cGy/30~34F/6~7 week period and for palliative aim, daily tumor dose of 300 cGy was given up to the range of 3,600~4,500 cGy/12~15F/2~3 week period. Postoperatively, mediastinum was treated for total dose of 5,040 cGy/28F/5.5 week period. 123 patients (123/207, 59%) were followed up after completion of radiotherapy for 14 months to 7 years. Local tumor response to the irradiation was measured by chest X-ray taken at one month follow up and was evaluated for response rate, if they were regressed more than 50% or less than 50% of the initial tumor size. The treatment results were as follows; 1. The median survival time was 8.5 months and survival rates for 1 year, 2 year and 5 year was 25%, 3.5% and 1% of nonsmall cell lung ca of 74 evaluable patients. 2. More than 50% of local tumor response rate was obtained in about half of overall cases; 90.5% for small cell ca, 50% for squamous cell ca, 25% for adenoca and 57% for large cell ca. 3. Response rate more than 50% was seen in the 50% of the patient group with tumor diameter more than 5cm and in the 55% of those with tumor diameter less than 5cm. 4. By total raidation dose given, patient group which was given 5,400~6,120 cGy equivalent dose or higher showed tumor response rate more than 50% in 53% of the patients, whereas the group with dose less than 5,400cGy equivalent, in 25% of the patients. 5. Survival rate for 6 month, 1 year and 2 year was compared between the group of local tumor response rate more than 50% vs. group with response rate less than 50%; 74% vs. 43%, 33% vs, 23%, 10% vs. 1%, respectively. 6. Local failure was seen in 21%(44/207) of the patients, which occured mostly within 15 months after completion of radiation therapy. Distant metastases were seen in 49.7%(103/207) of the patients, of which 43 cases were found before initiation of radiotherapy. The most common metastatic sites were bone and brain. In this sutdy, 1 year,2 year and S year survival rates were somewhat poor compared to the other studies. It mainly seems to be due to the poor general status of the patients and the far-advanced stage of the disease. In nonsmall cell cancer patients who had limited local disease and had small primary tumor size, we observed better local response. In addition, dose higher than 6,000 cGy group showed better tumor control than lower dose group. Survival rate was better for the local control group. For imporvement of local control of the lung cancer and hence, the survival of the patients with lung cancer, proper radical radiotherapy with high dose for localized disease is needed. New modality of treatment such as high LET beam in radiation therapy or drugs for the advanced disease as well as early diagnosis is also needed.

국한성 소세포 폐암에서 항암 화학 및 흉부 방사선치료의 병합요법 적응 (Combined Chemotherapy and Radiation Therapy in Limited Disease Small-Cell Lung Cancer)

  • 목적 : 국한성 병기의 소세포 폐암에 대하여 항암 화학 및 흉부 방사선치료의 병합요법을 적용하여 국소 반응율, 급성 부작용의 빈도, 단기 임상 추적 관찰 결과 등을 보고하고자 한다. 대상 및 방법 : 1994년 10월부터 1998년 4월까지 국한성 병기의 소세포 폐암으로 진단되어 VIP 요법(etoposide, ifosfamide, cis-platin) 또는 EP 요법(etoposide, cis-platin)의 복합 항암 화학 및 흉부 방사선치료의 동시 병합요법을 시행받은46명의 환자를 대상으로 하였다. 항암 화학요법은 3주 간격으로 모두 6회의 시행을 목표로 하였고, 흉부 방사선치료는 10MV X-ray를 사용하여 일일선량 2Gy씩 44Gy를 4.5주간에 첫번째 화학요법과 동시에 시행하고자 하였다. 관해율은 예정된 치료가 모두 종료된지 4주만에 판정하였으며, 완전 관해를 얻었던 경우에는 10회에 걸쳐 25Gy의 예방적 전뇌 방사선치료를 예정하였다. 급성 부작용의 빈도와 정도는 SWOG 부작용 판정 등급체계를 적용하였으며, 단기 추적 관찰 결과로서 1년 및 2년 생존율, 무병 생존율 등은 Kaplan-Meter법을 사용하였다. 결과 : 전체 환자에 대한 추적 기간의 중앙값은 16개월이었다(범위 2개월$\~$41개월). 완전 관해는 30명(65$\%$)에서 있었으며, 이 중 22명에서 예방적 전뇌 방사선치료를 시행하였다. 동시 병합요법의 3도 이상의 혈액학적 부작용의 빈도는 각각 백혈구감소증 23명(50$\%$), 적혈구감소증 17명(37$\%$), 혈소판감소증 9명(20$\%$)이었고, 비혈액학적 부작용은 탈모 9명(20$\%$), 오심 및 구토 5명(11$\%$), 그리고 말초신경염 1명(2$\%$)이었으며, 이로 인한 화학요법의 지연은 한명에서, 화학요법제의 용량 감소는 전체 246회의 화학요법 중 58회에서 있었다. 3도 이상의 방사선 식도염은 없었으나 화학요법의 부작용으로 인한 흉부 방사선치료의 일시 중단은 21명에서 평균 8.3일간 필요하였다. 국소 재발은 완전 관해 환자에서 8명, 국소 진행은 부분관해 및 불변 환자에서 6명이 확인되었고, 원격 전이는 17명에서 확인되었으며, 이 중 4명에서 국소 재발과 원격 전이가 함께 확인되었다. 원격 전이의 주요 장기로는 뇌가 10명으로 가장 많았고, 간이 4명으로 다음을 차지하였다. 전체 환자들의 생존기간의 중앙값은 23개월이었으며, 1년, 2년 생존율 및 무재발 생존율은 각각 79%$\%$ 45$\%$ 및 55$\%$, 32$\%$였다. 결론 : 국한성 소세포 폐암환자에서 항암 화학 및 방사선치료의 병합요법을 적용하여 만족할 만한 관해율 및 1년, 2년 생존율을 얻었으며, 대체적인 환자들의 치료 방침에 대한 순응도는 양호한 편으로 판단된다.

Lung RPO 선량전달시, Carbon Couch Side Rail과 Vac-lok이 미치는 영향 (Effect of Carbon Couch Side Rail and Vac-lok In case of Lung RPO irradiation)

  • 목 적 : 폐의 우측후사방향 선량전달시, Carbon Side Rail과 환자 고정기구인 Vac-lok이 미치는 영향을 보고자 한다. 대상 및 방법 : Vac-lok의 오른쪽 부분을 10, 20, 30 mm 두께로 제작하였다. 측정은 유리선량계를 이용하여 측정하였고, 측정점은 팬텀 우측 폐의 center Point를 기준으로 좌, 우, 하, 상 방향 각각 A, B, C, D Point로 설정 하였다. 각 point에 유리선량계를 삽입한 후 couch의 Side Rail을 외측(Out)으로 뺀 후 vac-lok을 놓지 않은 no vac-lok, 그리고 10, 20, 30 mm의 vac-lok 위에 팬텀을 세팅하였다. 중심점에 6 MV 광자선을 조사야 $10{\times}10cm^2$, SAD 100 cm, 겐트리 각도 $225^{\circ}$로 하여 300 MU/min 선량률과 100 MU 조사선량을 전달하였다. 측정은 5회씩 실시하였고, 마찬가지로 Side Rail을 내측(In)으로 넣은 후 각 point에 대해서도 같은 조건으로 5 회씩 측정하여 평균값을 산출하였다. 결 과 : side rail에 따라서는 중심점, A, B, C, D Point 각각 -11.8 %, -12.3 %, -4.1 %, -12.3 %, -7.3 %의 선량 감소를 보였다. Side-Rail-Out에서 10 mm vac-lok의 경우 약 -0.9 %가 감소되었고, 20 mm vac-lok 사용 시 약 -2.0 %, 30 mm vac-lock 사용 시 약 -3.0 %가 감소되었다. Side-Rail-In에서 10 mm vac-lok의 경우 약 -1.0 %가 감소되었고, 20 mm vac-lok 사용 시 약 -2.1 %, 30 mm vac-lok 사용 시 약 -3.0 %가 감소되었다. Side-Rail-In 상태의 no vac-lok 선량 값을 기준으로 Side-Rail-Out 상태의 10, 20, 30 mm vac-lok을 사용할 때, side rail에 대한 선량 감소에 더하여 중심점에서는 약 -0.9 %, -1.8 % -2.4 %, A point에서는 -0.5 %, -1.6 %, -2.1 %, B point에서는 약 -0.9 %, -2.0 %, -3.2 %, C Point에서는 -1.0 %, -2.1 %, -3.1 %, D point에서는 약 -1.0 %, -1.6 %, -3.1 %의 추가적인 선량 감소를 나타냈다. 결 론 : 폐를 비롯한 우측후사방향 방사선 치료 시 side rail에 대해 주의를 기울이고, vac-lok 제작 시 vaclok 두께에 대해 관심을 갖는다면 더 나은 치료 효과를 기대해 볼 수 있으리라 사료된다.

측모 두부방사선계측사진 분석과 측모 사진 분석의 비교 (Comparison of analysis of the lateral cephalogram and analysis of lateral facial photograph)

  • 오래 전부터 교정진단 및 치료계획 수립 시 골격 및 치열 관계와 안모를 분석하기 위한 필수적인 진단 자료로 측모 두부방사선계측사진을 이용하였다. 하지만 최근 들어 심미적인 측면이 강조되면서 좀 더 정량화된 연조직 분석법이 필요하게 되었고 좀 더 사실적인 연조직 측모를 얻기 위해서는 측모 두부방사선계측사진보다는 측모 사진이 더 유용할 것이다. 아직까지 측모 사진을 계측, 분석하는 경우는 드물었기 때문에 이 연구에서는 측모 사진의 계측 분석을 측모 두부방사선계측사진 분석과 비교해 봄으로써 연조직과 경조직 분석이 서로 상관관계를 갖는지를 알아보고 그 대체 가능성을 시험 평가해 보고자 하였다. 교정과에 내원한 환자들 중 측모 두부방사선계측사진의 계측치인 ANB를 기준으로 I급 부정교합$(0^{\circ}{\sim}4^{\circ})$ 32명, II급 부정교합$(4^{\circ}{\leq})$ 32명, III급 부정교합$(-5^{\circ}{\sim}0^{\circ})$ 31명을 선정하였다. 각 피험자들의 측모 두부방사선계측사진과 측모 사진을 각각 촬영하여 수평적, 수직적 계측 항목을 설정한 후 각 계측치들의 평균과 표준편차를 구하였고 통계적 유의성을 알아보기 위해 표준화 변수 Z를 이용한 t-test와 상관분석을 이용하였다. 분석 결과 측모 두부방사선계측사진 분석에서 세 그룹간에 뚜렷한 차이를 보이는 계측치들이 측모 사진에서도 유사한 차이를 보이는 것을 확인할 수 있었다. 본 연구 결과 측모 사진의 Su-N'-Pg'은 측모 두부방사선계측사진에서 상, 하악골의 전후방적인 관계를 나타내는 ANB를 대신하여 사용될 수 있으며, 측모 사진의 N'-Sn/Sn-Pg'은 측모 두부방사선계측사진의 NA/APg (facial convexity)을, 그리고 측모 사진의 N'-Tra-Me'는 측모 두부방사선계측사진의 FH-MP (mandibular plane)을 대신하여 사용될 수 있을 것으로 보인다.
