Objectives: To propose a risk-adjustment model from insurance claims data, and analyze the changes in cesarean section rates of healthcare organizations after adjusting for risk distribution. Methods: The study sample included delivery claims data from January to September, 2003. A risk-adjustment model was built using the 1st quarter data, and the 2nd and 3rd quarter data were used for a validation test. Patients' risk factors were adjusted using a logistic regression analysis. The c-statistic and Hosmer-Lemeshow test were used to evaluate the performance of the risk-adjustment model. Crude, predicted and risk-adjusted rates were calculated, and compared to analyze the effects of the adjustment. Results: Nine risk factors (malpresentation, eclampsia, malignancy, multiple pregnancies, problems in the placenta, previous Cesarean section, older mothers, bleeding and diabetes) were included in the final risk-adjustment model, and were found to have statistically significant effects on the mode of delivery. The c-statistic (0.78) and Hosmer-Lemeshow test ($x^2$=0.60, p=0.439) indicated a good model performance. After applying the 2nd and 3rd quarter data to the model, there were no differences in the c-statistic and Hosmer-Lemeshow $x^2$. Also, risk factor adjustment led to changes in the ranking of hospital Cesarean section rates, especially in tertiary and general hospitals. Conclusion: This study showed a model performance, using medical record abstracted data, was comparable to the results of previous studies. Insurance claims data can be used for identifying areas where risk factors should be adjusted. The changes in the ranking of hospital Cesarean section rates implied that crude rates can mislead people and therefore, the risk should be adjusted before the rates are released to the public. The proposed risk-adjustment model can be applied for the fair comparisons of the rates between hospitals.
In order to investigate the occurrence status of tiller at the upper node of rice, this trial was conducted with different eco-typed and 23 cultivars in 1983 and testified the effect of fertilizer levels with Indica x Japonica cultivars in 1984. In occurring of tiller at the upper nodes, upper 2nd, 3rd node and lower 2nd, 3rd node rose simultaneously. The tillering rates of the upper nodes of Japonica x Indica cultivars were more than Japonica cultivars and early maturing cultivars were more than medium or late maturing cultivars, respectively. Highly negative correlations were found between the tillering rate of upper nodes and days from transplant-ing to heading and bending angle degree of 2nd node. The tilleriiig rate of the upper nodes treated with different levels of fertilizer increased with non-fertilizer than those of standard (N-P-K =15-11-13kg/10a) or heavy (N-P-K =25-15-18kg/10a) levels.
Gamadi, a native rice cultivar from Nepal in which the panicle remains enclosed within its flag leaf sheath upto maturity, was crossed with different genetic marker testers of 12 linkage groups in order to analyze its linkage relationship. The results obtained from the experiment were summarized as follows: Normal segregations of all the genetic marker genes used in this experiment viz Cl, wx and Pla of linkage group I, Pn, Rd and Pub of linkage group III, and lg, g, Ps, gh, Hla, la, nl, bl, be and gl of linkage groups II, IV, V, VI, VII, VIII, IX, X, XI, and XII respectively confirmed the previous results, and also strongly indicated that the genetic constituent of the Gamadi and marker testers is same. 'Gamadiness' (the panicle enclosing character) was controlled by two complementary dominant genes with the segregation ratio of 9 Gamadi to 7 normal panicle-exserting types. These genes have been temporarily proposed as G-a and G-b for gamadiness. G-a gene was found to be linked with the neckleaf gene (nl) of linkage group Ⅸ with the crossover value of 0.3733$\pm$0.027. G-b gene appeared to be associated with the brittle culm gene (bc) of the linkage group XI with the crossover value of 0.2725$\pm$0.061.TEX>0.061.
Residential broadband networks (RBN) are quickly evolving from xDSL/Cable networks to FTTH services with lOOMbps bandwidth. With the high-speed Internet services, subscribers tend to use web, multimedia, game or file sharing applications. Therefore, performance of RBN is important to the quality of services for users. Typically, Internet service providers (ISPs) are providing their RBN performance results that are measured by subscribers with the dedicated measurement tools. However, they do not make their results public to people. In order to obtain the performance results measured by users, we have to induce a lot of subscribers, which will take a lot of cost and time. Monarch, that is used in this paper, could provide the 3rd party measurement method without the help of ISPs and subscribers. With the extensive experiments through Monarch, we have shown that 3rd party measurement results are not much different from those of user-initiated measurement. In addition, we have observed that bandwidth is around $80{\sim}90Mbps$ and RTT is less than 30ms in FTTH services provided by three major Korean ISPs.
PURPOSE. This study aimed to investigate the Vickers Hardness Number (VHN) of light- and dual cured resin cements cured through monolithic zirconia specimens (VITA YZ) of various translucencies: translucent (T); high translucent (HT); super translucent (ST); and extra translucent (XT) at 0, 24, and 48 h after curing. MATERIALS AND METHODS. Four zirconia specimens from each translucency were prepared. Two light-cured resin cements (Variolink N LC; VL and RelyX Veneer; RL) and two dual-cured resin cements (Variolink N DC; VD and RelyX U200; RD) were used. The cement was mixed and loaded in a mold and cured for 20 s through the zirconia specimen. The upper surface of cements was tested for VHN using a microhardness tester at 0, 24, and 48 h after curing. The VHN were analyzed using two-way repeated, Brown-Forsythe ANOVA with Games Howell post-hoc analysis and independent t-tests (P < .05). RESULTS. All cements showed significantly higher VHN from 0 h to 24 h (P < .001). At 48 h, the VHN of light-cured cements were significantly lower when cured under the T groups than under XT groups (P = .001 in VL, P = .014 in RL). At each post curing time of each translucency, VD showed higher VHN than VL (P < .05), and RD also showed higher VHN than RL (P < .05). CONCLUSION. The translucency of zirconia has an effect on the VHN for light-cured resin cements, but has no effect on dual-cured resin cements. Dual-cured resin cement exhibited higher VHN than the light-cured resin cement from the same manufacturer. All resin cements showed significantly higher VHN from 0 h to 24 h.

XML has become a do facto standard for structured document and data on the Web. An XML data deluge over the network will be more, since XML based standards such as Web Service and Semantic Web gets popular. There are efforts to store and query XML documents in a relational database system and recent efforts focus on how to provide such operations using XPath and XQuery. In this paper, we present study about those research efforts and we propose a new scheme to stoγe and query XML documents in a relational database using XPath query. The scheme uses a 'shred' method to store and translates XPath queries to SQL. We also present our empirical experiments using a RDBMS.
Coarctation of the aorta was rare condition among the congenital cardiovascular defects in Korea. We experienced a case of coarctatlon of the aorta [postductal type], which was successfully corrected by resection and end to end anastomosis. This patient, 21 years male patient, was admitted to the medical department for evaluation of hypertension, headache and exertional dyspnea during 4 years, and transferred to the department of chest surgery for operation. On physical examination, blood pressures were measured on both extremity, measuring 190/100mmHg on the arm and 100/80mmHg on the leg. Systolic murmur was heard on 2nd to 3rd left intercostal space and left sternal border. On simple chest x-ray, rib notching was seer/on low border of right 3rd and left 4th rib. Final preoperative diagnosis was made by the retrograde aortic catheterization and aortography, which showed the typical configuration of postductal type of coarctation with poststenotic dilatation of the aorta. On 20th, July, 1978, under the general anesthesia with endotracheal intubation, resection of coarctation of the aorta and end to end anastomosis was performed. During clamp for resection, blood pressure of upper extremity was elevated to 200/140mmHg, and controlled by Arfornad. During recovery, blood pressure over 160ramrig in systole was controlled by Reserpine for 8days postoperatively. At discharge, postoperative 8th day, brachial and femoral artery pressure was 145/85 mmHg and 135/80mmHg. After discharge, there was no evidence of specific symptoms and hypertension without antihypertensive drug.
Purpose: This study was performed to identify the effects of dance sports programs on the parameters of health promotion (blood pressure, pulse, glucose, blood lipids, musculoskeletal pain, fatigue, self-efficacy, and depression) in rural women. Methods: A non-equivalent control group pretest-posttest experimental design was used. The study subjects were 38 women who were aged 45-78 and cultivating perilla leaves in a rural area. An experimental group (n=20) received the dance sports program from December 3rd, 2009 to June 3rd, 2010 and a control group (n=18) did not. Data was collected 3 times (before the program, 12 during and 24 weeks after the program) and were analyzed with $X^2$ test, t-test, and Repeated Measures ANOVA using SPSS/WIN 18.0. Results: For the experimental group, significant improvement was found in high density lipoprotein cholesterol (HDL-C), low density cholesterol (LDL-C), triglyceride (TG) musculoskeletal pain, fatigue, self-efficacy, and depression as compared to the control group. Conclusion: The findings of the study indicates that the dance sports program is effective on health promotion for rural women. Therefore, dance sports programs can be recommended for health promotion of rural residents.
In this study, we present geometrical and kinematical analysis of Moreton wave observed in 2012 June 3rd and July 6th, recorded in H-${\alpha}$ images of Global Oscillation Network Group (GONG) archive. These large-scale waves exhibit different features compared to each other. The observed wave of June 3rd has angular span of about $70^{\circ}$ with a diffuse wave front associated to NOAA active region 11496. It was found that the propagating speed of the wave at 17:53 UT is about $931{\pm}80km/s$. The broadness nature of this Moreton wave can be interpreted as the vertical extension of the wave over the chromosphere. On the other hand, the wave of July 6th associated with X1.1 class are that occurred at 23:01 UT in AR NOAA11515. From the kinematical analysis, the wave propagated with the initial velocity of about $994{\pm}70km/s$ which is in agreement with the speed of coronal shock derived from type II radio burst, v ~ 1100 km/s. These two identified waves add the inventory of the large-scale waves observed in 24th Solar Cycle.
Objectives : The usage of acupuncture has gained popularity for certain chronic pain conditions. However, the efficacy of acupuncture in various diseases has not been fully established and the underlying mechanism is not clearly understood. In the present study, the effect of electroacupuncture (EA) applied to yangno$(SI_6)$ on the neuropathic pain was examined. Methods : A common source of persistent pain in human is a neuropathic pain. Neuropathic pain was induced by tight ligation of L5 spinal nerve. When rats developed pain behaviors, EA was applied for 30 min. under enflurane anesthesia with repeated train stimuli at the intensity of 10X of muscle twitch threshold. The foot withdraw latency of the hind limb was measured for an indicator of pain level after each manipulation. Results : EA increased the mechanical threshold of the foot in the rat model of neuropathic pain significantly for the duration of 1 hr. suggesting a partial alleviation of pain. EA applied to SI6 point produced a significant improvement of mechanical sensitivity of the foot lasting for at least 1 h. However, $ST_{36}$ point did not produce any significant increase of mechanical sensitivity. The improvement of mechanical threshold was interpreted as an analgesic effect. The analgesic effort was specific to the acupuncture point since the analgesic effect on the neuropathic pain model could not be mimicked by EA applied to a point, $ST_{36}$. In addition, this analgesic effect of EA is mediated by a adrenergic mechanism of descending control of spinal cord from the brain. Conclusions : The data suggest that EA produces a potent analgesic effect on the neuropathic pain model in the rat; and 2) that EA-induced analgesia is mediated by a adrenergic mechanism of descending control in a point specific manner.
