Wrap-around Free Flap in Acute Thumb Injury (족무지 유리피부편을 이용한 급성 수무지 손상의 치료)

  • Firstly the wrap-around free flap from the great toe was transferred successfully by Morrison and O'Brien in 1980 for reconstruction in chronic case of amputated thumb distal to the MP joint. Since then, significant bone peg resorption and grafted bone fracture have been reported in most of cases after operation. We have performed thumb reconstructions within 2 weeks after injury in 11 cases which included acute crushing injuries or failed replantation of thumb at Guro Hospital from September, 1983 to October, 1993. The results obtained from this study are as follows: 1. There were 8 males and 3 females and the mean age was 28.8 years old ranged from 3 years old to 50 years old. 2. The most common injury mechanism was machinary injury in 10 of 11 cases. 3. We have performed thumb reconstructions by using wrap-around free flap from the great toe without bone graft in 6 cases and with bone graft in 5 cases within 2 weeks after injury. 4. During 6 years and 1 month mean follow-up period, there were bone resorptions in 3, grafted bone fracture in 1, infections in 2 and soft tissue necrosis in 3 cases. 5. We obtained satisfactable results in respect of function and cosmesis and urged to use this flap for reconstruction in acute thumb injury.

Cost Analysis of Wrap Silage Production in the Paddy Field for Forage Crop Cropping System (답리작 사료작물의 랩사일리지 생산비 분석)

  • Mechanized operation model and mechanical cultivation technology for winter barley, rye, Italian ryegrass and sudan grass wrap silage production system at the paddy field for cropping system was developed. Also, a series of experiment were performed and lots of data were collected and analyzed to develope mechanical technology, coverage area, and optimum size of the farm (break-even point) for wrap silage production system. The coverage area for winter barley or rye wrap-silage production system is determined around 61.9, 73.4, 77.5, 88.2 ha in the case of drill seeding and different ripening species by tractor power 50, 75, 100, 130 ps, respectably. The break-even point of the farming size is analyzed as 20 ha and its production cost is estimated around 367, 383, 430, 443 won/TDN-kg in the case of winter barley wrap-silage by tractor power 50, 75, 100, 130 ps, respectably. The break-even point of the farming size is analyzed as coverage area and its production cost is estimated around 237, 215 won/TDN-kg in the case of winter barley wrap silage and sudan grass by the tractor power 50, 100 ps, respectably.

Implementation of Policing Algorithm in ATM network (ATM 망에서의 감시 알고리즘 구현)

  • In this thesis, a policing algorithm is proposed, which is one of the traffic management function in ATM networks. The proposed algorithm minimizes CLR(Cell Loss patio) of high priority cells and solves burstiness problem of the traffic caused by multiplexing and demultiplexing process. The proposed algorithm has been implemented with VHDL and is divided into three parts, which are an input module, an UPC module, and an output module. In implementation of the UPC module\`s memory access, memory address is assigned according to VCI\`s LSB(Lowest Significant Byte) of ATM header for convenience. And the error of VSA operation from counter\`s wrap-around can be recovered by the proposed method. ANAM library 0.25 $\mu\textrm{m}$ and design compiler of Synopsys are used for synthesis of the algorithm and Synopsys VSS tool is used for VHDL simulation of it

  • To modify product design easily, modern CAD systems adopt the feature-based model as their primary representation. On the other hand, the boundary representation (B-rep) model is used as their secondary representation. IGES and STEP AP203 edition 1 are the representative standard formats for the exchange of CAD files. Unfortunately, both of them only support the B-rep model. As a result, feature data are lost during the CAD file exchange based on these standards. Loss of feature data causes the difficulty of CAD model modification and prevents the transfer of design intent. To resolve this problem, a tool for recognizing design features from a B-rep model and then reconstructing a feature-based model with the recognized features should be developed. As the first part of this research, this paper presents a method for decomposing a B-rep model into simple volumes suitable for design feature recognition. The results of experiments with a prototype system are analyzed. From the analysis, future research issues are suggested.

Microsurgical Reconstruction of the Injured Limb (미세혈관 수술법을 이용한 결손사지의 재건술)

  • From Fabuary 1982 to May 1995, 396 patients had undergone reconstructive surgery of the upper and lower limb with microsurgical technique at department of orthopaedic surgery, Yonsei University of Medicine. The results were as follows; 1. Average age at the time of operation was 23.4years(2-64 years), and there were 277 male and 119 female patients. 2. Among 324 patients of soft tissue flap(87 inguinal flap, 132 scapular flap, 38 latissimus dorsi flap, 11 latissimus dorsi and scapular combind flap, 6 gracilis flap, 12 deltoid flap, 3 tensor facia lata flap, 11 dorsalis pedis flap, 6 lateral thigh flap, 12 wrap around flap, 1 lateral arm flap, 5 musculocutaneous flap), 274 cases(85.5%) were succeed. 3. Among 37 patients of vascularized bone graft(18 fibular bone graft, 11 iliac bone graft, 7 toe to finger transplantation,1 vascular pedicle rib graft), 30 cases(80.1%) were succeed. 4. In 26 cases of segmental resection and rotationplasty at lower extremity, 23 cases were succeed. 5. In 7 cases of Tikhoff-Linberg procedure and in 2 case of segmental resection and replantation, all case was succeed. Overall success rate of microscopic reconstructive surgery was 85.6%. In conclusion, microsurgical technigue is valuable for reconstruction of tissue defect or function loss of the limb.
