Advanced Formulation and Pharmacological Activity of Hydrogel of the Titrated Extract of C. Asiatica

  • Titrated extract of Centella asiatica (TECA) contains three principal ingredients, asiaticoside (AS), asiatic acid (AA), and madecassic acid (MA). These components are known to be clinically effective on systemic scleroderma, abnormal scar formation, and keloids. However, one problem associated with administration of TECA is its low solubility in aqueous as well as oil medium. In this study, various nonionic surfactants and bile salts as anionic surfactant were tested and screened for solubilizing TECA with a view to developing topical hydrogel type of ointment which is stable physicochemically, and has better pharmacological effects. When TECA was incorporated into various nonionic surfactant systems, labrasol had the most potent capacity for solubilizing TECA. In cases of bile salt systems, Na-deoxycholate (Na-DOC) had foremost solubilizing capacity, even more than labrasol. In differential scanning calorimetric study, the peaks of AA, MA, AS and Na-DOC disappeared at the coprecipitate of $1\%$ TECA and $1\%$ Na-DOC, suggesting the optimum condition of Na-DOC for solubilizing TECA. When the physicochemical stability of hydrogel containing this mixture was assessed, it was stable at room temperature for at least one month. Pharmacologically it significantly decreased the size of wound area at the $9^{th}$ day when applied to the wound area of rat dorsal skin. Taken together, solubility of TECA was dramatically improved by using non ionic and anionic surfactant systems, and Na-DOC was found to be the most effective solubilizer of TECA in formulating a TECA-containing hydrogel typed ointment. Moreover this gel was considered to be applicable to clinical use for wound healing effect.

Morphological Comparision of the Effect of Open and Occlusive Dressing on Rabbit Skin Wound (가토 피부상처에서의 개방드레싱과 밀봉드레싱의 효과에 대한 형태학적 비교)

  • This morphological study was carried out to investigate the healing effect of open and occlusive dressing on rabbit skin wounds. The male white rabbits were given incision wound measuring 1.0cm in length and excision wound measuring $1.0{\times}0.2cm$ respectively on backs. Five rabbits among their received sterilization with beta dine twice a day and kept wound open. Another group of rabbits received sterilization and occlusive dressing with application of 1% sulfadiazine ointment and covering of gauze. The rabbits were observed at 5, 10, and 15 days after dressing with light microscope. The results were as follows. Both incision and excision wounds received open and occlusive dressing respectively revealed formation of scab and regeneration of epidermal epithelial cells at wound margin. The superficial dermis showed infiltration of neutrophils, monocytes, and lymphocytes, fibroblastic proliferation, and edema. On 10 days after opera and occlusive dressing the scab had disappeared and regeneration of epidermal epithelium was completed. The capillaries and inflammatory cells were decreased in number. However, edema and fibroblastic proliferation were more pronounced. On 15 days after opera and occlusive dressing, there were keratinization of regenerated epidermal epithelium, loss of inflammatory infiltration, edema, and capillary proliferation, and increase of fibroblastic proliferation and collagen fibers. The amounts of scars were less in incision wound than of excision one. There were no differences in healing processes between both dressing methods. According to the above results, it was conceived that sterilization of wound is more important in wound healing than dressing method in trivial wounds.

Healing Effects of Ginsenoside Rg1 on Experimental Open Wound in Rat (흰쥐의 외과적 창상에 대한 Ginsenoside Rg1의 치료효과)

  • This study was performed to investigate the effect of ginsenoside Rg1 treatment on wound healing using SD rats by generating four full-thickness skin wounds on the dorsum. In the Rg1-treated groups (5,000 and 10,000 ppm), area of wounds and macroscopic inflammatory signs were significantly decreased compared to control group throughout the experimental period in a concentration dependent manner. Histological appearance after 20 days of treatment with Rg1 revealed the formation of epithelial layer, hair follicles and progressive angiogenesis and an increase in collagen and granulation as compared to control group. Rg1 treatment resulted in the increased expression of the vascular endothelial growth factor (VEGF) mRNA and reduced expression of transforming growth factor beta (TGF-$\beta$) mRNA in wounded skin compared to control group. The expression levels of VEGF and TGF-$\beta$ mRNA in the Rg1-treated groups were similar to those of Fucidin(R) ointment-treated group. These results suggested that Rg1 should be helpful for the promotion of wound healing.

Application of Rats According to Molecular Weight of Chitosan (키토산의 분자량에 따른 랫드에서의 적용)

  • Biodegradable films were prepared by solution blend method in the weight ratio of chitosan for the purpose of useful biomaterials. The possibility of biomaterials prepared from natural polymer as a skin substitute was evaluated by measuring biocompatibility. These films were inserts in the back of rat and their biodegradability was investigated by the film weight and hematology as a function of time for the biotransformation. The result of rat test showed that medium, high viscosity chitosan induced some suspects of inbiocompatibility in the tissue by goreign body reaction 48 and 72 hours after implantation. Also, we prepared the official burn ointment which is made by low viscosity chitosan. This burn ointment was covered on the skin wound of artificial burn and their effect of healing was investigated by the evaluation of the naked eye and hematological change as a function of time. The result of rats test showed that burn ointments made from chitosan was effective reductio of inflammation than negative group.

The Oxidation of Magnetic Particles in Medicinal Ointment

  • Magnetic particles in a novel, wound-healing ointment were studied using M$\ddot{o}$ssbauer spectroscopy and VSM to estimate the stability of the properties of the magnetic particles. The isomer shifts of $Fe_3O_4(A)$ were found to be 0.49-0.56 mm/s relative to iron metal, this indicates that the iron ions in $Fe_3O_4(A)$ are $Fe^{3+}$. On the other hand, the isomer shifts of $Fe_3O_4(B)$ were found to be 0.91-1.13 mm/s relative to iron metal, this shows that the ion state of $Fe_3O_4(B)$ is a mixed state of $Fe^{2+}$ and $Fe^{3+}$. It is noted that this composition, as well as that of the initial pure component in the form of a highly dispersed fraction (${\sim}10\;{\AA}$), differs from the stoichiometric one. It was found that the area ratio of the M$\ddot{o}$ssbauer subspectra of $Fe_3O_4(A)$ / $Fe_3O_4(B)$ taken at 87 and 181 K linearly increased in comparison to the initial pure magnetic particles, but the rate of increase of the area ratio at 181 K was about two times that at 87 K. From the magnetic hyperfine field, despite their small size, the particles exhibit no superparamagnetism.

The Effects of Jawoongo plus Flos Lonicerae on The Artificial Wound on Rats (자운고가(紫雲膏加) 금은화(金銀花)가 흰쥐의 창상(創傷)에 미치는 효과(效果))

  • 자운고가(紫雲膏加) 금은화(金銀花)가 창상치유과정(創傷治愈過程)에 미치는 영향(影響)을 살펴보고자 혈청중(血淸中)의 Leukocyte, CRP, Cortisol의 변화 및 상처크기의 축소율(縮小率)을 실험(實驗)한 결과 백혈구의 변화에서는 Sample II 가 Control에 비해 감소(減少)의 유의성을 나타내었고 CRP함량(含量)의 변화에서는 뚜렷한 유의성을 나타내지 못하였고 Cortisol함량(含量)의 변화에서는 Sample II 가 Control에 비해 감소(減少)의 유의성을 나타내었지만 Sample III와 비교시에는 Sample III의 감소(減少)의 유의성이 높게 나타났다. 상처(喪妻)의 축소율(縮小率)은 실험 10일과 15일째 Control에 비해 유의성이 인정(認定)되었다. 이상(以上)을 살펴볼 때 자운고가(紫雲膏加) 금은화(金銀花)가 창상치료(創傷治療)에 대해 효과적(效果的)이며 약재(藥材)로서의 효능을 가지고 있음을 알 수 있으나 자운고(紫雲膏)와 비교하였을 때는 금은화(金銀花)의 첨가가 창상치료(創傷治療)애 크게 유의성있는 변화를 나타내지 못하였지만 자운고(紫雲膏)의 단독(單獨) 투여(投與)시보다는 나은 효과를 나타내었으며 앞으로 한방(韓方) 연고(軟膏)의 개발을 위하여 더 많은 연구(硏究)가 필요할 것으로 사료(思料)된다.

A Case of Dissection and Resuture Performed at the Department of Korean Medicine for Excessive Granulation Tissue Caused by Suturing Failure (봉합 실패로 발생된 과다 육아조직에 대해 한의과에서 시행한 절제 및 재봉합 증례)

  • Objectives : The purpose of this study is to report case of hypergranulation tissue that occurred after laparotomy and dissected by CO2 laser. Methods : A 72-year-old female had hypergranulation tissue on the epigastrium. Local anesthesia was achieved with bufonis venenum pharmacopuncture. The CO2 laser was used for dissection. After removal of granulation, irrigation with soyeom pharmacopuncture solution and simple interrupted suture were performed. Yeonkyopaedok-san were administered for 7 days and the suture was removed after 14 days. At last, saengkigo was applied. Results : Hypergranulation tissue sized 1.0×1.0×1.0cm was clearly removed. Including excessive growing of granulation tissue, adverse effects were not reported until the clear skin adhesion. Conclusions : Hypergranulation tissue was surgically removed and sutured completely without any complications by using pharmacopuncture, oral herbal medicine and ointment that have anti-inflammatory effects and wound healing. Through this study, it is hoped that surgery including suture will be actively performed in more diverse diseases in the Korean medicine community.
