Effect of LEDs Light of 633 nm Wavelength in Skin of Organism (633 nm 파장의 LED 광원이 생체 피부에 미치는 영향)

  • Low power laser therapy is internationally certified and is known to be effective in stimulating DNA in living organisms, increasing protein synthesis and activating cell division, smoothing blood circulation, promoting cell activation, cell regeneration and function. It also has anti-inflammatory, anti-edemic, anti-fibrous dysplastic and neuralogic hyperfunctional effects. This study was intended to verify the effect of LED irradiation therapy on wound healing in cell and animal tests by applying LED irradiator using a laser and laser diode, which was independently designed and developed to emit beams of similar wavelength to that of a laser. This equipment was fabricated using a micro-controller and a high brightness LED, and designed to enable us to control light irradiation time, intensity and reservation. In case of cell proliferation experiment, each experiment was performed to irradiation group and non-irradiation group for tissue cells. MTT assay method was chosen to verify the cell increase of two groups and the effect of irradiation on cell proliferation was examined by measuring 590 nm transmittance of micro-plate reader. In the wound healing experiment, 1$cm^2$ wounds on the skin wound of SD-Rat(Sprague-Dawley Rat) were made. Light irradiation group and none light irradiation group divided, each group was irradiated one hour a day for 9 days. As a result, the cell increase of tissue cells was verified in irradiation group as compared to non-irradiation group. And, compared with none light irradiation group, the lower incidence of inflammation and faster recovery was shown in light irradiation group.

A Case Study of Wound Dressing Treatment in a Patient with Diabetic Foot Ulcer (당뇨병성 족부 궤양 환자의 창상 드레싱 치료 증례)

  • Objectives : The purpose of this study is to report on the treatment of wound dressing in patients with diabetic foot ulcers in the dermatology clinic of Korean medicine. Methods : First, sufficient marginal resection of the ulcer and necrotic tissue on the foot of a patient with diabetic peripheral neuropathy was performed. Inflammation was treated with Anti-inflammatory pharmacopuncture solution, and dressings were applied for one month using Cornu Cervi Parvum pharmacopuncture solution, and Haeboo ointment for granulation tissue formation and skin regeneration. Results : After 10 days of the treatment, the formation of new granulation tissue was observed in the necrotic tissue area, and the extent of the lesion decreased sharply from the 14th day. On the 40th day, epithelialization progressed so that the treatment was terminated, and complete keratinization of the site was observed at follow-up 3 weeks after the end of the treatment. Conclusions : In the treatment of wound dressings in diabetic foot ulcer patients, marginal resection treatment with the application of pharmacopuncture solution and herbal ointment showed good effect.

  • 피부의 관리를 위한 방법 중 광을 이용한 케어는 비침습적인 방법으로 세포를 활성화 시켜 도움을 주는 매우 유용한 방법이다. 최근에는 낮은 에너지의 LED를 광원으로 이용하는 LLLT (Low Level Light Therapy)가 많이 적용이 된다. 본 논문에서는 피부 미용기기에서 많이 사용되고 있는 가시광 영역의 적색과 황색, 그리고 적외선 영역의 LED를 이용하여 array를 제작하였다. 제작된 LED array는 5V의 전압과 50mA의 전류로 구동되며, 적색이 평균 $10.7mW/cm^2$, 황색이 평균 $1.8mW/cm^2$, 적외선이 평균 $5.5mW/cm^2$의 광출력을 갖는다. 이를 배양된 human cell plate에 각각 10, 20, 40분의 시간으로 조사하였으며, LED array와 조사면 사이에 20 mm의 거리를 두어 균일한 광량이 조사 될 수 있도록 하였다. 실험결과, wound healing의 경우는 적색광을 조사하였을 때, injury line의 길이가 $604.36{\mu}m$에서 10분 조사 시 $368.92{\mu}m$, 20분 조사 시 $298.04{\mu}m$, 40분 조사 시 $491.22{\mu}m$로 줄어드는 효과를 보였으며 특히 20분간 조사하였을 경우의 효과가 가장 좋았다. Cell regeneration의 경우는 황색광을 20분간 조사하였을 경우에 UV 에 의해 손상된 세포 수가 가장 크게 줄어드는 것을 확인할 수 있었다. 결과적으로 photorejuvenation 효과를 줄 수 있는 wound healing과 cell regeneration에는 적색 및 황색광의 LED array를 20분간 조사하였을 때가 가장 효과적이었으며, 적외광의 경우에는 모든 실험에서 효과가 없음을 확인할 수 있었다.

Histologic evaluation of the regenerated bone using bone graft materials (수종의 골이식재를 이용한 유도재생골의 조직학적 평가)

  • This study was performed to evaluate the effect of bone graft materials including demineralized freeze-dried bone, freeze-dried bone, deproteinized bovine bone on space-making capacity and bone formation in guided bone regeneration with titanium reinforced ePTFE membrane(TR-ePTFE). Adult male rabbits(mean BW 2kg) were used in this study. Intramarrow penetration defects were surgically created with round bur on calvaria of rabbits. TR-ePTFE membrane was adapted to calvarial defect and bone graft materials were placed. Animals were sacrificed at 2, 8, 12 weeks after surgery. Non-decalcified specimens were processed for histologic analysis and prepared with Villaneuva bone stain. The results of this study were as follows: 1. TR-ePTFE membrane was biocompatible and capable of maintaining the space-making. 2. Tissue integration was not good at TR-ePTFE membrane. Fixation was not enough. so, wound stabilization was not good. 3. In animals using deproteinized bovine bone, demineralized freeze-dried bone, bone formation was little. 4. In animals using freeze-dried bone, bone formation was better. Within the above results, bone formation may be inhibited when wound stabilizafion was not good.

Development of LED Module for Tooth Care with Effect of Promoting Scar Treatment and Analysis of Optical Properties (흉터 치료 촉진 효과를 갖는 치아 케어용 LED 모듈 개발과 광학적특성 분석)

  • In this paper, we analyzed the optical properties of the LED module for dental care, which has the effect of promoting cell regeneration and scar treatment. The LED module is a U-shaped module suitable for the shape of teeth. It is manufactured with an LED module (632 nm) and an LED module (632 nm + 870 nm), analyzes the results of optical properties, sets the irradiation distance, irradiation time, and is effective in healing skin wounds. Evaluation was conducted. It was tested in 6 test groups according to the light irradiation conditions, and light was irradiated to the scar site every other day for 7 days, 1 day and 3 days. As a result, it was confirmed that the effect of scar treatment was the highest when the combined wavelength of 632 nm + 870 nm was irradiated in pulse mode than when the single wavelength was irradiated and the composite wavelength was continuously irradiated. In group C group irradiated with PW Mode: pulse mode (period 36 ms, pulse width 35 ms) using a composite wavelength with LED module (632 nm + 870 nm) than group A without light irradiation, the length of scar reduction was 19 %, the area of the scar was further reduced by 10%, and it was confirmed that it is effective in treating scars in the wound area.

Wnt/β-catenin signaling activator restores hair regeneration suppressed by diabetes mellitus

  • Diabetes mellitus is one of the most prevalent diseases in modern society. Many complicationssuch as hepatic cirrhosis, neuropathy, cardiac infarction, and so on are associated with diabetes. Although a relationship between diabetes and hair loss has been recently reported, the treatment of diabetic hair loss by Wnt/β-catenin activators has not been achieved yet. In this study, we found that the depilation-induced anagen phase was delayed in both db/db mice and high-fat diet (HFD) and streptozotocin (STZ)-induced diabetic mice. In diabetic mice, both hair regrowth and wound-induced hair follicle neogenesis (WIHN) were reduced because of suppression of Wnt/β-catenin signaling and decreased proliferation of hair follicle cells. We identified that KY19382, a small molecule that activates Wnt/β-catenin signaling, restored the capabilities of regrowth and WIHN in diabetic mice. The Wnt/β-catenin signaling activator also increased the length of the human hair follicle which was decreased under high glucose culture conditions. Overall, the diabetic condition reduced both hair regrowth and regeneration with suppression of the Wnt/β-catenin signaling pathway. Consequently, the usage of Wnt/β-catenin signaling activators could be a potential strategy to treat diabetes-induced alopecia patients.

The Effects of rhBMP-2/ACS on the Periodontal Healing of 1-Wall Intrabony Defects in Dogs (성견 1면 골결손부에서 rhBMP-2/ACS가 치주조직 치유에 미치는 영향)

  • The ultimate goal of periodontal therapy is the regeneration of periodontal tissue and repair of function. For more than a decade there have been many efforts to develop materials and bioactive molecule(such as growth factor and differentiation factors) to promote periodontal wound healing. Among the bioactive molecules, bone morphogenetic protein(BMP) was studied for periodontal wound healing. Since Urist demonstrated that demineralized bone matrix could induce the formation of cartilage and bone in ectopic site, many studies on BMP have been reported. Among those BMPs, it was reported that rhBMP-2 enhanced the healing of bone defects in animal studies and clinical studies. However, its efficacy in periodontal regeneration, especially 1-wall intrabony defects is still unknown. The purpose of this study was to examine the effect of rhBMP-2/ACS on the epithelial migration, gingival connective tissue adhesion, cementum formation, alveolar bone regeneration in intrabony defects of dogs. Four millimeter deep and four millimeter wide 1-wall defects were surgically created in the mesial aspects of the 3rd incisors. The test group received rhBMP-2/ACS with a flap procedure and the control underwent buffer/ACS with a flap procedure. Histologic analysis after 8 weeks of healing revealed the following results: 1. The length of epithelial growth(the distance from alveolar crest to the apical end of JE) was $0.9{\pm}1.5mm$ in the control group and $1.2{\pm}1.4mm$ in the test group. There was no statistically significant difference between the two groups. 2. The length of connective tissue adhesion was $2.4{\pm}1.3mm$ in the control group and $1.2{\pm}1.1mm$ in the test group. The control group showed significantly enhanced adhesion(P<0.05). 3. The length of new cementum was $0.9{\pm}1.0mm$ in the control group and $1.7{\pm}0.8mm$ in the test group. The test group showed significantly enhanced cementum regeneration(P<0.05). 4. The length of new bone height was $1.9{\pm}0.6mm$ in the control group and $2.4{\pm}0.9mm$ in the test group. There was no statistically significant difference between the two groups. 5. The new bone area was $4.7{\pm}1.7mm^2$ in the control group and $8.0{\pm}2.0mm^2$ in the test group. The test group showed significantly enhanced bone formed area(P<0.05). 6. The new bone density was $73.0{\pm}8.6%$ in the control group and $66.6{\pm}15.3%$ in the test group. There was no statistically significant difference between the two groups. These results suggest that the use of rhBMP-2 in 1-wall intrabony defects has significant effect on new cementum and new bone formation area, but doesn't have any significant effect on the prevention of junctional epithelium migration and new bone formation height.

Effect of the Water Extract of Persicae Semen on Promotion of Axon Regeneration (도인(桃仁)이 중추신경 재생 촉진에 미치는 영향)

  • Following CNS injury, inhibitory influences at the site of axonal damage occur. Glial cells become reactive and form a glial scar, gliosis. Astrocyte-rich gliosis relates with up-regulation of GFAP and CD81, and eventually becomes physical and mechanical barrier to axonal regeneration. It is postulated that the astrocytic reaction is absent, regeneration of axons can occur. And it was reported that treatment with anti CD81 antibodies enhanced functional recovery in the rat with spinal cord injury. So in this current study, the author investigated the effect of the water extract of Persicae Semen on the regulation of GFAP and CD81 that increase when gliosis occurs. Persicae Semen decreased the expression of GFAP and CD81 in astrocyte cell by ELISA method. Persicae Semen decreased the RNA expression of CD81 and GFAP. The proteins that separate in whole cell were analaysed by western blot, and the expression of GFAP and CD81 was decreased. In vivo, rats brains were peformed cortical stab wound, the water extracts of Persicae Semen were injected for 7 days, 30 days. As a result, GFAP and CD81 expression were decreased in immunohistochemistry. These findings demonstrate that Persicae Semen decreases GFAP and CD81 expression. Accordingly, Persicae Semen could be a candidate for promotion of axon regeneration after CNS injury.

The Effect of Platelet Rich Plasma Combined with Bovine Bone on the Treatment of Grade II Furcation Defects in Beagle Dogs (혈소판 농축 혈장이 치근이개부 병변에 미치는 효과)

  • Current acceptable methods of promoting periodontal regeneration are basis of removal of diseased soft tissue, root treatment, guided tissue regeneration, graft materials, biological mediators. Platelet Rich Plasma have been reported as a biological mediator which regulate activities of wound healing progress including cell proliferation, migration, and metabolism. The purpose of this study is to evaluate the possibility of using the Platelet Rich Plasma as a regeneration promoting agent for furcation involvement defect. Five adult beagle dogs were used in this experiment. With intrasulcular and crestal incision, mucoperiosteal flap was elevated. Following decortication with 1/2 high speed round bur, degree II furcation defect was made on mandibular third(P3), forth(P4) and fifth(P5) premolar. 2 month later experimental group were PRP plus bovine bone and bovine bone only. After 4, 8 weeks, the animals were sacrificed by perfusion technique. Tissue block was excised including the tooth and prepared for light microscope with Gomori's trichrome staining. At 4 weeks after surgery, there were rapid osteogenesis phenomenon on the defected area of the Platelet Rich Plasma plus bovine bone group and early trabeculation pattern was made with new osteoid tissue produced by activated osteoblast. Bone formation was almost completed to the fornix of furcation by 4 weeks after surgery. In conclusion, Platelet Rich Plasma can promote rapid osteogenesis during early stage of periodontal tissue regeneration.

The effect of smoking on the healing response following flap debridement surgery (흡연이 치주 판막술후 치유에 미치는 영향)

  • The ultimate goal of periodontal therapy is the regeneration of periodontal tissue which has been lost due to destructive periodontal disease. Various periodontal procedures have been used throughout the years in an attempt to reestablish attachment of periodontal tissues to root surfaces affected by periodontitis. Flap debridement surgery has been demonstrated to be a successful procedure in gaining the probing attachment level and reducing probing depth. A tendency towards impaired wound healing following periodontal procedures in smokers has been clinically documented. But, previous clinical studies on healing response in smokers are based on a retrospective design. The purpose of this study was to evaluate the treatment outcome following flap debridement surgery in smokers compared to nonsmokers. 25 patients with moderate to advanced periodontitis were included for study. Among these patients, 13 patients were smokers, and 12 patients were nonsmokers. Mucoperiosteal flap was raised with the sulcular incision. No antibiotic treatment was administered postsurgery. The patients was recalled at monthly intervals during a period of 6 months following the surgery. The patients were received supragingival scaling and oral hygiene reinforcement. All the recordings, including modified O' Leary plaque control record, bleeding on probing, probing pocket depth, probing attachment level,were recorded, presurgery and 6 months postsurgery. The changes of all the recordings at 6 months after flap debridement surgery revealed the following results: 1. PI on all the dentitions and surgical sites showed no statistical significance between smokers and nonsmokers at presurgery. But, smokers demonstrated a significantly lower % of PI than nonsmokers at 6 months postsurgery. 2. Smokers demonstrated a greater % of BOP sites than nonsmokers on the surgical sites and all the dentitions, presurgery and 6 months postsurgery. But, there was no statistical significance between two groups. 3. Smokers exhibited significantly less reduction of probing depth in the 3 mm or less probing pocket depth(PPD) group, 6mm or more PPD group and total PPD group when compared to nonsmokers at 6 months postsurgery. 4. Smokers exhibited significantly less gain of probing attachment level(PAL) in the 3mm or less PPD group, 6 mm or more PPD group and total PPD group when compared to nonsmokers at 6 months postsurgery.
