임플란트와 골 사이의 결합력을 증가시키기 위하여 타이타늄 표면에 변화를 주기위한 많은 연구들이 진행되고 있다. 타이타늄의 표면 구조나 미세 지지도의 변화가 임플란트에 대한 세포의 반응에 영향을 미치며, 골아 유사세포는 표면 조도가 높은 타이타늄 표면에 더 잘 부착하며, 세포외 기질의 합성과 광물화 결정이 더 잘 일어난다고 알려져 있다. 그러나 대부분의 연구들은 마이크로 단위의 미세 지지도에 대한 연구들이고 나노 단위의 미세 지지도에 대한 연구들은 미미하다. 이에 본 연구에서는 ROS 17/2.8 cell line을 이용하여 기계적 처리만한 군을 대조군으로 하여 blasting 처리한 마이크로 단위의 미세 지지도 표면과 알칼리 처리된 나노 단위의 미세 지지도 표면에 대한 골아 유사세포의 세포 부착양상, 증식 그리고 골아 유사세포의 표식인자 발현양상 등을 상호 비교하여 골아 유사세포에 미치는 영향을 관찰하고자 하였다. As we considered in the main subjects, investigations on the theory of 'Doctrine on five elements' motion and six kinds of natural factors(運氣學說)' through 'Wang Bing's Commentary(王氷 注本)' of 'The seven great chapters in The Yellow Emperor's Internal Classic Su Wen' ("黃帝內經素問 七篇大論") are as follows. (1) In The seven great chapters("七篇大論")' Wang Bing supplement theory and in the academic aspects as a interpreter, judging from 'forget(亡)' character. expressed in the 'The missing chapters("素問遺篇")', 'Bonbyung-ron("本病論")' and 'Jabeob-ron(刺法論)', 'The seven great chapters("七篇大論")' must be supplementary work by Wang Bing. Besides, he quoted such forty books as medical books, taoist books, confucianist books, miscellaneous books, etc in the commentary and the contents quoted in the 'Su Wen(素問)' and 'Ling Shu("靈樞")' scripture nearly occupy in the book. As a method of interpreting scripiure as scripture, he edited the order of 'Internal Classic("內經")' ascended from the ancient time and when he compensated for commentary, with exhaustive scholarly mind and by observing the natural phenomena practically and writing the pathology and the methods of treatment. We knew that the book is combined with the study of 'Doctrine on five elements motion and six kinds of natural factors(運氣學說)' (2) When we compare, analyze the similar phrase of 'The seven great chapters in The Yellow Emperor's Internal Classic Su Wen'("黃帝內經素問ㆍ七篇大論") through 'Wang Bing's Commentary(王氷 注本)', he tells abouts organized 'five elements(五行)' and 'heaven's regularly movement(天道運行)' rather than 'Emyangengsangdae-ron("陰陽應象大論")' in 'The seven great chapters("七篇大論")'. Also the 'Ohanunhangdae-ron("五運行大論")' because the repeated sentences with 'Emyangengsangdae-ron("陰陽應象大論")' is long they are omitted. And in the 'Youkmijidae-ron("六微旨大論")', 'Cheonjin ideology(天眞四象)' based on the 'Sanggocheonjin- ron("上古天眞論")', 'Sagijosindae-ron("四氣調神大論")' is written and in the 'Gigoupyondae-ron("氣交變大論")', the syndrome and symptom are explained in detail rather than 'Janggibeobsi-ron("藏氣法時論")', 'Okgijinjang-ron ("玉機眞藏論")' and in the 'Osangieongdae-ron("五常政大論")', the concept of 'five element(五行)' of the 'Gemgwejineon-ron("金櫃眞言論")' is expanded to 'the five elements' motion concept(五運槪念)' and in the 'Youkwonjeonggidae-ron("六元正紀大論")', explanations of 'The five elements' motion and six kinds of natural factors(運氣)' function are mentioned mainly and instead systematic pathology is not revealed rather than 'Emyangengsangdae-ron("陰陽應象大論")'. And in the 'Jijinyodae-ron("至眞要大論")', explanations of the change of atmosphere which correspond to treatment principle by 'The three Yin and Yang(三陰三陽)' as a progressed concepts are revealed. Therefore there are much similarity between the phrase of 'Emyangengsangdae-ron("陰陽應象大論")' and 'chapters of addition(補缺之篇)'. Generally, the doctrine which 'The seven great chapters("七篇大論")' are added by Wang Bing(王氷) is supported because there are more profound concepts rather than the other chapter in 'The seven great chapters("七篇大論")'. (3) When we study Wang Bing's(王氷) 'Pattern on five elements motion and six kinds of natural factors(運氣格局)' in 'The seven great chapter("七篇大論")', in the 'Cheonwongi-dae-ron("天元紀大論")', With 'Cheonjin ideology(天眞思想)' and the concepts of 'Owang(旺)'
Up to the present the scholastic achievements in the history of the medical system have been rather scare despite its importance in the Korean History. Hence, this dissertation attempts to examine the significance of the institute in the Korean History, covering the period from the ancient times through the early Chosun-Dynasty. In the ancient times, the medical practice relied primarily upon human instincts and experiences at the same time, shaman's incantations were widely believed to cure diseases, the workings of evil spirits supposedly. For the period from the Old Chosun through Samhan(巫堂), Chinese refugees brought a long medical knowledge and skills of the continent. New Chinese medicine, traditional practices and incantations were generally used at this time. Medicine and the medical system were arranged by the period of the Three Countries(三國時代). No definite record concerning Koguryo remains now. As for Paekje, however, history shows that they set up the system under the Chinese influence, assigning medical posts such as Euibaksa(medical doctor), Chaeyaksa(pharmacist), and Jukeumsa(medicine man) within Yakbu(department of medicine). Scientifically advanced, they sent experts to Japan, giving a tremendous influence on the development of the science on ancient Japan. After the unification of the three countries, Shilla had their own system after the model of Dang(唐). This system of the Unified Shilla was continued down to Koryo and became the backbone of the future ones. In the ancient time religion and medicine were closely related. The curative function of the shaman was absolute. Buddhism played a notable part in medical practice, too, producing numerous medical monks. The medical system of Koryo followed the model of Dang with some borrowings from Song(宋). Sangyakkuk(尙藥局) was to deal exclusively with the diseases of the monarch whereas Taeeuigam(太醫監) was the central office to handle the national medical administration and the qualification test and education for doctors. In addition, Dongsodaebiwon(東西大悲院), Jewibo(濟危寶), and Hyeminkuk(惠民局) were public hospitals for the people, and a few aristocrats practiced medicine privately. In 987, the 6th year of Songjong(成宗), local medical operations were installed for curing the sick and educating medical students. Later Hyonjong(顯宗), established Yakjom(clinics, 藥店) throughout the country and officials were sent there to see patients. Foreign experts, mainly from Song, were invited frequently to deliver their advanced technology, and contributed to the great progress of the science in Korea. Medical officials were equipped with better land and salary than others, enjoying appropriate social respect. Koryo exchanged doctors, medicine and books mainly with Song, but also had substantial interrelations with Yuan(元), Ming(明), Kitan(契丹), Yojin(女眞), and Japan. Among them, however, Song was most influential to the development of medicine in Koryo. During Koryo Dynasty Buddhism, the national religion at the time, exercised bigger effect on medicine than in any other period. By conducting national ceremonies and public rituals to cure diseases, Taoism also affected the way people regarded illness. Curative shamanism was still in practice as well. These religious practices, however, were now engaged only when medication was already in use or when medicine could not held not help any more. The advanced medical system of Koryo were handed down to Chosun and served the basis for further progress. Hence, then played well the role to connect the ancient medicine and the modern one. The early Chosun followed and systemized the scientific and technical achievement in medicine during the Koryo Dynasty, and furthermore, founded the basis of the future developments. Especially the 70 years approximately from the reign of Sejong(世宗) to that of Songjong(成宗) withnessed a termendous progress in the field with the reestablishment of the medical system. The functions of the three medical institute Naeeuiwon(內醫院), Joneuigam(典醫監), Hyeminkuk(惠民局) were expanded. The second, particualy, not only systemized all the medical practices of the whole nation, but also grew and distributed domestic medicaments which had been continually developed since the late Koryo period. In addition, Hyeminso(惠民局, Hwarinwon(活人院)) and Jesaenwon(濟生院)(later merged to the first) played certain parts in the curing illness. Despite the active medical education in the capital and the country, the results were not substantial, for the aristocracy avoided the profession due to the social prejudice against technicians including medical docotors. During the early Chosun-Dynasty, the science was divided into Chimgueui (acupuncturist), Naryogeui(specialist in scrofula) and Chijongeui (specialist in boil). For the textbooks, those for the qualification exam were used, including several written by the natives. With the introduction on Neoconfucianism(性理學) which reinforced sexual segregation, female doctors appeared for the female patients who refused to be seen by male doctors. This system first appeared in 1406, the sixth year of Taejong(太宗), but finally set up during the reign of Sejong. As slaves to the offices, the lowest class, female doctors drew no respect. However, this is still significant in the aspect of women's participation in society. They were precedents of midwives. Medical officials were selected through the civil exam and a special test. Those who passed exams were given temporary jobs and took permanent posts later. At that time the test score, the work experience and the performance record of the prospective doctor were all taken into consideration, for it was a specialized office. Most doctors were given posts that changed every six months, and therefore had fewer chances for a goverment office than the aristocracy. At the beginning the social status of those in medicine was not that low, but with the prejudice gradully rising among the aristocracy, it became generally agreed to belong to the upper-middle technician class. Dealing with life, however, they received social respect and courtesy from the public. Sometimes they collected wealth with their skills. They kept improving techniques and finally came to take an important share in modernization process during the late Chosun-Dynasty.
도시 보건소 직원의 보건소 업무에 대한 인식 및 태도를 파악하기 위하여 대구직할시 7개 보건소 직원 310명을 대상으로 1994년 8월 15일부터 9월 30일까지 설문조사를 실시하여 252명(회수율 81.3%)의 자료를 분석하여 다음과 같은 결과를 얻었다. 조사대상은 남자가 95명(37.3%), 여자가 157명(62.3%)이고, 60.3%가 대졸이상자였다. 현재 근무부서의 시설이 보건사업을 수행하는데 적합하다고 한 의견이 28.6%, 적합하지 않다가 51.1%였고, 보유 기자재가 사업수행에 적합하다가 19.4%, 적합하지 않다가 39.0%였으며, 보건소의 인력수가 적정하다가 28.6%, 적합하지 않다가 44.8%였다. 근무부서의 예산이 보건사업 수행에 적합하다고 한 의견이 13.1%, 적합하지 않다가 38.5%였다. 지방자치제 실시후 사업내용이 바뀌어야 한다고 한 의견이 51.9%, 지방자치제의 실시가 자신의 근무부서의 업무에 도움이 된다고 한 의견이 25.4%, 도움되지 않는다가 24.6%였다. 지방자치제 실시에 따라 보건소의 조직과 기능이 개선되어야 한다는 의견은 78.6%였다. 사업 목표량의 설정이 해당 부서나 지역의 실정에 비추어 맞게 책정되어 있다는 의견이 11.1%, '그렇지 않다'가 43.3%였다. 업무 수행을 위한 전문적인 지식이나 기술에 대한 교육을 더 받아야 한다고 한 의견이 57.5%, 더 받을 필요없다가 20.6%였고, 자신의 업무수행에 자율성이 있다고 생각하는 견해가 35.7%, 자율성이 없다가 25.8%였으며, 현재 하고 일에 만족한다가 39.3%, 만족하지 못한다가 16.3%였다. 보건소의 인사관리에 대해서는 11.5% 합리적이라고 하였고, 47.3%가 불합리적 이라고 하였으며, 보건소가 주민들로부터 신뢰를 받고 있다는 의견이 41.3%, '그렇지 않다'는 의견이 13.1%였다. 보건소에서 지역주민에게 제공하는 서비스 중에서 잘 시행되고 있는 사업은 결핵관리, 일반진료, 모자보건사업의 순이었으며, 부족한 사업은 보건교육, 치과진료, 위생, 통합보건사업의 순이었다. 향후 보건소에서 주민에게 제공해야 할 서비스로는 노인보건사업, 가정의료사업, 재활보건사업, 당뇨병관리, 고혈압관리, 학교보건사업, 정신보건사업의 순으로 지적하였다. 보건소 근무자들은 시설, 기자재, 인력, 예산, 인사관리, 사업목표량의 설정 및 평가, 인사관리 등에 대해서는 부정적인 의견이 많았으며, 업무수행을 위한 보수교육, 지방자치제 실시를 통한 업무의 변화, 업무의 자율성, 업무의 만족도 면에서는 대체로 긍정적인 의견을 가진 것으로 나타났다.
본 연구는 문헌연구와 옛지도 및 항공사진의 분석, 현장 관찰과 드론 사진 촬영과 고저측량 그리고 주민인터뷰를 통해 익산시 금마면 신용리 황각동에 소재한 '유상곡수(流觴曲水)' 암각서 유적의 입지성과 장소성을 추적하여 이곳에서의 유상곡수 수계(修禊) 등의 향유 가능성을 타진함으로써, 이곳의 장소성을 분명히 함으로써 국내 유상곡수로 정원 유적의 조명과 보존을 목적으로 시도되었다. 본 연구의 결론은 다음과 같다. 유상곡수 유적이 존재하는 황각동 일대는 여러 문헌에서 익산에서 가장 아름다운 가경(佳景)으로 손꼽혀왔다. 황각동의 유래는 의정부(議政府)의 별칭과 밀접하게 관련된 것으로 판단된다. 즉 좌찬성(左贊成)을 역임한 양곡(陽谷) 소세양(蘇世讓)과의 관련성에 주목하였는데 특히 그의 태생지와 인접하고 있을 뿐 아니라 별서인 태허정(太虛亭), 은거당인 퇴휴당(退休堂)과 묘소 그리고 사후 배향 서원 등이 인근에 다수 분포한 점을 그 근거로 제시하였다. 밭일 후 바위에 호미를 걸어 놓는 넓은 바위인 하서대(荷鋤臺)는 중국과 한국의 한시의 용례로 볼 때 한가로운 전원생활과 유유자적하게 살아가는 소박하고 은자적인 삶을 표현한 것으로 보인다. 본 유적의 핵심이 되는 '유상곡수' 바위글씨가 있는 수석바위 상부의 암혈(巖穴)은 수계를 지원하기 위한 차일공(遮日孔)으로 파악되며 인근의 일간정과 모정은 유상곡수를 지원하기 위한 공간 기능을 수행했을 것으로 보인다. 등잔바위 전면에 새겨진 '황각동(黃閣洞)'바위글씨는 황각동천(黃閣洞天)에 이르는 관문으로, 향촌에 존재하는 이상세계를 관념화한 표식으로 파악하였다. 본 연구를 통해서 '황각동'과 '하서대' 바위글씨는 대한제국 광무(光武) 5년인 1901년 3월 29일에 익산군수 오횡묵과 지인인 김인길(金寅吉)이 새긴 것으로 확인되었다. 또한 이봉구의 「황각동운(黃閣洞韻)」과 양곡의 후손인 소진덕의 「황각동시회(黃閣洞詩會)」란 시제로 볼 때 황각동에서 곡수연과 관련되었을 것으로 추정되는 시사 모임이 최소한 일제강점기 초기까지도 행해졌음을 유추할 수 있다. 한편 현재 곡수로의 최대폭은 11.3m, 횡단구배는 15.04%로 계상되었으며 곡수로로 추정되는 수로 구간의 연장거리는 약 27.6m, 종단구배는 3.51%로 측정되었으나 현재 양안이 석축으로 처리된 점을 감안한다면 곡수로의 폭원과 연장거리는 훨씬 길었을 것으로 추정된다. 황각동 유상곡수 유적과 관련된 봄(삼월삼짇날) 모춘(暮春) 이용, 음주와 시 짓기, 시제 「황각동시회」, 유상곡수 바위글씨 그리고 인근 유상정으로 추정되는 일간정이라는 정자의 존재 등으로 볼 때 최소한 조선 말기까지 유상곡수연이 펼쳐진 공간이었음이 확인된다. 아쉽게도 '유상곡수'바위글씨에 대한 암각 주체와 조선 말기 이전의 향유자 등에 대해서는 자료 부족으로 확인하지 못함은 연구 한계로 남는다. 이는 향후 이에 대한 꾸준한 자료 발굴 노력을 통해 구명해야 할 부분이다.
Much has teed changed in the field of hospital administration in the It wake of the rapid development of sciences, techniques ana systematic hospital management. However, we still have a long way to go in organization, in the quality of hospital employees and hospital equipment and facilities, and in financial support in order to achieve proper hospital management. The above factors greatly effect the ability of hospitals to fulfill their obligation in patient care and nursing services. The purpose of this study is to determine the optimal methods of standardization and quality nursing so as to improve present nursing services through investigations and analyses of various problems concerning nursing administration. This study has been undertaken during the six month period from October 1971 to March 1972. The 41 comprehensive hospitals have been selected iron amongst the 139 in the whole country. These have been categorized according-to the specific purposes of their establishment, such as 7 university hospitals, 18 national or public hospitals, 12 religious hospitals and 4 enterprise ones. The following conclusions have been acquired thus far from information obtained through interviews with nursing directors who are in charge of the nursing administration in each hospital, and further investigations concerning the purposes of establishment, the organization, personnel arrangements, working conditions, practices of service, and budgets of the nursing service department. 1. The nursing administration along with its activities in this country has been uncritical1y adopted from that of the developed countries. It is necessary for us to re-establish a new medical and nursing system which is adequate for our social environments through continuous study and research. 2. The survey shows that the 7 university hospitals were chiefly concerned with education, medical care and research; the 18 national or public hospitals with medical care, public health and charity work; the 2 religious hospitals with medical care, charity and missionary works; and the 4 enterprise hospitals with public health, medical care and charity works. In general, the main purposes of the hospitals were those of charity organizations in the pursuit of medical care, education and public benefits. 3. The survey shows that in general hospital facilities rate 64 per cent and medical care 60 per-cent against a 100 per cent optimum basis in accordance with the medical treatment law and approved criteria for training hospitals. In these respects, university hospitals have achieved the highest standards, followed by religious ones, enterprise ones, and national or public ones in that order. 4. The ages of nursing directors range from 30 to 50. The level of education achieved by most of the directors is that of graduation from a nursing technical high school and a three year nursing junior college; a very few have graduated from college or have taken graduate courses. 5. As for the career tenure of nurses in the hospitals: one-third of the nurses, or 38 per cent, have worked less than one year; those in the category of one year to two represent 24 pet cent. This means that a total of 62 per cent of the career nurses have been practicing their profession for less than two years. Career nurses with over 5 years experience number only 16 per cent: therefore the efficiency of nursing services has been rated very low. 6. As for the standard of education of the nurses: 62 per cent of them have taken a three year course of nursing in junior colleges, and 22 per cent in nursing technical high schools. College graduate nurses come up to only 15 per cent; and those with graduate course only 0.4 per cent. This indicates that most of the nurses are front nursing technical high schools and three year nursing junior colleges. Accordingly, it is advisable that nursing services be divided according to their functions, such as professional, technical nurses and nurse's aides. 7. The survey also shows that the purpose of nursing service administration in the hospitals has been regulated in writing in 74 per cent of the hospitals and not regulated in writing in 26 per cent of the hospitals. The general purposes of nursing are as follows: patient care, assistance in medical care and education. The main purpose of these nursing services is to establish proper operational and personnel management which focus on in-service education. 8. The nursing service departments belong to the medical departments in almost 60 per cent of the hospitals. Even though the nursing service department is formally separated, about 24 per cent of the hospitals regard it as a functional unit in the medical department. Only 5 per cent of the hospitals keep the department as a separate one. To the contrary, approximately 12 per cent of the hospitals have not established a nursing service department at all but surbodinate it to the other department. In this respect, it is required that a new hospital organization be made to acknowledge the independent function of the nursing department. In 76 per cent of the hospitals they have advisory committees under the nursing department, such as a dormitory self·regulating committee, an in-service education committee and a nursing procedure and policy committee. 9. Personnel arrangement and working conditions of nurses 1) The ratio of nurses to patients is as follows: In university hospitals, 1 to 2.9 for hospitalized patients and 1 to 4.0 for out-patients; in religious hospitals, 1 to 2.3 for hospitalized patients and 1 to 5.4 for out-patients. Grouped together this indicates that one nurse covers 2.2 hospitalized patients and 4.3 out-patients on a daily basis. The current medical treatment law stipulates that one nurse should care for 2.5 hospitalized patients or 30.0 out-patients. Therefore the statistics indicate that nursing services are being peformed with an insufficient number of nurses to cover out-patients. The current law concerns the minimum number of nurses and disregards the required number of nurses for operation rooms, recovery rooms, delivery rooms, new-born baby rooms, central supply rooms and emergency rooms. Accordingly, tile medical treatment law has been requested to be amended. 2) The ratio of doctors to nurses: In university hospitals, the ratio is 1 to 1.1; in national of public hospitals, 1 to 0.8; in religious hospitals 1 to 0.5; and in private hospitals 1 to 0.7. The average ratio is 1 to 0.8; generally the ideal ratio is 3 to 1. Since the number of doctors working in hospitals has been recently increasing, the nursing services have consequently teen overloaded, sacrificing the services to the patients. 3) The ratio of nurses to clerical staff is 1 to 0.4. However, the ideal ratio is 5 to 1, that is, 1 to 0.2. This means that clerical personnel far outnumber the nursing staff. 4) The ratio of nurses to nurse's-aides; The average 2.5 to 1 indicates that most of the nursing service are delegated to nurse's-aides owing to the shortage of registered nurses. This is the main cause of the deterioration in the quality of nursing services. It is a real problem in the guest for better nursing services that certain hospitals employ a disproportionate number of nurse's-aides in order to meet financial requirements. 5) As for the working conditions, most of hospitals employ a three-shift day with 8 hours of duty each. However, certain hospitals still use two shifts a day. 6) As for the working environment, most of the hospitals lack welfare and hygienic facilities. 7) The salary basis is the highest in the private university hospitals, with enterprise hospitals next and religious hospitals and national or public ones lowest. 8) Method of employment is made through paper screening, and further that the appointment of nurses is conditional upon the favorable opinion of the nursing directors. 9) The unemployment ratio for one year in 1971 averaged 29 per cent. The reasons for unemployment indicate that the highest is because of marriage up to 40 per cent, and next is because of overseas employment. This high unemployment ratio further causes the deterioration of efficiency in nursing services and supplementary activities. The hospital authorities concerned should take this matter into a jeep consideration in order to reduce unemployment. 10) The importance of in-service education is well recognized and established. 1% has been noted that on the-job nurses. training has been most active, with nursing directors taking charge of the orientation programs of newly employed nurses. However, it is most necessary that a comprehensive study be made of instructors, contents and methods of education with a separate section for in-service education. 10. Nursing services'activities 1) Division of services and job descriptions are urgently required. 81 per rent of the hospitals keep written regulations of services in accordance with nursing service manuals. 19 per cent of the hospitals do not keep written regulations. Most of hospitals delegate to the nursing directors or certain supervisors the power of stipulating service regulations. In 21 per cent of the total hospitals they have policy committees, standardization committees and advisory committees to proceed with the stipulation of regulations. 2) Approximately 81 per cent of the hospitals have service channels in which directors, supervisors, head nurses and staff nurses perform their appropriate services according to the service plans and make up the service reports. In approximately 19 per cent of the hospitals the staff perform their nursing services without utilizing the above channels. 3) In the performance of nursing services, a ward manual is considered the most important one to be utilized in about 32 percent of hospitals. 25 per cent of hospitals indicate they use a kardex; 17 per cent use ward-rounding, and others take advantage of work sheets or coordination with other departments through conferences. 4) In about 78 per cent of hospitals they have records which indicate the status of personnel, and in 22 per cent they have not. 5) It has been advised that morale among nurses may be increased, ensuring more efficient services, by their being able to exchange opinions and views with each other. 6) The satisfactory performance of nursing services rely on the following factors to the degree indicated: approximately 32 per cent to the systematic nursing activities and services; 27 per cent to the head nurses ability for nursing diagnosis; 22 per cent to an effective supervisory system; 16 per cent to the hospital facilities and proper supply, and 3 per cent to effective in·service education. This means that nurses, supervisors, head nurses and directors play the most important roles in the performance of nursing services. 11. About 87 per cent of the hospitals do not have separate budgets for their nursing departments, and only 13 per cent of the hospitals have separate budgets. It is recommended that the planning and execution of the nursing administration be delegated to the pertinent administrators in order to bring about improved proved performances and activities in nursing services.
국유림경영(國有林經營)은 어느 나라를 막론(莫論)하고 그 사명(使命)과 경영목적(經營目的)으로 봐서 중요시(重要視)되고 있다. 한국(韓國)의 국유림(國有林)도 또한 한국경제(韓國經濟)의 비약적(飛躍的)인 발전(發展)에 따라 목림수요(木林需要)의 계속적(繼續的)인 증가(增加)로 국가적(國家的)인 사명(使命)과 산업경제적(產業經濟的)으로 더욱 중요(重要)한 위치(位置)에 놓이게 되었다. 그러나 지금(只今)까지 한국임정(韓國林政)의 주요목표(主要目標)가 산림자원(山林資源)의 보존(保存)과 국토보전기능(國土保全機能)의 회복(回復)에만 급급(汲汲)한 나머지 임업(林業)의 경제생산성(經濟生產性)을 높이는 산업정책적의의(產業政策的意義)가 적었음을 우리는 부인(否認)할 수 없다. 그리하여 한국(韓國)의 임업(林業)도 한국경제구조중(韓國經濟構造中)의 일환(一環)으로서 산업적(產業的)으로 발전(發展)시킬 필요(必要)에 직면(直面)하게 되어 국유림(國有林)도 합리적(合理的)인 산림시업(山林施業)에 기초(基礎)를 둔 산림생산력(山林生產力)의 증강(增强)이 절실(切實)하게 되었고, 그렇게 하므로써 결과적(結果的)으로 우수(優秀)한 산림(山林)이 조성(造成)되어 자연(自然), 산림(山林)의 국토보전기능(國土保全機能) 기타(其他)의 공익적기능(公益的機能)도 발휘(發揮)될 수 있을 것으로 본다. 한국(韓國)의 국유림(國有林)은 1908년(年) 임적계출시(林籍屆出時)의 역사적(歷史的) 소산(所產)으로서 그 후(後) 국토보존(國土保存)과 산림경영(山林經營) 학술연구(學術硏究) 기타(其他) 공익상(公益上) 국유(國有)로 보존(保存)할 필요(必要)가 있는 요존림(要存林)과 이에 속(屬)하지 않는 부요존림(不要存林)으로 구분(區分)하고 요존국유림중(要存國有林中) 국가(國家)가 직접(直接) 임업경영(林業經營)을 목적(目的)으로 하는 산림(山林)은 3개영림서(個營林署)에서 관리(管理)하고 있으며 기타(其他)는 각시도(各市道) 및 타부처소관(他部處所管)으로 되어있는데 국유림(國有林)은 1971년말현재(年末現在) 전국산림면적(全國山林面積)의 19.5%(1,297,708 ha)를 점(占)하고 있으나 임목축적(林木蓄積)은 전국산림총축적량(全國山林總蓄積量)의 50.1%(
정부는 보건진료원으로 하여금 지역주민들에게 보다 더 의욕적으로 양질의 보건의료서비스를 제공하도록 하기 위하여 1992년 4월 1일부터 보건진료원을 별정직 공무원으로 정규직화 하였다. 본 연구는 보건진료원의 정규직화가 보건진료원의 업무활동과 보건진료소의 관리운영체계에 미친 영향을 분석하기 위해 경상남도와 경상북도의 보건진료소 중 집락추출법과 단순확률추출법으로 50개소를 뽑아 보건진료원을 대상으로 직접 면담조사하고 제반기록 및 보고서에서 필요한 자료를 발췌하였다. 조사기간은 1992년 1월 1일에서 3월 31일까지(정규직화 이전)와 1993년 1월 1일에서 3월 31일까지(정규직화 이후)였다. 보건진료원들의 96%가 정규직화를 원했는데 그 이유는 신분보장과 보수가 좋아지리라는 것이었다. 정규직화 후 보건진료원직을 자랑스럽게 생각한다는 사람이 24%에서 46%로 증가하였다. 신분보장에 대해서는 항상 불안하다는 사람이 30%에서 10%로 감소하였다. 정규직화 후 월평균 급여액은 802,600원에서 1,076,000원으로 34% 증가했으며 90%가 만족한다고 했다. 업무 내용별 자율성 인지정도는 업무계획, 업무수행, 진료소관리(재정)운영, 업무평가 영역에 대한 자율성 인지도가 정규직화 후에 증가되었다. 보건진료원의 활동내용 중 지역사회 자원파악, 지도작성상태, 지역사회조직 활용정도, 인구구조 파악정도와 가정건강기록부 작성은 정규직화 후에 특별한 변화는 없었다. 또한 집단보건교육, 개인보건교육, 학교보건교육의 실시도 정규직화 후에 변화가 없었다. 그러나 가정방문 실시현황은 1인당 월평균 13.6%회에서 정규직화 후에는 27.5%회로 늘었다. 모성보건 및 가족계획 사업 그리고 예방접종도 정규직화 후에 타기관에 의뢰하는 것이 더 늘었다. 통상질병관리 가운데 성인병관리는 3개월 동안 1개 진료소당 평균 고혈압환자는 12.7%명에서 11.6명으로, 암환자는 1.5명에서 1.2명으로, 당뇨병환자가 4.3명에서 3.4명으로 줄었다. 각종 기록부 비치상황은 장비대장, 약품관리 대장, 환자진료기록부는 100% 비치되었으나 기타 기록부는 그렇지 않았고 정규직화 후에도 변화는 없었다. 보건진료소가 보건소로부터 지원을 받는 내용은 약품 14.0%에서 30%로, 소모품 22.0%에서 52.0%로, 건물유지 및 보수가 54.0%에서 68% 로, 보건교육 자료가 34.0%에서 44.0%로 증가하였고, 장비는 58.0%에서 54.0%로 감소했다. 보건진료소의 월평균 수입은 진료수입이 약 22,000원 증가했고, 국비 또는 지방비 보조금이 4,800원에서 38,508%원으로 증가했으나 회비 및 기부금은 줄어 총수입은 약 50,000원 증가했다. 지출총액은 큰 변동이 없었다. 보건소로부터 3개월 동안 받은 지도감독 중 지시공문을 받은 진료소가 20%에서 38%로 늘었고, 방문지도는 79%에서 62%, 회의소집은 88%에서 74%로 감소하였다. 전화지도는 보건진료소당 평균 1.8회에서 2.1회로 늘었다(p<0.01). 면보건요원과의 협력관계가 있다고 한 보건진료원은 42%에서 36%로 감소하였다. 보건소장과의 관계가 좋다는 보건진료원이 46%에서 24%로 감소하였고, 보건행정계장과 관계가 좋다는 사림이 56%에서 36%로 감소하였다(p<0.05). 보건진료소 운영협의회 회장과의 관계가 좋다는 사람은 62%에서 38%로 감소되었고 보건진료소 운영협의회가 보건진료소에 별로 도움이 안된다와 전혀 도움이 되지 않는다는 사람이 정규직화 전과 후에 각각 92.0%, 82.0%였다. 운영협의회가 필요 없다는 사람은 정규직화 전에 4%에서 16%로 증가되었다(p<0.05). 보건진료원제도 발전을 위해 제안된 사항은 보건교육중심의 활동, 보건진료소운영의 자율성 보장 보건소에 경험이 풍부한 보건진료원을 두어 지도감독하게 할 것과 사용하는 약품의 종류를 늘려 줄 것 등이었다. 이상의 결과로 보하 정규직화 후 보건진료원의 역할, 기능 등의 업무활동의 변화는 거의 없었으나 신분보장과 봉급에 대한 만족도는 향상이 되었고 또한 자율성도 증가하였다. 보건소의 지원은 약간 늘었으며, 지도감독체제에서 지시 공문의 증가로 사무보고 업무가 많아지고, 근무 확인을 위한 전화감독은 늘었으나 업무치진을 위한 행정직 지도 또한 기술적 지도는 거의 없었다.

Based on the function of 'Nei Qu Li '(內驅力) the 'Kao Zheng Pai', in the spirit of 'use confucianism as the base', researched letters, meanings and historical origins. Because they were influenced by the methodology of historical research(考證學) of the Qing era, they valued the evidential research of classic texts, and there was even one branch that did only historical research, the 'Rue Xue Kao Zheng Pai'(儒學考證派). Also, the 'Yi Xue Kao Zheng Pai'(醫學考證派) appeared by the influence of Yoshida Kouton and Kariya Ekisai(狩谷掖齋). 3. In the 'Kao Zheng Pai(考證派)'s theories and views the 'Yi Xue Kao Zheng Pai' did not look at medical scriptures like the "Huang Di Nei Jing"("黃帝內經") and did not do research on 'medical' related areas like acupuncture, the meridian and medicinal herbs. Since they were doctors that used medicine, they naturally were based on 'formulas'(方劑) and since their thoughts were based on the historical ideologies, they valued the "Shang Han Ja Bing Lun" which was revered as the 'ancestor of all formulas'(衆方之祖). 4.
