폐경후 여성의 관상동맥질환 위험요인과 자각증상, 관련지식 및 예방적 건강실천 (Risk Factors, Subjective Symptoms, Knowledge of Coronary Artery Disease and Preventive Health Practices in Postmenopausal Women)

  • Purpose: The purpose of this study was to investigate postmenopausal women's risk factors and subjective symptoms of coronary artery disease, their knowledge about the risk factors and their performance of health behaviors. Method: The subjects were 285 women aged between 50~60 who had naturally reached menopause. We interviewed them using a questionnaire on the subjects' risk factors of coronary artery disease, their knowledge about the risk factors and their performance of preventive health behaviors. Collected data was analyzed through t-test, ANOVA & Duncan test. Results: Risk factors of coronary artery disease observed in the subjects were diabetes (10.9%), hypertension (22.8%) and hyperlipidemia (14.7%). Symptoms of coronary artery disease were experienced more by those who had diabetes, smoked, had a history of coronary artery disease, and thought that they might have coronary artery disease. With regard to the subjects' knowledge about the risk factors of coronary artery disease, the average score was 13.88 out of 20 points. Conclusion: The results of this study suggest that postmenopausal women have several risk factors of coronary arterial disease but they did not have sufficient knowledge about the risk factors and did not manage the factors appropriately.

성별에 따른 관상동맥질환자의 삶의 질 영향 요인 (Gender Differences in Factors Influencing Quality of Life among Patients with Coronary Artery Disease)

  • Purpose: The purpose of this study was to identify the level of quality of life and gender differences in predictors of quality of life among patients with coronary artery disease. Methods: Participants for this descriptive survey were 67 men and 65 women who signed informed consents. They were patients who had undergone coronary artery bypass graft surgery or percutaneous transluminal coronary angioplasty, or were on medication therapy after a heart attack. The Short-Form Health Survey (SF-36), Personal Resource Questionnaire-part (II), and the Center for Epidemiologic Studies Depression Scale were used to measure quality of life, social support, and depression respectively. Gender and age were controlled because they were reported as influencing factors in previous studies. Results: There were significant differences in depression and quality of life between men and women however, social support was not significantly different by gender. In multiple regression analysis, depression was a significant predictor and explained 51.9% of quality of life for men. In women, depression and social support were significant predictors and explained 50.9% of quality of life. Conclusion: Factors influencing quality of life for men and women were different, and therefore, nurses need to consider their patients' gender and use specific strategies to improve quality of life for patients with coronary artery disease.

관상동맥질환자의 혈청 지방산 조성에 관한 연구 (Serum Fatty Acids in Patients with Angiographically-Documented Coronary Artery Disease)

  • To study the relation between serum fatty acids and coronary artery disease(CAD), 194 subjects were randomly selected and divided into three groups(control, single vessel disease group(SVD), muliple vessel disease group(MVD)) according to the angiographic results. Total serum levels of fatty acids and serum phospholipid(PL)-fatty acids(FAs)were analysed using gas chromatography and their associations with CAD were examined. Different patterns of total serum fatty acid levels were found in men and women. Levels of most fatty acids of SVD and MVD were significantly lower in men, while those of MVD were significantly higher in women. In terms of PL-FAs in both men and women, the levels of PL-FAs follow the order of control < SVD < MVD and in women, the difference was significant. Various ratio(including ratios denoting the activites of desaturase and elongase) of total serum-and PL-FAs were similar in the three groups. In the relation of serum fatty acids to serum lipid profiles, PUFA & LDL showed a negative correlation, while, SFA & LDL-cholesterol and PUFA and HDL-cholesterol showed positive correlatons. correlations. Systolic blood pressure and alcohol intake levels negatively affected the levels of serum LA, AA, EPA and DHA in the risk factor analysis. These findings are consistent with other evidence indicating that fatty acid compositions are changed in CAD, especially on the concentration base and the change was related to the severity of the disease. Therefore, for the purpose of disease prevention and therapeutic use, balanced intakes of various fatty acids must be seriously considered.

종합검진 수검자의 관상동맥 전산화단층 혈관조영술 검사에서 관상동맥질환의 위험요인과 관련성 분석 (A Study on the Analysis of Risk Factors and Correlations of Coronary Artery Disease of the Examinee taking Coronary Computed Tomography Angiography in a Comprehensive Health Improvement Center)

  • 본 연구는 건강검진 목적으로 시행된 관상동맥 CT에서의 성별, 연령에 따른 관상동맥질환 발견율에 대한 유용성과 글루코스, 총 콜레스테롤, 내장지방, 체질량지수, 중성지방, 고밀도지단백질(HDL), 저밀도지단백질(LDL) 각각의 인자들과 관상동맥질환과의 관련성에 대해 알아보았다. 총 299명의 수검자를 대상으로 후향적 분석을 실시 한 결과 관상동맥질환 발견율에 대해서는 남성, 50세 이상에서의 유병율이 높은 것으로 나타났으며, 통계적으로 유의성을 보였다. 특히 남성의 유병율(37.9%)은 여성의 유병율(17.0%) 보다 약 2배가 높은 것으로 확인되었다. 또한 관상동맥질환과 관계되는 혈액학적 요인으로는 글루코스와 고밀도지단백질(HDL)이 통계적으로 유의성을 보였다. 관상동맥질환의 예방 및 관리를 위해서는 글루코스와 고밀도지단백질(HDL)의 조절이 필요할 것으로 보이며, 다른 혈액학적 인자들과는 통계적으로 유의성은 낮았지만, 관상동맥질환에 대한 관리의 필요성이 확인되었다. 주요 결과로서 선별검사 목적으로 관상동맥 CT의 사용은 방사선 피폭 등의 문제점으로 인해 수검자의 성별, 나이를 고려하여 시행하는 것이 좋을 것으로 생각되며, 관상동맥 CT 시행 이전에 비침습적인 방법들의 선행검사를 통한 관상동맥 질환 검사가 필요함을 제안한다.

Sex Differences in Coronary Artery Disease: Insights From the KoRean wOmen'S chest pain rEgistry (KoROSE)

  • Interest in sex differences in coronary artery disease (CAD) has been steadily increasing. Concurrently, most of the data on these differences have primarily been Western-oriented. The KoRean wOmen'S chest pain rEgistry (KoROSE), started in 2011, has since published numerous research findings. This review aims to summarize the reported differences between men and women in CAD, integrating data from KoROSE. Cardiovascular risk in postmenopausal women escalates dramatically due to the decrease in estrogen levels, which normally offer cardiovascular protective effects. Lower estrogen levels can lead to abdominal obesity, insulin resistance, increased blood pressure, and endothelial dysfunction in older women. Upon analyzing patients with CAD, women are typically older and exhibit more cardiovascular risk factors than men. Diagnosing CAD in women tends to be delayed due to their symptoms being more atypical than men's. While in-hospital outcome was similar between sexes, bleeding complications after percutaneous coronary intervention occur more frequently in women. The differences in long-term prognosis for CAD patients between men and women are still a subject of ongoing debate. Pregnancy and reproductive factors also play a significant role as risk factors for cardiovascular disease in women. A notable sex disparity exists, with women found to use fewer cardiovascular protective drugs and undergo fewer interventional or surgical procedures than men. Additionally, women participate less frequently than men in clinical research. Through concerted efforts to increase awareness of sex differences and mitigate sex disparity, personalized treatment can be provided. This approach can ultimately improve patient prognosis.

관상동맥질환자의 규칙적 운동이행 영향요인의 성별 비교: 국민건강영양조사 자료(2013~2015년) 활용 (Gender Comparison Factors Influencing Regular Exercise Adherence in Patients with Coronary Artery Disease: Data from the 6th Korea National Health and Nutrition Examination Survey (2013~2015))

  • Purpose: The purpose of this study was to investigate gender related factors that influence regular exercise adherence in patients with coronary artery disease. Methods: This secondary analysis study used data from the 2013~2015 Korea National Health and Nutrition Examination Survey (KNHANES). Three hundred and eighty patients over 20 years of age and having coronary artery disease were selected for this study. Measures included questions about regular exercise, general characteristics, health behaviors, and health status. Data were analyzed using descriptive analysis, Rao-Scott $x^2$ statistic, and multiple logistic regression analysis with the SPSS 21.0 program. Results: The results were as follows. Men did more regular exercise than women (58.8% vs 40.1%). The multivariate-adjusted Odds Ratios (OR) for regular exercise adherence in men were 2.93 (95% CI: 1.39~6.17) for non smokers and 3.06 (95% CI: 1.53~6.13) for men with 0 to 1 comorbidities. Women had a high odds ratios of 0.38 (95% CI: 0.18~0.82) for not using alcohol and 2.10 (95% CI: 1.03~4.29) for no back pain. Conclusion: To improve regular exercise adherence in patients with coronary artery disease, it is necessary to develop different approaches considering gender.

관상동맥질환자에서 병변의 협착정도에 따른 일반영양소 및 개별지방산의 섭취양상 (Nutrients and Individual Fatty Acids Intake Patterns in the Coronary Artery Disease Patients with Different Degrees of Stenosis)

  • Coronary artery disease (CAD) such as angina pectoris and myocardial infarction(MI) have been considered the major cause of death for decaddes . THeir incidence and prevalence are still increasing . Numerous studies have been done on the risk factor analysis of CAD in Western countries. Since the diet in Western countries is different from that in Korea it is difficult to assume that the Korean diet has the same effects as its western counterpeart on the development of CAD . THus the gudidelines for the Western CAD patients can no totally be applied to Koreans. This study was conducted to investigate the relationship between diet and CAD in Koreans. Subjects were comprised of patients admitted to the hospital for chest pain with poxxilbe CAD (men 129, women 65) . They were divided into the following three groups according to angiographic results ; angiogrphically normal coronary artery group (control), single vessel disease group(SVD) and multiple vessel disease group (MVD) . Intakes of dietary fatty acids and other nutrients were assessed by the semiquantitative food frequency method. Blood was also obtained from subjects for serum lipid analysis. Serum lipid profiles of men were clearly different form those of women. For men, serum choesterol levels (or LDL or LDL/HDL) seemed to be higher in the MVD group compared to the control and SVD groups, while TG level was significantly higher in the MVD group for women. Both men and women showed significantly higher caloric , fiber and vitamins C and A intakes in the MVD group than in the other two groups. Higher caloric intakes was due to higher intakes of carbohydrate and protein rather than fat intake. In terms of fatty acids intake, there were no significant differences among the three groups . Smoking seemed to have great effect on eating patterns in CAD patients. Smokers and ex-smokers showed significantly higher intakes of most nutrients including individual fatty acids that non-smokers. The results of stepwise regression showed that moderate alcohol intake decreases LDL levels and increases HDL levels and smoking seems to lowe HDL levels in CAD patients.

한국인 급성 심근경색증 환자의 관상동맥 중증도에 영향을 미치는 대사증후군 위험요인 (Metabolic Syndrome Risk Factors related to Severity of Coronary Artery Diseases in Patients with Acute Myocardial Infarction)

  • Purpose: This study was conducted to identify the clinical characteristics and risk factors on the occurrence of metabolic syndrome (MS), and to examine factors affecting the severity of coronary artery diseases in patients with acute myocardial infarction (AMI). Methods: A total of 894 patients who had admitted C national university hospital from 2008 to 2010 participated in this study. Collected data were lipid profiles, abdominal circumference, blood pressure, fasting blood sugar (FBS) level, participants' demographic data and other risk factors by interview, measurement, and review of participants' medical records. MS was defined according to modified National Cholesterol Education Program Adult Treatment Panel III and Asia-Pacific Criteria. Results: The participants' mean age was 64.7 (${\pm}11.0$) years and 65% was male patients. The participants' with MS was 37.6% in men and 71.4% in women. According to binary logistic regression analysis, high FBS (95% CI 1.7-2.0) and lower high-density lipoprotein (HDL) cholesterol (95% CI 1.1-1.9) were independent predictors of severe coronary artery disease. Conclusion: These risk factors of severe coronary artery disease will be utilized as an important basic data in part of management, education, and countermeasure of patients with both MS and AMI.

디피리다몰 부하 $^{99m}Tc$-MIBI 심근 SPECT 극성결손지도를 이용한 관동맥질환 진단의 남녀 비교 (Comparison of Diagnostic Accuracy for Detecting Coronary Artery Disease of Dipyridamole $^{99m}Tc$-MIBI Myocardial SPECT and It's Defect Map between Men and Women)

  • To evaluate the usefulness and differences in diagnosing coronary artery disease (CAD) between men and women of intravenous dipyridamole $^{99m}Tc$-MIBI myocardial SPECT, we obtained $^{99m}Tc$-MIBI myocardial SPECT and compared with the findings of coronary angiographies. Ninety eight male and 37 female patients who underwent dipyridamole $^{99m}Tc$-MIBI myocardial imaging within one month of cardiac catheterization were studied. Scans were considered abnormal if perfusion defect was detected and the defect size was more than 12% for left anterior descending artery (LAD) and circumflex (LCX) and 8% for right coronary artery (RCA) territories. Lesions${\geqq}$50% luminal diameter narrowing were considered significant CAD. Overall sensitivity for detection of CAD was 94.3% in men and 96.4% in women; specificity was 70% in men and 52.6% in women (P=not significant, ns). Vessel-matched sensitivity was 75.3% in men and 72.7% in women (P=ns): specificity was 84.6% in men and 67.9% in women (P < 0.025). For individual coronary artery, the sensitivity in men and women was 87.7%, 81.8% for LAD; 78%, 83.3% for RCA and 52.2%, 46.7% for LCX (P=ns): the specificity was 80%, 40% for LAD (P<0.01), 82.5%, 68.4% for RCA, 88.9%, 86.4% for LCX (P=ns). The hemodynamic parameter after intravenous dipyridamole in men and women were significantly changed; the heart rate was increased and systolic, diastolic blood pressure was decreased. Adverse effects were reported in 58.8% of men and 72.7% in women (P=ns). The incidence of chest pain and headache were higher in women. There was no significant difference in the incidences of nausea, abdominal pain, dizziness, facial flushing, dyspnea. In conclusion, dipyridamole $^{99m}Tc$-MIBI myocardial SPECT is a safe, noninvasive test for evaluation of CAD. There was no gender difference to detect CAD, but more false-positive rate in women especially in the territory of LAD.

관상동맥질환자의 삶의 질에 영향을 미치는 요소 (Factors explaining Quality of Life in Individuals with Coronary Artery Disease)

  • Purpose: The study was done to compare quality of life by gender, and to identify factors which explain quality of life in individuals with coronary artery disease. Methods: For the survey, 91 individuals (53 men and 38 women) agreed to participate in the study. Cardiovascular risk factors, systolic blood pressure, body mass index, total cholesterol, triglyceride, high density lipoprotein-cholesterol, and low density lipoprotein-cholesterol, health behavior as well as quality of life, were measured. Descriptive statistics, t-test, correlation and hierarchical multiple regression with SPSS WIN 12.0 were used to analyze the data. Results: Significant gender differences were found for education, smoking status, chronic disease, perceived health status, and quality of life within sub-dimensions. Hierarchical regression analysis showed gender (men), age, perceived health status, cardiovascular risk scores, and health behaviors together explained 40.2% (adjusted $R^2$) of variance in quality of life. Conclusion: As the factors explaining quality of me in individuals with coronary artery disease have been identified as gender (men), age, perceived health status, and health behaviors, health promotion programs designed for this population should focus on these factors for effective behavioral modification, and consequent improvement in quality of life.
