Comparative Analysis of Sperm Motility Using Cell Soft System-3000 and Sperm Quality Analyzer-V (Cell Soft System-3000과 Sperm Quality Analyzer-V를 이용한 정자 운동성 비교 분석)

  • Objective: To evaluate the results of CASA systems and to compare its results. Methods: Fifty semen sampales were analysed. Concentration, motility and forward progression were evaluated simultaneously on the same semen samples using Cell Soft System-3000 (CS system) and Sperm Quality Analyzer-V (SQA system). Results: Mean semen volume was $2.8{\pm}1.2\;ml$. Mean value of sperm concentration, motility, forward progression using CS system were $83.4{\pm}45.7{\times}10^6/ml$, $52.3{\pm}16.4%$ and $48.6{\pm}13.4%$, respectively. And mean value of sperm concentration, motility, forward progression using SQA system were $78.2{\pm}42.9{\times}10^6/ml$, $57.0{\pm}24.0%$ and $50.6{\pm}21.9%$, respectively. There were no statistical significancy of sperm concentration, motility, forward progression between the two devices. Conclusion: SQA system variables well correlated with the CS system. As a screening test for semen quality, CS system and SQA system is considered as useful in the management of male infertility.

The Effect of Oriental Medicine Therapy on Results of In Vitro Fertilization-Embryo Transfer (한방치료가 체외수정시술에 미치는 영향)

  • Objectives: We investigated whether oriental medical therapy influences the results of in vitro fertilization-embryo transfer. Methods: 19 women with infertility were treated by oriental treatment from August 1999 to June 2000 in the department of obstetrics and gynecology of an oriental medical center. The women were planning in vitro fertilization-embryo transfer after oriental medicine treatment. The data from the women was analyzed, we obtained the following results. Results: The average age of infertile women was $35.42{\pm}4.86$; infertility caused by male factor or tubal factor were the most frequent. The average number of previous failed IVF treatments was $2.21{\pm}1.81$. The number of aspirated oocytes was $9.00{\pm}6.09$ in IVF cycle before oriental therapy, increasing to $9.80{\pm}5.41$ after therapy, but it was not statistically significant. In IVF before oriental therapy, the average number of embryos transfer was $3.20{\pm}1.90$, and it significantly increased to $4.40{\pm}1.45$ after treatment. The quality points of embryos were $10.00{\pm}6.02$ before treatment, but statistically significantly improved to $14.07{\pm}4.98$ after. The characteristics of women being pregnant and non-successful women were compared with age, period of taking herbal medicine and the number of failed IVF treatments, but there was no significant difference. Conclusion: Oriental treatment before IVF could be expected to have good results in treatment of infertility.

Efficacy of testicular sperm chromatin condensation assay using aniline blue-eosin staining in the IVF-ET cycle

  • Objective: This study was performed to evaluate testicular sperm chromatin condensation using aniline blue-eosin (AB-E) staining and its effects on IVF-ET. Methods: Chromatin condensation was analyzed using AB-E staining in 27 cases of testicular sperm extraction. There were 19 cases of obstructive azoospermia (OA) and 8 cases of non-obstructive azoospermia (NOA) in IVF-ET. Mature sperm heads were stained red-pink whereas immature sperm heads were stained dark blue. The percentage of sperm chromatin condensation was calculated from the ratio of the number of red-pink sperm to the total number of sperm analyzed. Results: The overall percentages of chromatin condensation in OA and NOA were $31.1{\pm}11.2%$ and $26.3{\pm}14.4%$, respectively. The fertilization rate was significant higher in OA than NOA ($p$ <0.05); however, the rates of good embryos and clinical pregnancy did not show statistical differences. In OA and NOA, statistical differences were not observed in the rate of chromatin condensation, fertilization, good embryos, and clinical pregnancy between the pregnant group and non-pregnant group. Conclusion: Chromatin condensation is less stable than OA and showed a low fertilization rate in NOA. While there were no significant differences in chromatin condensation results between NOA and OA, we propose that a pattern of decreased chromatin condensation in NOA is one of the factors of low fertilization results requiring further study.

Development of a Coping Scale for Infertility-Women (CSI-W) (여성의 난임 대처 측정도구 개발)

  • Purpose: The purpose of this study was to develop a Coping Scale for Infertility-Women (CSI-W). Methods: The initial items were based on an extensive literature review and in-depth interviews with seven infertile women. Forty-three items were derived from a pilot survey. Data were collected from 216 women who had experienced intrauterine insemination (IUI) and in-vitro fertilization (IVF) more than once. The data were analyzed to verify the reliability and validity of the scale. Results: Seven factors containing 28 items (four factors containing 17 items for active coping and three factors containing 11 items for passive coping) were extracted from the exploratory factor analysis to verify the construct validity. The four factors of active coping were confrontation, self-control, seeking social support (spouse), and seeking social support (colleagues and experts). The three factors of passive coping were distancing, escape, and avoidance. These items were verified through convergent, discriminant, and concurrent validity testing. The internal consistency reliability was acceptable (active coping: Cronbach's α = .78; passive coping: Cronbach's α = .81). Conclusion: As its validity and reliability have been verified through various methods, the CSI-W can contribute to assessing the coping strategies of infertile women.

Correlations between embryo morphokinetic development and maternal age: Results from an intracytoplasmic sperm injection program

  • Objective: It is widely accepted that aging decreases women's fertility capacity. The aim of this study was to assess correlations between maternal age and the morphokinetic parameters and cleavage pattern of embryos. Methods: The morphokinetics of embryos derived from women < 30, 30-35, 36-40, and > 40 years of age were compared retrospectively in terms of time of second polar body extrusion, time of pronuclei appearance, time of pronuclei fading, and time of two to eight discrete cells (t2-t8). Furthermore, abnormal cleavage patterns such as uneven blastomeres at the two-cell stage, cell fusion (Fu), and trichotomous mitoses (TM) were assessed. Results: Only t5 occurred later in women aged 36-40 and > 40 years when compared with those aged < 30 and 30-35 years (p< 0.001). Other morphokinetic timing parameters, as well the presence of uneven blastomeres, were comparable between the groups (p> 0.05). However, Fu and TM were more common in women aged > 40 years than in younger women (p< 0.001). Conclusion: Maternal age was correlated with the cleavage pattern of embryos. Therefore, evaluating embryo morphokinetics may contribute to optimal embryo selection, thereby increasing fertility in patients with advanced maternal age.

Health Policy Regarding Pregnancy Care in two "Lowest-Low" Fertility Social Contexts: A Comparison between Korean and Japanese Policies

  • To develop policies regarding fertility and pregnancy that will be effective in preventing further declines in fertility rates in the context present-day Korea, current policies in Japan were analyzed and compared with those now being implemented in Korea. This study was structured to involve (a) comparison of maternal health projects in Korea and Japan, and (b) comparison of infertility support policies based in regional cities in Korea and Japan. Korea's Health Plan 2030 emphasizes strengthening healthcare for high-risk pregnant women, expanding investments to benefit vulnerable groups, and establishing a support system for infertile couples. In Japan, government programs involving treatment targeting infertility specifically were implemented nationwide in 2006. Wide dissemination of accurate knowledge related to pregnancy is emphasized. Also, counseling centers specializing in infertility were established by 67 local governments. We have confirmed that Korean policies include decentralization, while Japan is implementing the central government's infertility policy uniformly in all regions. Japan also adjusted its policy out of concern that problems related to infertility and childbirth will worsen due to the social disaster of COVID-19. The results indicate that providing additional support for psychological counseling may be preferable to increasing the number of in-vitro fertilization procedures. The physical burden on women may be minimized by benchmarking policies in Japan. Step-by-step application of these procedures should be systematically supported to achieve the best results.

Acceleration of Early Embryonic Development by Induction of Acrosome Reaction in Intracytoplasmic Sperm Injection (세포질내 정자주입술 시행시 정자의 첨체반응이 수정란의 초기 발생과 임신율에 미치는 영향)

  • Bypassing acrosome reaction and fusion process in intracytoplasmic sperm injection(ICSI), most of injected spermatozoa still contain intact acrosome contents and plasma membrane. It Is not known yet what acrosome contents and plasma membrane of spermatozoa have effect on the development of embryo. For further understanding of fertilization process after ICSI, we studied the time of pronucleus formation, disappearance and first cleavage in human zygote, and pregnancy rate in relation to acrosome reaction rate of spermatozoa after ICSI. Seventy cycles undergoing ICSI program were randomly selected. Sperm suspension from 38 cycles were treated 50% human follicular fluid(hFF) for 3 hours in order to induce acrosome reaction, others were not treated as control. Acrosome reaction in hFF treated and non-treated group was assessed by fluorescein isothiocyanate(FITC)-conjugated Arachis hypogea(PNA) and Pisum sativum agglutinin(PSA). Oocytes were classified into 'good' and 'poor' according to their morphology. After ICSI, fertilization of oocytes were assessed by detection of two pronuclei at 16 hours. The pronuclei disappearance and first cleavage of zygotes were observed at 24 hours, and then embryos were transferred to uterus after culture for 72 hours. The rate of acrosome reaction of spermatozoa in hFF treated group was significantly higher than that in control(p<0.01). Fertilization rates of good oocytes were not different both control and hFF treated group(81.3%(174/206) vs. 72.1%(102/130)). But, in poor oocytes, the fertilization rates in hFF treated group(72.1%(149/183)) were increased compared than those of control group (63.6%(98/140), p<0.01). In either good or poor oocytes, the rates of pronuclei disappearance in hFF treated-spermatozoa injected oocytes were higher than control (59.1%(103/174), 56.4%(84/149) vs. 32.4%(33/102), 37.8%(37/98), p<0.01). Also, the rates of thirst cleavage were increased in hFF treated group (31%(54/174), 24.1%(36/149)) compared than those of control group (10.8%(11/102), 13.2%(13/98), p<0.01). The pregnancy rates of hFF treated group (42.1%(16/38)) were slightly higher than control group (28.1%(9/32), p>0.05). But, the pregnancy rate of group which possessed more than one cleavaged zygote at 24 hours was higher than group which did not (45.2%(19/42) vs. 21.4%(6/28), p<0.05). From these results, the development of zygotes were faster in higher acrosome reacted sperm group than lower acrosome reacted sperm group after ICSI. Our results may be explained that acrosomal membrane and plasma membrane are easily detached from spermatozoa in acrosome reacted spermatozoa compared with acrosome intact sperm in the cytoplasm of oocyte during pronuclear formation. We conclude that the injection of acrosome reacted spermatozoa will increase the pregnancy rate as they can induce fast embryonic development in ICSI.

Relationship between Microdeletions on the Y Chromosome and Defect of Spermatogenesis (Y 염색체 미세결실과 정자형성장애의 연관성에 대한 연구)

  • Objective s: To estimate the frequency of Y chromosome microdeletions in the Korean population of infertile men and to evaluate the relationship between microdeletion on the Y chromosome and clinical phenotypes of infertile men with idiopathic azoospermia and oligozoospermia. Materials and Methods: Genomic DNA was extracted from blood samples collected from 330 infertile men attending the Infertility Clinic at Samsung Cheil Hospital, Korea. Six sequence tagged sites (STSs) spanning the azoospermia factor (AZF) regions of the Y chromosome were amplified by polymerase chain reactions (PCRs). Results: Microdeletions on Y chromosome were detected in 35 (10.6%) of the 330 infertile men. Most of the microdeletions (91.4%) involved AZFb or AZFc. The high incidence of microdeletions were found in AZFc region (57.1%), but the low in AZFa (8.6%) and AZFb (5.7%). Larger microdeletions involving two or three AZF regions were detected in 28.6% of cases. All patients (6 patients) with deletion of AZFa region showed no germ cell phenotypes, Sertoli cell only syndrome or Leydig cell hyperplasia in histopathologic examinations. Conclusion: Microdeletions on the Y chromosome, especially, at AZFc/DAZ regions may be the major cause of azoospermia and severe oligozoospermia. We suggest that idiopathic infertile men have genetic counselling and microdeletion analysis on the Y chromosome before IVF-ET and ART program.

Early Mouse and Human Embryonic Development in vitro by Co-culture with Human Oviduct Epithelial Cells (인간 난관 상피세포와의 공동배양이 생쥐와 인간수정란의 체외발달에 미치는 영향에 관한 연구)

  • We examined effects of co-culture with human oviduct epithelial cells (HOEC) on the development of mouse and human embryos from early embryonic· stage to late morula or blastocyst stage (LM or B). In human, embryos were transferred and pregnancy rate was investigated. The HOEC, collected from surgically removed fallopian tube, were cultured in medium-199 supplemented with 20 % fetal cord serum (FCS). The HOEC were characterized by using immunocytochemical staining with anticytokeratin antibody and then used for cultures of mouse and human embryos. Results obtained from co-culture system were as follows. Development rate of mouse embryos was improved by co-culture system at late developmental stage (p<0.025). Human supernumerary embryos remained after transfer, unsuitable for freezing because of their poor quality, were co-cultured for 72hrs. Co-culture (78.79%) or conditioned medium (78.26%) system improved the developmemt rate, significantly, in comparision with control (11.11%)(p<0.00l). Co-cultured (85.71%) human zygotes for 24hrs showed the better development rate in comparision with control (50.00%) (p<0.01). When we transferred embryos cultured with the HOEC to patients, we obtained one pregnancy. Co-cultured human zygotes for 24hrs showed the better quality and viability for the replacement in comparision with control (p<0.01). In addition, improved pregnancy rate was obtained. Our results suggest that the co-culture system can rescue early degenerating embryos by improving early development and yield a resonable number of blastocyst for the appropriate replacement. The effect provided by cultured HOEC is not species specific for the development of embryos and it can be used to overcome in vitro blocks for the development. And also the co-culture system offers the possibility to freeze embryos at blastocyst stage which is more sucessful stage for the freezing. The HOEC monolayer may provide some stimulus via specific factor, which is unknown, to the development of embryos. Our results showed that the co-culture system with HOEC can be an alternative to conventional culture system.

$In$ $vitro$ development and gene expression of frozen-thawed 8-cell stage mouse embryos following slow freezing or vitrification

  • Objective: This study was performed to compare the efficiency of slow freezing and vitrification based on survival, development to blastocysts, and cell numbers of blastocysts. Changes in embryonic gene expression in fresh and frozen-thawed embryos were also examined. Methods: Eight-cell stage embryos were collected from superovulated female BDF1 mice. The collected embryos were randomly divided into three groups. One group was maintained as fresh controls (n=42), one was frozen by slow freezing (n=43), and one was cooled by vitrification (n=43). After thawing or cooling, survival rates, development to blastocyst, and cell numbers and inner cell mass (ICM) cell numbers of blastocysts were compared with those of the control group. The expressions of eight genes ($Rbm3$, $Birc5$, $Sod1$, $Sod2$, $Cirbp$, $Caspase3$, $Trp53$, $Hsp70.1$) were examined by real time-quantitative polymerase chain reaction in the fresh and frozen-thawed embryos. Results: There were no significant differences in the slow freezing and vitrification groups' survival rate after thawing (88.4% vs. 88.4%), development to blastocyst (100% vs. 97.4%), cell numbers ($107.0{\pm}21.0$ vs. $115.0{\pm}19.7$), or ICM cell numbers of blastocysts ($11.3{\pm}5.2$ vs. $11.1{\pm}3.7$). Cell numbers of blastocysts were significantly ($p$ <0.05) lower in the frozen-thawed embryos than the fresh embryos. There were no significant differences in the slow freezing and the vitrification groups' expressions of the eight genes. The expressions of $CirbP$ and $Hsp70.1$ were higher in the frozen-thawed embryos than in the fresh embryos but there were no significant differences. Conclusion: These results suggest that there were no significant differences between embryos that underwent slow freezing and vitrification.
