Background: Stainless steel wiring remains the most popular technique for primary sternal closure. Recently, a multifilament cable wiring system (Pioneer Surgical Technology Inc., Marquette, MI, USA) was introduced for sternal closure and has gained wide acceptance due to its superior resistance to tension. We aimed to compare conventional steel wiring to multifilament cable fixation for sternal closure in patients undergoing major cardiac surgery. Methods: Data were collected retrospectively on 1,354 patients who underwent sternal closure after major cardiac surgery, using either the multifilament cable wiring system or conventional steel wires between January 2009 and October 2010. The surgical outcomes of these two groups of patients were compared using propensity score matching based on 18 baseline patient characteristics. Results: Propensity score matching yielded 392 pairs of patients in the two groups whose baseline profiles showed no significant differences. No significant differences between the two groups were observed in the rates of early mortality (2.0% vs. 1.3%, p=0.578), major wound complications requiring reconstruction (1.3% vs. 1.3%, p>0.99), minor wound complications (3.6% vs. 2.0%, p=0.279), or mediastinitis (0.8% vs. 1.0%, p=1.00). Patients in the multifilament cable group had fewer sternal bleeding events than those in the conventional wire group, but this tendency was not statistically significant (4.3% vs. 7.4%, p=0.068). Conclusion: The surgical outcomes of sternal closure using multifilament cable wires were comparable to those observed when conventional steel wires were used. Therefore, the multifilament cable wiring system may be considered a viable option for sternal closure in patients undergoing major cardiac surgery.

We herein present the unique case of a 68-year-old male diabetic patient who developed sterile necrosis of the sternum 1 month after myocardial revascularization with the use of bilateral internal thoracic artery grafts. The sternum had been closed by the bilateral Robicsek wiring technique. The sternum was removed, and bilateral pectoralis major flaps were used to cover the defect. The patient had an uneventful recovery.
As device sizes are scaled to submicron dimensions, planarization technology becomes increasing1y important, both during device fabrication and during formation of multilevel interconnects and wiring. Chemical Mechanical Polishing (CMP) has emerged recently as a new processing technique for achieving a high degree of planarization for submicron VLSI applications. This paper is presented the results of CMP process window characterization studies for 0.35 micron process with 6 metal layers.

In here, improved apparatus is the learning equipment of combined use to study the principle and the technique of the originated sequence control circuit and the auto program of PLC.
Integrating micro-machined sensors and actuators on the conventional devices with the copper power lines was incompatible to fabricate the mass produced micro electromechanical system (MEMS) devices. To achieve the compatibility of the wiring method between MEMS parts and devices, we developed the three-dimensional sputter-evaporation system that coats micropatterned thin copper films on the surface of the MEMS element. The system consists of a process chamber, two branch chambers, the substrate holder, and a linear-rotary motion feedthrough. Thin copper film was sputtered and evaporated on the biocompatible polymer, Pellethane$^{circed{R}}$ and silicone, catheter that is 2 mm in diameter and 700 mm in length. The metal film coating technique with three-dimensional thin film sputter-evaporation system was developed to apply the power and signal lines on the micro active endoscope. In this paper, we developed the three-dimensional metal film sputter-evaporation system operated on the low temperature for the biopolymeric substrates used in the medical MEMS devices.
The train control and monitoring system(TCMS) is an on board computer system in railway vehicles performing the control, supervisory and diagnostic functions of the complete train. This system replaces a lot of hardwired relays and minimizes the necessaries vehicle wiring thus increasing the reliability of the train. In this paper, we proposed a software design technique of TCMS using CASE tool (SAO+). The TCMS functions are implemented in software easily programmed, using a functional block, graphic programming language. We applied the compressor air module to a case study.

The factors determine the planarization performance are slurry and pad type, insert material, conditioning technique, and choice of polishing tool. Circuit density, pattern size, and wiring layout also affect the performance of a CMP planarization process. This paper presents the results of studies on CMP process window characterization for 0.35 micron process with 5 metal layers.
Posterior transarticular screw fixation for atlantoaxial instability due to trauma or rheumatoid arthritis provides immediate rigid fixation of the C1-2 vertebral segment while preserving motion between the occiput and C1. This technique provides more resistance to translational and rotational forces than wiring technique. However, the technique of transarticular screw fixation is inherently demanding because of the complex anatomy of the occipitocervical region and vertebral artery(VA) at risk for arterial damage. VA injury may lead to serious subsequent neurological deficits and possibly death from bilateral VA injury. We report a case of a vertebral artery-to-epidural venous plexus fistula after posterior transarticular screw fixation which was treated with balloon occlusion.
Objective : In the retrospective analyzing 19 consecutive patients with subaxial cervical spine(C3~T1) injury treated by posterior cervical fixation and fusion, clinical manifestation, radiologic finding, operative technique, and postoperative results following 6 months were analyzed. Materials and Methods : Most common fracture level was C4-5, mean age 41, and male to female ratio 13 : 6. The most common cause of injury was motor vehicle accident(17 cases). In 19 cervical procedures, interspinous triple wiring was done in 14 cases, lateral mass plating in 5 cases, and additional anterior fusion in 2 cases. Results : Twelve weeks after operation, all cases were reviewed by plain cervical radiogram. In 17 cases that treated by posterior fusion only, 14 cases(81%) had kyphotic angle change less than $5^{\circ}$, 2 cases(12%) $5-20^{\circ}$, and 1 case(6%) more than $20^{\circ}$. Overall fusion rate was 88%, and there was no significant difference of bone fusion rate between autogenous bone graft and allogenous bone graft. Conclusion : In the case of severe posterior column injury or displacement, posterior approach seems superior to anterior approach, but in the case of combined anterior column injury, anterior approach is considered necessary. In this study, posterior fixation and fusion might be acceptable procedure for subaxial cervical fracture and dislocation, owing to its high fusion rate, low kyphotic angulation and low operation related complication rate.
배경: 통상적인 정중 흉골 절개를 이용한 개심술 후 흉골의 불안정성과 흉골 창상 감염은 대부분 동반되어 발생하며 수술 후 경과에 심각한 영향을 끼치게 된다. 본 연구에서는 각기 다른 흉골 봉합 방법을 사용함에 따른 흉골 감염 빈도를 비교하였다. 대상 및 방법: 2004년 4월부터 2008년 12월까지 흉골 절개를 이용한 개심술을 시행 받은 성인 환자 157명을 대상으로 하였다. 이들 중 86명의 A군 환자들은 통상적인 흉골 봉합법을 사용하였고, 나머지 71명의 B군 환자에게는 변형 Robicsek 흉골 봉합법을 사용하였으며. 두 군간의 수술 후 흉골창상 합병증의 빈도에 대해 조사하였다. 결과: 평균 연령은 A군보다 B군이 더 높았다($61{\pm}10$세 vs $57{\pm}13$세). 수술 전 좌심실 구출 율 30% 미만의 좌심실 기능 부전, 만성 폐쇄성 폐질환, 투석이 필요한 신부전 및 당뇨의 빈도가 B군에서 유의하게 높았으나 그 외 수술 전후 감염의 위험 인자는 두 군간에 유의한 차이가 없었다. A군 환자 중 2명에게서 표재성 흉부 창상 감염이 발생하였고 B군에서는 4명의 환자에게서 발생하였으나 통계적 유의한 차이는 보이지 않았다(p=0.255). 그러나 A군 환자들 중 6명의 환자들에게서 흉골 감염이 발생한 반면 B군에서는 한 명도 발생하지 않았으며 이는 유의한 차이를 보였다. 결론: 고위험 환자에게 변형 Robicsek 흉골 봉합법을 사용함으로써 흉골의 안전성을 확보할 수 있었으며, 흉골 감염 빈도가 줄어든 것을 확인할 수 있었다.
