Effects of Moxibustion on Physiological Indices and Autonomic Nervous Symptoms in Adults with Prehypertension (뜸요법이 전고혈압 성인의 생리적 지수와 자율신경계 증상에 미치는 효과)

  • Purpose: This study was done to examine the effects of moxibustion on physiological indices, such as 24-hr blood pressure, pulse pressure, and pulse rate, and autonomic nervous symptoms among adults with prehypertension. Methods: Participants were adults whose systolic and diastolic blood pressures were in the prehypertension stage and were not under treatment for the condition. Both experimental and control group consisted of 13 participants who were taking classes on acupuncture and moxibustion at a private institute. The experimental group received moxibustion once a day for 15 min, at least 5 days a week for 8 weeks. The results were analyzed using $x^2$-test, t-test, Fisher's exact test, Wilcoxon rank sum test and repeated measures ANOVA with the SAS program. Bonferroni correction method was adopted for multiple comparisons. Results: No significant change was observed in the 24-hr systolic and diastolic blood pressure between the two groups. Differences in the 24-hr pulse pressure and pulse rate were also not significant. However, significant decreases in headache, symptoms of flushing and fatigue were observed in the experimental group. Conclusion: The results suggest that moxibustion contributes to alleviating symptoms such as headache, flushing and fatigue associated with the autonomic nervous system among adults with prehypertension.

  • The purpose of this study is, first, to develop a Therapy Competency Enhancement Program for Novice Play-therapists Using Traditional Play to enhance therapy Competency and second, to examine the program's effectiveness. The program consists of 10 sessions and each session employs a variety of traditional plays. The Therapeutic relationship Scale and Playfulness Scale were used as a pre-test and post-test respectively. The data were analyzed by the SPSS 18.0 program, which applied the Mann-Whiteney U test and Wilcoxon rank sum test. Twelve novice play therapists participated in ten sessions of the Therapy Competency Enhancement Program, and were compared with the control group of twelve novice play therapists who did not receive treatment. The treatment group statistically showed significant enhancement in therapeutic relationships, and playfulness in comparison to the control group. It was concluded that the Therapy Competency Enhancement Program for Novice Play-therapists Using Traditional Play was effective for enhancing therapeutic relationships and playfulness. This study has significance in its attempt to develop a Therapy Competency Enhancement program to improve the therapeutic relationship, and playfulness of novice play therapists by Traditional Play.

Delayed Enhancement Magnetic Resonance Imaging Findings in Cardiac Amyloidosis (심장 아밀로이드증의 지연 조영증강 MR 영상소견)

  • Purpose : To evaluate late gadolinium enhancement (LGE) pattern of left ventricular (LV) myocardium and presence or absence of LGE in other regions of the heart on cardiac magnetic resonance (CMR) imaging in patients diagnosed with cardiac amyloidosis. Materials and Methods: From 2009 to 2011, 9 patients who were suspected cardiac amyloidosis underwent CMR. We retrospectively analyzed the presence or absence of LGE and enhancement pattern in LV myocardium, and the presence or absence of LGE in other chambers as well. Also we measured interatrial septal thickness (IST), relative signal intensities of atrial septum and epicardial fat over the left atrial (LA) cavity on delayed enhanced images. MRI parameters in these patients were compared to those of control group of patients with ischemic heart disease by Wilcoxon rank sum test. Results: Of nine patients, LGE were found in 8; subendocardial circumferential pattern in 4 and diffuse pattern in 4. LGE in right ventricle was observed in 7. IST was significantly increased in patients with cardiac amyloidosis (P = 0.02). Ratio of atrial septum to LA cavity and ratio of epicardial fat to LA cavity showed a significant difference (P = 0.0002 and P = 0.0006, respectively). Conclusion: In LGE CMR, subendocardial or diffuse enhancement pattern is a typical finding for patients with cardiac amyloidosis. Atrial septum and epicardial fat show relatively increased signal intensities over LA blood cavity.

A Pilot Study on Korean Version Development of the Rearing Knowledge and Practice for Infant Parents (부모의 양육지식과 실천 도구 개발 예비연구)

  • Rearing knowledge is a significant factor of relevance for rearing practice, and assessment of rearing knowledge and practice is important for infant parents. The author adopted the Child Rearing Knowledge Scale (CRKS) and Child Rearing Practice Scale (CRPS) that were developed and validated by Saramma & Thomas. According to the international linguistic validation process, pilot testing was done based on 20 infant's mothers in one Oketani massage center and one public health center. Descriptive statistic methods and Wilcoxon Rank Sum Test were employed to evaluate the level and compare the mean score according to general characteristics of subjects of the tools. Reliability and validity were tested by Cronbach's alpha and Spearman Correlations. The mean age of the subjects was 33.94 (${\pm}2.99$) years and the mean age of babies was 3.35 (${\pm}0.58$) months. The understanding level of the Korean version of the CRKS and CRPS was 1.80 (${\pm}0.65$) and 1.33 (${\pm}0.54$), respectively. In addition, the CRKS and CRPS were relatively easy to use. The mean score of the CRKS was 22.50 (${\pm}4.89$), which was a moderate score, while the mean score of the CRPS was 30.75 (${\pm}2.04$), which was high. The Cronbach's alpha values of the CRPS were as follows: feeding, 0.71; growth and development, 0.64; cleaning and protection 0.68; infant stimulation, 0.77. There was a significant correlation between infant stimulation of the CRPS and growth and development of the CRKS (r=0.530, p=0.016). The CRKS score of medical staff was significantly higher than that of non-medical staff (p=0.04). The CRKS and CRPS are expected to be used in clinical or community care practice as easy-to-use tools that are easy to respond to.

The Effects of a Community Psychiatric Nursing Program on the Rehabilitation of Home-based Long-term Psychiatric Patients

  • In the last few years, psychiatric nurse practitioners have shown a growing an interest in community psychosocial rehabilitation, caring for chronic psychiatric patients as case manager in South Korea. The purpose of this study was to evaluate the effectiveness of a community psychiatric rehabilitation nursing program on self-care activity and quality of life and to suggest this program as an effective nursing intervention in a group of chronic home-based psychiatric patients in a poor town. A nonequivalent control group, pretest-posttest design was used. Of the twenty women that started the program, sixteen finished it. The data were analyzed by the Wilcoxon Rank Sum Test. The program included the process of case management which consisted of four phases: the first was an active case finding and pre-test, the second was home visiting and contacted by phone. the third was group activity therapy of 12 sessions, and the fourth phase was terminal and post-test. The effects of the program were assessed by quality of life and self-care activity. The quality of life and the self-care activity, especially, area of nutrition. elimination, dressing. leisure activity, and follow-up clinic visiting showed greater improvement than those of the control group. The results of this study suggest that this program was effective in improving the quality of life of chronic home-based psychiatric patients.

Effects of a Proper Positioning on Prevention of Musculoskeletal Complication on Patients with Stroke. (뇌졸중 환자의 적절한 체위 유지가 근골격계 합병증 예방에 미치는 영향)

  • This study was done to identify the effectiveness of a proper positioning on musculoskeletal complication on patient with Stroke by using a quasi-experimental study. A total of 18 patients were selected as a subject from June 1st to October 31th 1998 who had been hospitalized at Intensive Care Units in K medical center. A experimental group consists of 8 patients who were given proper positioning every two hours. A control group consists of 10 patients who were given traditional positioning. The collected data were analyzed by SPSS windows including $X^2$-test (Fisher's exact method), Wilcoxon Rank Sum test and McNemar test. The result of this study was summerized as follows : 1) The experimental group with proper positioning has shown lower shoulder pain score, dorsiflexion contracture score, Shoulder subluxation score and higher Range of Motion and than the control group. 2) The experimental group with proper positioning has shown lower muscle pain score and edema score than the control group, but it was not statistically significant.Therefore, proper positioning could be applied as an independent nursing intervent ion for patients on Stroke in order to facilitate rehabilitation.

Sleep Patterns and Factors Associated with Excessive Daytime Sleepiness in University Students (대학생의 수면양상과 주간졸음증에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to investigate sleep patterns and predictors of excessive daytime sleepiness (EDS) in university students. Methods: Participants were 120 university students who were attending two universities in S-city and C-city. Data were collected from May 20 to June 15, 2012 using self-report questionnaires which included Johns' Epworth Sleepiness Scale, Yi's Sleep Quality Scale, and Beck Depression Inventory. Data were digitalized and analyzed using frequency, percentages, means and standard deviations, Wilcoxon rank sum test, Chi-square test, Fisher's exact test, and multiple logistic regression with SAS 9.0. Results: Mean total sleep time was 6.6 hours on weekdays, 8.1 hours on weekends. Mean sleep latency was 19.1 minutes and the score for mean sleep quality was 22.6. Prevalence of EDS was 12.5%. Depression was significantly different between EDS and Non-EDS students (t=2.17, p=.030). Multiple logistic regression showed that the only factor associated with EDS was depression (adjusted odds ratio of depression=5.33, 95% Confidence Interval=1.49-19.04). Conclusion: Results of this study indicate that university students experience short sleep time, low sleep quality, and common EDS with depression, suggesting that students with complaints of EDS should be completely assessed for depression as well as sleep problems.

Knowledge, Performance, and Educational Needs of Infection Control among Nurses in Long-term Care Hospitals: A Focus on Jeju Province (요양병원 간호사의 감염관리에 대한 지식, 수행도 및 교육요구 -제주 지역을 중심으로-)

  • Purpose: This study aimed to provide basic data for the development of education programs which improve the nurses' infection control performance by investigating the knowledge, performance, and educational needs of infection control among nurses in long-term care hospitals, and analyzing the relationship between these parameters. Methods: This was a descriptive study. A self-reported questionnaire was provided to 153 nurses in 210 long-term care hospitals on Jeju Island. Their knowledge, performance, and educational needs of infection control, data were analyzed using SAS Window(ver. 9.4), t-test, Wilcoxon rank-sum test, one-way ANOVA, Scheffe test, and Pearson's correlation coefficient. Results: Both knowledge (r=0.16, p=.042) and performance (r=0.52, p<.001) of infection control had positive correlations with the educational needs of the infection control. Conclusion: The higher the knowledge of infection control was, the higher the educational needs of the nurses were. However, knowledge of infection control did not correlate with performance of infection control. Therefore to increase the knowledge and performance of infection control, infection control education programs should suit the educational needs and the actual conditions of long-term care hospitals.

Outcomes of Home Care Service for Coronary Artery Bypass Graft (관상동맥우회술 환자의 가정간호 효과)

  • This study was to compare the functional status. complication and readmission rates. and client satisfaction with nursing care of home-based care and hospital-based care for clients with Coronary Artery Bypass Graft. Raw data were collected by interviewing and reviewing charts of 41 clients with Coronary Artery Bypass Graft between June 2001 and July 2002 at an university hospital located in Seoul. Korea. Out of 41 clients. 15 were in home care group and 26 were in hospitalization group. The baseline characteristics of the groups were almost identical. Mean age was 61.7 and 75.6% of clients being male. For home care group. the data collection was made at discharge and at termination of home care. and for hospitalization group. at discharge and at the first visit of outpatient department. Complication and readmission rates were investigated at one month after operation. Collected data were then analysed by conducting Chi-square test. Wilcoxon rank sum test. and Wilcoxon signed ranks test with SPSS program. The level of significance was .05. The results of the study are summarized as follows: 1. Postoperative length of stay of the home care group was shorter than that of hospitalization group by 1. 14 days(8.45 days vs. 9.59 days). On average. 1.8 home visits per client were observed. 2. The functional status (Barthel Index) at the termination of home care was significantly increased from that at discharge. For hospitalization group. a significant increase was observed between the functional status at the discharge and that at the first visit of outpatient department. The differencies in incremental of the scores. between the groups. were however not significant. 3. Complication and readmission rates; no statistically significant difference between the groups was observed. 4. The client satisfaction with nursing care (CSS) at termination of home care was significantly higher than that at hospital discharge. In conclusion. the outcomes of the analysis suggest that the home care benefits clients with Coronary Artery Bypass Graft. Client satisfaction with nursing care rises at termination of home care as compare to that measured at hospital discharge. Meanwhile. there was no significant differences in functional status. and complication and readmission rates. Further. home care reduced the length of stay in hospital.

Clinical Study of Tetracycline HCl Conditioning in the Treatment of Gingival Recession. A Comparative Study (치근 피개 술식시 치근에 도포된 테트라사이클린의 효과에 관한 임상적 연구)

  • Histological studies indicate a tetracycline HCl similar to citric acid to induce connetive tissue repair in animals. When tetracycline HCl was used as a root conditioning agent in humans, there was a trend toward more connective tissue attachment than in root planing alone. The purpose of this study was to evaluate clinical effect of tetracycline HCl in the treatment of gingival recession. 44 teeth in 12 patients with bilaterally gingival recession & Miller classification I, II gingival recession were selected and 22 teeth were treated with 125mg/ml tetracycline HCl , the others was not treated with tetracycline HCl. Gingival recession, pocket depth, clinical attachment level, width of keratinized gingiva were observed at baseline, postoperative 4, 12, 20weeks. Both groups were statistically analyzed by Wilcoxon signed Ranks Test & Wilcoxon's rank sum test(Mann-whitney test) using SPSS program.(5% significance level) The results were as follows: 1........The change of gingival recession, clinical attachment level, keratinized gingiva in both groups were increased significantly at 4, 12, 20 weeks. 2.......The pocket depth exhibited no marked changes throughout the entire investigation in both groups. 3........The change of gingival recession in tetracycline group was increased significantly than control group at 4, 12, 20 weeks and the percentage of root coverage was 93% in tetracycline group and 83% in control group. 4........The change of clinical attachment level, pocket depth, keratinized tissue from baseline to 4, 12, 20 weeks was not differ significanltly in both group.
