Development of a predictive model for hypoxia due to sedatives in gastrointestinal endoscopy: a prospective clinical study in Korea

  • Background/Aims: Sedation has become a standard practice for patients undergoing gastrointestinal (GI) endoscopy. However, considering the serious cardiopulmonary adverse events associated with sedatives, it is important to identify patients at high risk. Machine learning can generate reasonable prediction for a wide range of medical conditions. This study aimed to evaluate the risk factors associated with sedation during GI endoscopy and develop a predictive model for hypoxia during endoscopy under sedation. Methods: This prospective observational study enrolled 446 patients who underwent sedative endoscopy at the Korea University Ansan Hospital. Clinical data were used as predictor variables to construct predictive models using the random forest method that is a machine learning algorithm. Results: Seventy-two of the 446 patients (16.1%) experienced life-threatening hypoxia requiring immediate medical intervention. Patients who developed hypoxia had higher body weight, body mass index (BMI), neck circumference, and Mallampati scores. Propofol alone and higher initial and total dose of propofol were significantly associated with hypoxia during sedative endoscopy. Among these variables, high BMI, neck circumference, and Mallampati score were independent risk factors for hypoxia. The area under the receiver operating characteristic curve for the random forest-based predictive model for hypoxia during sedative endoscopy was 0.82 (95% confidence interval, 0.79-0.86) and displayed a moderate discriminatory power. Conclusions: High BMI, neck circumference, and Mallampati score were independently associated with hypoxia during sedative endoscopy. We constructed a model with acceptable performance for predicting hypoxia during sedative endoscopy.

Effect of operating temperature using Ni-Al-$ZrH_2$ anode in molten carbonate fuel cell (Ni-Al-$ZrH_2$ 연료극을 사용한 용융탄산염 연료전지의 온도의 영향)

  • Fuel cell is a device that directly converts chemical energy in the form of a fuel into electrical energy by way of an electrochemical reaction. In the anode for a high temperature fuel cell, nickel or nickel alloy has been used in consideration of the cost, oxidation catalystic ability of hydrogen which is used as fuel, electron conductivity, and high temperature stability in reducing atmosphere. Most MCFC stacks currently operate at an average temperature of $650^{\circ}C$. There is some gains with decreased temperature in MCFC to diminish the electrolyte loss from evaporation and the material corrosion, which could improve the MCFC life. However, operating temperature has a strong related on a number of electrode reaction rates and ohmic losses. Baker et al. reported the effect of temperature (575 to $650^{\circ}C$). The rates of cell voltage loss were 1.4mV/$^{\circ}C$ for a reduction in temperature from 650 to $600^{\circ}C$, and 2.16mV/$^{\circ}C$ for a decrease from 600 to $575^{\circ}C$. The two major contributors responsible for the change in cell voltage with reducing operation temperature are the ohmic polarization and electrode polarization. It appears that in the temperature range of 550 to $650^{\circ}C$, about 1/3 of the total change in cell voltage with decreasing temperature is due to an increase in ohmic polarization, and the electrode polarization at the anode and cathode. In addition, the oxidation reaction of hydrogen on an ordinary nickel alloy anode in MCFC is generally considered to take place in the three phase zone, but anyway the area contributing to this reaction is limited. Therefore, in order to maintain a high performance of the fuel cell, it is necessary to keep this reaction responsible area as wide as possible, that is, it is needed to keep the porosity and specific surface area of the anode at a high level. In this study effective anodes are prepared for low temperature MCFC capable of enhancing the cell performance by using zirconium hydride at least in part of anode material.

Design Optimization of Heat Exchangers for Solar-Heating Ocean Thermal Energy Conversion (SH-OTEC) Using High-Performance Commercial Tubes (고성능 상용튜브를 사용한 태양열 가열 해양온도차발전용 열교환기 설계 최적화)

  • In this study, the optimal design of heat exchangers, including the evaporator and condenser of a solar-heating ocean thermal energy conversion (SH-OTEC), is investigated. The power output of the SH-OTEC is assumed to be 100 kW, and the SH-OTEC uses the working fluid of R134a and high-performance commercial tubes. The surface heat transfer area and the pressure drop were strongly dependent on the number of tubes, as well as the number of tube passes. To solve the reciprocal tendency between the heat transfer area and pressure drop with respect to the number of tubes, as well as the number of tube passes, a genetic algorithm (GA) with two objective functions of the heat transfer area (the capital cost) and operating cost (pressure drop) was used. Optimal results delineated the feasible regions of heat transfer area and operating cost with respect to the pertinent number of tubes and tube passes. Pareto fronts of the evaporator and condenser obtained from multi-objective GA provides designers or investors with a wide range of optimal solutions so that they can select projects suitable for their financial resources. In addition, the surface heat transfer area of the condenser took up a much higher percentage of the total heat transfer area of the SH-OTEC than that of the evaporator.

Numerical Analysis of Added Resistance and Vertical Ship Motions in Waves for KVLCC2 (KVLCC2에 대한 파랑 중 부가저항과 수직운동에 대한 수치해석)

  • The present study provides numerical simulations to predict the added resistance and ship motion of the KVLCC2 in regular waves using the unsteady Reynolds-Averaged Navier-Stokes (URANS) and 3-D potential methods. This numerical analysis is focused on added resistance and vertical ship motions (heave and pitch) under a wide range of wave conditions at three ship speeds (design, operating and zero speeds). Firstly, the characteristics of the CFD and 3-D potential flow methods are presented to predict added resistance and ship motions in regular waves taking into account various wave conditions at design speed to provide a validation study as well as at operating and zero speeds. Secondly, analyses of added resistance and ship motion with unsteady wave patterns and time history results as simulated by CFD were performed at each ship speed. Systematic validation and verifications of the numerical computations in this study were made against available Experimental Fluid Dynamics (EFD) data including grid convergence tests to demonstrate that reliable numerical results were obtained for the prediction of added resistance and ship motion in waves. Relationships between added resistance, vertical motion and changes in ship speeds were also found.

Development of Tacho Generator for Application of Anti-aircraft Weapon System (대공무기체계 적용을 위한 타코제너레이터 개발)

  • This paper presents the development of a tacho generator that is applicable to a DC motor for anti-aircraft weapon systems. In general, devices such as tacho generators and resolvers are used as feedback devices for controlling DC motors. A tacho generator with a wide operating temperature range was developed, which has robust characteristics against shock loads and vibrations according to the operational characteristics of anti-aircraft weapon systems. The target specifications were set based on the requirements of the tacho generator currently in operation. A rotor coupled to the shaft of the motor and a stator coupled to the housing of the motor were then designed and manufactured. The inductance was 31.0 mH, the terminal resistance was 147.7 ohms, and the rotational measurement factor was satisfactory under both normal operation and operating conditions after the maximum speed for the standard of 9.500 ± 0.475 V/krpm. In addition, the environmental suitability of the applied equipment was confirmed through the rate of change in unit temperature, and it was found that the temperature characteristics were all within 0.03 %/℃.

  • Recently, various studies have been conducted to optimize the pump operation scheduling and/or the pump/tank size minimizing the system cost of water distribution network. Prior to that, it is important to understand the sensitivity of pump/tank size on the system cost and overall water quality. Here, we have performed the sensitivity analysis to investigate the effect of pump/tank size on the economic cost (construction and operation) and water quality (water age). The analysis was applied on a real, large-scale water transmission network currently operating in South Korea. The results revealed that the pump/tank size has a strong influence on system construction/operation costs. Especially, the tank size has a significant effect on the system-wide water quality. In the case of applied networks, the operating cost decreases as the capacity of the facility increases, but the design cost increases. Using a sensitivity analysis, a suitable range of pump/tank size could be suggested to minimize costs and stabilize the water quality at the same time prior to a system design.

  • This work describes a re-configurable 10MS/s to 100MS/s, low-power 10b two-step pipeline ADC operating at a power supply from 0.5V to 1.2V. MOS transistors with a low-threshold voltage are employed partially in the input sampling switches and differential pair of the SHA and MDAC for a proper signal swing margin at a 0.5V supply. The integrated adjustable current reference optimizes the static and dynamic performance of amplifiers at 10b accuracy with a wide range of supply voltages. A signal-isolated layout improves the capacitor mismatch of the MDAC while a switched-bias power-reduction technique reduces the power dissipation of comparators in the flash ADCs. The prototype ADC in a 0.13um CMOS process demonstrates the measured DNL and INL within 0.35LSB and 0.49LSB. The ADC with an active die area of $0.98mm^2$ shows a maximum SNDR and SFDR of 56.0dB and 69.6dB, respectively, and a power consumption of 19.2mW at a nominal condition of 0.8V and 60MS/s.

A study on the improvement of work flow and productivity in complex manufacturing line by employing the effective process control methods (복잡한 생산라인에서 효율적 공정관리 기법 도입에 따른 공정흐름 및 생산성 개선 연구)

  • Due to the change from small volume production to small quantity batch production systems, individual companies have been attempting to produce a wide range of operating strategies, maximize their productivity, and minimize their WIP level by operating with the proper cycle time to defend their market share. In particular, using a complex workflow and process sequence in the manufacturing line has some drawbacks when it comes to designing the production strategy by applying analytical models, such as mathematical models and queueing theory. For this purpose, this paper uses three heuristic algorithms to solve the job release problem at the bottleneck workstation, product mix problem in multi-purpose machine(s), and batch size and sequence in batch machine(s). To verify the effectiveness of the proposed methods, a simulation analysis was performed. The experimental results demonstrated that the combined application of the proposed methods showed positive effects on the reduction of the cycle time and WIP level, and improvement of the throughput.

Clinical Microscopy: Performance, Maintenance and Laser Safety (임상에서의 현미경: 작동, 유지보수 및 레이저 안전)

  • A microscope is the fundamental research and diagnostic apparatus for clinical investigation of signaling transduction, morphological changes and physiological tracking of cells and intact tissues from patients in the biomedical laboratory science. Proper use, care and maintenance of microscope with comprehensive understanding in mechanism are fully requested for reliable image data and accurate interpretation for diagnosis in the clinical laboratory. The standard operating procedure (SOP) for light microscopes includes performance procedure, brief information of all mechanical parts of microscopes with systematic troubleshooting mechanism depending on the laboratory capacity. Maintenance program encompasses cleaning objective, ocular lenses and inner optics; replacement and calibration of light source; XY sample stage management; point spread function (PSF) measurement for confocal laser scanning microscope (CLSM); quality control (QC) program in fluorescent microscopy; and systematic troubleshooting. Laser safety is one of the concern for medical technologists engaged in CLSM laboratory. Laser safety guideline based on the laser classification and risk level, and advisory lab wear for CLSM users are also expatiated in this overview. Since acquired image data presents a wide range of information at the moment of acquisition, well-maintained microscopes with proper microscopic maintenance program are impulsive for its interpretation and diagnosis in the clinical laboratory.

Turbine Efficiency Analysis of Steady Flow in a Twin Scroll Turbocharger (트윈 스크롤 터보과급기에서 정상유동의 터빈 효율 분석)

  • The turbochargers used widely in diesel and gasoline engines are effective devices to reduce fuel consumption and emissions. In this study, the isentropic turbine efficiency of the steady flow in a twin-scroll turbocharger for the passenger vehicle gasoline engine was analyzed. The cold gas test bench was designed and made. The pressure and temperature of the inlet and exit of the turbine were measured at 60,000, 70,000, 90,000, and 100,000rpm under the steady-state flow. The isentropic turbine efficiency was calculated. The efficiency was the range of 0.53 to 0.57. The BSR and expansion ratio were changed from 0.71 to 0.84 and from 1.24 to 1.72, respectively. The isentropic turbine efficiency decreased with increasing BSR and expansion ratio. The operation of only scroll A or B was compared with that of the twin-scroll turbine. The isentropic efficiency of using only scroll B was higher than those of only scroll A at 60,000rpm. The isentropic efficiency of using only scroll A was higher than those of only scroll B at 100,000rpm. Therefore, the twin-scroll turbine used in this study is operating effectively in the wide speed range.
