Grain Refinement를 통한 Mg-Ca-Zn합금의 기계적 특성 및 부식 특성 향상 (Improvement of Mechanical and Corrosion Properties of Mg-Ca-Zn Alloy by Grain Refinement)

  • 마그네슘은 다른 금속들에 비해 비강도가 높으며, 우수한 진동 흡수능, 전자파 차폐성 등으로 산업 전반에 많이 활용되고 있다. 예를 들면, 자동차 시트 프레임, 실린더 헤드 등 자동차 부품으로도 사용되며 노트북 케이스, 핸드폰 케이스 등의 전자제품에도 널리 사용되고 있다. 또한 신체의 뼈가 금이 가거나 부서졌을 경우, 그 손상된 뼈의 치료를 보조하기 위해 사용되는 골 접합용 임플란트 재료로 각광받고 있다. 현재 임플란트 재료로는 Ti alloy, Stainless steel, Co-Cr-Mo alloy등이 사용되어 지고, Mg 합금은 연구단계에 머물러 있다. 현재 골접합용 임플란트 재료의 문제점으로는 한번 접합 수술 후에 회복이 되면 이것을 제거하기 위한 수술을 또 해야 한다는 점이 있다. 하지만 마그네슘은 현재 재료와 비교하여 충분한 강도를 실현 할 수 있으며, 일정시간 후 자체적으로 분해되고 인체에 무해한 원소기 때문에 연구가 진행되고 있다. 그러나 순수 Mg으로는 충분한 강도와 내식성을 가질 수 없기 때문에 합금원소를 첨가하여 Mg 합금을 설계한다. 본 논문에서는 마그네슘에 인체에 무해한 Ca, Zn를 첨가하여 합금을 설계하였고, 추가적으로 강도와 내식성을 향상시키기 위해서 Grain refiner로서 Sr을 소량 첨가하여 최종 합금을 설계하였다. Sr은 방사선 원소는 인체에 유해하지만, 그 외 천연 Sr 원소는 인체에 무해하다. 이렇게 제조된 합금을 광학현미경, 주사전자현미경을 통해 미세조직분석을 수행하고, 인장시험, 동전위 분극 시험, 침지시험을 통해 기계적 특성 및 부식특성을 평가하였다. 결론적으로, Sr 첨가에 의해 조직이 미세해졌으며, 기계적 특성 및 내식성이 향성된 것을 확인 할 수 있었다.

얼굴의 측면 윤곽선 개선을 위한 미용 코성형술 (Aesthetic Rhinoplasty for the Improvement of the Lateral Facial Profile; Image-up Rhinoplasty)

  • There are several characteristics of the nose of orientals. The dorsum of nose is flat and low, the skin is thick with severe tension, the nasal tip is bulbous, the nostril is wide, and the projection of the nose is limited due to a poorly developed alar cartilage with a short columella. In order to improve these untoward characteristics of the nose of Orientals aesthetically, plain augmentation of the dorsum and tip-plasty with conventional methods has been performed by many plastic surgeons. However these conventional rhinoplasty is not enough to obtain satisfactory results when transforming into a more beautiful and aesthetically charming appearance. In order to produce the optimal nasal shape and profile, it is extremely important to consider the aesthetic surgical factors, which are; the position of the nasion, the optimal nasolabial angle(95-100 degree in Orientals), the natural exposure of infra-tip lobule with and columella, the position of the tip defining point in harmony with the dorsal profile and the smooth and natural silhouette of the lateral nasal profile as it descends into the inferior portion of the nose. From April, 2003 to August, 2004, a total of 52 patients underwent open rhinoplasty, adhering to the strict aesthetic principles considered and described priorly. Surgical approach was done through a transcolumella incision and an alar rim incision. The nasal dorsum was augmented with a silicone implant and the shape of the columella and the nasolabial angle were finessed with a silicone strut implant which was placed in between the medial crurae in a manner of a non-visible graft. The nasal tip was corrected by alar cartilage suture technique and onlay graft of shield shaped Alloderm and Gore-Tex. Author obtained the optimal nasal shape and profile aesthetically, and the results, considered satisfactory in all patients without any complications, are as follows; 1. the average increase in nasal length was 2.5 mm, 2. the average decrease in nasal width was 2.1 mm, 3. the average increase in nasal tip projection was 3.2 mm, 4. the changes of nasolabial angles were from 85.5 degree to 94.7 degree, 5. the changes of the angle between the long axis of the external naris was from 101.3 degree to 89.5 degree. In conclusion, this surgical procedure is an effective, reliable and a valuable method in improving the nasal shape, tip projection, nasolabial angle and especially, the lateral facial profile of Orientals aesthetically.

이온 주입법을 이용한 ZnO 박막의 As 도핑 (Arsenic Doping of ZnO Thin Films by Ion Implantation)

  • ZnO with wurtzite structure has a wide band gap of 3.37 eV. Because ZnO has a direct band gap and a large exciton binding energy, it has higher optical efficiency and thermal stability than the GaN material of blue light emitting devices. To fabricate ZnO devices with optical and thermal advantages, n-type and p-type doping are needed. Many research groups have devoted themselves to fabricating stable p-type ZnO. In this study, $As^+$ ion was implanted using an ion implanter to fabricate p-type ZnO. After the ion implant, rapid thermal annealing (RTA) was conducted to activate the arsenic dopants. First, the structural and optical properties of the ZnO thin films were investigated for as-grown, as-implanted, and annealed ZnO using FE-SEM, XRD, and PL, respectively. Then, the structural, optical, and electrical properties of the ZnO thin films, depending on the As ion dose variation and the RTA temperatures, were analyzed using the same methods. In our experiment, p-type ZnO thin films with a hole concentration of $1.263{\times}10^{18}cm^{-3}$ were obtained when the dose of $5{\times}10^{14}$ As $ions/cm^2$ was implanted and the RTA was conducted at $850^{\circ}C$ for 1 min.

Scar Wars: Preferences in Breast Surgery

  • Background The uptake of breast reconstruction is ever increasing with procedures ranging from implant-based reconstructions to complex free tissue transfer. Little emphasis is placed on scarring when counseling patients yet they remain a significant source of morbidity and litigation. The aim of this study was to examine the scarring preferences of men and women in breast oncoplastic and reconstructive surgery. Methods Five hundred men and women were asked to fill out a four-page questionnaire in two large Irish centres. They were asked about their opinions on scarring post breast surgery and were also asked to rank the common scarring patterns in wide local excisions, oncoplastic procedures, breast reconstructions as well as donor sites. Results Fifty-eight percent of those surveyed did not feel scars were important post breast cancer surgery. 61% said that their partners' opinion of scars were important. The most preferred wide local excision scar was the lower lateral quadrant scar whilst the scars from the deep inferior epigastric artery perforator (DIEP) flap were most favoured. The superior gluteal artery perforator flap had the most preferred donor site while surprisingly, the DIEP had the least favourite donor site. Conclusions Scars are often overlooked when planning breast surgery yet the extent and position of the scar needs to be outlined to patients and it should play an important role in selecting a breast reconstruction option. This study highlights the need for further evaluation of patients' opinions regarding scar patterns.

  • In this study, three treatment options to replace two posterior missing teeth were investigated using three dimensional finite element analysis: two wide(${\phi}5.0mm$) implants(the experimental model I), two standard(${\phi}3.75mm$) implants(the experimental model II), and three standard(${\phi}3.75mm$) implants(the experimental model III). Two kinds of load case were applied ; 1) perpendicular on occlusal surface(axial load), parallel on occlusal surface(lateral load). 2) perpendicular on occlusal surface(3mm lateral to central point). The results obtained from this study were as follows; value of Von-mises stress (equivalent stress) was smallest in the two wide implant among the three experimental models. It was reported that the diameter is the efficient factor than osseointegrated surface area.

변형된 Y형 해부학적 금속판을 이용한 상완골 원위부 골절의 치료 (Treatment for Distal Humeral Fractures by Modified Anatomical Y-Plate)

  • Comminuted fracture of the distal end of the humerus in adults is very rare and difficult to treat. In operative treatment, an implant which can achieve accurate anatoraical reduction and rigid fixation is needed. But the pre­existing Y -shaped plate had wide and thick limbs and some problems in fixation for most distal humeral fractures. So we devised a modified plate which is more narrow and th1n and has a different hole distance and wing length (long lateral wing). The aim of the study was to evaluate the clinical result of this modified anatomical Y -plate. From 1991 to 1997, we treated 23 cases of distal humeral fractures using modified anatomical plate and the results were as follows. 1. Fracture type in 18 patients(78%) was C-type(C1,C2,C3) which were intraarticular and mostly displaced or comminuted. 2. Bony union was obtained in 22 patients(96%) through rigid fixation and observed radiologically at 3.5 months(2-6months) on the average. 3. In 23 patients, 19 patients(82%) showed satisfactory results after 34 months(6-73months) follow-up. So if the modified anatomical Y-plate is used in the treatment of distal humeral fracture, a satisfactory result can be obtained through a more accurate, easy, and rigid fixation than preexisting plate.

RESONANCE FREQUENCY ANALYSIS OF IMPLANTS WITH ANODIZED SURFACE OXIDES

  • The present experimental study was designed to address two issues. The first was to investigate whether oxidation voltage of titanium implants influenced bone tissue responses after an in vivo implantation. The second aim was to investigate secondary stability change after 1 to 3months period. Screw-shaped implants with a wide range of oxide properties were prepared by electrochemical oxidation methods, where the oxide thickness varied in the range of $3-15{\mu}m$. The micro structure revealed pore sizes of $1-3{\mu}m$, the crystal structures of the titanium oxide were amorphous, anatase and a mixture of anatase and rutile type. Bone tissue responses were evaluated by resonance frequency measurements that were undertaken 1 to 3months after insertion in the rabbit tibia. It was concluded that no statistical difference of RFA values was found between the groups, RFA gains after Imonth and 3months were calculated.

Root coverage with a modified laterally positioned flap combined with a subepithelial connective tissue graft in advanced recession

  • Purpose: A laterally positioned flap (LPF) combined with a subepithelial connective tissue graft (SCTG) is one of the conventional approaches for resolving gingival recession defects, with the advantages of flap flexibility and extended coverage of the tissue graft. However, thus far, evidence is lacking for the use of this technique for the treatment of advanced gingival recession defects. This report discusses three Miller class III cases with interproximal bone loss and wide and deep defects treated with a combination procedure of a modified laterally positioned flap (mLPF) and SCTG. Methods: mLPF combined with SCTG was performed for each case. The defect size and the degree of hypersensitivity at baseline and the final appointment in each case were documented. Results: The three cases had a mean initial defect of $7.7{\pm}1.5mm$ and a mean residual defect of $1.7{\pm}1mm$ at the 6-, 3-, and 36-month follow-up, respectively, after the root coverage surgery. The symptom of hypersensitivity was improved, and the patients were satisfied with the clinical outcomes. Conclusions: The results demonstrated that the combination of mLPF with SCTG is promising for treating these advanced cases with respect to obtaining the expected root coverage with the gingival tissue.

심한 치과공포증 환자에서 임플란트 식립을 위한 Propofol과 Remifentanil 진정법 -증례 보고- (Sedation for Implant Surgery using Propofol and Remifentanil in Severe Dental Phobia Patient -A Case Report-)

  • Anxiety and fear is two main factors that keep patients from going to dental clinic. Especially, patients may feel implants operations are more traumatic. Intravenous conscious sedation for dental treatment can make patient comfortable and relaxable. Midazolam is more popular for sedation for dental treatment, but target-controlled infusion (TCI) of propofol and remifentanil is gaining wide popularity. A 54-year-old female patient who had severe dental phobia was referred to our dental hospital. She had past history of 2 times of hyperventilation and syncope during dental treatment. The patient showed a lot of dental anxiety and fear to dental treatments and stress reduction protocol was needed. We administered intravenous conscious sedation using target controlled infusion system with remifentanil and propofol. During sedation, we monitored the status of consciousness with bispectral index and vital signs. Dental treatment could be finished successfully without any problems.

Medpor Craniotomy Gap Wedge Designed to Fill Small Bone Defects along Cranial Bone Flap

  • Objective : Medpor porous polyethylene was used to reconstruct small bone defects (gaps and burr holes) along a craniotomy bone flap. The feasibility and cosmetic results were evaluated. Methods : Medpor Craniotomy Gap Wedges, V and T, were designed. The V implant is a 10 cm-long wedge strip, the cross section of which is an isosceles triangle with a 4 mm-long base, making it suitable for gaps less than 4 mm after trimming. Meanwhile, the Medpor T wedge includes a 10 mm-wide thin plate on the top surface of the Medpor V Wedge, making it suitable for gaps wider than 4 mm and burr holes. Sixty-eight pterional craniotomies and 39 superciliary approaches were performed using the implants, and the operative results were evaluated with respect to the cosmetic results and pain or tenderness related to the cranial flap. Results : The small bone defects were eliminated with less than 10 minutes additional operative time. In a physical examination, there were no considerable cosmetic problems regarding to the cranial bone defects, such as a linear depression or dimple in the forehead, anterior temporal hollow, preauricular depression, and parietal burr hole defect. Plus, no patient suffered from any infectious complications. Conclusion : The Medpor Craniotomy Gap Wedge is technically easy to work with for reconstructing small bone defects, such as the bone gaps and burr holes created by a craniotomy, and produces excellent cosmetic results.
