Comparative study on patterns and symptoms of disharmony(病機病症) between the internal organs(藏府) and external bodily form(身形) (장부(藏府)와 신형(身形)의 병기(病機) 및 병증(病症)에 대한 비교고찰(比較考察))

  • All living things including human being consist of soul(spirit) and body. Soul is the root of a life and body frames it. I wrote this paper to tell how internal injury due to endogenous etiological factors and affection due to exogenous pathogenic factors, affect the internal organs and the external bodily from. This paper begins with description of the patterns of disharmony of the internal organs. General disorders of each Five-Jang(五藏) an be classified into two types of soul and body. The Liver and the Heart which lead changing to Yang(陽) of vital energy, have close relationship with spiritual symptoms because spirit is related to Yang. The Lung, Kidneys, the Spleen which lead changing to m(陰) of vital energy, mainly connected with physical disorders because body is relatively close to m. The Five-Jang are ruled by the Five-Phases(五行) system and cause troubles with Oche(五體) and the nine body orifice. Otherwise the main function of the Six-Bu(六府) is to receive food, absorb the usable portions, and transmit and excrete waste. Therefore they can cause such problems as abdominal pain, distention, difficulty in urination, and constipation. The spleen is responsible for sending Grain-Ki(穀氣) so that is closely connected with the six-Bu. The Gall bladder takes care of control of giving out spirit. That's why it presents many symptoms related to the spirit that is ruled by the Five-Jang. Patterns of disharmony of external bodily form is influenced by the state of Meridians. Bodily forms get divided into many parts by the function of six-meridians(六經) to which they belong. Six-meridians have their own function related to excretion, related to excretion, retention, and balance(開闔樞). If local bodily forms get affected by pernicious influences, the Meridians to which they are attached will lose harmony and cannot fulfil their own functions. Because the meridian system unifies all parts of the body, the whole body will be under the influence of the affection although local parts of the bodily forms get affected. Therefore the symptoms of disharmony should be considered in a synthetic view. There are however, also cases which should be focused on the very meridian channels affected. Disorder within a Meridian generates derangement in the pathway make it not to benefit the muscles and skin belong to it. Because the meridians connect the interior organs with the exterior bodily form, they are influenced by each other.

Management of the Intractable Huge Intracranial Osteoma Based on the 3D Printing Model

  • Osteoma is one of the benign tumor that occurs on the bones all over the body. Mostly the simple excision is known to be enough. However, sometimes we encounter the troublesome situation where the osteoma is located in very challenging area, which results in the recurrence. 26 year female presented with the intractable intracranial osteoma. Given the disease entity of the osteoma, the simple excision would be enough or conservative management. But this osteoma turned out to be huge and recurrent in spite of the endoscopic resections, which causes the facial disappearance accompanied by the orbital vertical dystopia. Moreover, the patient's main concern was the pain. We performed the intracranial resection of the whole lesion and reconstructed the skull base and frontal bone as well as the part of the orbital wall. In order to restore the original bony anatomy, the 3D printing model was used based on the titanium mesh. I report this unusual case of the intractable intracranial huge osteoma. This report may be helpful for the other surgeons to make a decision on their similar cases in the future.

Palliative Sedation for Terminal Cancer Patient (말기 암 환자에서 완화적 진정 증례 토론)

  • Palliative sedation has been used in patients who undergo intractable suffering at the end of life. Its implementation, however, may be complicated due to resistance of clinicians and barrier of bioethical issues. Here, we present 50-year-old man with stomach cancer and multiple bone metastasis who was treated with palliative chemotherapy and radiotherapy. He suffered from refractory pain on the whole body even after standard analgesics and multidisplinary effort to relieve. Upon shared decision for sedation, he was given midazolam until discharge. Literature reviews reveal cases similar to the present case.

  • The condition contrary to the physiological conditions obtained by the normal functioning of seven constituents of the body is defined as disease. The primary causes of the disease are emotional defilements of desire, hatred and delusion. The secondary causes of diseases are seasonal climatic changes, the agency of harmful demons, and improper diet or conduct. Once a disease occurs, it brings the disfunctioning of the three humors of wind, bile, and phlegm. Therefore all the disease should be examined and determined which of theses three humors effect its nature the most. There are five main techniques of external therapy, namely bloodletting, which extract the impure blood produced by disease of heat; moxibustion, which debilitates cold diseases, medicinal compresses, fomentation, massage with ointment, and minor surgery. Among these, the former three are the mild treatment and the latter three the severe ones because of the pain the patient has to suffer during the whole procedure.

  • Objective : The aims of this study were to show the clinical state and to investigate depression and anxiety by BDI and STAI scale in tension headache patients. Method : The patient group was consisted of the 20 patients with Tension Headache who were treated in Department of Neuropsychiatry, Daejeon University Oriental Hospital from 3 March 2003 to 30 September 2003. The control group was consisted of the 20 patients on physical therapy with cerebaral vascular disorder in the Daejeon University Cheonan Oriental Hospital. The patient group was investigated by various characteristics, and compared with the control group by BDI and STAI scores. Result : 1. The ratio of female was higher, the 40 aged were higher frequence, in distribution of the period of the clinical history, short term within 1 week was the most.. 2. Tension headache was most frequent at whole portion, neck stiffness, shoulder pain, general body weakness were mainly coexited, and the prescription invigorating spleen supplementing qi and tonifying qi and blood. 3. The BDI score was higher in patient group, but there was no significant difference 4. The STAI score was higher in patient group, but there was no significant difference.

  • Postural balance of temporomandibular joint (TMJ) reflects the neuromuscular balance. TMJ posture treatment for meridian yinyang balance has been asserted to have an impact upon cervical spine alignment with an episodic good clinical result for refractory torticollis cases. A retrospective study with one-group pretest-posttest design was conducted to review cervical spine imaging data and clinical symptom changes in cervical dystonia cases to explore the effect of TMJ balance treatment. Review of clinical symptoms and cervical spine imaging data including computed tomography and simple x-ray of 9 cervical dystonia cases revealed significant improvement of cervical spine alignment and clinical symptoms. Improvement of cervical spine alignment may be an underlying mechanism of TMJ balance treatment, as a way of balancing the whole-body meridian system.

  • The following results were obtained. 1. The Urticaria is similar to the korean medical name of yeun-jin, yum-leu, pung-jin-geu(風疹槐), Pung-sa, Pung-so-yeunjin. 2. The most part of the causes that induce Urticaria are like that, the sup-yeul(濕熱) that caused by the sa-gi(邪氣)'s invasion, food and behavior's fault, is gathering in the gastrointestinal tract. 3. The symptomes are like that, the severe pruritic blisters are spread on the whole body, and the color of the blisters is red or white, and nausea, vomitting, abdominal pain, chest discomfort are induced with the Urticaria. 4. The Su-Chok yangmyong Kyong(手 足陽明經), Chok taeum Kyong(足太陰經) and Chok taeyang Kyong(足太陽經) are used for the acupuncture therapy of Urticaria. 5. The acupuncture points of the Kokchi(曲池), Hyolhae(血海), Chok-Samni(足三里), Samumgyo(三陰交), Hapkok(合谷) are used for the acupuncture therapy of Urticaria. 6. the Pye-area(肺區), Shinmun(神門), dammajin-area(蕁麻疹區), Shinsangsun(腎上腺), Chimbu(枕部) are used for the acupuncture therapy of Urticaria in the ear-acupuncture therapy of Urticaria.

  • Purpose : This study was designed to investigate the effect of herbal medication prescribed for patients with febrile diseases in the pregnancy. Method : We prescribed Kungsosankakambang for 18 patients who had got cold, at Wonkwang University Gwangju Oriental Hospital from January 2001 to September 2002. They had the symptoms of cough, sputum, snivel, headache, whole body pain, etc. Results : The most of patients were improved. This herbal medicine did 9 patients good right after dosage. And 8 patients have improved much in health.

  • Liquid biopsy, a minimally invasive procedure that causes minimal pain and complication risks to patients, has been extensively studied for cancer diagnosis and treatment. Moreover, it facilitates comprehensive quantification and serial assessment of the whole-body tumor burden. Several biosources obtained through liquid biopsy have been studied as important biomarkers for establishing early diagnosis, monitoring minimal residual disease, and predicting the prognosis and response to treatment in patients with cancer. Although the clinical application of liquid biopsy in gastric cancer is not as robust as that in other cancers, biomarker studies using liquid biopsy are being actively conducted in patients with gastric cancer. Herein, we aimed to review the role of various biosources that can be obtained from patients with gastric cancer through liquid biopsies, such as blood, saliva, gastric juice, urine, stool, peritoneal lavage fluid, and ascites, by dividing them into cellular and acellular components. In addition, we reviewed previous studies on the diagnostic, prognostic, and predictive biomarkers for gastric cancer using liquid biopsy and discussed the limitations of liquid biopsy and the challenges to overcome these limitations in patients with gastric cancer.
