In recent years, clinicians' and dentists' esthetic demands in dentistry have increased rapidly. The ultimate goal in modern restorative dentistry is to achieve "white" and "pink" esthetics in the esthetically important zones. Therefore, modern esthetic dentistry involves not only the restoration of lost teeth and their associated hard tissues, but increasingly the management and reconstruction of the encasing gingiva with adequate surgical techniques. Interdental space are filled by interdental papilla in the healthy gingiva, preventing plaque deposition and protecting periodontal tissue from infection. This also inhibits impaction of food remnants and whistling through the teeth during speech. These functional aspects are obviously important, but esthetic aspects are important as well. Complete and predictable restoration of lost interdental papillae remains one of the biggest challenges in periodontal reconstructive surgery. One of the most challenging and least predictable problems is the reconstruction of the lost interdental papilla. The interdental papilla, as a structure with minor blood supply, was left more or less untouched by clinicians. Most of the reconstructive techniques to rebuild lost interdental papillae focus on the maxillary anterior region, where esthetic defects appear interproximally as "black triangle". Causes for interdental tissue loss are, for example, commom periodontal diseases, tooth extraction, excessive surgical periodontal treatment, and localized progressive gingiva and periodontal diseases. If an interdental papilla is absent because of a diastema, orthodontic closure is the treatment of choice. "Creeping" papilla formation has been described by closing the interdental space and creating a contact area. In certain cases this formation can also be achieved with appropriate restorative techniques and alteration of the mesial contours of the adjacent teeth. The presence of an interdental papilla depends on the distance between the crest of bone and the interproximal contact point, allowing it to fill interdental spaces with soft tissue by altering the mesial contours of the adjacent teeth and positioning the contact point more apically. The interdental tissue can also be conditioned with the use of provisional crowns prior to the definitive restoration. If all other procedures are contraindicated or fail, prosthetic solutions have to be considered as the last possibility to rebuild lost interdental papillae. Interdental spaces can be filled using pink-colored resin or porcelain, and the use of a removable gingival mask might be the last opportunity to hide severe tissue defects.
Human teeth vary widely in color. Practitioner and patients are concerned with preventing and correcting discolored or dark teeth to achieve and maintain stain-free, white teeth. Tooth brushing cannot alter tooth color but it can remove adhering films and stains. The esthetics of natural dentition can be improved by bleaching and this process can be applied to intrinsically and extrinsically stained teeth. The need for a brighter, more attractive smile has made rapid growth in the market for tooth whiteners. There is no doubt these products work as whiteners, at least on mild to moderate stains, but the safety of these products are unclear. In this experiment, the effect of tooth whitener application on the color and microhardness of extracted human enamel was measured. RMS, RMT and NWT were used as tooth whiteners, and tooth paste(ETQ) and hydrogen peroxide solution(HPO) were used as controls. 35 caries-free extracted human molars were embedded and polished with the exposed enamel diameter of 4 mm. The tooth whiteners and control agents were applied according to the manufacturers' instructions or clinically simulated procedures for eight weeks, and measurements were repeated every two weeks. Value(L*) difference was measured using Differential Colorimeter(Model TC-6FX, Denshoku Co., Japan), and microhardness was measured using microhardness tester(Mitsuzawa Seiki Co., Japan). The results were as follows; 1. After application of agents for eight weeks, the Vickers hardness increased significantly in the ETQ, RMS and RMT application group(p〈0.01), and that decreased significantly in NWT application group(p〈0.01), but in HPO application group there was no significant change. The change in microhardness was greatest in NWT application group(p〈0.01). 2. After application of tooth whiteners and controls for eight weeks, the value change of toothpaste application group was significantly lower than those of other agents groups(p〈0.01), and there was no significant difference in value(L*) change among tooth whitener groups(p〉0.01). 3. The application of tooth paste and paste type tooth whitener made gradual value change, but hydrogen peroxide gel type tooth whitener and hydrogen peroxide solution made rapid value change during initial application period.

Objectives: This study evaluates the bond strength and marginal adaptation of mineral trioxide aggregate (MTA) Repair HP and Biodentine used as apical plugs; MTA was used as reference material for comparison. Materials and Methods: A total of 30 single-rooted teeth with standardized, artificially created open apices were randomly divided into 3 groups (n = 10 per group), according to the material used to form 6-mm-thick apical plugs: group 1 (MTA Repair HP); group 2 (Biodentine); and group 3 (white MTA). Subsequently, the specimens were transversely sectioned to obtain 2 (cervical and apical) 2.5-mm-thick slices per root. Epoxy resin replicas were observed under a scanning electron microscope to measure the gap size at the material/dentin interface (the largest and smaller gaps were recorded for each replica). The bond strength of the investigated materials to dentin was determined using the push-out test. The variable bond strengths and gap sizes were evaluated independently at the apical and cervical root dentin slices. Data were analyzed using descriptive and analytic statistics. Results: The comparison between the groups regarding the variables' bond strengths and gap sizes showed no statistical difference (p > 0.05) except for a single difference in the smallest gap at the cervical root dentin slice, which was higher in group 3 than in group 1 (p < 0.05). Conclusions: The bond strength and marginal adaptation to root canal walls of MTA HP and Biodentine cement were comparable to white MTA.

Objective: Esthetic improvements during orthodontic treatment are achieved by changes in positions of the lips and surrounding soft tissues. Facial soft-tissue movement has already been two-dimensionally evaluated by cephalometry. In this study, we aimed to three-dimensionally assess positional changes of the adult upper lip according to simulated maxillary anterior tooth movements by white light scanning. Methods: We measured changes in three-dimensional coordinates of labial landmarks in relation to maxillary incisor movements of normal adults simulated with films of varying thickness by using a white light scanner. Results: With increasing protraction, the upper lip moved forward and significantly upward. Labial movement was limited by the surrounding soft tissues. The extent of movement above the vermilion border was slightly less than half that of the teeth, showing strong correlation. Most changes were concentrated in the depression above the upper vermilion border. Labial movement toward the nose was reduced significantly. Conclusions: After adequately controlling several variables and using white light scanning with high reproducibility and accuracy, the coefficient of determination showed moderate values (0.40-0.77) and significant changes could be determined. This method would be useful to predict soft-tissue positional changes according to tooth movements.

The author studied the age distribution, etiology, affected site and several radiographic features of periapical granulomas, cysts, and abscesses. The material consisted of 928 films obtained from the patients who were diagnosed and treated under the diagnosis of periapical granulomas, cysts, and abscesses during the past 8 years (1979-1986) at the Infirmary of Dental School, Chosun University. The obtained results were as follows: 1. The order of incidence was as follows: periapical abscess (67.2%), granuloma, and cyst. 2. The age distribution revealed the highest incidence around the age of 30 and relatively higher incidence over the age of 60 in the case of periapical abscess and granuloma. 3. In the frequency of location: Periapical abscesses occured most frequently in the mandibular molars. Granulomas showed relatively higher incidence in maxilla than in mandible. Cysts were most common in the maxillary anterior teeth. 4. The mean diameter of dental granuloma was 5.9㎜, however, all dental granulomas were less than 9.3㎜ in diameter. The mean diameter of periapical cyst was l3.8㎜. 5. Periapical cyst revealed well circumscribed radiolucent lesions and 77.8% of the lesion showed white line. 86.0% of dental granuloma showed well circumscribed border, 54.5% sclerosis on surrounding bone and 38.5% partial white line. Periapical abscess revealed diffuse radiolucent lesion, 89.6% of the lesions had sclerosis on surrounding bone, and 38.0% sinus tract.

The purpose of this study was to evaluate the difference of remineralization effects of various anti-cariogenic toothpastes on artificial carious lesions in primary and permanent teeth using quantitative light-induced fluorescence-digital (QLF-D) system. Sound human primary (n = 48) and permanent teeth (n = 48) were randomly divided into following groups : control group (Group 1), fluoride toothpaste (Group 2), functionalized tricalcium phosphate (fTCP) + fluoride toothpaste (Group 3), and casein phosphopeptide-amorphous calcium phosphate (CPP-ACP) toothpaste (Group 4). Specimens were prepared by exposure in a demineralizing solution and then treated using the different toothpastes twice daily during 14 days. All specimens were analyzed with the QLF-D system. QLF data analysis indicated three different toothpastes showed significant remineralizing effects compared to Group 1 in both primary and permanent teeth. Also, the remineralizing effects in Group 3 and 4 were significantly higher than in Group 2. This study suggested that the toothpastes containing fTCP + fluoride and CPP-ACP have the significant anti-cariogenic effects on enamel demineralization in both primary and permanent teeth, and QLF-D is an useful device to assess the incipient carious lesion and remineralization effects of the anti-cariogenic materials quantitatively. Therefore, clinicians can consider the QLF-D system for the evaluation of demineralization and remineralization in primary and permanent teeth.
The purpose of this study was to determine pH value of some alcoholic drinks sold in Korea and to provide the basic information which can cause dental erosion. Alcoholic drinks commercially sold were purchased from various big markets in Korea. The sorts of drinks tested in this study consisted of 5 brands of beers, 24 brands of makgeollis, 9 brands of wines and 12 brands of sojus. The test groups were selected randomly and the pH of each beverage was determined using a pH meter he each pH was measured. For statistical data, Mann-Whitney test was used to analyze difference for red wine and white wine and Kruskal-Wallis test was used to compare the pH of each test group. The result of this study was as followings: the mean pH of 5 brands of beers was 4.21, that of 24 brands of makgeollis 3.88, of 9 brands of wines 3.34 and of 12 brans of sojus 7.86. Each test group was significantly different (p<0.05). Except for soju groups, the test groups had a low pH value which can cause dental erosion. In terms of comparing between pH value of red wine and white wine, the result of this study represented that the mean pH of red wine was 3.45 and that of white wine was 3.21. This result showed the pH of two kinds of wine had a low pH which can lead to dental erosion and the difference of two wine were significantly different (p<0.05). As a result, some drinks sold in Korea have a high erosive potential on teeth since they have a comparatively low pH expect soju. Hence, when we consume some kinds of alcoholic drinks, we make sure to remember that the alcoholics which had a low pH, can have an effect on dental erosion that mean we should avoid to drink some alcoholic drinks with low pH for long time.

Purpose: The purpose of this study was to compare and evaluate the masking ability of three types of high translucent zirconia according to the various thicknesses and backgrounds. Materials and Methods: Using three types of high-translucency zirconia (Ceramill zolid fx white, Ceramill zolid ht+ white, Ceramill zolid ht+ preshade A2), 10 cylindrical specimens were fabricated in 10mm diameter and each with four thicknesses (0.6 mm, 1.0 mm, 1.5 mm, 2.0 mm), respectively by CAD/CAM method. The background was 10 mm in diameter and 10 mm in thickness. A1, A2, A3 flowable resin backgrounds, blue-colored core resin background, and Ni-Cr alloy background were prepared, and black, white backgrounds provided by the spectrophotometer manufacturer (x-rite, Koblach, Austria) were used. zirconia specimens and the background specimen were stacked to measure L, a*, b* with Spectrophotometer (Color i5, x-rite, Koblach, Austria) and the ΔE value with the other background is calculated. The Calculated mean ΔE values were compared based on perceptibility threshold 1.0 and acceptability threshold 3.7. Nonparametric tests such as Kruskal-Wallis test were performed to verify statistical significance (α = 0.05). Results: There was a significant difference in the mean ΔE value according to the zirconia type, background and thickness change (P = 0.000). Conclusion: According to the results of this study, the pre-colored high-translucent zirconia can obtain the desired zirconia shade when it is restored on teeth, composite resins, and abutments except for the blue resin core.
Freshly extracted permant teeth were examined by electron microscope according to the Filmy replica method. Preparatory method with hydrochloric acid or proteolytic enzyme treatment was used in order to injure the structural composition of enamel surface to compare between the natural carious defects and the etched samples used in this study. The natural carious defects were chalky white or brown discolored portions at the smooth enamel surface without having visible loss of enamel substances. The findings were as follows: 1. The first caries attact initiated from the mineralized portion of rod sheath which is located at the top of enamel rod. 2. The caries developed downward along the rod sheath interprismatic substances and enamel rods subsequently. However, the caries involvement of interprismatic substances and enamel rods occured almost at the same time. 3. The rod sheath attached to the remineralized smooth emael surface was located at the top of enamel rod.
The authors have studied the marginal leakage of various dental cements; Fynal cement (Caulk), Zinc cement (S.S. White) Carbo cement (Shofu, Poly-carboxylate cement), EBA cement (Opotow alumina cement), Propac (G-C's zinc oxide euginol cement), and Super cement (Shofu oxyphosphate zinc cement), by means of penetration of 2% methylene blue solution between orden crown restorations and human teeth in 150 cases. The result revealed as follow
1. None of the cements tested were free from dye penetration.
2. There was a wide difference in dye penetration among the trade brands of dental cement when immersed in dye solution before setting of cements.
3. EBA cement and Carbo cement showed comparatively low grade of dye penetration at the restorative margins.
