The Effects of a Context-based Drawing Task on the Language Expression of Severe Wernicke's and Broca's Aphasic Patients (문맥적 상황중심의 그림 그리기 과업이 중증의 베르니케 실어증과 브로카 실어증에 미치는 영향)

  • This study attempted to determine the effects of a context-based drawing task on the language expression of a severe Wernicke's and Broca's aphasic. The subjects in this study showed a poor auditory comprehension and naming performance. They also showed paraphasia and perseveration. This study focused on improving language expression by a drawing task based on conversation at hand. Ten target words were chosen which were easily drawnable and familiar to the subjects. The results showed that the context-based drawing task was effective on improving the subjects' confrontation naming ability and expressive language ability in terms of explanation of sentences. In addition, the Broca's aphasic showed improved naming ability when the contextual cues were given and he was supposed to spontaneously name words.

  • Miller Fisher syndrome (MFS) is characterized by ataxia, areflexia, and ophthalmoparesis. Here we present a case of MFS mimicking Wernicke encephalopathy (WE) during pregnancy. A 31-year-old woman at 8 weeks of gestation presented with diplopia and ataxia after experiencing nausea and vomiting for several weeks. We initiated thiamine based on a suspicion of WE, which produced no clear effects. However, her symptoms began to improve following intravenous immunoglobulin treatment, and other findings finally lead to a diagnosis of MFS. Because ataxia and ophthalmoparesis can be misdiagnosed as WE during pregnancy, clinicians should consider MFS in the differential diagnosis.

While thiamine deficiency and repletion are often considered prior to dextrose infusions in patients with chronic alcohol abuse to prevent WE, they are often overlooked in non-alcoholic patients who are also at risk for malnutrition. In this paper we describe a case of a non-alcoholic 28-year-old female status post-sleeve gastrectomy who developed SNHL in the setting of thiamine deficiency and WE, with ongoing hearing impairment requiring hearing aids despite thiamine repletion.

A Case Report of Wernicke Aphasia by Korean Medicine Treatment, Including Scalp Acupuncture (두침요법을 포함한 한의 복합치료로 호전된 감각성 실어증 치험 1례)

  • The purpose of this case study is to report the effectiveness of scalp acupuncture and Cheungsindodam-tang on Wernicke's aphasia in patients with left middle cerebral territory infarction. Scalp acupuncture, herbal medicine (Cheungsindodam-tang), and moxibustion were administered for 66 days to improve the patient's symptoms. The degree of improvement in symptoms was evaluated using the Korean version of the Western Aphasia Battery (K-WAB) and Mini-Mental State Examination-Korea (MMSE-K). Quality of life was evaluated with the Korean version of the Stroke and Aphasia Quality of Life Scale-39 (K-SAQOL-39). After 66 days of treatment, the aphasia quotient score of the K-WAB test increased from 19 to 34.6 after treatment. The K-SAQOL-3 score increased from 2.77 to 3.18, and the MMSE-K score increased from 4 to 21. This case report suggests that Korean medical therapy can be effective in improving the language functions of patients with Wernicke's aphasia.

  • Leigh syndrome or subacute necrotizing encephalomyelopathy is a rare, rapidly progressive neurodegenerative disorder. In general, symptoms such as shortness of breath and decreased cardiac function usually occur within 1 year of life. It is a serious disease with a mortality rate of 75% in 2-3 years. The cause of Leigh syndrome is DNA mutation. Approximately 75% of patients have nuclear DNA mutations while 25% have mitochondrial DNA mutations. Clinical symptoms vary depending on the affected brain area. Neuroimaging plays an important role in diagnosing patients with Leigh syndrome. Late-onset Leigh syndrome is rarer and progresses more slowly compared to the classic form. Here, we report a case of late-onset Leigh's syndrome mimicking Wernicke's encephalopathy.

  • Thiamine (vitamin $B_1$) is a water-soluble vitamin that is not endogenously synthesized in humans. It is absorbed by the small intestine, where it is activated. Its active form acts as a coenzyme in many energy pathways. We report a rare case of thiamine deficiency in a 3.5-year old boy with short bowel syndrome secondary to extensive bowel resection due to necrotizing enterocolitis during his neonatal age. The patient was parenteral nutrition-dependent since birth and had suffered from recurrent central catheter-related bloodstream infections. He developed confusion with disorientation and unsteady gait as well as profound strabismus due to bilateral paresis of the abductor muscle. Based on these and a very low thiamine level he was diagnosed and treated for Wernicke encephalopathy due to incomplete thiamine acquisition despite adequate administration. He fully recovered after thiamine administration. After 1999 eight more cases have been reported in the PubMed mostly of iatrogenic origin.

  • To evaluate the usefulness if functional MR imaging(MRI) for the determination of language dominance system and to assess differences in the visual and auditory instrument language generation task according to activation task or activated area. Functional maps of the language area were obtained during visual and auditory instructions in word generation tasks in 6 healthy volunteer with right-handness were examined on a 1.5T scanner and the EPI BOLD technique, and three pulse sequence technique get of the true axial planes. Both task consisted of 96 phases including 6 activations and rests contents. Postprocessing were done on MRDx program by using cross correlation method. Two task compare the blain activation area surveyed of 1anguage lateralization index. To evaluated of the detection rates of Broca. Wernicke, pre-frontal lobe, Supplementary Motor Area (SMA) and pre-motor cortex areas and the differences of language lateraliaztion among two word generation task To lateralization index survey in 1anguage area on right and left in brain get to activation area pixel in brain. Compared to visual and auditory instrument task in the language areas get to the lateralization index. Two language generation task high detection rates of Broca and Wernicke areas. The visual instruction no detected in the auditory area, and auditory instruction no detected in the visual area. There was statistics significant different of them among language generation task. 1'his indicated that language area obtained image of the brain functional MR imaging usefulness in the visual and auditory task instrument.

  • 목적 : 베르니케실어증환자의 치료에 있어서 전통적 기법은 청각적 이해력을 높이기 위해 청각변별력을 직접적으로 치료한다. 그에 반해 Helm-estabrooks 등(1991)에 의해 개발된 TWA(Treatment for Wernicke's Aphasia)는 읽고 이해하기(reading comprehension), 구두읽기(oral reading), 따라 말하기(repetition)등의 과정을 통해 재청각화(reauditorization)를 유발하여 청각이해력(auditory comprehension)을 향상시키는 언어치료 프로그램이다. 본 연구자들은 TWA가 베르니케실어증을 가진 한국인의 재청각화에도 효과적인지를 알아보고자 하였다.
