The Status of Managing Posttraumatic Stress in Life Managers for Elderly People Living Alone and Measures for its Improvement: Focusing on Employees in Seoul (독거노인생활관리사의 외상 후 스트레스 관리 실태와 개선 방안: 서울 지역 종사자를 중심으로)

  • This study aims to examine Life Managers for Elderly People Living Alone (LMEPLAs) in Seoul regarding their traumatic experience and the status of their posttraumatic stress disorder and also how they are coping with it in order to find out ways to improve it. As a study method, we investigated LMEPLAs in Seoul through a self-administered survey regarding whether they had faced any traumatic experience, types of their traumatic experience, diagnosis on posttraumatic stress, and the status of their coping with traumatic experience. According to the study results, 186 respondents (37.57%) have been found to indicate either partial or complete posttraumatic stress symptoms, but the status of their coping with it is very poor. The followings are the results of our discussion. First, it is needed to find out life managers suffering from posttraumatic stress disorder and build up a system to manage them consistently. Second, it is necessary to vitalize education about traumatic experience and posttraumatic stress management. Third, it is urgently needed to build up a system to support life managers who have faced any traumatic experience or been diagnosed to have posttraumatic stress disorder. Fourth, it is demanded to cultivate and arrange experts equipped with specialized knowledge and technique. Fifth, it is needed for them to build a network with medical institutes to receive a prompt diagnosis and specialized treatment.

Status and Issues on Disaster Preparation Programs in Public Health Center (보건소의 재난관련 대비-대응 사업의 실태와 과제)

  • Objectives: The purpose of this study was to review the disaster preparation and response programs and the status of disaster preparation in public health center. Methods: In depth interview was performed in September 2017 using 5 open questions to the persons who are in charge of disaster response services in 5 public health centers of different levels in Korea. The questions included general characteristics of public health center, disaster programs and future issues. The research hired a quality method. Results: In general, the persons in charge recognized the cooperative agency of local government in disaster management. There were no disaster preparation programs developed by the public health centers. Most of the preparation were passive activities such as emergency support, crisis management on communicable disease and quarantine, participation in biological disaster response training, and education etc. The persons in charge emphasized necessity of disaster preparation programs. Conclusions: Disaster preparation and responsiveness is an evolving issue in public health centers in Korea. Medical support system and communicable disease management system are being set up in the national level. A comprehensive system covering health management, nutritional support, mental health, environment management of shelter, and volunteers supports on public health center level needs to be developed along with a easy-to-follow manual.

Awareness of Dementia National Responsibility of Elders: Oral Health Items

  • Background: Dementia is a condition in which a person who has been living a normal life suffers from various cognitive impairments in memory, words, and judgment that considerably disrupt daily life. The oral care ability and subjective oral status of elderly individuals with dementia are lower than those of a healthy person. The oral health care of individuals admitted to nursing homes inevitably falls to nursing assistants and nursing care staff. This study aimed to investigate the need for oral health management items of and to provide basic direction for the future of the Dementia National Responsibility System. Methods: Elders aged 65 years and over were selected from a comprehensive welfare center. A total of 155 questionnaires were analyzed. The questionnaire consisted of 15 items about general status, 9 items about recognition of the Dementia National Responsibility System, 5 items of the subjective recognition of oral health, and 6 items of the correlation between oral health and dementia. Results: Among our subjects, 71.0% answered that they did not know about the Dementia National Responsibility System, 78.7% answered that they think they need the system, and 81.9% think that they should add dental health items to the Dementia National Responsibility System. The response to the need for dementia national responsibility, oral health items in the Dementia National Responsibility System, and oral specialists all showed scores of >4 points. The need for the Dementia National Responsibility System, oral health items, and specialists were found. Conclusion: It is necessary to include oral health care items in the Dementia National Responsibility System so that elderly individuals with dementia can receive the needed oral health care.

  • For systematic pet management, the pet registration system has established. It makes the owner of the pet is given greater responsibility through the pet registration system but it also provided with welfare. One of the advantages of the pet registration system is that prepare system what return to owner quickly and safety through information inquiry when the pets are lost. But there are some conflict with the law in the information inquiry, so it occurs interfere the system. In this paper, we propose using the Smart Contract to finding the missing animal effectively. Proposed method discloses only the partially information. Thus it will be eliminated conflict parts, and increases the information accessibility to increase the number of people who can inquiry information. In addition, keeping the RFID inquired feature for compatible with the existing system. The smart contract querying enables quickly and precision access to information. Lastly, compare the proposed method with existing method to see the improvement.

Possibility of Risk Accidents and Response Strategies for Participants in Agencies in Charge of Employment for the Elderly: Focusing on occupational risk accidents and COVID-19 risk accidents (노인일자리 전담기관 참여자의 위험사고 발생가능성과 대응 전략: 업무상 위험사고와 코로나-19 위험사고를 중심으로)

  • This study identified the level of awareness of dangerous accidents by empirically analyzing the possibility of hazardous accidents at work and the possibility of dangerous accidents due to COVID-19 targeting the participating seniors who are active in the Senior Club, an organization dedicated to job placement for the elderly. The purpose of this study is to provide basic data for preparing measures to improve risk accidents that may occur in organizations in charge of job placement for the elderly and to suggest countermeasures to prevent dangerous accidents. For this purpose, data were collected from the elderly participating in 16 senior clubs in Busan, and 425 of the collected 430 were ultimately used. As a result of this study, first, it was found that the elderly who participated in a job-dedicated institution for the elderly generally perceived the possibility of occupational risk accidents and risk factors due to COVID-19 to be low. Second, it was found that there was a statistically significant difference in the probability of occurrence of hazardous accidents at work according to demographic characteristics during the participation period, and the probability of occurrence of dangerous accidents with COVID-19 was found to have statistically significant differences in the classification and age of the participating elderly. Based on the results of this study, the project manager suggested creating a safety accident management record sheet, preparing a risk management manual, and establishing a risk management system reflecting the individual characteristics of the participants.

Studies on the Demands of Development Program of Silver Industrial Professionals in the Northern Gyeonggi-do Area (경기 북부 실버관리인력 양성 과정에 관한 수요도 조사)

  • The principal objective of this study was to assess the demands of the development program of silver health care professionals for elderly individuals residing in the Northern Gyeonggi-do area. To this end, a survey was conducted to investigate college students and employees related with elderly individuals in the area regarding their recognition, interest, and involvement in the labor training program. In the case of college students, the health education they had received was only 1 to 2 hours, from a school lecture(35%). The content of health education they desired was exercise(34%) and stress management skills(28%). The sources of health information they received included mass media(77%) and the internet(12%), and they trusted the information they received from health professionals(45%), and the mass media(34%). In the case of health professionals who were working at silver care facilities, the sources of health information to which they had access were mass media(51%), internet(14%), reliable health professionals(56%), mass media(22%), and books related to health (18%). The principal issues they reported as being relevant to the elderly were dementia(39%), hypertension(14%), arthritis (11%), and they reported that the most important personnel for elderly in the future would be care managers(44%), and care helpers(21%). 88% of subjects believed that there was a need for a silver welfare integrated information system. 43% of subjects used the internet, 77% of them required in-service training programs for the welfare of the elderly. Via this developmental program of silver health care professionals, a variety of new job opportunities can be provided in the future, and a program related to the silver service industry must be established as soon as possible.

A Study on Lives of the Young Elderly in Low Cost Small Housing Communities in America and Canada : Focused on Suburban Areas of Small Cities along Rural Counties (미국과 캐나다 연소노년층의 저비용 소형 주거단지 생활에 관한 연구 : 농촌지역 소도시 외곽을 중심으로)

  • This study has been performed to explore process of forming low cost small site housing communities and residential lives of the young elderly around small cities along rural counties of America and Canada, and suggest future implications for Korea. In this study, five low cost small housing zones such as cottage and mobile home parks in non-metropolitan counties of California and Ontario State such as Weed, Paradise, Sun City,, Wellington on the Lake, and Trenton were visited and the elderly residents and service experts were interviewed. The senior concentrating housing clusters were formed primarily from influx of both metropolitan and rural residents for the purpose of seeking warmful localities, traffic connections, and business purposes in theme focused production areas. On the other hand, residential lives in the zones are occasionally negatively influenced by unbalanced concentrations of elderly facilities such as nursing stations and funeral homes. For the future of Korean rural elderly policies, suggestions are made as follows: first, integrated forms of urban and rural township need to be settled as attracting places for early retiring people who seek low cost pastoral oriented but culturally activated environment. Second, a model town of mobile housing structures needs to be initiated as a measure of evaluating adaptation process of those movers. Third, a cooperation system among governmental ministries needs to be formed in order to integrate a long term master plan of establishing traditional rural town of independent housing districts. fourth, productive and active lifestyles need to be maintained for government lead retirement rural villages by limiting expansion of nursing related facilities around the independent areas. Fifth, visiting welfare service programs and volunteer groups need to be further developed for the housing area especially in winter time, when social integration and emotional comforts are extremely limited.

Socioeconomic impact of traditional Korean medicine, Pyeongwee-San (KMP6) as an anti-allergic inflammatory drug

  • The prevalence of allergic disease has been increasing over the past few decades in the majority of Western industrialized nations. There are some socioeconomic disparities regarding allergic disease status and management. Pyeongwee-San (KMP6) is Korean medicine for the treatment of gastrointestinal tract disease. It is known that KMP6 has an improving effect on the spleen and stomach functions in traditional Korean medical theory. Here, we hypothesized that KMP6 could be used to regulate the inflammatory reaction. We show the molecular mechanisms of Pyeongwee-San (KMP6) on inflammatory reactions. A molecular docking simulation showed that hesperidin, component of KMP6, regulate the enzymatic activity by interaction in the active site of caspase-1. KMP6 control the activity of caspase-1 in activated human mast cell line (HMC-1 cells). KMP6 reduced the expression of receptor interacting protein (RIP)-2 in HMC-1 cells. Thymic stromal lymphopoietin protein production and mRNA expression were inhibited by KMP6. In the activated HMC-1 cells, KMP6 suppressed the activation of mitogen-ativated protein kinase and nuclear factor-kappaB. In addition, KMP6 significantly inhibited the expression of inflammatory cytokines. Our findings indicate that KMP6 may attenuate allergic reactions via the regulation of caspase-1/RIP-2 signaling pathway. These studies will help advance the social welfare system.

The Daily Life Experiences of Community Dwelling People with Mental Disabilities (재가 정신장애인의 일상생활경험)

  • Purpose: As the daily life experiences of community-dwelling people with mental disabilities are closely correlated to mental health recovery, this study aimed to analyze their daily life experiences in depth to determine their significance. Method: Participants included eight community-dwelling people with mental disabilities. A total of 12 in-depth interviews were conducted once or twice per participant. The collected data were analyzed using the phenomenological analysis method described by Giorgi. Results: The daily life experiences of community-dwelling people with mental disabilities were categorized into the following four components: "encountering shocking reality in the outside world", "life tied down by the mental illness", "happiness and gratefulness encountered in daily life", and "wishing for a change and self-sustaining life". Conclusion: This study found that the recovery and welfare of community-dwelling people with mental disabilities were promoted and that these patients made necessary efforts to become a member of the local community. These findings could be used as a reference for establishing social welfare policies which enable people with mental disabilities to participate as functioning members of the community that facilitate the prevention of relapse, to promote their integration into the community. Furthermore, the present results will contribute to the development of nursing intervention programs to promote recovery and prevent relapse, to ultimately establish a mental health management system.

  • This study aims to understand main contents of the National Mental Health Strategy and changes after mental health reform through the National Mental Health Strategy in Australia, and to find some convergence policy implications on mental health policy in South Korea. The direction of reform has changed considerably over the 20 years that the National Mental Health Strategy has been in place including the National Mental Health Policy(1992, 2008), the National Mental Health Plan(1993~2014), COAG National Action Plan on Mental Health(2006~2011), the Roadmap for National Mental Health Reform 2012-2022. The National Mental Health Strategy has advocated fundamental change in the national spending on mental health, mental health service provision, the expansion of community-based mental health services and care system, NGO, consumer and carer participation in mental health care.
