우리나라 청소년의 건강위험요인이 구강건강에 미치는 영향 (Impact of Health Risk Factors on the Oral Health of Korean Adolescents: Korea Youth Risk Behavior Web-Based Survey, 2013)

  • 본 연구는 제9차(2013)청소년건강행태 온라인조사를 바탕으로 우리나라 청소년 총 66,951명을 대상으로 청소년의 건강위험요인과 구강건강과의 관련성을 확인하고, 구강건강에 미치는 위험도를 알아보고자 하였다. 수집된 자료는 통계 분석 프로그램인 PASW Statistics ver. 18.0을 사용하여 분석하였고 주요 결과는 다음과 같다. 남학생은 33,777명(50.5%), 여학생은 33,174명(49.5%)으로 남학생 비율이 높았고, 학년 분포는 중학생과 고등학생 모두 1학년이 가장 높았다. 학업성적은 '하위권'이 24,468명(36.6%)으로 가장 높았고, 경제 상태는 중위권이 32,374명(48.4%)으로 가장 높았다. 어머니 학력은 고졸이 가장 높았다. 성별에 따른 6가지 구강증상경험 분포의 차이를 알아본 결과 '치아가 깨지거나 부러짐'은 성별의 차이가 유의하지 않았고(p=0.135), '먹을 때 치아가 아픔', '치아가 욱신거리고 쑤심', '잇몸이 아프고 피가 남', '혀와 입 안쪽 뺨이 아픔'은 여학생이 남학생보다 더 많이 경험한 것으로 나타났다(p<0.001). 그러나 '입 냄새가 남'은 남학생(52.5%)이 여학생(47.5%)보다 더 많이 경험한 것으로 나타났다(p<0.001). 건강위험요인과 구강증상경험과의 관련성 분석결과 음주, 흡연, 학교폭력 경험, 주중 인터넷 사용이 관련성이 있었으며 로지스틱 회귀분석 결과 음주경험이 있는 대상자의 구강증상경험 위험도가 OR 1.33배, 흡연 경험이 있는 대상자의 구강증상경험 위험도가 OR 1.2배로 높아졌으며 인터넷 사용시간은 '1시간 이내 사용'보다 '7시간 이상' 사용자가 구강증상경험 위험도가 OR 1.25배 높아졌으며 학교폭력 경험도 경험이 없는 대상자에 비해 '3~4번' 경험이 있는 대상자의 구강증상경험 위험도가 OR 1.54배 높아짐을 알 수 있었다. 따라서 본 연구 결과를 토대로 청소년의 올바른 건강습관을 도모하고, 건강위험요인을 적절히 중재하여 청소년의 구강건강을 증진시킬 수 있는 포괄적 건강증진 프로그램개발에 기여할 수 있을 것이다.

청소년의 금지 약물 사용에 따른 관련 요인 분석 (Analysis of Related Factors According to Using Illicit Substances Among Adolescents)

  • Objectives: The purpose of this study was to analyze related factors in relation to usage of illicit substances for Korean adolescents. Methods: This was a cross-sectional study, which used data from the Twelfth Korean Youth Risk Behavior Web-based Survey (KYRBWS). A total of 65,528 students in 2016 were included in this study. Chi-square test and logistic regression analysis were used for analyzing related factors using illicit substances. Results: Overall, 553 (0.8%) students responded as having ever used illicit substances during their lifetime and 133 (0.2%) students responded as current illicit substance users. It was found that electronic cigarette (EC) use, conventional cigarette smoking, second hand smoking, sad/despair mood, and economic status were significantly associated factors with usage of illicit substances. Comparing 'current EC users' with 'never EC users', the adjusted odds ratios (OR) was 7.76 (95% confidence interval (CI): 5.56-10.82) for life-time use of illicit substances, and 18.74 (95% CI: 8.01-43.87) for current use of illicit substances among adolescents. Additionally, comparing 'students who ever were exposed to second hand smoking during the past week', with 'students who never were exposed to second hand smoking', the adjusted OR was 2.60 (95% CI: 2.13-3.17) for life-time use of illicit substances, and 2.85 (95% CI: 1.68-4.83) for current use of illicit substances among adolescents. Conclusions: Based on the results, electronic cigarette use, second hand smoking, conventional cigarette smoking, sad/despair mood, and economic status were significantly associated with increased odds of substance experience and current substance use. Therefore, the results of this study can provide a useful evidence about adolescent behaviors in predicting substance use.

청소년의 치과진료기관 이용여부에 영향을 미치는 요인 (Affecting factors to using oral health of korean adolescents)

  • Objectives : This study was performed to enhance a rate of Using oral clinic and to search related factors of using oral clinics. Methods : The subjects of the study were 74,698 subjects who answered to the 2007 korea Youth Risk Behavior Web-based survey. Answers of 73,836 subjects were used for the final analysis. We used spss of version 12.0. This study conducted a Chi-square test to identify using oral clinics rate according to characteristics of subjects and a multivariate logistic regression analysis to identify whether they are independently significant. Results : Tooth scaling experiences were responded by 10,060 subjects(25.6%). Grade and metropolis, economic level had significant associations with using oral clinic experiences. The rate of using oral clinic showed significant difference in point, depending on general characteristics. Their oral health showed significant difference in point between using oral clinic experience and non-experience groups. According to the multiple logistics regression analysis whose significant independent variables were sex, grade, metropolis, economic level, blooding gum, tooth brushing, oral malodor. Conclusions : The policy is needed to prevent economic constraints from worsening oral health by supporting the poor. And our government should eventually prepare a public relation program suitable for public.

부모의 국적에 따른 다문화가정 청소년의 구강 증상 경험: 제16차 청소년건강행태조사 (Oral symptom experiences in adolescents among multicultural families according to the parental nationality: The 16th Korean youth health behavior survey)

  • Objectives: This study aimed to evaluate the oral symptoms experiences of Korean adolescents among multicultural families. Methods: Data on 985 participants from the 16th Korea Youth Risk Behavior Web-based Survey were collected and analyzed using complex sample descriptive statistics. Results: The foreign-born mother, foreign-born parents, and foreign-born father rates were 77.2%, 17.3%, and 5.4%, respectively. The toothache, gingival bleeding, tooth fracture, and tooth sensitivity experience rates were 21.3%, 18.8%, 13.3%, and 30.4%, respectively. The tooth fracture rate was highest in the foreign-born parents group (24.6%). The toothache odds ratio (OR) was 1.71 (95% CI: 1.24-2.37) times higher in girls. The foreign-born mother group was 2.47 (95% CI: 1.09-5.60) times higher than that of the foreign-born father or both parents foreign-born groups. The smoking group was 2.03 (95% CI: 1.20-3.43) times higher than that of the non-smoking group. The gingival bleeding OR was 5.11 (95% CI: 1.80-14.53) times higher in the lowest economic status group. The tooth fracture OR was 3.44 (95% CI: 1.01-11.70) times higher in the lowest economic status group. The tooth sensitivity OR was 1.53 (95% CI: 1.14-2.04) times higher in girls. Conclusions: It is necessary to establish a program to promote oral health of adolescents from multicultural families.

Factors Affecting Scaling Experiences of Adolescent Children from Multicultural and Native Families

  • Background: Multicultural families are constantly on the rise as marriage migrants and foreign workers increase. Multicultural families appear to record poor health levels compared to native families. As health is a social issue, children of surviving generations of multicultural families are also a growing interest. This study was conducted to confirm the effect of multicultural families and parents' educational level on the scaling experience of children. Methods: For this study, the 2016~2018 Korea Youth Risk Behavior Web-based Survey data were used. In order to make the two groups of adolescents belonging to multicultural and native families similar, a total of 5,362 people were included in the survey, consisting of 2,681 individuals each from multicultural and native families using the propensity score matching method. Logistic analysis was performed to identify factors influencing the scaling experience of adolescent children. Results: The results confirm that, even after controlling for factors such as parents' educational level, household income, and children's oral health behavior, parents' nationality appeared to have a statistically significant effect on their children's scaling experience. In addition, it was confirmed that the experience of oral health education had a significant effect. Conclusion: Cultural heterogeneity and the lack of adequate language ability of immigrants affects health behavior and medical accessibility. Therefore, children from multicultural families are more likely to be exposed to unhealthy environments compared to the children of native Korean families. Based on an understanding of the socioeconomic multicultural background of individuals, education and public policy should be prepared to improve the awareness for the need for preventive oral health and provide unhindered accessibility to dental services.

The Impact of Internet Addiction on Depression among Korean Adolescent Middle and High School Students

  • It is a well-known fact that Internet addiction adversely affects mental health of adolescents. This study was conducted to determine whether there is a difference in the experience of depression according to the level of Internet addiction. Participants included 73,238 middle and high school students from the Korean Youth Risk Behavior Web-based Survey (KYRBWS) conducted in 2010. The level of Internet addiction and the experience of depression were assessed using self-diagnosis questionnaires. Multiple logistic regression analysis was used to identify the association between Internet addiction and depression. High-risk and potential-risk Internet users were 1.61 times and 1.21 times more likely to experience depression, respectively, than normal Internet users. The increase in depression was more significant in girls students. Acknowledging the connection between Internet addiction and depression, the problem should be tackled from the perspective of school health by providing systematic Internet addiction prevention and treatment programs.

Changes in Dietary Behavior Among Adolescents and Their Association With Government Nutrition Policies in Korea, 2005-2009

  • Objectives: This study was conducted to observe recent changes in adolescents' dietary behavior and indirectly evaluate the effects of the government's nutritional policies in Korea. Methods: We analyzed the secular trends in seven dietary behaviors using the Korea Youth Risk Behavior Web-based Survey data from 2005 to 2009. Through literature review, we included the policies implemented for the improvement of adolescents' dietary behaviors during the same periods. Results: The significant linear trends were observed in all dietary behaviors (p<0.05). Overall, all behaviors except the fruit intake rate were desirably changed during five years but undesirable changes were observed between 2008 and 2009 in all behaviors. Within those periods, several policies were implemented including 'Ban on carbonated-beverages in school', 'Green Food Zone', etc. Despite confirmed evidence of their effects, the policies on individual behavior such as nutrition education didn't influence the prevalence of dietary behaviors because they were conducted to too limited persons. Polices on the school environmental improvement, such as ban on carbonated beverage in school, were more effective because they decreased the exposure of undesirable food environment. However, for effect of Green Food Zone improving community environment we couldn't come to a conclusion because of too short period after full implementation. Conclusions: Among government nutrition policies conducted from 2005 to 2009, those on environmental improvement, especially in school, were more effective than those on individual behavior. Therefore, the development and implement of policies on school environmental improvement are needed in Korea.

청소년의 인터넷 사용 수준이 구강증상에 미치는 융합 영향 (The Convergence Effect of Internet Usage Levels on Oral Symptoms of Adolescents)

  • 본 연구목적은 청소년의 인터넷 사용 수준이 구강증상에 미치는 영향을 분석하고자 제14차(2018년) 청소년건강행태온라인조사자료를 이용하여 60,040명을 대상으로 분석하였다. 인터넷 사용 수준이 구강증상에 미치는 영향요인을 로지스틱 회귀분석한 결과 먹을 때 치아 아픔 증상경험, 치아가 쑤시고 욱신거림 증상, 잇몸통증 증상에서는 스트레스가 높은 경우, 일일잇솔질횟수가 없을 경우, 통증의 인지가 높았다. 또한 인터넷 사용시간은 적을수록, 주관적 건강인지, 수면만족도는 높을수록, 흡연, 음주경험이 없을수록 통계적으로 유의하게 낮았다. 결론적으로 청소년의 과도한 인터넷 사용을 완화시키는 보건교육은 적절한 건강행위를 가능하게 할 수 있을 것으로 사료되고 이러한 구강증상을 예방할 수 있는 보건교육프로그램 개발에 도움을 주고자 한다.

남녀 중.고등학생의 자살시도 예측요인 (Factors on the Suicidal Attempt by Gender of Middle and High School Student)

  • Purpose: The suicide rate of adolescents in Korea is increasing annually. Therefore, this research was done to identify the suicide attempt rate of middle and high school students and to identify factors that influence suicidal attempts. Methods: The Korea Youth Health Risk Behavior Web-based Survey (2007) was used as data. Discriminant analysis and logistic regression were performed to analyze the data depending on gender to consider the gender difference in assessing the influence of each independent variable on suicidal attempts. Results: Discriminant analysis according on gender showed that 13 factors correlated with suicidal attempts for boys, and 20 factors for girls. The most highly correlated factors were smoking, depression and inhalation experience. For inhalation experience, boys had 2.7 times higher possibility of suicide attempts (95% CI 1.8-3.0) and girls, a 2.4 times higher possibility (95% CI 1.7-3.5). Conclusion: The results of the study indicate a need to classify adolescents for expectation of suicide risk and high danger for suicidal attempts through, and introduce suicide prevention programs for these adolescents. In particular, it is necessary to start intervention with students who smoke, have sexual and inhalation experiences and high levels of depression.

청소년의 알레르기비염이 구내염에 미치는 영향 (Impact of allergic rhinitis on stomatitis in Korean adolescents)

  • Objectives: This study aimed to determine whether adolescents with allergic rhinitis are at an increased risk of stomatitis and to ascertain possible sex-specific differences. Methods: The data for this cross-sectional study was obtained from the 18th Korea Youth Risk Behavior Web-based Survey (2022). Of the 56,213 students surveyed, 51,850 (boys 26,397 and girls 25,453) were included in the final analyses. Multivariate logistic regression was performed to identify the impact of allergic rhinitis on the risk of stomatitis among Korean adolescents. Results: In Model II, which was adjusted for allergic disease, boys with allergic rhinitis had a 1.53-fold (adjusted odds ratio [aOR]=1.53, 95% confidence interval [CI]=1.40-1.67), and girls had a 1.35-fold (aOR=1.35, CI=1.25-1.46) elevated risk of stomatitis than in those without. In Model III, after adjusting for all covariates, boys with allergic rhinitis had a 1.48-fold (aOR=1.48, CI=1.36-1.62) elevated risk of stomatitis than in those without, and girls had a 1.32-fold (aOR=1.32, CI=1.22-1.43) elevated risk, which remained statistically significant after adjusting for covariates. Therefore, allergic rhinitis in adolescents exerts a detrimental effect on the risk of experiencing stomatitis symptoms; the risk was higher in boys than in girls. Conclusions: These findings suggest that oral health care attention and intervention are needed for adolescents with allergic rhinitis.
