Maze 수술 후 동율동 및 좌심방 수축능 회복에 대한 좌심방 부피 감소 성형술의 영향 (Effect of Left Atrial Volume Reduction with Maze Operation on Sinus Conversion and Recovery of Left Atrial Transport Function)

  • 배경: Maze술식은 심방 세동의 치료에 있어 중요한 방법으로 받아들여지고 있으나 몇 가지 요인, 특히 좌심방 크기가 클수록 전환율이 감소하는 것으로 알려져 있다. 본 연구에서는 류마티스성 승모판막 질환과 동반된 심방 세동 환자에서 좌심방 부피 감소 성형술과 함께 시행한 Maze술식의 효과에 대해 알아보고자 하였다. 대상 및 방법 : 2001년 12월부터 2004년 7월까지 류마티스성 승모판막 질환과 동반한 심방 세동에 대해 총 45명의 환자에서 수술을 시행하였다. 좌심방 부피 감소 성형술을 시행하지 않은 경우를 L군, 시행한 경우를 2군으로 구분하고, 수술 후 7일, 3개월, 그리고 6개월에 동율동 전환과 A파 발견율을 비교 분석하였다. 평균 추적 관찰 기간은 1군과 2군에서 각각 $15.8\pm10.1$개월, $6.1\pm2.7$개월이었다. 결과: 수술 후 동율동 전환기간은 1군과 2군에서 각각 $9.88\pm12.2$일, $1\pm3.6$일 (p<0.01)이었다. 동율동 전환율은 1군과 2군에서 각각 수술 후 7일에 $65\%,\;75\%$ (p=0.07), 수술 후 3개월에 $70.5\%,\;100\%$ (p<0.01),수술 후 6개월에 $93\%,\;100\%$(p<0.01)였다. A파 발견율은 1군과 2군에서 수술 후 7일에 $31.2\%,\;83.3\%$, 3개월에 $63.6\%,\;100\%$, 6개월에 $71.4\%,\;100\%$였다. 결론: 본 연구에서 류마티스성 승모판 질환에 동반된 심방 세동 환자에서 좌심방 부피 감소 성형술과 함께 시행한 Maze 술식이 수술 후 동율동 전환율이나 좌심방 수축능 회복에 좋은 결과를 보여주었다 그러나 향후 대상환자들에 대한 중장기적인 추적 관찰이 필요하리라 생각한다.

  • 목 적: 심근의 단독 비경화증은 성인을 대상으로 산발적인 보고가 있었으나 소아에서는 정확한 혈역학적인 양상, 장기적인 예후, 유전 양상 등에 대해서는 많이 알려지지 않은 상태이다. 저자들은 심근의 단독 비경화증을 보이는 환자군의 임상 양상, 혈역학적인 성상과 경과, 형태학적 특성 및 유전양상 등을 평가하고자 본 조사를 시행하였다. 방 법 : 충남대학교병원 소아과에서 1997년부터 2001년까지, 심초음파 검사를 통하여 심근의 단독 비경화증으로 진단받은 10명(남아 5명, 여아 5명)을 대상으로 임상 양상, 혈역학적인 성상과 경과, 형태학적 특성 및 유전양상 등을 조사하였다. 결 과: 환자들의 진단시의 평균 연령은 $45{\pm}53$개월(1일-14세)이었으며, 평균 추적 관찰 기간은 $33{\pm}19$개월이었다. 남녀비는 동일하였다. 심초음파 검사시 발현 증상은 부정맥 3명, 태아 초음파 검사상 서맥 2명, 심잡음 4명이었다. 다른 심기형이 동반된 환자는 6명으로, 3명은 심실중격결손 또는 심방중격결손이 있었고, 2명은 승모판 탈출증이 동반된 동맥관 개존증, 1명은 승모판 열극이 있었다. 2명에서 심전도상, II, III, aVF 유도에서 T파의 하강이 관찰되었고, WPW 증후군의 환자는 1명으로 발견 당시 발작성 상실성 빈맥이 동반되었다. 2명에서 가족성 동기능 장애가 발견되었으며, 제 3도 방실 차단이 1명의 환자에서 있었다. 심초음파 검사 상 지주 간 고랑의 정도는 승모판 수준에서 심첨부로 갈수록 심하였다. 가족적 발생을 보인 2명에서 가족성 동기능 장애를 동반하였다. 추적관찰 중 대부분의 환자는 무증상이나, 2명에서 좌심실수축기능 저하가 있었고, 이 중 1명은 심한 동기능 장애로 영구 심박 조율기 삽입 시술이 필요한 상태이다. 결 론 : 소아에서 심근의 단독 비경화증은 아주 드문 질환은 아니며, 성인과 마찬가지로 심근기능 장애 및 심각한 합병증의 발생 가능성이 있기 때문에 꾸준한 추적 관찰이 필요하다. 또한 가족적 발생을 할 수 있으므로 환자 가족 내 선별 검사를 해야 하며, 심근의 단독 비경화증의 정확한 빈도, 장기적 예후, 진단기준의 객관성을 확보하기 위하여 국내에서도 광범위한 다기관 연구가 필요하다.

  • 목 적 : 본 연구는 $4{\sim}10\;Hz$의 빛과 소리자극이 인체의 단기기억력 및 긴장이완에 어떠한 영향을 주는지 관찰하기 위한 예비연구로 시행되었다. 방법:총 10명의 대상을 무작위로 동기화된 파형의 빛과 소리를 가한 처치군(R 군)과 동기화되지 않은 무작위 파형의 빛과 소리를 가한 허위군(P 군)으로 분류하고, 하루 15분씩 5일간 빛과 소리 자극을 가하였다. 단기기억력을 측정하기위한 척도, 불안 척도 그리고 자율신경심장 기능 검사 등을 시행하였다. 2일간 휴식 후 R군과 P군을 교차하여 5일간 같은 실험을 반복하였다. 결 과:R군에서 빛과 소리자극 1회 적용 후 단기기억력의 유의미한 향상이 관찰되었다. 자율신경심장기능 검사에서는 R군에서 유의미한 이완 효과가 관찰되었다. 결 론:이 연구 $4{\sim}10\;Hz$의 빛과 소리자극이 단기기억 및 긴장이완과 연관됨을 암시한다. 향후 실험대상수를 늘려 예비연구결과를 확인함이 필요하다.

부산광역시 10대전략산업과 디자인산업의 디자인경영에 관한 연구 (A Study on design management of the design industry and 10 strategic industries in Busan Metropolitan City)

  • 본 연구는 부산산업 성장과 고도화를 기반으로 3단계에 걸쳐 실행하는 10대전략산업에 디자인경영 측면에서 해석한 미래전략 디자인산업의 위치와 부산 디자인산업 육성방안을 제시하였다. 나아가 미래가치에 대한 창조혁명의 시대에 핵심 역할로서 디자인산업의 역할과 디자인경영 관점에서 조망하였으며, 부산 10대 전략산업에서 미래전략의 구성과 디자인산업 연관관계를 분석하였다. 특히, 전략산업과의 연관관계를 해석하였다. 디자인경영의 관점은 가치기반으로서의 디자인이 핵심 전략적 기능과 역할을 수행하는 미래가치의 산업이며, 디자인경영의 주요 기능과 비즈니스경영의 관계성을 이론적으로 고찰하였다. 한편, 부산 디자인 육성방안에서 제시된 항목을 정리하여 전략산업과의 연관관계에서 디자인산업 여건과 부산 디자인전략 수립기반의 조성을 위한 추진목표와 체계를 살펴보았다. 다음으로, 전략산업의 디자인 경영 요소를 분석하고, 부산디자인 육성 컨셉을 분석하여 핵심전략으로서의 의미와 위치관계, 정책우선 등 부산산업 30대 프로젝트의 분석과 그룹핑을 통하여 단계별 실현가능성의 순위를 도출하였다. 본 연구의 주제는 창의성 산업은 미래 산업 결정조건과 핵심가치라는 점과, 디자인 경영에 대한 인식의 전환, 그리고 부산 디자인산업의 비전과 추전전략으로서의 의미를 평가해 보는 것이다. 21세기 초에 정보혁명의 지식경제가 도래하였다. 2000년대의 전반적인 축이 디자인 전략 경영에 의해 창조적 혁신의 성장기를 준비하고 있다. 창조 패러다임의 도래와 지금은 창조경제 시대의 기능과 역할의 중심에 디자인경영의 새로운 이노베이션 DNA을 창출 할 것이며, 개별산업디자인에서 산업의 컨버전스화로 산업간 융합을 거쳐 디지털의 정보화, 지식화를 통해 디자인 프로세스가 변화가 되었으며, 정보, 기술을 활용한 가치창출의 통합디자인이 부산디자인산업 육성과 10대 전략산업의 핵심적 역할을 할 것으로 기대되고 있다.

얀 라우어스(Jan Lauwers) 공연의 탈서사적 특징들 -<이사벨라의 방(Isabella's Room)>, <랍스터 가게(The Lobster Shop)>, <사슴의 집(Deer House)>을 중심으로- (The post-epic characteristics in Jan Lauwers' theatre -, and -)

  • This study aims to analyze the characteristics of post-epic theatre in the Belgian theatre director Jan Lauwers' trilogy titled in "Happy Face/Sad Face": (2004), (2006) and (2008). I regard that it played a very important junction for him to create his own theatrical style compared to earlier years. From this period, Lauwers has tried to create his original plays in order to concentrate the story of our era and has showed to combine a variety of media such as dance, installation, video, singing etc. In this context, I would like to study his own theatricality from the three perspectives of dramaturgy, directing and acting largely based on Hans-Thies Lehmann's theory of post-epic theatre, who pointed out the significance of Lauwer's theatrical leading role very early. First, from the dramaturgical point of view, we need to pay attention to the theme of translunary death; where the living and the dead coexist on the stage. In fact, death is the theme that Lauwers has been struggling to research for quite long time. In his trilogy, the dead never exits the stage. The dead, who is not a representative tragic character, even meddles the things among or with the living and provide comments to people. As a consequence, it happens to reduce a dramaturgical strong tension, leads depreciation of suspense and produces humanism in a way. This approach helps to create his unique comical theatrical atmosphere even though he deals with the contemporary tragic issues such as war, horror and death. Second, from the directing point of view, it is worth to take a look at the polyphonic strategy in terms to applying various media. Among all the things, the arts of dancing and singing in chorus are actively applied in Lauwer's trilogy. The dance is used in individual and microscopic way, on the other hand, singing shows collective and is a macroscopic quality. The dance is the representing media to show Lauwer's simultaneous microscopic mise-en-scene. While main plot takes place around the center-stage, actors perform a dance around the off-centered stage. Instead of exiting from the stage during the performance, the actors would continue dance -sometimes more like movements- around the off-centered stage. This not only describes the narrative, but also shows how each character is engaged to the main plot or incident, and how they look into it as a character. Its simultaneous microscopic mise-en-scene intends to function such as: showing a variety moments of lives, amplifying some moments or incidents, revealing character's emotion, creating illusionary theatrical atmosphere and so on. Meanwhile, singing simple lyrics and tunes are an example of the media to stimulate the audiences' catharsis. As the simple melody lingers in the audiences' mind, it ends up delivering a theatrical message or theme after the performance. This message would be transferred from the singing in chorus functions as a sort of leitmotive in order to make an impression to the audience. This not only richens their emotion but also creates an illusionary effect. Third, from the acting perspective, I'd like to point out the "detachment" aesthetic which Lehmann has pointed out. The actors never go deep into the drama by consistently doing recognize a theatrical illusion. The audience happens to pay attention to their presence through the actor's deliberate gesture, business, movement, rhythm, language, dance etc. The actors are against forming closed action by speaking in various languages or by revealing deliberately stage directions or acts, and by creating expressive mise-en-scene with multiple media. As a consequent, the stage can be transformed to not a metaphoric but a metonymic place. These actions are the ultimate intention for a direct effect to the audience. So to speak, Lauwers uses the anti-illusionary theatrical method: the scenes of fantastic death, interruption of singing and dance, speaking many kinds of languages, acting in detachment-status and so on. These strategies function to make cracks in spectators' desire who has a desire to construct a linear narrative. I'd like to say that it is the numerous potentiality to let the reality penetrate though and collide the reality with a fiction. By doing so, it induces for spectators to see the reality in the fiction. As Lehmann says, "when theatre presents itself as a sketch and not as a finished painting, the spectators are given the chance to feel their own presence, to reflect on it, and to contribute to the unfinished character themselves". In this sense the spectators can perform an objective criticism on our society and world in Lauwer's theatre because there are a number of gaps and cracks in his theatrical illusion where reality can penetrate. This is also the point that we can find out the artists' responsibility in this era of our being.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. The maximum adsorbed amount was obtained in the neighborhood of pH 4.5. Isoelectric point of bovine albumin is pH 5.0, so experimental result shows that it shifted to acid area. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. We propose a simple adaptive algorithm to increase capability of noise cancelation in QRS complex detection with two stage adaptive filter.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. This electrical modelling of the basilar membrane (BM) is clearly useful for the next approach to the further. Third, best-fit boundary is detected by the cost function which is based on the best-fit elliptic model. We use combination of MLP and fuzzy filter for pattern recognition to estimate the direction of a muscle and Hogan's method for the force estimation. EMG signals are acquired by using a amputation simulator and 2 dimensional joystick motion. The bedside monitor consists of a main unit and module cases with various parameter modules. The main unit includes a 12.1" TFT color LCD, a main CPU board, and peripherals such as a module controller, Ethernet LAN card, video card, rotate/push button controller, etc. The main unit can connect at maximum three module cases each of which can accommodate up to 7 parameter modules. They include the modules for electrocardiograph, respiration, invasive blood pressure, noninvasive blood pressure, temperature, and SpO2 with Plethysmograph.

  • Engineers have developed new instruments that aid in diagnosis and therapy Ultrasonic imaging has provided a nondamaging method of imaging internal organs. A complex transducer emits ultrasonic waves at many angles and reconstructs a map of internal anatomy and also velocities of blood in vessels. Fast computed tomography permits reconstruction of the 3-dimensional anatomy and perfusion of the heart at 20-Hz rates. Positron emission tomography uses certain isotopes that produce positrons that react with electrons to simultaneously emit two gamma rays in opposite directions. It locates the region of origin by using a ring of discrete scintillation detectors, each in electronic coincidence with an opposing detector. In magnetic resonance imaging, the patient is placed in a very strong magnetic field. The precessing of the hydrogen atoms is perturbed by an interrogating field to yield two-dimensional images of soft tissue having exceptional clarity. As an alternative to radiology image processing, film archiving, and retrieval, picture archiving and communication systems (PACS) are being implemented. Images from computed radiography, magnetic resonance imaging (MRI), nuclear medicine, and ultrasound are digitized, transmitted, and stored in computers for retrieval at distributed work stations. In electrical impedance tomography, electrodes are placed around the thorax. 50-kHz current is injected between two electrodes and voltages are measured on all other electrodes. A computer processes the data to yield an image of the resistivity of a 2-dimensional slice of the thorax. During fetal monitoring, a corkscrew electrode is screwed into the fetal scalp to measure the fetal electrocardiogram. Correlations with uterine contractions yield information on the status of the fetus during delivery To measure cardiac output by thermodilution, cold saline is injected into the right atrium. A thermistor in the right pulmonary artery yields temperature measurements, from which we can calculate cardiac output. In impedance cardiography, we measure the changes in electrical impedance as the heart ejects blood into the arteries. Motion artifacts are large, so signal averaging is useful during monitoring. An intraarterial blood gas monitoring system permits monitoring in real time. Light is sent down optical fibers inserted into the radial artery, where it is absorbed by dyes, which reemit the light at a different wavelength. The emitted light travels up optical fibers where an external instrument determines O2, CO2, and pH. Therapeutic devices include the electrosurgical unit. A high-frequency electric arc is drawn between the knife and the tissue. The arc cuts and the heat coagulates, thus preventing blood loss. Hyperthermia has demonstrated antitumor effects in patients in whom all conventional modes of therapy have failed. Methods of raising tumor temperature include focused ultrasound, radio-frequency power through needles, or microwaves. When the heart stops pumping, we use the defibrillator to restore normal pumping. A brief, high-current pulse through the heart synchronizes all cardiac fibers to restore normal rhythm. When the cardiac rhythm is too slow, we implant the cardiac pacemaker. An electrode within the heart stimulates the cardiac muscle to contract at the normal rate. When the cardiac valves are narrowed or leak, we implant an artificial valve. Silicone rubber and Teflon are used for biocompatibility. Artificial hearts powered by pneumatic hoses have been implanted in humans. However, the quality of life gradually degrades, and death ensues. When kidney stones develop, lithotripsy is used. A spark creates a pressure wave, which is focused on the stone and fragments it. The pieces pass out normally. When kidneys fail, the blood is cleansed during hemodialysis. Urea passes through a porous membrane to a dialysate bath to lower its concentration in the blood. The blind are able to read by scanning the Optacon with their fingertips. A camera scans letters and converts them to an array of vibrating pins. The deaf are able to hear using a cochlear implant. A microphone detects sound and divides it into frequency bands. 22 electrodes within the cochlea stimulate the acoustic the acoustic nerve to provide sound patterns. For those who have lost muscle function in the limbs, researchers are implanting electrodes to stimulate the muscle. Sensors in the legs and arms feed back signals to a computer that coordinates the stimulators to provide limb motion. For those with high spinal cord injury, a puff and sip switch can control a computer and permit the disabled person operate the computer and communicate with the outside world.

가루깍지벌레(Pseudococcus comstocki Kuwana)의 온도별 발육기간 및 발육단계 전이 모형 (Temperature-dependent Development of Pseudococcus comstocki(Homoptera: Pseudococcidae) and Its Stage Transition Models)

  • 본 연구는 가루깍지벌레 방제적기 예측을 위한 모형을 개발하고자 수행하였다. 포장에 서 가루깍지벌레 발생시기 조사 및 온도별 발육기간을 조사하였으며 각 발육단계 전이(우화)모형 을 작성하였다. 성충발생 최성기는 1세대 6월 중하순,2세대 8월 중하순,3세대는 10월 하.순으로 수원지방에서는 연 3회 발생하였다. 가루깍지벌레 각 발육단계의 발육기간은 $25^{\circ}C$ 까지는 온도가 증가할수록 감소하였으나 그 이상 온도에서는 증가하였다. 발육영점온도 추정결과 알 14.5$^{\circ}C$, 1령 약충+2령 약충 8.4$^{\circ}C$, 3령 약충 10.2$^{\circ}C$, 산란전기간 11.8$^{\circ}C$, 그리고 1령 약충부터 산란전까지는 10.1$^{\circ}C$ 이었다. 발육완성을 위한 적산온도(DD)는 알 105 DD, 1령 +2령 315 DD, 3령 143 BD, 산란전기간 143DD이었다. 알부터 산란기까지 필요한 적산온도는 599DD이었다. 생물리적 발육모형과 발육완료시기 분포를 나타내는 Weibull함수를 이용 가루깍지벌레의 특정 발육단계에서 다음 발육단계로 전이되는 개체수의 비율을 추정하는 발육단계 전이모형을 작성하였다. 1령부터 산란전기간까지 적산온도를 이용하여 성충발생 세대별 50%산란시기를 예측한 결과 Mean-minus-base 추정법을 사용한 경우 실측일과 비교하여 1992년과 1993련 1세대와 2세대 모두 2-3일의 편차를 보였고,Sinewave추정법을 이용한 경우는 1-7일의 편차를 보였다. Rectangle추정법은 0-6일의 편차를 보였다. 발육모형을 이용 일별 발육률을 추정하고 이것을 누적하는 발육률 적산모형의 경우 1세대와 2세대의 성충산란 시기 예측 결과 모두 50%산란시기까지는 1-2일의 편차를 보였다.

디지털 흉부 촬영에서 구리필터사용에 따른 환자 표면선량 감소효과에 관한 연구 (The Study on the Reduction of Patient Surface Dose Through the use of Copper Filter in a Digital Chest Radiography)

  • 의학적으로 방사선을 사용 시 가장 중요한 점은 환자의 병을 진단 가능하되 피폭선량을 최소화시켜야 한다는 점이다. 진단용 X-선 중 장파장부분은 대부분 흡수에 관여하게 되어 환자의 피폭선량을 증가시키므로 불필요하다. 이러한 장파장영역을 제거하기 위한 방법으로 가장 효과적인 방법은 여과 판의 사용이다. 이에 부가여과가 없는 경우와 0.3mmCu filter 사용 시의 화질과 피부입사선량에 대해 실험을 해 보았다. ROC 신호용 지름 4mm, 두께 3mm의 아크릴 디스크를 chest phantom에 랜덤하게 위치시키면서 non filter 및 Cu filter시 각 각 20매씩 총 40매의 신호+잡음영상을 촬영하고, 또한 신호가 없는 영상(잡음만의 영상)을 각 각 20매씩 40매 촬영하여 총 80매의 영상을 준비한 후 5명의 방사선종사자가 판독 후 P(S/s), P(S/n) 구하여, ROC곡선을 그리고, 감도와 특이도 및 곡선하면적을 구해 평가하였다. 또한 AAPM(American Association of Physicists in Medicine by the American Institute of Physics)권고에 따른 ANSI chest phantom과 Contrast-detail phantom을 이용하여 non filter 또는 Cu filter사용 시 시각적으로 관찰 가능한 병소의 갯수를 파악하였으며, 또한 두 경우에서 피부입사선량을 측정하였다. 그 결과 Cu filter사용 시에 ROC곡선이 좌상방향에 위치하고, 감도, 곡선하면적 및 CD phantom 병소의 갯수에서 모두 좋은 결과를 나타내었다. 또한 피부입사선량은 감소가 되어 여러 측면에서 부가필터의 사용을 검토해 볼 필요가 있으리라 사료된다.
