Background : In many Nursing Delivery System, Nursing Department at D Hospital had used to traditional nursing practice model what is called functional activities based system. It has a lot of merit that carried out specialized and rapid works but tend to ignore indivisual professional responsibility and task-based work assignments. In addition this system showed high turnover rates due to heavy workload, timesum of handing over duties, lack of support from peers and interstaff communication. So we performed conversion of Nursing Delivery System to My Patients Nursing Care System for providing comprehensive nursing to patient and reducing turnover rates and increasing job satisfaction to nurse. Method : 1. 1st step(96.4.9): Detected the problem of Nursing delivery System and estabilished improving planning 2. 2nd step(96.4.26): Visited other hospital on job training 3. 3th step(96.4.29): Discussed to premonitoring problem after conversion Nursing Delivery System and prepared structure 4. 4th step(96.5.6): My Patients Nursing Care System practical application 5. 5th step(96.7.20): Held complementary meeting 6. 6th step(96. 7): The other ward application 7. 7th step(96. 10): Extended application to whole wards Results: 1. Workload: (1) reduction(55.6%) (2) addition(44.4%) 2. Presently, there is increasing demand for geriatric nursing care because of increasing preparation of elderly population due to extended age. Of particular importance in determining the effectiveness of nursing care given to the elderly is the attitude of the nurse toward the elderly patient. Knowledge of the various changes that usually occur with aging will enable the nurse to help an aged Person maximize his potential in illness and in health. The objectives of this study were : 1. To delineate the attitudes of nurses toward the geriatric nursing care. 2. To learn the influencing factors affecting the attitudes of nurses toward the geriatric nursing care. The study population defined and randomly selected is 225 nurses at one general hospital, one private hospital, two national hospitals in Seoul during the period of October l0th -20th, 1975. The questionnaire method was used. Respondents were 140 nurses. X$^2$- test and t- test were employed in analyzing the data. The questionnaire form included 54 statements which concerned the attitudes of nurses to-ward geriatric nursing care. It was divided into five areas : 1) General characteristics of study population. 2) The nurse's concepts of elderly. 3) Care of the geriatric patient. 4) Interpersonal relationship with geriatric patient. 5) Teaching of the geriatric patient. Each of the 54 statements of the questionnaire was considered to be either Positive or Negative. A Positive response was assigned the value of+1 , and a Negative response or no response was assigned the value of O. The results of the study were as follows : 1. The Investigation of attitudes of nurses toward geriatric nursing care. a. Data indicated , respondents have negative attitudes in their. concepts of elderly (74.3%) b. Data indicated respondents have positive attitudes in care of geriatric patient (64.3%). c. Data indicated respondents have positive attitudes in interpersonal relationship with geriatric patient (85% ). 4. Data indicated respondents have positive attitudes in teaching of geriatric patient (89.3%). 2. The results of study regarding the five hypothesis were as follows : a There was significant difference in the mean scores between the attitudes of nurses toward the geriatric nursing care and level of education of the nurse. b. There was no significant difference between the attitudes of nurses toward the geriatric nursing care and clinical experience, but there was significant difference between care of the geriatric patient and clinical experience. c. There was no significant difference between the attitudes of nurses toward the geriatric nursing care and shift most frequently worked, but there was significant difference between care of the geriatric patient and shift most frequently worked. d. There was no significant difference in the mean scores between the attitudes of nurses toward the geriatric nursing care and marital status, but there was significant difference in the mean scores between interpersonal relationship, teaching of geriatric patient and marital status. e, There was no significant difference in the mean scores between the attitudes of nurses toward the geriatric nursing care and experience with elderly, but there was significant difference in the mean scores between care of geriatric patient and experience with elderly.
As a descriptive survey, this study was attempted to get basic data necessary to recognize the factors that disturb the therapeutic atmosphere of hospital wards as perceived by nurses and hospitalized patients, to identify differences between the perceptions of the nurses and of patients. The subjects, 159 patients in Pusan National Hospital and 68 nurses working there were sampled between March 18 and April 13, 1996. The tool used to measure the disturbing factors was an amended form of the one developed by Kim, Mae Ja(1983). The differences between each subject's score for each factor were analyzed using means & SD. and the highest 3 items above the mean score for each factor were collected and compared. The results are described below : 1. Subject's perception of main disturbing factors : patients reported that the main factors were 'loss of role & economic trouble', 'the prognosis of disease', 'the change of daily life' but nurses replied that the main factors were' the prognosis of disease', 'the communication trouble with the medical team & interpersonal relationships'. 'The change of daily life' was not a perceved factor by nurses, but ranked third by the patients. 2. Subject's perception degree of each disturbing factor : (1) among the items related to interpersonal relationship. the patient group reported that the worst disturbance was dur to severely ill patients in the same room' but the nurse group regarded 'greed to monopolize wheelchairs or other supplies' as the worst disturbance. (2) among the items related to physical factors. the patient group regarded 'limitations to wash their body, physical pain and limitations in physical activity' as the worst disturbance, but the nurse group regarded' physical pain', and 'limitations to activity or change of appearance' as the worst disturbance. (3) among the items related to the change of daily activity, the patient group regarded 'the boredom of hospitalization or infavorable diet' as the worst disturbance, but the nurse group regarded 'too much noise or unclean room' as the worst disturbance. (4) among the items related to the communication trouble with medical team, the patient group regarded 'the ignorance of their disease due to poor information. the inability to understand the language of the medical team or the difficulty in seeing physician in time' as the worst disturbance, but the nurse group regarded 'the inability to trust physicians and physician's poor attention to patients' as the worst disturbance.
Objectives : The aim of this study was to evaluate the effect of sociodrama for reducing burnout among health care workers and to investigate relationship between the job satisfaction, conflict with other professions, and burnout. Methods:15 nurses in general hospital participated in sociodrama directed by psychiatrist. One week after acting sociodrama, they were requested to complete ward organizational feature scale(WOFS) and maslach burnout inventory(MBI). The other twenty nurses in same ward requested to complete questionnaire without participating in sociodrama at the same time. Results : Sociodrama group(N=15) showed significantly lower score in burnout when compared to control group(N=17). Nurse/physician relationship was significantly correlated with job satisfaction and burnout. After controlling for interaction between variables by multivariate regression analysis, only job satisfaction was significant predictive factor to burnout. Conclusion : Our result suggests that the sociodrama can be effective in reducing burnout of health care workers in general hospital. These finding also underline the importance of relationship with physicians in job satisfaction and burnout of healthcare workers in general hospital.
This study was aimed to prepare the evidences for establishing policies on nursing manpower by identifying the relationship among delegation level of nursing activities, nursing professionalism and turnover intention in long-term care hospital nurses. The study was a descriptive survey research that used a structured questionnaire. 146 nurses agreed to take part in the research. They worked in six long-term care hospitals located in A and B province. Data were collected from July to August 2016 and analyzed through SPSS 20.0 program, using descriptive statistics, t-test, ANOVA and pearson's correlation coefficients. As a results, first, as for the delegation of nursing activities by long-term care hospital nurses, the levels of delegation of direct nursing activities were 34.63±14.12, and the level of delegation of indirect nursing activities were 1.71±1.75. Second, the mean score of nursing professionalism and the turnover intention of long-term care hospital nurses were 3.62±0.77 and 2.71±0.97, respectively. Third, the delegation of direct nursing activities by long-term care hospital nurses significantly differed depending on the number of nurses in the ward and the number of caregivers in the ward. The delegation of indirect nursing activities significantly differed depending on the number of nurses in the ward, the presence of conflicts and stress related to delegation. Forth, there was no significant correlation of the delegation of nursing activities by long-term care hospital nurses with nursing professionalism and with turnover intention. Most of nursing activities, were delegated to nurse aids, since there were no clear guidelines or legal procedures on nursing activities in long-term care hospital nurses. Therefore it is required to find how to clearly establish positive nursing professionalism and to develop a proper nursing manpower arrangement and the guidelines for the delegation of nursing activities in a long-term care hospital in terms of policy.
Caring is the essential and universal concept of underlying nursing. Exactly how caring attributes relate to nurses' perception in the context of a contemporary technological environment have yet to be operationlized. In total. 560 Registered Nurses from 5 different university hospitals were involved in this study which aimed to : investigate technological influence and caring attributes as perceived by nurse. Data were obtained using Technological Influence Questionnaire (TIQ) and Caring Attributes Questionnaires(CAQ) which developed by Arthur et al(1999). Data were analyzed using SAS program for frequency. t-test. ANOVA and Tukey's Studentized Range test. and Pearson correlation coefficients. The results were as follows : 1) The mean score of TIQ was 48.84 and special unit nurses' TIQ was higher than that of general ward nurses. 2) The mean score of CAQ was 237.02 and it showed significant differences in the following variables : age. religion. educational background. marital status. career, and position. 3) With regard to the mean item score of CAQ by 3 perspectives : theoretical perspectives score was 3.76. practical perspectives score was 4.03, and pedagogical perspectives score was 3.84. 4) The correlation between TIQ score and CAQ score was slightly moderate (r=.35), The CAQ mean scsre was significantly different accordig to the level of TIQ meen score. The CAQ mean score was $247.19(\pm18.17)$ in high TIQ(>mean+1SD) group, $236.21(\pm19.71)$ In moderate $TIQ(between\;mean{\pm}1SD) $ group, and $228.47(\pm18.07)$ in low TIQ( (mean-1SD) group. In conclusion, nurses perceived the CAQ differently by several demographic characteristics which relating to age. religion, eduacational background. marital status. career. and position. This study provides an important message for administrators and nurse educators by highlighting factors which can be addressed by education programs. staff development and appraisal. Further research is necessary to improve and refine the Korean version of TIQ and CAQ instead of Arthur et al's scales directly to Korean nurses.
The purpose of this study was conducted to determine the working conditions, post-traumatic stress syndrome and the self-resilience of nurses, and find out the relationship between the post-traumatic stress syndrome and the self-resilience. Data were collected by questionnaire method among 220 General Hospital nurses at Gyeongnam G and J city. Data were analyzed by using SPSS 21.0 program WIN. The results of working conditions of the nurses majority were shift work 70.5%, currently working in the ward 54.6%. Scores were shown as Post-traumatic stress syndrome (28.20) and self resilience (39.98). There is a negative correlation (r =.226, p=.001) between the post-traumatic stress syndrome and the self-resilience. Based on the result, since working conditions of nurses caused psychological stress and nurses with high self-resilience eased the degree of post-traumatic stress, in terms of the personnel management of hospitals, a psychological intervention strategy should be considered.
Purpose: This study was conducted to identify relationships among circadian types, sleep quality, and adaptation to night shifts among nurses working on two or three day night duties. Methods: The participants were 199 ward nurses from two university hospitals in Gyeongnam province. Each nurse worked on two or three day night duties. Data were collected between September 24 and October 14, 2012 and analyzed using t-test, ANOVA, $Scheff{\grave{e}}$ test, Pearson correlation, and multiple regression with SPSS/WIN 14.0 program. Results: Mean scores for sleep quality and adaptation to night shifts was $4.92{\pm}1.46$ on a 10-point scale and $2.66{\pm}0.47$ on a 5-point scale respectively. There were no significant differences in sleep quality and adaptation to night shifts according to circadian types of nurses. There were significant correlation between sleep quality and adaptation to night shifts (r=.25, p<.001). Factors affecting adaptation to night shifts were preference for night shifts, and subjective health status, which together explained about 35% of the total variance. Conclusions: Nurses working on two or three day night duties had low level of sleep quality and adaptation to night shifts. It is necessary to improve preference for night shifts and subjective health status for improving adaptation to night shifts of nurse.
The purpose of this study was to identify job stress, resilience, and nursing performance, and to investigate affecting factors of nursing performance on hospital nurses. The subjects consisted of 141 general hospital nurse in the K city. The variables were measured using questionnaires, analyzed using descriptive statistics, t-test, ANOVA, and multiple regression analysis. The average score of job stress was 44.21±9.15, resilience was 2.86±0.65, and nursing performance was 3.78±0.48. The factors influencing on nursing performance were Total Clinical career, Career in present ward, and resilience. The explanation of nursing performance was 37%. These results suggest that it is necessary to consider these factors at the nursing organizational level to promote nursing performance. Also, it is necessary to construct education program that can enhance resilience and find a strategy to realize it.

Purpose : This study was conducted to examine the Experience of Adaption to Work in Comprehensive Nursing Service Units of Nurses. Methods : Empirical data were collected from 8 nurses through in-depth interviews. Eight participants who had more than 6 months of clinical experience in the general ward and had more than 6 months of work experience in Comprehensive Nursing Service Units were selected as participants. The main question was what does it mean to work in Comprehensive Nursing Service Units? All interviews were recorded and transcribed, and then analyzed using Giorgi's interview. Results: As a result of the analysis, five topics were derived. 'Facing a changed work environment', 'A sense of unexpected heterogeneity in work', 'Evaluation of immediate reality.', 'Finding strategies and promoting balance', 'Growing up as a professional nurse.' Conclusion: Based on these findings, There is a need to build a system that can adapt to work and continue to develop programs for capacity building in Comprehensive Nursing Service Units of Nurses.
