노인의 야간보행 불안감에 영향을 미치는 요인 (Factors Affecting Walking at Night Anxiety among the Elderly)

  • 이 연구는 노인의 야간보행 불안감에 영향을 미치는 요인을 탐색적으로 살펴보는데 목적이 있다. 분석에 사용된 표본은 65세 이상 노인 9,120명이다. 노인의 야간보행 불안감에 영향을 미치는 요인은 인구사회학적 요인, 건강관련 요인, 그리고 사회적 인식 요인으로 구성되었다. 인구사회학적 요인은 성별, 연령, 지역, 교육수준, 배우자 유무, 가구소득을 포함한다. 건강 관련 요인은 보행능력, 시력, 주관적 건강상태, 인지기능을 포함한다. 사회적 인식 요인은 타인의 준법수준 인식과 범죄에 대한 안전인식을 포함한다. 주요 분석결과는 다음과 같다. 인구사회학적 요인 중 노인의 야간보행 불안감에 유의미한 영향을 미친 변수는 성별, 연령, 지역, 교육수준으로 나타났다. 건강 관련 요인 중 노인의 야간보행 불안감에 유의미한 영향을 미친 변수는 보행능력, 시력, 주관적 건강상태, 인지기능으로 나타났다. 사회적 인식 요인 중 노인의 야간보행 불안감에 유의미한 영향을 미친 변수는 타인의 준법수준 인식과 범죄에 대한 안전인식으로 나타났다. 본 연구의 결과는 노인을 위한 안전하고 편안한 보행환경을 조성하기 위한 다각적인 방안들이 필요함을 보여준다

The Effect of Hinged Ankle-Foot Orthosis on Walking Function in Children With Spastic Diplegic Cerebral Palsy: A Cross-Sectional Pilot Study

  • Purpose: The aim of the current study was to examine the effects of hinged ankle-foot orthosis (HAFO) on walking function in children with spastic diplegic cerebral palsy (CP). Methods: Thirty-two children (mean age: $6.79{\pm}0.35years$, age range: 5-7 years) who were diagnosed with spastic diplegic cerebral palsy participated in the study. Each subject typically walked through 10 meters of a gait platform with markers on the subject's proper body segments and underwent 3-D motion analysis system with and without hinged ankle-foot orthosis. The HAFOs were all custom-made for individual CP children and had plantarflexion stop at $0^{\circ}C$ with no dorsiflexion stop. The interventions were conducted over three trials in each group, and measurements were performed on each subject by one examiner in three trials. 3-D motion analysis system was used to measure gait parameters such as walking velocity, cadence, step-length, step-width, stride-length, and double support period in two conditions. Results: The walking velocity, cadence, step-length, and stride-length were significantly greater for the HAFO condition as compared to the no HAFO condition (p<0.05). However, no significant difference in step-width and double support period was observed between two conditions. Conclusion: These findings suggest that using the HAFO during walking would suggest positive evidence for improving the spatiotemporal parameters of gait in children with spastic diplegic cerebral palsy.

  • Purpose: We had executed the 3 3 5 5 walking club program for 12 weeks for one hour three times per week to evaluate the effect of the program on health of the elderly. Methods: A pre and post measurement tool of the program was applied to measure obesity, rate of body fat, blood pressure, vital capacity, standard vital capacity, intake capacity of maximum oxygen, grasping power, reaction of whole body, pulse, body mass index (BMI), total bodily score, age of bodily strength, age of health. Data collected from this tool was analyzed by SPSS in paired t-test. Results: The 3 3 5 5 walking club program has affected the weight control of the participants. The participants of 3 3 5 5 walking club program reduced their weight from 61.7 kg to 61.25 kg, rate of body fat from 21.89 kg to 21.22 kg, standard vital capacity from 2,393.56 ml to 2,380.44 ml, reaction of whole body from 475.38 msec to 439.33 msec, and increased grasping power from 22.06 to 23.16 ml (p<.05), total bodily score 68.91 points to 71.38 points significantly (p<.05). Conclusion: The result indicated that the 3.3.5.5 Walking club program has an positive effect on improving health care of the elderly without special exercise tools.

Influence of mobile phone texting on gait parameters during ramp ascent and descent

  • Objective: The purpose of this study was to examine the influences on gait features during mobile phone use while ramp walking. Design: Cross-sectional study. Methods: Thirty-three healthy adult subjects performed four walking conditions on an outside ramp with a 5 m length, 1.5 m width, and a $5^{\circ}$ angle. All participants were touch screen mobile phone users. Four walking conditions were used: 1) ramp ascent, 2) ramp descent, 3) texting during ramp ascent, and 4) texting during ramp descent. In conditions 3) and 4), subjects texted the words of "Aegukga"-the song of patriotism-while walking. Upon the signal of start, the subjects walked the ramp during texting. Gait parameters were measured at the length of 3 m excluding 1 m of the start and end of the total length. Each situation was repeated three times for each subject, and mean values were calculated. For gait examination, a gait analyzer was used (OptoGait). Results: Subjects ranged in age from 23 to 38 years (mean age, 27.73). Eighty-three percent of subjects in our study had experienced an accident during mobile phone use. Texting on a mobile phone while walking significantly decreased ramp gait, speed, cadence, stride length, step length, and single support (p<0.05) and significantly increased stride time, step time, gait cycle, and double support (p<0.05). There was a significant difference in cadence, step length, stride time, step time, and single support during ramp ascent and descent (p<0.05). Conclusions: Texting on a mobile phone while walking significantly decreased gait quality.

실내·외 보행환경의 변화가 40대와 노인의 보행속도와 다리 근활성도에 미치는 영향 (Effects of Changes in the Indoor and Outdoor Environmental on the Walking Speed and Lower Extremity Muscle Activities in People Aged Forty and Older than Seventy Years)

  • PURPOSE: This study compared the effects of indoor and outdoor environmental changes on the activity of the major lower limb muscles and walking factors in people 40-50 years and those aged older than 70 years. METHODS: Ten middle-aged people in their forties (age:$44.2{\pm}2.7$, BMI:$21.8{\pm}1.8$) and 10 elderly aged more than 70 years (age:$76.4{\pm}5.9$, BMI:$22.2{\pm}1.9$) with a normal walking ability were included. The participants walked 100 m both indoors and outdoors at their own speed. Using a 3D motion analyzer and EMG, the walking speed, angle of the ankle and activity changes of the lower limb muscles were compared. RESULTS: Significant differences in walking speed and peak-plantar flexion angle were observed between the two groups (p<.05). The muscular activity of the gastrocnemius muscle (GCM) was significantly different outdoors in the swing phase between the two groups (p<.05). In the people aged in their forties, the muscular activity of the rectus femoris (RF) was significantly higher outdoors than indoors (p<.05). In the elderly, however, the muscular activity of the RF was lower outdoors than indoors (p<.05). When compared to those in there forties, the muscular activity of the outdoor RF significantly decreased in the elderly group (P<.05). The muscular activity of the biceps femoris (BF) in the elderly decreased significantly outdoors compared to indoors (p<.05). CONCLUSION: For the elderly, increasing the exposure to the new environments or focusing on the performance of repeated movements for gradual speed control and precise movements is required to maintain normal gaits and movements that are less affected by environmental changes.

중년의 체질량지수에 따른 건강관련 삶의 질 영향요인 (Influencing Factors on Health-related Quality of Life According to Body Mass Index in Middle Age People)

  • Purpose: The purpose of this study was to investigate the influencing factors on health-related quality of life according to body mass index (BMI) in middle age people. Methods: Using the 2017 Korea Health Panel Data, a total of 4,711 subjects age 40-64 were selected and classified into four groups based on BMI levels (underweight, normal weight, overweight, obese). Influencing factors according to BMI in middle age people was analyzed using multiple regression. Results: Activity limited was the most influential factor for health-related quality of life in middle age people. Walking and stress were factors influencing health-related quality of life in all weight groups. In the overweight and obese group, suicidal impulse was a significant influencing factor on health-related quality of life. Conclusion: To improve the health-related quality of life of middle age people, effective walking and stress management for maintaining and managing health, as well as managing suicide risk in the overweight group and obese group are required.

Analysis of Changes in Electrical Signals and Ground Reaction Force in Muscles According to Exercise Method

  • With an increasing aging population and improved standards of living, more attention has been paid to health. Although walking exercise is known as an aerobic exercise, it imposes repeated and continuous impacts on the joints of the lower extremities. Therefore, when overweight gives a burden to the lower extremity or there is a joint disease, exercise limit occurs. The articular cartilage, weakened with age, also makes it difficult for the elderly to perform walking exercises. Accordingly, this study conducted a comparative analysis between regular walking using only the lower extremity and Nordic walking, which has been known as stable. For analysis, electromyography (EMG) was performed and the ground reaction force of the upper and lower extremities were measured in the same exercise. Integrated EMG (iEMG) revealed that the upper extremity muscles were more active in Nordic walking than in regular walking, where lower extremity muscles were relatively more inactive. In addition, when EMG measurements were performed at each measurement point during walking exercise, the pattern was different. Nevertheless, the result was the same as in iEMG. The load that occurs in each exercise was measured using the ground reaction force system. As a result, Nordic walking had a lower load than regular walking. Therefore, it was found that Nordic walking minimized the load on the lower extremities owing to the effect of whole-body exercise and was a safer and more efficient exercise method.

보행운동이 당뇨환자의 혈당 및 운동에 대한 동기에 미치는 효과 (The Effects of Walking Exercise on Blood Glucose Level and Motivation of Exercise in the Diabetes Mellitus Patients)

  • This study was performed to explore the effects of walking exercise on blood glucose level in the diabetes mellitus patients. The design of this study was a equivalent pre-post test experiment. The subjects consisted of eighteen diabetes mellitus patients. The walking exercise consists of 22.5minutes, speed of 120walking per minute. Data were analyzed with mean, standard deviation, Wilcoxon signed rank test, Spearman correlation and percentage using SAS program. The results were as follows. 1. The blood glucose level has significantly decreased after walking exercise. 2. Strength of exercise and change of blood glucose level was revealed correlation. 3. Age and strength of exercise was revealed high correlation. 4. Satisfaction after walking exercise was revealed all patients. In conclusion, the walking exercise can decrease blood glucose level. This exercise was show a positive effect on the exercise compliance.

고령자 보행자립도(능력) 측정모형 개발 (Development of Elderly Walking Independence Index Model)

  • 앞으로 10년 뒤인 2026년 우리나라는 초고령화 사회(super aged society)로 진입할 것으로 예상되며, 이로 인해 사회 전반적인 모든 분야에서 고령자 분석 및 대책 마련에 심혈을 기울이고 있다. 교통 분야에서도 고령자의 보행 교통사고가 급격히 증가하고 있는 점을 고려하여 이를 줄이기 위한 연구개발과 정책적 보완을 위한 노력을 기울이고 있다. 그러나 고령자의 보행행태, 보행능력, 보행영향 요인 등의 기초적인 연구가 전무하여 어려움을 겪고 있다. 이에 본 연구에서는 기존 논문 검토를 통해 보행능력(보행속도)을 일반, 건강, 운동요인 등 3개 분야로 구분하였다. 평균나이 72.6세의 고령자 52명을 대상으로 면접, 국민체력 100, SPBB, Cybex 등의 다양한 방법과 측정도구를 종합하여 3개 분야 총 12개의 세부 요인에 대한 측정을 시행하였다. 그 결과 일반요인인 연령, 체격 등은 보행속도에 미치는 영향이 적었으며, 건강요인 중에는 하지 신근, 운동요인 중에는 평형성, 협응성, SPPB가 가장 보행속도에 가장 많은 영향이 있음을 도출하였다. 이를 활용하여 고령자의 보행능력을 측정하는 보행자립도 모형을 개발하였다. 개발된 모형은 고령자의 보행능력의 진단하여 교통 분야 고령자 보행 연구의 기초자료로 활용될 수 있을 것으로 기대한다.

보행스피드에 대한 상체 공헌도의 연령에 따른 변화 (Age-Related Change of Upper Body Contribution to Walking Speed)

  • The purpose of this study was to investigate the effect of the upper body in order to increase a propulsive force in the old's walking. The subjects were each 10 males, the latter term of the aged and former term of the aged. There were three walking speeds of slow(about 5km/h), medium(about 6km/h), and maximum speed(about 7km/h). The subjects walking 11m were filmed the 5m section (from 3m to 8m) by 2-video cameras using three dimensional cinematography. And we computed different mechanical quantities and especially computed the relative momentum in order to achieve this study's aim. In this study, we was able to acquire some knowledge. The step length and step frequency increased in proportion to the walking speed, and the faster walking speed, the shorter ratio of supporting time( both legs supporting time/one step length time). When it was one leg support phase, the torso was indicated to generate the momentum in order to produce the propulsive force of walking. The upper and lower body had a cooperative relation for walking such as keeping step rate with the arms to legs and maintaining the body balance. The opposition phase for upward-and-downward direction of the torso and arms in walking was functioned to prevent the increase rapidly toward vertical direction of the center of gravity. The arms had contributed to coordinate the tempo of legs and the posture maintenance of the upper body. And by absorbing the relative momentum from the upper torso with arms to the lower torso, it had the rhythmical movement on upward-and-downward direction reducing the vertical reaction force. On account of the relations of absorption and generation of the propulsive force and the production of vertical impulse in the lower torso when walking by maximum speed, it was showed that the function of lower torso was come up as important problem for the mechanical posture stability and propulsive force coordination.
