낙상 경험 유무에 따른 여성 노인의 보행과 균형 능력에 관한 연구 (A Comparison Study of Walk and Balance Ability of Women Elderly with or Without Falls Experience)

  • Background: The elderly are likely to fall due to physical and mental atrophies, and experiencing falls may result in fear of falls and lack of self-confidence, which also leads to hesitation to physical activities and changes in walk and balance, the major variables in independent daily life. Methods: In three senior citizen centers located in D city, 22 elderly women aged 65 or older were chosen, and they filled in the questionnaire which included their agreement to voluntarily participate in the survey and medical histories. As to whether they had falls experience, the medical history items in the questionnaire asked them if they had falling down or falls once a year, twice for the three years. The objects were divided to NFE (non-falls experience) and FE (falls experience). Results: 1. As to walking abilities, significant difference was found between NFE and FE regarding walk width, stepping with two feet, and stride length while there was no significant different in terms of cadence. 2. As to balancing abilities, was significant difference between NFE and FE in terms of the physical body center area. As to the sit-to-stand, tandom gait test, no significant difference was found, and neither in the timed up and go test. Conclusions: The elderly with falls experience has inferior walking and balancing abilities to those without falls experience, and thus they are more exposed to the risks of falls.

Immediate Effect of Foot Drop Stimulator in Outpatients with Chronic Stroke: A Mixed Method Study

  • Background: The foot drop stimulator is designed to improve the walking ability of foot drop in patients after stroke, however, studies on clinical effects are still lacking. Objective: To investigate the effect of a foot drop stimulator on the walking and balancing abilities of foot drop patients after a stroke. Design: One-Group (Pretest-Posttest) Design. Methods: All subjects walked in all three conditions: foot drop stimulator (FDS) ankle foot orthosis (AFO) and barefoot. Primary outcome measures were assessed for walking and balance using a 10-m walking test (10MWT) and a timed up and go test (TUG). Secondary outcome measures consisted of a brief user interview, and the patients recorded the advantages and disadvantages of each condition. Results: FDS, AFO, and barefoot conditions showed a statistically significant difference in 10MWT and TUG (P<.001) as a result of comparing three conditions. FDS and AFO were significantly different from the barefoot condition as post-hoc results; however, there was no significant difference between the two conditions (P>.05). Conclusion: In this study, the foot drop stimulator contributed to improving the balance ability, and the walking ability was similar to the effect of the ankle-foot orthosis.

Relationships Between Cognitive Function and Gait-Related Dual-Task Interference After Stroke

  • Previous studies have reported that decreased cognitive ability has been consistently associated with significant declines in performance of one or both tasks under a dual-task walking condition. This study examined the relationship between specific cognitive abilities and the dual-task costs (DTCs) of spatio-temporal gait parameters in stroke patients. The spatio-temporal gait parameters were measured among 30 stroke patients while walking with and without a cognitive task (Stroop Word-Color Task) at the study participant's preferred walking speed. Cognitive abilities were measured using Computerized Neuropsychological Testing. Pearson's correlation coefficients (r) were calculated to quantify the associations between the neuropsychological measures and the DTCs in the spatio-temporal gait parameters. Moderate to strong correlations were found between the Auditory Continuous Performance test (ACPT) and the DTCs of the Single Support Time of Non-paretic (r=.37), the Trail Making A (TMA) test and the DTCs of Velocity (r=.71), TMA test and the DTCs of the Step Length of Paretic (r=.37), TMA test and the DTCs of the Step Length Non-paretic (r=.36), the Trail Making B (TMB) test and the DTCs of Velocity (r=.70), the Stroop Word-Color test and the DTCs of Velocity (r=-.40), Visual-span Backward (V-span B) test and the DTCs of Velocity (r=-.41), V-span B test and the DTCs of the Double Support Time of Non-paretic (r=.38), Digit-span Forward test and the DTCs of the Step Time of Non-paretic (r=-.39), and Digit-span Backward test and the DTCs of the Single Support Time of Paretic (r=.36). Especially TMA test and TMB test were found to be more strongly correlated to the DTCs of gait velocity than the other correlations. Understanding these cognitive features will provide guidance for identifying dual- task walking ability.

시각적 피드백과 리듬청각자극을 통한 트레드밀 보행훈련이 뇌졸중 환자의 보행능력에 미치는 영향 (Effects of Visual Feedback and Rhythmic Auditory Stimulation on Walking of Stroke Patients Induced by Treadmill Walking Training)

  • Background: Stroke patients show abnormal walking patterns due to brain injury. In order to have the desired walking pattern, appropriate stimulation is required to activate the central pattern generator. For this reason, our study performed treadmill ambulatory training with rhythmic auditory stimulation. However we did not consider the influence of visual feedback. Objects: The purpose of this study was to compare the gait abilities in chronic stroke patients following either treadmill walking training with rhythmic auditory stimulation and visual feedback (TRASVF) or treadmill walking training with rhythmic auditory stimulation (TRAS) alone. Methods: Twenty-one stroke patients were divided into two groups: A TRASVF group (10 subjects) and a TRAS group (11 subjects). They received 30 minutes of neuro-developmental therapy (NDT) and walking training for 30 minutes, five times a week for three weeks. Temporal and spatial gait parameters were measured before and after the training period. The Biodex gait trainer treadmill system measured gait parameters. Results: After the training periods, the TRASVF group showed a significant improvement in walking speed, the step length of the affected limb, and time on each foot of the affected limb when compared to the TRAS group (p<.05). Conclusion: The results of this study showed that the treadmill walking training with rhythmic auditory stimulation and visual feedback improved individual gait ability more than the treadmill walking training with rhythmic auditory stimulation alone. Therefore, visual feedback should be considered along with rhythmic auditory stimulation training.

리듬적 청각자극이 편마비환자들의 보행에 미치는 영향 (The Effects of Rhythmic Auditory Stimulation (RAS) on Hemiplegia Patient' Gait)

  • 본 연구는 신경학적 음악치료의 한 방법인 RAS가 편마비 환자의 회전과 직선보행 시 보행요소에 어떠한 영향을 미치는지를 살펴보고자 하였다. 연구는 음악치료소거와 개입의 차이를 4주간 비교하는 반복측정 설계로 디자인되었고 측정 보행요소에 대해 21가지로 세부 항목화 하였다. 연구의 대상은 보행훈련이 필요한 4명을 물리치료사에게 의뢰받았으며, 사용된 음악은 행진곡, 가곡을 중심으로 연구자가 선택하였다. 각 세션은 워밍업, RAS보행훈련, 마무리로 구성되었다. RAS보행훈련에서 사용된 음악들은 음악치료처치 전 측정한 각 환자의 본래 보행속도에 맞추어진 빠르기로 제공되었다. 속도는 점차적으로 빠르게 제공하였으며 각 세션이 50분간 진행되었다. 연구결과 음악치료 소거구간(A)보다 음악치료처치구간(B)에서 보행능력이 증가됨을 관찰할 수 있었고, 다른 치료가 음악치료와 병행될 때 환자들의 보행능력 향상에 더욱 긍정적인 영향을 준다는 선행연구를 지지하는 결과를 보였다. 또한 본 연구는 RAS음악치료처치가 직선보행 시 뿐만 아니라 회전보행 시 보행능력 향상에 도움을 준다는 것을 보여준다는 의의를 가진다.

Seafarers Walking on an Unstable Platform: Comparisons of Time and Frequency Domain Analyses for Gait Event Detection

  • Wearable sensor-based gait analysis has been widely conducted to analyze various aspects of human ambulation abilities under the free-living condition. However, there have been few research efforts on using wearable sensors to analyze human walking on an unstable surface such as on a ship during a sea voyage. Since the motion of a ship on the unstable sea surface imposes significant differences in walking strategies, investigation is suggested to find better performing wearable sensor-based gait analysis algorithms on this unstable environment. This study aimed to compare two representative gait event algorithms including time domain and frequency domain analyses for detecting heel strike on an unstable platform. As results, although two methods did not miss any heel strike, the frequency domain analysis method perform better when comparing heel strike timing. The finding suggests that the frequency analysis is recommended to efficiently detect gait event in the unstable walking environment.

Comparison of the Effects of Talus Stabilization Taping and Kinesio Taping on Balance and Walking Speed in Persons with Chronic Stroke

  • Objective: The purpose of this study was to compare walking speed and balance abilities according to various taping methodsin patients with stroke. Design: Cross-sectional study Methods: Twenty patients with stroke who were hospitalized at a rehabilitation hospital were allotted to the either the talus stabilization, Kinesio or sham taping, or barefoot conditions by drawing lots. Assessment tools used were the Functional Reach Test(FRT), Timed Up and Go test(TUG), One-Legged Stance Test (OLST), and the 10-Meter Walk Test(10MWT).After each test was measured three times, the mean values of each test was used for analysis. Results: Significant results were observed with thetalus stabilization and Kinesio taping groupcompared to the barefoot and sham taping method for theFRT,TUG, the OLST, and the 10MWT(p<0.05). Also, significant differences in the resultswere seen in the OLST, TUG, and the 10MWTwith the talus taping compared to the Kinesio taping method(p<0.05). Conclusions: The use oftalus stabilization taping applied to the ankle of patients with stroke was more effective for balance and walking ability improvement than Kinesio taping through the correction of an abnormal position of the talus.It is considered that these methods of taping can be applied effectively in the clinic.

고령자 보행 지원을 위한 지능형 6족 로봇 (Intelligent Hexapod robot for the support walking of the aged)

  • 본 논문은 시력이 떨어지고 보행이 불편한 고령자의 이동을 지원하는 지능형 6족 이동 로봇에 대한 연구이다. 센서 및 소형카메라를 장착한 6족 지능형 이동 로봇은 다양한 보행 및 회전, 이동 속도 조절 등의 이동 능력과 다양한 센서를 이용한 장애물 회피, 위협 상황 인식, 경사보행, 물체영역의 인식 및 추적 등의 기능을 가지며, 로봇에 연결된 선을 이용해 고령자의 보행을 유도한다. 로봇의 안정된 보행을 위한 모터의 제어방법, 센서 및 영상인식을 통한 로봇의 환경인식, 그에 따른 로봇의 지능적이고 능동적인 대응, 감지된 환경정보를 신속하게 무선통신을 통해 필요한 고령자 또는 보호자에게 전송하는 방법 등을 구현하였다. 한 다리에 3자유도를 가진 18관절 6족 로봇으로 구현되어 비평탄 지형에서도 안정된 보행구현은 물론 다양하고 부드러운 동작을 취할 수 있으며 강력한 구동 능력을 위해 디지털 슈퍼토크 서보모터를 사용하였고, 위험 상황 인식을 위해 3축 가속도 센서, PIR(초전)센서, 불꽃센서, 연기센서, 적외선, 초음파 거리감지 센서, 조도센서, 터치센서, 온도/습도 센서, 음성인식센서와 카메라를 통한 영상인식 등을 센서 융합에 의해 구현함으로써 상황인식의 정확도를 높이고 고령자들의 길안내 시 위험을 조기에 예측하여 전달하는 에이전트 역할을 수행하도록 설계되었다.

수중걷기운동이 여성노인의 보행 및 평형능력에 미치는 영향 (Effect of Aquatic Walking Exercise on Gait and Balance Parameters of Elderly Women)

  • Objective: The purpose of this study is to analyze the effects of aquatic walking exercise on gait and balance parameters of elderly women. Method: 15 elderly people were recruited for this study (age: 73.20±5.19 yrs, height: 153.87±3.36 cm, mass: 60.33±5.73 kg). All variables were measured using Gaitview AFA-50. The variables were the heel contact time ratio, gait angle, and M/P change ratio for gait patterns and ENV, REC, RMS, Total Length, TLC, Sway velocity, and Length/ENV for balance abilities. A paired t-test and the Wilcoxon signed-rank test were carried out to verify the differences in the test scores after participating in the water walking program. The significance level for all statistical analyses was set to α=.05. Results: As for the changes in their walking function after the exercise, heel contact time ratio (p<.01) showed a statistical significance, while gait angle and M/P change ratio did not reveal statistically significant differences. In the test of balance ability on both feet and with eyes opened, statistical significance was found in ENV, REC, RMS, TLC (p<.01), and sway velocity (p<.05), while the test with eyes closed showed statistical significance in length/ENV as well as ENV, REC, RMS, sway velocity (p<.01) TLC, and total length (p<.05). As for the single-leg stance balance ability, ENV and REC revealed statistically significant differences. Conclusion: These results show that water walking is effective for improving the function of the ankle flexor muscles, providing stability to the ankle joint during walking and helping efficient walk. In addition, it is also expected to help prevent falls due to loss of balance by improving the stability of lower extremity muscles and trunk.

척추관 협착증 환자의 보행능력과 요추 주변 근육 단면적의 상관관계 연구 (The Correlation between Cross-sectional Area of Lumbar Paraspinal Muscles and Walking Ability in the Patients with Lumbar Spinal Stenosis)

  • Objectives The purpose of this study was to investigate the correlation between walking ability of lumbar spinal stenosis patients and the cross-sectional area (CSA) of lumbar paraspinal muscles. Methods This study was carried out on 62 lumbar spinal stenosis patients who had limited walking abilities because of neurogenic claudication (NC). All patients received more than 2 weeks of complex treatment at Mokhuri Neck&Back Hospital. CSA of lumbar paraspinal muscles was measured from axial T2-weighted MRI and divided by CSA of adjacent vertebral body to avoid influence of body statues (RCSA-Relative CSA). Pain Free Walking Distance and Numerical Rating Scale (NRS) was measured before and after treatment. Results The Pain Free Walking Distance had significantly increased in patients who had bigger RCSA of psoas muscle (r=0.313, p<0.05). Conclusions The psoas muscle can be a predictive factor for restoring walking ability of lumbar spinal stenosis patients who have limitations walking.
