Walking test for assessing lung function and exercise performance in patients with cardiopulmonary disease (심폐질환 환자에서 걷기검사를 이용한 폐기능 및 운동기능의 평가)

  • BACKGROUND : Dyspnea is common among patients with cardiopulmonary disease, and "daily disability" is defined as a functional impairment resulting from exercise intolerance. The maximal oxygen uptake(VO2max) during exhausting work is not only the best single physical indicator of the capacity of a man for sustaining hard muscular work, but also the most objective method by which one can determine the physical fitness of an individual as reflected by his cardiovascular system. However, the expense, time and personnel requirements make this procedure prohibitive for testing large group. The walking test is well-known type of exercise and it cost nothing to perform and have good reproducibility. Thus we performed the walking test and investigated correlations with spirometry, ABG and exercise test. METHOD: We observed the walking test and exercise test by cycle ergometer in 37 patients who visited our hospital because of dyspnea. Arterial blood gas analysis and spiromety, dyspnea index were performed, too. RESULT : (1) The VO2max was significantly lower in patients with COPD and cardiovascular disease than asthma and dyspnea on exertion group(p<0.05). The walking test distance was also lower in former. (2) The 12 minute walking test was significantly correlated with VO2max, PaCO2, FVC(%), FEV1(%) in all patients(p<0.05), and the walking test was only conelated with VO2max in patients with COPD(p<0.05). (3) In COPD patients, the VO2max was best correlated with FEV1(%) and FVC(%) and significantly correlated with walking test. But there was no correlation between walking test and FEV1(%) & FVC(%). (4) The 6 minute walking test was well correlated with 12 minute walking test(r=0.92. p<0.01). CONCLUSION : The walking test is the simple method for assessing exercise performance in patient with cardiopulmonary disease and a reliable indicator for VO2max. And the walking test is practical method for assessing on everyday disability rather than maximal exercise capacity. The 6 minute walking test is highly correlated with 12 minute walking test and a less exhausting for the patients and a time-saving for the investigator.

A Study on User Behavior and Satisfaction with Neighborhood Parks within Walking Distance with Consideration for Interior and Exterior Environments - Focusing on the Case Study Hwarang and Gwanum Park, Daegu - (도보권 근린공원의 내·외부 환경을 고려한 이용행태 및 만족도에 관한 연구 - 대구광역시 화랑공원과 관음공원을 대상으로 -)

  • Recently, having neighborhood parks within walking distance has grown in importance. The purpose of this study is to analyze the effect of user satisfaction with neighborhood parks within walking distance considering interior and exterior environments. To do so, a field survey and GIS were conducted to construct data which were then compared with result of the analysed environment. Finally, amultiple regression analysis was conducted to confirm impact on user satisfaction of environment. By summarizing the study results, it was found that users of Hwarang Park exhibited a high level of satisfaction with 'park facilities' and 'safety of park use'. In the case of Gwanum Park, users exhibited a high level of satisfaction with 'green space' and 'amount of shade'. On the contrary, two park users exhibited low levels of satisfaction with 'facilities for children' and 'various attractions' within the parks. The pedestrian environment of Hwarang Park was rated higher than Gwanum Park within the park service area. User satisfaction was also rated higher than for Gwanum Park. However, two park users exhibited low levels of satisfaction with 'various attractions' within the pedestrian environment. From the result regression analysis of the total satisfaction factors, 'environment of access route', 'park facilities' and 'space for walking' positively influenced park use satisfaction. It was found that improvement of the pedestrian environment would be more effective than the improvement of the internal environment on Hwarang Park. This study investigated correlation with the access road environment as well as the interior environment of the parks. The results of this research will be used to improve accessibility and availability for neighborhood parks within walking distance.

Road Safety Message Providing Methodology for Considering the Elderly Walking Behavior (고령자 보행행태를 고려한 상충위험정보 제공방법 설계)

  • In this paper, we introduce a warning message providing methodology by considering the elderly's walking behavior in V2P(: Vehicle-to-Pedestrian) communication systems. Using the elderly's accident records in Kyounggi-do area from 2011 to 2014, the elderly's accident behaviors are analyzed by categorizing accidents as road type (single road/ crossing road), existence of traffic signal (with signal/ without signal), existence of crosswalk (crosswalk/ jaywalk), cause of accident (jaywalk/ signal violation/ carelessness accident/ Etc.). In addition, the elderly's behavior variable and vehicle speed are applied, then some threshold values are utilized in a warning message providing in conflict area. Finally, we propose a distance which can protect an accident depending on the elderly's walking speed, safety margin and vehicle speed. By applying the results of this research to the V2P/P2V communication based road safety message providing system with the elderly's walking behavior, it may be helpful to reduce accident rate of the elderly.

  • The following study reviewed the walking patterns of stroke patients with hemiplegia, which is called hemiplegic gait of stroke patients. Focusing is given to the changes in the distance and temporal factors of walking, which is called spatiotemporal characteristics, throughout the walking cycle. First, we introduced the definitions of essential terms related to gait and its measure. Second, we reviewed the spatiotemporal characteristics of hemiplegic gait. A main issue was that hemiplegic gait showed significant deviations from normal healthy gait. Although hemiplegia is primarily associated with unilateral motor disorder, changes in almost all spatiotemporal parameters used to assess walking were evident on both the involved and uninvolved sides of the body. Last, we reviewed the changes of spatiotemporal parameters of hemiplegic gait according to the prognosis or status of stroke patients, which may help to give a specific intervention for rehabilitation of stroke.

  • In recent years, over 100 trails were created in Korea, and Jeju Olle and Jiri Mt. Doolle trail are the most familiar and successful trails in Korea. The research aimed to find relationship between fair tourism and long distance trails. In the first step, we investigated the concept of fair tourism, and deducted 8 key-words; 1)local participation 2)resource sharing 3)provision of unique social, cultural resources 4)sovereignty of local culture 5)small_scale, low impact, eco-friendly 6)sustainable infrastructure 7)revitalize local community 8)comprehensive accommodation of a diverse tourists. Through the analysis of 3 representative sites of long distance trails, we tried to find out the distinct characteristics of fair tourism of the selected sites with evaluating criteria that was provided by UNEP and related organizations. The finding of the study could contribute to defining concept of fair tourism, and by doing so, attribute in setting up a framework of designing and planning of long distance trail under the concept of fair tourism that diverse parties could co-benefit, including local community, local government and tourists.

The exercise-distance measuring system with high precision considering of altitude (고도를 고려한 정밀도 높은 운동거리 측정시스템)

  • To measure the athletic information of exercisers, the applications of smartphone are programmed based on the sensing data from GPS device. These applications provide exercisers for running or walking distance, exercising time, calorie consumption, average speed, and so on. Among them, the exercising distance should measure accurately because it directly affects the other athletic information for exercisers. However, the existing methods for measuring the exercising distance makes errors because they are worked on the simple sphere or ellipse earth models. Actually, the surface of real earth is composed of inclined ground like hills and valleys. In this paper, a new exercising distance measuring algorithm is proposed to compensate the errors of existing method. It considers the altitude of slopes in exercising routes. To evaluate exercising distance measuring algorithms, we implement the athletic life-guide system based on the smartphone platform. In experiments, the proposed method shows that it provides more accurate distance measurement.

  • In this paper, we implemented the electornic moving aid system for safe walking of the blind. An obstacle detecting of each sector used ultrasound and a distance measurement used time of flight. The alarm is designed to have a sound and a tactile function that can be selected on an user's convenience. This system can detect and obstacle of upward, forward, downward and optimally warn to the blind with vibration, beep sound by appling warning algorithm on object detection. Experimental testing and performance evaluation have been successfully carried out with a prototype cane, and the experiment shows the capability of the function to detect unknown objects within an assigned distance, under knees, over head height, and crushed puddles.

Load Carrying Effect on the Biomechanical Parameters of Slips and Falls (미끄러져 넘어짐의 생체학적 연구에 있어서 부하이동이 끼치는 영향)

  • The biomechanical analysis of the load carrying effect on different floor surfaces has been conducted. Four different floor surfaces were prepared for ten subjects with each walking at a fixed velocity(1.33 m/sec) while carrying five different loads. The results showed that because of the significant interaction effect between floor slipperiness and the load carrying task, the load carrying effect should be analyzed according to different levels of the floor slipperiness, especially contaminant floors. On oily surfaces, slip distance(SD) and heel velocity (HV) increased whereas stride length(SL) decreased as load increased. In other words, significantly longer SD, faster HV, and no normal gait were found as load increased. As a result, a different protocol should be applied to measure floor slipperiness on oily floors as compared to dry surfaces for tribological approach.

  • The purpose of this study is to carry out the quantitative and qualitative analysis of an urban park in the landscape ecological view and to provide the important basic data in the urban development process. The results of this study are as follows. First, in the case of disposition distance analysis in quantitative plans, very necessary regions of the children park are turned out to be 24 sites, of the neighboring park are turned out to be 30 sites, and walking park are turned out to be 22 sites. Second, in the case of undevelopment neighborhood park analysis, priority orders are Daebong Park, Suseong Park Third, in the case of area, shape, and isolation in qualitative plans, interior area of Bummul park is larger than that of Chimsan park and isolation of Bummul park higher than that of Chimmsan.

Korean Medical Treatment for Improving Symptoms of Four Patients Diagnosed with Grade II Lumbar Spondylolisthesis

  • Objectives : To investigate the clinical results of patients who were diagnosed with grade II spondylolithesis treated with Korean medicine of relaxative Chuna and Gangchuk herbal medicine. Methods : We reviewed medical records of four patients diagnosed with grade II spondylolisthesis. Each patient has taken intensive Korean medical admission treatment at Mokhuri Hospital. All patients received treatments of Gangchuk herbal medicine, relaxative Chuna and acupuncture during admission period. Before treatment and every after 7-day treatment the patient's pain and walking distance without pain were assessed. The zero minimum to ten maximum pain numeric rating scale(NRS) was used to determine the degree of pain. Results : Average admission duration was 23 days. All the patients' NRS decreased from median figure of 7.5[the minimum 7 to the highest 8] to median figure of 1[the minimum 0 to the highest 2]. Walking distance without pain increased from a median of 55 m[the minimum 20 m to the highest 100 m] to median of 165 m[the minimum 100 m to the highest 250 m]. Conclusions : Intensive conservative korean medical treatment was effective in reducing pain and functional improvement for grade II lumbar spondylosisthesis patients.
