Association between serum alanine amino­transferase level and obesity indices in Korean adolescents

  • Purpose: To analyze the correlation between serum alanine aminotransferase (ALT) and obesity indices including body mass index (BMI), body fat percentage (BFP), total fat mass (FM), truncal fat mass (TFM), waist circumference (WC), and waist-to-height ratio (WHtR) in Korean adolescents. Methods: This was a cross-sectional study based on data derived from the 2010-2011 Korean National Health and Nutrition Examination Surveys (KNHANES). Subjects were Korean adolescents aged 10-18 years (871 total; 475 boys and 396 girls) who participated in KNHANES. Results: In both sexes, BMI, FM, TFM, WC, and WHtR were higher when ALT levels were in the 4th quartile. In boys, there was a significant positive correlation between ALT level and BMI, BFP, FM, TFM, WC, and WHtR (r=0.55, P<0.0001 for BMI; r=0.52, P<0.0001 for BFP; r=0.58, P<0.0001 for FM; r=0.61, P<0.0001 for TFM; and r=0.56, P<0.0001 for WC; r=0.62, P<0.0001 for WHtR), and the correlation coefficient was higher than that in girls. Conclusion: Our results suggest a significant positive association between serum ALT level and obesity indices in male adolescents.

담석증 환자의 식생활양식 및 영양 실태 조사 (A Descriptive Study of Gall Stone Patient's Dietary Habits and Nutritional Status)

  • Gallstone composition has changed over the past decades in the Korean with a prominent increase in the prevalence of cholesterol gallstone. This trend is possibly due to the westernization of dietary habits. The purpose of this study was descriptive of GB patient's health related eating behaviors and nutrient consumption patterns. One hundred and six gallstone patients who have had cholecystitis surgery enrolled in this study. Anthropomertic indices, such as height, weight, waist circumference, and hip circumference were measured and calculated BMI and WHR. As the biomarker, TC, TG, LDL-C, HDL-C, FBG level and SBP/DBP were measured and analyzed the relationship with GB stone formation. The structured checklist of health related eating behavior and the semiquantitative food frequency questionnaire after pre-test was used in the face to face interview study. The mean age was $54.9{\pm}13.3$ and gallstone disease was more frequent in the fifties and sixties. Mean BMI was $24.3{\pm}2.8$ in males, and $23.4{\pm}3.9$ in females, the average of waist circumference was $91.1{\pm}7.0cm$ in males and females were $85.4{\pm}9.6cm$. The WHR of men and women was $0.93{\pm}0.0,\;0.90{\pm}0.1$, respectively. The obesity and overweight trend was observed in gallstone patients. The mean blood sugar was researched at $114.5{\pm}35.0mg/dL$. And the ratio of both symptoms DM and gallstone was 26.4%. The rate of dramatic eating repast was significantly higher in the females (p<0.01). Males tended to more frequently consume meat, of high fat content meats and greasy food consumption when eating out. The CPF ratio was 57:16.5:26.5. There was a significant positive correlation between WC and energy (r=0.252, p<0.05), carbohydrate (r=0.255, p<0.05) and niacin (r=0.227, p<0.05). In addition, carbohydrate were significantly correlatied with TC (r=0.230, p<0.05). BMI appeared positive in correlation of protein (r=0.201, p<0.05) and fat (r=0.205, p<0.05). These findings provide a little association that dietary habits are related with cholesterol gallstone formation.

대구.경북지역 20, 30대 남성 근로자의 연령 및 BMI에 따른 건강상태 (The Health Status according to the Age and BMI of Male Workers in Daegu.Gyeongbuk Region)

  • 본 연구에서는 대구 경북지역 20, 30대 남성근로자 224명을 대상으로 BMI에 따른 신체적 특성, 건강상태를 조사하였다. BMI는 20대 $23.67\;kg/m^{2}$, 30대 $23.95\;kg/m^{2}$로 조사대상자 모두 대한비만학회에서 제시한 정상 범위인 $18.5{\sim}23\;kg/m^{2}$에서 벗어난 과체중으로 조사되었다. BMI에 따른 신체적 특성은 체중, 이상체중비율, BMI 모두에서 유의적인 차이(p<0.001)를 나타냈다. 그리고 신체둘레 측정 결과 허리 둘레, 엉덩이 둘레, 허리/엉덩이 둘레비는 비만군, 과체중군, 정상군 순으로 유의적인 차이(p<0.001)를, 허리/엉덩이 둘레는 30대가 20대보다 크게(p<0.01)나타났다. 체구성성분 측정 결과 체지방률{body fat(%)}과 체지방량{body fat(kg)}은 비만군, 과체중군, 정상군 순으로 유의적인 차이(p<0.001)를 보였다. BMI가 증가함에 따라 관리요망군과 질환의심군의 비율이 유의적으로(p<0.001) 증가하였고, 평균 혈압은 비만군이 정상군 및 과체중군보다 높은 유의적인 차이(p<0.001)가 있었으며, 공복시 혈당은 30대가 20대보다 높은 유의적인 차이(p<0.05)를 보였다. 간질환의 유무를 알 수 있는 지표인 SGOT, SGPT 및 $\gamma-GTP$는 BMI별로 보면 비만군은 정상군 및 과체중군보다 높아 유의적인 차이(SGOT, SGPT; p<0.001, $\gamma-GTP$; p<0.01)를 보였으며 비만군의 SGOT는 43.58 U/L로 정상범위에서 벗어났다. $\gamma-GTP$를 연령별로 보면 30대가 34.74 U/L로 정상군에는 속하지만 20대의 28.06 U/L보다 높은 유의적인 차이(p<0.05)를 보였다. 건강상태와 음주 및 흡연 유무에 따른 생화학적 상태를 살펴보면, 건강상태에 따른 수축기혈압은 관리요망군, 질환의심군, 정상군 순으로 유의적인 차이(p<0.01)를 보였으며, 확장기혈압, 총콜레스테롤, 공복 시 혈당, SGOT, SGPT 및 ${\gamma}-GTP$는 정상군 및 관리요망군과 질환의심군 간에 유의적인 차이(p<0.001)를 보였다. $\gamma-GTP$는 음주 근로자가 33.79 U/L, 비음주 근로자가 26.64 U/L로 집단 간의 유의적인 차이(p<0.05)를 보였고, 수축기 혈압과 헤모글로빈은 각각 흡연 근로자가 124.65 mmHg, 15.82 g/dL이고, 비흡연근로자는 120.43 mmHg, 15.38 g/dL로 나타나 집단 간의 유의적인 차이(p<0.01)를 보였다. 체중이 많이 나갈수록 BMI, 이상체중비율(%), 허리둘레 및 엉덩이둘레, 허리/엉덩이 둘레비, 체지방량, 체지방률(%)이 높으며, 이에 따라 수축기혈압과 확장기혈압, 총콜레스테롤, 공복 시 혈당, SGOT, SGPT, $\gamma-GTP$ 모두 높아진다(p<0.001, p<0.01, p<0.05). 이상의 결과를 종합해보면, 조사대상자인 20, 30대 남성 근로자 대부분이 과체중에 속하며, 체중이 증가할수록 BMI, 허리둘레, 엉덩이둘레, 체지방률(%) 등이 증가하고 BMI가 높아질수록 수축기혈압과 이완기혈압, SGOT, SGPT, $\gamma-GTP$ 등의 생화학적 수치들이 높아져 건강상태가 좋지 않게 된다. 음주와 흡연 역시 혈압, $\gamma-GTP$ 등을 높여 건강상태 악화에 한 몫을 하는 것으로 나타났다. 따라서 20, 30대 남성근로자들의 연령 및 비만도의 증가에 따라 건강상태가 나빠지며 성인병 유병률의 가능성이 높아짐을 인식하고, 이러한 건강 위험요인들을 개선할 수 있는 노력이 필요할 것이라 사료된다. 정기적인 건강검진과 더불어 규칙적인 운동과 음주, 흡연, 업무 과중, 스트레스와 같은 생활습관 및 불규칙한 식습관을 개선할 수 있는 방안의 모색이 요구된다.

중년 남성의 식생활행동과 혈청 인슐린 및 지질성분과의 관계에 관한 연구 (The relation of the Eating Behavior and fasting serum Insulin and Lipids in Middle-aged Men)

  • The purpose of this research is to investigate how obesity and eating behavior are related to the concentration of serum insulin and lipids and blood pressure of middle-aged men The sample for this study utilizes 240 middle-aged men between th4 age of 40 and 60 living in the city of Kumi. Through a process of anthropometry and categorization by relative body weight, the sample is devide into two groups, the obese group (51 men) and the normal group (129 men). The results of the study were as follows : 1111111. The men from the obese group with the exception of height, subs-capular/triceps skinfold girth ratio(STR) had a higher level of body mass index (BMI), waist and hip circumference, waist/hip girth ratio(WHR), subscapular and triceps skinfold thickness than the normal group (P=0.0001). 2. In comparing the serum insulin and lipids between the obese and normal group, the obese group had a higher level of insulin and triglyceride (P=0.016, P=0.050) but a lower concentration of HDL-cholesterol and HDL-cholesterol/total cholesterol (P=0.034, P=0.004). Also, the obese group had a higher level of systolic and diastolic blood pressure (P=0.001, P=0.029). When looking at the relationship between the serum insulin and lipids and anthropometric measurements, the relationship between waist/hip girth ratio(THR), the concentration of fasting serum insulin and lipids were stronger than the obesity index, body mass index(BMI). 3. Men who exercised regularly had a lower total cholesterol and systolic blood pressure 9P=0.049, P=0.041), and a higher level of HDL-cholesterol/total cholesterol (P=0.004). There was no observable relationship between the food habit score, the preference for fiber foods, and the concentration of serum lipids. Also, the concentration of serum lipids had no apparent effect on the preference for salty, sweet, and greasy foods.

복부비만 노년 여성의 의복패턴설계를 위한 체형연구 (An Analysis of Body Shapes in Aged Abdominal Obese Women for Apparel Pattern Design)

  • 본 연구는 복부가 비만한 노년 여성의 의복의 패턴설계를 위한 기초 자료를 제공하기 위한 목적으로 수행되었으며 이들의 체형특성을 분석하고 일반 노년 여성과의 상반신 및 하반신 체형특성의 차이점을 파악하고자 하였다. 연구대상은 60세 이상 서울 및 서울 근교에 거주하는 노년 여성 318명-복부비만 251명, 일반인 67명-이며 직접측정을 실시하였다. 복부비만의 기준은 허리엉덩이둘레비(WHR)가 0.85 이상인 피험자로 하였다. 높이항목, 두께항목, 너비항목, 길이항목 및 몸무게로 구성된 총 33개 측정치 및 측정치를 토대로 한 계산치 및 지수치를 이용하여 기술통계분석, 상관분석, T검정을 실시함으로써 이들의 복부돌출요인에 따라 의복구성에 고려해야 할 체형특성을 파악하고, 이를 일반 노년 여성과 비교분석하였다. 연구결과 복부비만 노인은 일반 노인보다 높이항목과 어깨너비 등 어깨관련항목을 제외한 두께, 둘레, 길이 항목에서 유의한 차이를 보이며, 로러지수와 버벡지수, 체간부의 편평률에서도 유의차를 보여 전체적 인 비만도가 높으며 체간부 형태가 원통형을 나타낸다. 또한 복부비만 노년 여성의 경우 상관관계 분석결과 엉덩이둘레보다 배둘레나 엉덩이외포둘레가 패턴 설계에 필요한 주요 항목들과 더 높은 선형적 상관성을 보이므로 패턴의 기준항목 설정시 이를 고려하여 제작하여야 할 것이다.

30대 - 50대 성인남성의 상반신 신체 치수 및 체형 유형 변화 연구 (A Study on the Changes in Upper Body Sizes and Body Types of 30-50's Adult Men)

  • This study aimed to provide basic data for establishing a sizing system for menswear by investigating body size changes and body shapes in adult men in their 30s to 50s. Data on 52 different upper body categories collected from 4,314 people during the 5-7th Size Korea surveys were analyzed. The waist height, hip height, torso and arm lengths, and torso and upper limb circumferences increased while the arm joint sizes decreased. According to factor analysis, factors 1 and 2 referred to 'the horizontal size of the upper body and limbs' and the 'torso height and upper limb length', respectively, while factors 3 and 4 represented 'shoulder size' and 'upper body length' respectively. In clustering analysis, types 1 and 2 were 'short and fat' and 'tall and thin,' respectively, while types 3 and 4 represented 'tallest and chubby' and 'shortest and small' respectively. Torso length increased in the 30 to 40s in type 1 while torso circumference increased in the 30s and late 40s in type 2. In type 3, shoulders got bigger in the 30s. In type 4, torso circumference increased in the early 30s, and torso length increased in the late 40s. 'Thin' was a typical body shape in the early 30s. Among early 40s men, distribution evolved from 'small' to 'thin.' In the late 40s, while the percentage of 'small' decreased, the ratio of 'obese' increased. In the 50s, 'small' was the most common.

Comparison of Waist-to-height Ratio (WHtR), Body Mass Index (BMI) and Waist Circumference (WC) as a Screening Tool for Prediction of Metabolic-related Diseases

  • The present study showed WHtR to be significantly better than BMI and WC for prediction of metabolic-related diseases in the middle-aged and older people in Korea, based on Bayesian ordered probit model analysis. The variations of WC, BMI and WHtR were compared according to the number of metabolic-related diseases such as hypertension, dyslipidemia, stroke, myocardial infarction, angina pectoris and diabetes. It was found that the three measures showed the similar variation except a very few extreme cases for age less than 40. For subjects over the age of 40, WC was not significant and WHtR gave more influence in greater variability than BMI on the number of metabolic diseases. Also, the rate of change for WHtR was higher than for BMI as the number of metabolic-related diseases increased. Specifically, the difference of the marginal effect of WHtR between no disease and only one disease was 1.81 times higher than that of BMI. Moreover, it was pointed out that the threshold value of WHtR for obesity should be considered differently by age.

인슐린 비의존형 당뇨병 남성 환자의 영양소 섭취량, 혈중 지질 및 비만도에 관한 연구 (Study of Nutrient Untake, Blood Lipids, and Obesity in Non Insulin Dependent Diabetes Mellitus Male Individuals)

  • This study was an attempt to investigate various factions, such as the calorie, nutrient intake, physical activity, blood lipids, obesity prevalence and body fat distribution on NIDDM male diabetics. General characteristics, physical activity and exercise levels of subjects were invesigated by interviewing, daily calorie and nutrient intake were measured by convenient method. The following anthropometric measurements were made on all participats : weight, height. Also waist and hip circumference were measured on 174 male diabetics to get waist-to-hip circumference ratio as index of the body fat distribution. For measurement of plasma lipids, 12-hour fasting blood samples were drawn The results of this study were summarized as follows : 1. At the onset of diabetes, the major self-diagnosed symptoms were polydipsia, fatigue, and body weight redution 2. The average of daily energy intake of male diabetics was 2106 Kcal which is 96% of the RDA Percentage of energy is that carbohydrate:protein : pat=70:14:16. 3. Among the NIDDM male subjects, 59% was exercise regularly. 4. Obese subjects above in the ideal body weight of 120% are presently 17%, but 39% of subjects were reported to be obese in the past. The mean BMI of the male NIDDM diabetics is 23.3${\pm}$2.6 and the past mean BMI was 25.2${\pm}$2.7 The mean WHR was 0.93${\pm}$0.10. 5. When diabetics were divided into obese and nonobese group according to RBW, energy intake, blood pressure, blood glucose and total cholesterol were not significantly different between the two groups, but LDL and VLDL-cholesterol were significantly higher in the obese group. 6. RBW did not correlate with cholesterol and triglyceride, but WHR correlated significantly with cholesterol and triglyceride. In conclusion, these results from a present study support previous findings indicating that not only the degree of obesity but also the localization of fat is a risk factor for diabetes.

농촌 폐경 후 여성의 비만 지표에 따른 비만율 비교 및 혈중 지질과의 상호관련성 (Correlation of the Rate of Obesity and Blood Lipids According to Obesity Index in Rural Post-menopausal Women)

  • This study was performed to examine the relationship between the body mass index (BMI), the body fat, and the serum lipids of post-menopausal women in rural areas. The subjects were 510 women aged 50 and over. As a result of this study, we found a trend of decreasing BMI as age increased, but body fat increased. In addition, there was a significant decreasing of the lean body mass than an increasing of the body mass index according to increasing age. Therefore, this study confirmed that a main cause of rural women being classified as obese is a decrease in lean body mass, rather than an increase in of body fat. Of all subjects, 36.3% ($18.5{\leq}BMI$ < 23) of all subjects were classified as having normal BMI, whereas only 21.4% were classified as having normal body fat. Out of 190 subjects who were body fat 30% and over, 38 subjects were classified as obese ($BMI{\geq}25$) and 113 subjects were classified as overweight ($23{\leq}BMI$ < 25). The percentile of those with a BMI of $25kg/m^2$ was 70, and they had 30.82% body fat. HDL cholesterol showed a negative correlation with anthropometric factors (height, weight, body mass index, waist circumference, waist hip ratio, body fat), and total cholesterol, LDL cholesterol, and triglycerides showed a positive correlation. Especially, total cholesterol, LDL cholesterol, and hip circumference showed significant correlations. Because of differences in the body fat and lean body mass by age group, it seems difficult to assess obesity via BMI only. The elderly especially should have a higher significance placed on body fat or abdominal fat than only BMI.

씨름 선수와 규칙적 중등도 및 간헐적 저강도 운동실천자의 체격지수, 영양소 섭취 및 혈중 지질의 비교 연구 (Comparison of Anthropometric Measurements, Dietary Quality and Blood Lipid Levels in Ssireum Players, Regular Moderate and Intermittent Light Exercisers)

  • Ssireum, the traditional Korean sport, is very popular at both amateur and professional levels. Ssireum players are prone to be obese which related to the chronic disease in their later life. The purpose of the study was to compare the anthropometric measurements, quality of diet, and blood parameters of Ssireum players with those of subjects who were matched body mass index, gender and age. Participants in the 3 groups, Ssireum players (SP, n=15), regular moderate exerciser (RME, n=15, >3 times /wk, >20 min/time) and intermittent light exerciser (ILE, n=14) groups. Anthropometric measurements included height, weight, fat mass (by Dual energy X-ray absorptiometry), lean body mass, triceps and thigh skin-fold thickness, mid-arm, waist, hip and thigh circumference. Dietary assessments were accomplished using 3-days food records, diet quality index (DQI), dietary variety score (DVS). Blood levels of lipids, leptin and insulin were analyzed. As a results lean body mass and mid-arm circumference were significantly higher in Ssireum players than those of other groups (p<0.01). Total body fat, trunk fat, abdominal skin fold thickness and waist-hip ratio were significantly lower in SP group than those of other groups (p<0.01). DVS were higher however, dietary quality was low in Ssireum players than in other groups. There were no differences among the 3 groups in regard to blood total cholesterol, LDL-cholesterol and glucose levels however, leptin level was low in Ssireum players. These results indicate that Ssireum players had significantly higher lean body mess and lower body fat when these were compared with regular moderate exerciser and intermittent light exerciser. Blood leptin levels of Ssireum players were low but blood lipid profiles were not significantly different.
