전기근육자극 훈련이 복부비만 중년 여성의 복부지방, 체간 근 두께와 활성도에 미치는 영향 (Effects of Neuromuscular Electrical Stimulation Training on Abdominal Fat, Trunk Muscle Thickness and Activity in Middle-Aged Women with Abdominal Obesity)

  • PURPOSE: This study examined the effects of low frequency neuromuscular electrical stimulation (NMES) training on abdominal obesity in middle-aged women through electromyography and ultrasound. METHODS: Twenty-two middle aged women with abdominal obesity participated in the study. A low-frequency NMES device was used on the abdomen and waist of each subject for 20 minutes each (a total of 40 minutes) three times a week for eight weeks. The waist-hip ratio (WHR), weight and BMI (Body Mass Index) were measured. Electromyography (EMG) and ultrasound measurements were performed three times in total (pre-intervention, four weeks into the intervention, and eight weeks post-intervention) to examine the effects of low-frequency NMES on the abdominal muscle activity, muscle thickness, and subcutaneous fat. RESULTS: The results indicated a difference in the WHR and waist circumference before and after intervention (p<.05). The external oblique muscle (EO) showed a significant increase in muscle activity during all measurements taken post-intervention (p<.05). The abdominal subcutaneous fat thickness also showed a significant decrease between each measurement (p<.05). The test results showed that the abdominal subcutaneous fat thickness values taken eight weeks post-intervention were significantly lower than those taken pre-intervention and four weeks into the intervention (p<.05). CONCLUSION: These findings show that low-frequency NMES device training can be applied to middle-aged women with abdominal obesity to improve their body shape and exercise performance.

고령남녀의 체형특성 연구 -다양한 비만지수의 적용을 중심으로- (A Study on Body Shape Characteristics of Elderly Men and Women -Focusing on the Application of Various Obese Indices-)

  • This study examines the multilateral characteristics of elderly body shapes by applying body measurement items and obesity indices (BMI, WHR, WHtR, WC, and flatness). The subjects were 397 males, 378 females aged 70-85 from the $6^{th}$ Size Korea and 132 males, 212 females aged 60-69, 596 males, 650 females aged 20-29 from the $7^{th}$ Size Korea. In order to examine the body size differences among age groups, age was divided into 7 groups (20-24, 25-29, 60-64, 65-69, 70-74, 75-79, and 80-85). Both elderly males and females showed lower values of height and weight than those in their 20s. However, abdominal obesity indices (WHR and WHtR) showed higher values in the elderly than those of their 20s; in addition, they showed larger mean values as age increased. The chest and arm circumference tended to decrease in elderly males as age increased; however, elderly females tended to decrease in thigh and calf circumference size. There were gender differences in body changing due to aging. Obesity frequency was significantly higher in the elderly than in the 20s. The study represents basic data for product design for the elderly.

Genetic Variants of CYP11B2 and CYP1A1 Among the North-Indian Punjabi Females with Polycystic Ovary Syndrome

  • Polycystic ovary syndrome (PCOS) is a complex endocrinopathy in women of reproductive age. The genetics of PCOS is heterogeneous with the involvement of number of genes in the steroid synthesis pathway. The CYP11B2 encodes aldosterone synthase and the genetic variants might increase aldosterone secretion in PCOS cases. CYP1A1 is known to enhance the intraovarian catechol estrogen production and thus the propensity for PCOS. The present case-control study analyzed a total of 619 females for CYP11B2 (rs1799998) and CYP1A1 (rs4646903) polymorphisms. Obesity was examined according to body mass index (BMI) and waist hip ratio (WHR) categorization. Biochemical (lipid profile) analysis was performed in PCOS females. BMI (P=0.0001) and WHR (P=0.0001) revealed a statistically significant difference between PCOS cases and controls. The overall levels of triglycerides were higher in PCOS females. The genotype frequency distribution of CYP11B2 (rs1799998) polymorphism revealed statistically significant difference between PCOS cases and controls (P=0.017). However, CYP1A1 (rs4646903) polymorphism did not showed any association with PCOS. The present case-control association analysis is first from our region for CYP1A1 and CYP11B2 polymorphisms and is suggestive of genetic predisposition of steroidogenic genes among PCOS patients in the North-Indian Punjabi females.

서울 지역 중.노년 여성의 골밀도와 식생활 태도 (Bone Density and Food Behavior of Middle- and Old-Aged Korean Women)

  • This study was conducted to investigate bone density and food behavior of middle-aged (<65-years-of-age) and oldaged women (${\geq}$65-years-of-age) in the Seoul area of Korea. Average height and weight, body mass index, body fat percentage, and waist-hip ration (WHR) of the middle-aged participants were 158.0 cm, 58.4 kg, 23.3, 29.7%, and 0.88. Corresponding measurements for old-aged participants were 152.6 cm, 56.3 kg, 24.1, 33.1%, and 0.93. T- and Z-scores for middle-aged women were -1.20 and -0.21, and those of old-aged women were -2.02 and -0.49. Osteopenia and osteoporosis percentage of middle- and old-aged women were 41.2% and 14.7%, and 45.2% and 37.1%, respectively. Significant differences were evident concerning between-meal, overeating, exercise, and nutritional supplementation among the two groups. Middle-aged women displayed a positive relationships between bone density and the parameters of height (p<0.05), weight (p<0.05), exercise (p<0.05), consumption of soybean and soybean-processed food (p<0.05), and negative relationships between bone density and the parameters of WHR (p<0.001), consumption of meat and meatprocessed food (p<0.05), and tea drinking (p<0.05). In old-aged women group, positive relationships were evident for bone density and the parameters of height (p<0.05), weight (p<0.05), exercise (p<0.05), nutritional supplementation (p<0.05), and consumption of soybean and soybean-processed food (p<0.05). The results reveal a link between healthy bones and desirable food behavior, indicating the importance of a practical and systematically organized food behavior education program for middle- and old-aged Korean women.

Relationship of sodium index with the obesity indicators of university students in Daegu, South Korea: a cross-sectional study

  • Objectives: The sodium index is an index that converts the estimated sodium intake calculated using a verified and reliable sodium estimation formula. This study aimed to determine the relationship between the sodium index and obesity indicators and the potential impact of excessive sodium consumption on obesity. Methods: Obesity indicators, such as body mass index (BMI), body fat percentage, waist-to-hip ratio (WHR), and visceral fat levels, were analyzed in 120 university students (60 men and 60 women). The sodium index was calculated by indexing the estimated sodium intake according to age, sex, BMI, salt-eating habits, and salt-eating behaviors. The relationship between sodium index and obesity indicators was analyzed using multiple logistic regression. Results: The estimated sodium intake was 3,907.1 mg, with 76.7% of the participants categorized under the "careful" level of sodium index and 10.8% under the "moderate" level. As the sodium index increased, the BMI, body fat percentage, WHR, and visceral fat levels significantly increased. All obesity indicators significantly increased in patients with a "severe" sodium index than in those with a "moderate" sodium index. In addition, a strong positive correlation was identified between obesity indicators and sodium index. When the "severe" sodium index was compared with the "moderate" sodium index, the risk of obesity based on body fat percentage increased by 2.181 times (95% confidence interval [CI], 1.526-3.118), while the risk of obesity based on visceral fat level increased by 4.073 times (95% CI, 2.097-7.911). Conclusions: Our findings suggest a correlation between excessive sodium intake and obesity. Moreover, the sodium index can be used to determine sodium intake.

퇴행성 슬관절염과 비만과의 상관성에 관한 임상 연구 (The Clinical Study on Correlation between Knee Osteoarthritis and Obesity)

  • Objectives : The purpose of this study is to investigate the correlation between assessment measurements of knee osteoarthritis and obesity. Methods : Data on assessment measurements of knee osteoarthritis and obesity were obtained from 63 patients suffering from knee osteoarthritis from February to April, 2005. The assessment measurements consisted of BMI (body mass index), WHR (waist-hip ratio), two disease-specific questionnaires (Western Ontario and McMaster Universities (WOMAC) index and Lequesne's Functional Severity Index (LFI)), one generic instrument (Korean Health Assessment Questionnaire (KHAQ)), and VAS (Visual Analogue Scale). Statistical correlations among assessment measurements were evaluated by examining the Pearson's correlation coefficients. Results : 1. The sexual ratio in this study was 1: 5.3 (male: female= 10: 53). The average age of all patients was $59.1{\pm}6.6$ (male: $58.3{\pm}7.1$, female: $59.2{\pm}6.5$), and the group of 60-69 (34 patients, 54%) was biggest in the age distribution. 2. The average BMI of all patients was $25.4{\pm}3.1\;kg/m^2$ (male: female= $23.6{\pm}2.6\;kg/m^2:\;25.8{\pm}3.0\;kg/m^2$), and the group of $20-25\;kg/m^2$ was biggest in the BMI distribution. According to clinical definition of obesity by WHO (1997), 52.4% of all patients was within normal weight, 42.9% was overweight, and 4.8% was obese. 3. The average WHR of all patients was $0.91{\pm}0.06$ (male: female= $0.90{\pm}0.05:\;0.92{\pm}0.06$). According to definition of abdominal obesity, 73.0% of all patients (46 patients were all female) was in the state of abdominal obesity. 4. There were no significant correlations in statistics among assessment measurements except between BMI and WHR. Conclusion : Though there is significant correlation between knee osteoarthritis and obesity according to many clinical and experimental researches, there is no assessment measurement reflecting knee osteoarthritis and obesity simultaneously. For this, further studies on correlation between knee osteoarthritis and obesity and development of assessment measurement or questionnaire on this are needed.

베타 3-아드레날린 수용체의 유전자형이 고혈당증 유발에 미치는 영향 (Effects of β3-Adrenergic Receptor Polymorphism on the Hyperglycemia in Korean Subjects)

  • 한국인 남녀 530명을 대상으로 $\beta$3-AR의 유전자 다형성이 비만과 혈당의 증가에 미치는 영향을 연구한 결과 연구대상자들의 평균 연령은 26.55$\pm$0.31세이었고, 남성이 9.1%,여성이 90.9%이었다. $\beta$3-AR의 유전자 다형성의 분포는 WW형 0.75, WR형 0.22, RR형 0.03이었고, BMI 25 kg/$m^2$를 기준으로 하여 정상군에서 WW, WR, RR형의 빈도수는 각각0.75, 0.23, 0.02이었고, 비만군에서는 각각 0.76, 0.21, 0.03이었다. 유전자 다형성에 따라 혈당은 WR+RR형에서 WW형에 비해 유의적으로 증가하였다(p=0.001). 혈당 6.105 mmol/L을 기준으로 $\beta$3-AR의 유전자 다형성의 빈도수를 분석한 결과 WR+RR형의 빈도수가 고혈당군에서는 35.6%이었고 정상혈당군에서는 23.3%으로 변이형의 빈도가 고혈당군에서 유의적으로 높았다(p=0.011). 혈당에 따라 비만도와 체지방율은 고혈당군에서 유의적으로 증가하였다(p=0.044, 0.046). HDL 콜레스테롤은 정상혈당군에서 유의적으로 증가하였고(p=0.006), 중성지방은 고혈당군에서 유의적으로 증가하였다(p=0.000). 혈당의 증가에 가장 영향을 미치는 지표를 분석하기 위해 다단계 로지스틱 회귀분석한 결과 중성지방(p=0.000), 혈중 알부민(p=0.008), $\beta$3-AR의 유전자 다형성 (p=0.011), HDL 콜레스테롤(p=0.059) 순으로 나타났다. 특히 중성지방의 증가와 $\beta$3-AR의 유전자의 WR+RR형은 고혈당의 유발 위험율을 각각 2.165배, 2.015배 증가시키고, HDL 콜레스테롤의 증가는 위험율을 0.491배 감소시키는 결과를 보인다. 각각의 유전자다형성 군에서 혈당과 BMI, WHR, 체지방량의 상관성을 분석한 결과에서 정상군에서는 비만의 판정지표인 BMI, WHR, 체지방량과 혈당이 유의적인 정의 상관관계를 나타내었으나 변이형인 WR, RR형에서는 이들 변수간에 상관성이 보이지 않았다 결과적으로 $\beta$3-AR 유전자의 변이형에서 혈당은 증가하였고, 고혈당에서는 체지방 및 중성지방이 증가하였고, HDL 콜레스테롤은 감소하였다. 또한 고혈당과 변이형의 빈도는 유의적인 상관성을 나타내었다. 따라서 $\beta$3-AR 유전자의 변이형은 혈당과 혈중 지질의 조성변화에 영향을 미치며 이는 고혈당의 위험성을 예견할 수 있는 독립적인 지표로 나타났다.

경남지역 초등학생들의 음료수 섭취와 영양소 섭취량 및 BMI와의 관련성 (The Relationship between Beverage Consumption, Nutrient Intake and Body Mass Index in Elementary School Students in Gyeongnam Area)

  • 초등학생들의 음료수 섭취실태를 살펴보고 우유 및 가당 음료수섭취가 아동들의 영양소섭취와 비만도에 미치는 영향을 알아보기 위해 경남의 초등학생 160명을 대상으로 2006년 6월부터 7월까지 신체계측을 실시하고, 정량적 빈도법을 이용하여 음료수섭취량을 조사하였다. 또 24시간 회상법을 사용하여 비연속적으로 3일간의 영양소섭취량을 조사하였다. 조사대상 아동들의 음료수의 총 섭취빈도는 1개월에 93.5회로 하루 평균 3.1회 정도였다. 음료수 중, 우유섭취빈도가 가장 높았는데 1개월에 36회로 하루 평균 1회 이상 우유를 섭취하였으며 다음으로 주스, 발효유, 탄산음료의 순이었다. 또 총 음료수 섭취량은 하루 평균 626.6 mL였고, 이 중 우유를 가장 많이 섭취하였고(253.8 mL), 다음으로 주스(133.6 mL), 탄산음료(77.7 mL)의 순이었다. 또 우유와 두유를 제외한 가당 음료수 섭취량은 359.7 mL였다. 전체 음료수 섭취열량은 328.5 kcal로, 하루 섭취열량의 20.3%였으며, 우유 및 두유로부터 섭취한 열량은 하루 섭취열량의 9.8%였고, 가당 음료는 10.5%였다. 남녀 아동별로 비교해 보았을 때 가당 음료로부터 얻는 열량의 비율이 여자가 13.5%, 남자가 7.7%로 여자가 남자에 비해 유의하게 높았다. 가당 음료수를 섭취하는 빈도와 이로 부터 얻는 열량이 높을수록 단백질, 비타민 A, 나이아신의 섭취량이 낮아서 아동들의 식사의 질이 낮았을 가능성이 있었다. 그러나 우유 및 두유 섭취빈도는 아동들의 영양섭취와 상관이 없었다. 우유 및 두유 섭취빈도가 높을 수록 키는 컸고, 체지방비와 WHR은 낮았으며 우유 및 두유섭취량이 많을수록, 체지방비와 WHR은 낮았다. 또 우유 및 두유로 부터의 섭취열량이 많을수록 WHR은 낮았지만, 가당 음료수 섭취는 신체계측치와 상관이 없었다. 결론적으로 초등학생들의 경우 우유 및 두유 섭취빈도는 아동들의 영양소섭취와 상관이 없었고, 우유 및 두유섭취량이 많을수록 체지방비와 WKR은 낮았다. 반면에 가당 음료수 섭취가 영양적인 측면에서 식사의 질을 저하시킬 가능성은 있었지만, 아동의 비만도와는 관련이 없었다.

일부 지역 대학생의 생활 스트레스와 수면, 신체계측, 영양소섭취 상태와의 상관관계 (Correlations among Life Stress, Sleep, Anthropometric Measurement and Nutrient Intakes of College Students)

  • 대학생의 생활 스트레스와 수면, 신체계측, 영양소섭취 상태와의 상관관계를 알아보고자 자기 기입식 설문조사와 신체계측 및 체성분 검사, 식이섭취조사를 실시한 결과는 다음과 같다. 대학생들의 평균 수면 시간은 남학생 7.0시간, 여학생 7.1시간이었고, 수면 점수는 남학생 40.6${\pm}$5.7, 여학생 41.5${\pm}$5.3으로 수면 시간과 수면 점수에 있어 남녀 간에 유의한 차이를 보이지 않았다. 수면과 생활 스트레스 간의 상관관계에서 남녀 모두에게 수면 시간은 생활 스트레스의 수준(스트레스 경험 빈도, 중요도, 총 스트레스 점수)과 상관관계를 보이지 않았으나, 수면의 질을 나타내는 수면 점수와 생활 스트레스 수준(스트레스 경험 빈도, 중요도, 총 스트레스 점수)을 살펴보았을 때, 남녀 모두에게 생활 스트레스와 수면의 질 간에 매우 유의한 음의 상관관계를 보여(p<0.01) 수면의 양보다는 수면의 질이 생활 스트레스와 더 상관이 있음을 보여주었다. 생활 스트레스 경험 빈도, 중요도, 총점과 신체계측 간에 상관성을 보았을 때 남학생은 생활 스트레스 경험 빈도, 중요도, 총점과 신체계측 및 체성분 간에 유의한 상관관계가 나타나지 않았으나 여학생은 생활 스트레스 경험빈도, 총점이 신체발달 점수와 유의한 음의 상관관계를 보였다(p<0.05). 남학생의 경우 경제 문제와 신체계측과의 상관관계에서 체중(p<0.05), 허리둘레(p<0.05), 엉덩이둘레(p<0.05)와 유의적인 음의 상관관계를 보였다. 여학생은 이성 문제에 대한 스트레스와 BMI(p<0.05), 체지방률(P<0.01), 복부지방률(p<0.01), 비만도(p<0.05) 간에 유의적인 음의 상관관계를 보였고, 친구 문제와 BMI(p<0.05), 비만도(p<0.05), 엉덩이둘레(p<0.05)간에 유의적인 양의 상관관계를 보였으며 학업 문제에서 체지방률(p<0.05), 복부지방률(p<0.05)과 유의적인 양의 상관관계를 보였다. 영양소 섭취량과 생활 스트레스와의 상관관계에서 몇몇 영양소와 생활스트레스 간에 유의한 상관성을 보였으며, 생활 스트레스 원인별로 그 양상이 달랐다 남학생은 대체로 동물성 식품으로부터 얻을 수 있는 영양소인 칼슘, 철분 등과 생활 스트레스 간에 양의 상관관계를 보였다. 이상에서 조사된 바에 의하면 대학생의 생활 스트레스는 수면의 양보다는 수면의 질과 상관성이 있었으며 체성분, 영양소 섭취상태와도 상관성을 보였다. 생활 스트레스에 적절히 대처하는데 올바른 식습관 및 생활습관의 확립이 도움이 될 것으로 사료되며 대학생을 대상으로 운동, 자신이 좋아하는 취미활동 하기, 상담제도 이용 등의 올바른 생활스트레스 관리 방법에 대한 교육과 바람직한 식생활을 위한 영양교육이 필요하다고 사료된다.

체중조절 내담자의 영양섭취 및 식행동 실태 분석 (Analysis on Nutrient Intakes and Eating Behaviors of Female Students Visiting Nutrition Counselling Office)

  • The purpose of this study was to analyze health-related habits, weight control experience and body composition of 344 female students visiting nutrition counselling office. Dietary habits of the subjects were assessed by means of interview with questionnaire and nutrient intakes were evaluated by the simple dietary assessment method. Weight and height were measured to get body mass index(BMI) and waist-hip ratio and their body composition were measured by Inbody 3.0. The obtained results were as follows: 1) Among students, 14.5% were exercising on a regularly basis, of which the exercises were walking and gymnastic exercise(22.1%), jogging(14.1%), and rope-skipping(4.7%), 48.4% of students were little drinking and 30.5% were once a week and smokers were 4.1%. 2) 28.3% of students were little having breakfast and 15.1% were once or twice a week. Two third of total students were not having breakfast regularly. Also only 59.6% of students were having dinner everyday, which means many of them were even skipping dinner. The reasons why they were not having breakfast were because they don't have enough time to eat(66.0%) and for a diet(2.8%) and the reasons for skipping dinner were because they were not hungry(23.0%), for the weight loss(18.2%). It was shown that they would skip dinner rather than breakfast for a weight control. 51.6% of students were taking snack 1~2 times a day and 5.3% were having little snack. 55.4% of students were dining out once a day and 15.4% more am 2 times a day. 3) 46.7% of students were already experiencing weight control before visiting the counselling office. 78.5% of students tried on one kind of weight control method, 11.4% on two kinds, and 10.1 % on more than three kinds. The method they tried for a weight control most was the one food diet using egg, fruits and beans, which is the most popular among them, and the next were an aerobic exercise(23.6%). a diet tried by a famous entertainer (15.5%), and the fasting(14.5%). 4) The average BMI was 21.2 and the body fat rate was 28.1%. As a result of grouping BMI, 12.8% were underweight 67.6% normal weight 11.6% overweight and 8% obesity. Less than standard for the body fat rate were 0.6%, 50.6% standard, 48.5% more than standard. 49.2% of students as normal weight on a BMI were assessed the so-called "skinny obesity. 50% of consulted students situated fat intra-abdominally at the umbilical level(WHR>0.8). 5) The energy intake was 76.6$\pm$17.8% of RDA and constituent ratio of carbohydrate, fat and protein were 64.2$\pm$5.2%, 21.6$\pm$3.7% and 14.3$\pm$2.3%, respectively, which is little over of 20% of recommended ratio of fat. Protein and niacin intake were more than 90% of RDA and riboflavin and vit. C were taken more than 100% of RDA. But Fe intake was 69.4$\pm$19.3%, Ca 76.6$\pm$23.6%, which were the least constituents. There was not remarkable significance between energy intake and nutrient density based on the groups of underweight, normal weight, overweight and obesity of BMI. 6) It was shown that body fat rate had remarkably significant correlation(p<0.000) with BMI(r=0.760) and WHR(r=0.817) respectively, but body fat rate was more correlated with WHR than with BMI. There was not much significant difference between body fat rate and WHR whether they exercised or not. However. BMI was significantly higher in the exercise group because one who showed higher BMI started to exercise since they looked fat in appearance and perceived as they were fat. fat.
