Analysis of the Rate of Discrepancy between Preliminary Reports by Radiology Residents and Final Reports by Certified Radiologists for Emergency Radiology: Studies in a University Hospital (대학병원 응급 영상검사에서 영상의학과 전공의 가판독과 전문의 최종 판독 간의 불일치 발생률 분석)

  • Purpose In the adult emergency department of a university hospital, we investigated the frequency of major discrepancies between the preliminary reports by radiology residents and the final reports by certified radiologists. Materials and Methods Based on CT and MRI scans obtained between December 2016 and November 2019, we selected cases with diagnoses or treatment plans that could be changed due to discrepancies between preliminary and final reports and classified them by the type of discrepancy. We also examined the distributions of the major discrepancies and stratified them by residents' working time zone, experience, and subspecialty. Results Based on the 72137 preliminary reports evaluated, 1348 tests (1.9%) showed major discrepancies. Most of the major discrepancies were false negatives (72.0%), followed by misdiagnosis (26.3%) and false positives (1.7%). Acute findings (87.2%) were more common than non-acute findings (12.8%). The major discrepancy rate increased toward the second half of the 24-hour shift, with the highest rate of 2.9% occurring between 2 am and 4 am. The major discrepancy rate did not vary with experience, and it varied from 0.6% to 4.5% for each subspecialty. Conclusion The major discrepancy rate was less than 2%, and it increased with longer working hours during a 24-hour shift.

Mediastinal Lymph Node Metastasis from Head and Neck Cancer: Predictive Factors and Imaging Features (두경부암의 종격동 림프절 전이: 예측인자 및 영상 소견)

  • Purpose To assess the predictive factors and describe the imaging features of mediastinal lymph node (MLN) metastases in patients with head and neck cancer. Materials and Methods We compared the clinical features and disease characteristics (sex, age, site of primary tumor, histologic type, history of prior treatments, TNM stages, and metastasis in cervical LNs) of patients with head and neck cancers between the MLN metastasis and no MLN metastasis groups. We also evaluated the chest CT (distribution and maximum dimension of the largest LN) and PET/CT (maximum standardized uptake value) features of MLN metastases based on the MLN classification. Results Of the 470 patients with head and neck cancer, 55 (11.7%) had MLN metastasis, involving 150 mediastinal stations. Hypopharynx cancer, recurrent tumor, T4 stage, N2/N3 stages, and M1 stage were found to be significant predicting factors for MLN metastasis. The most common location of MLN metastasis was ipsilateral station 2 (upper paratracheal LNs, 36.4%), followed by ipsilateral station 11 (interlobar LNs, 27.3%) and ipsilateral station 10 (hilar LNs, 25.5%). Conclusion Metastasis to MLNs should be considered in patients with head and neck cancer, especially in cases that are associated with a hypopharyngeal cancer, recurrent tumor, and high TNM stages.

MR Imaging Characteristics of Primary T-Cell Lymphoma of the Cauda Equina: A Case Report and Literature Review (말총의 원발성 T세포 림프종에서 MR 영상 소견: 증례 보고와 문헌 고찰)

  • Primary central nervous system lymphoma is a rare form of extranodal non-Hodgkin lymphoma, and primary T-cell lymphoma of the cauda equina is extremely rare. We describe a case involving a 56-year-old female who presented with low back pain and radiating leg pain for 4 months. MRI of the lumbar spine revealed an elongated, multinodular intradural lesion of approximately 10 cm from the L4 body to the S2 body level with iso-signal intensity on T1-weighted imaging, heterogeneous iso- and high-signal intensity on T2-weighted imaging, and a heterogeneous intense enhancement on gadolinium contrast-enhanced T1-weighted imaging. A peripheral T-cell lymphoma of the cauda equina was diagnosed on the basis of immunohistochemical and T-cell receptor gamma gene rearrangement analysis after intradural biopsy of the mass.

  • Bladder cancer is a relatively common cancer type, with a high recurrence rate, that can be often encountered in the imaging study. Accurate diagnosis and staging have a significant impact on determining treatment and evaluating prognosis. Bladder cancer has been evaluated by transurethral resection of bladder tumor for clinical staging and treatment, but it is often understaged when compared with final pathologic result by radical cystectomy. If the location, size, presence of muscle invasion, lymph node metastasis, distant metastasis, and presence of upper urinary tract cancer can be accurately diagnosed and evaluated in an imaging study, it can be treated and managed more appropriately. For an accurate diagnosis, radiologists who evaluate the images must be aware of the characteristics of bladder cancer as well as its types, imaging techniques, and limitations of imaging studies. Recent developments in MRI with functional imaging have improved the quality of bladder imaging and the evaluation of cancer. In addition, the Vesical Imaging Reporting and Data System was published to objectively assess the possibility for muscle invasion of cancer. Radiologists need to know the types of bladder cancer treatment and how to evaluate the changes after treatment. In this article, the characteristics of bladder urothelial carcinoma, various imaging studies, and findings are reviewed.

Agreement and Reliability between Clinically Available Software Programs in Measuring Volumes and Normative Percentiles of Segmented Brain Regions

  • Objective: To investigate the agreement and reliability of estimating the volumes and normative percentiles (N%) of segmented brain regions among NeuroQuant (NQ), DeepBrain (DB), and FreeSurfer (FS) software programs, focusing on the comparison between NQ and DB. Materials and Methods: Three-dimensional T1-weighted images of 145 participants (48 healthy participants, 50 patients with mild cognitive impairment, and 47 patients with Alzheimer's disease) from a single medical center (SMC) dataset and 130 participants from the Alzheimer's Disease Neuroimaging Initiative (ADNI) dataset were included in this retrospective study. All images were analyzed with DB, NQ, and FS software to obtain volume estimates and N% of various segmented brain regions. We used Bland-Altman analysis, repeated measures ANOVA, reproducibility coefficient, effect size, and intraclass correlation coefficient (ICC) to evaluate inter-method agreement and reliability. Results: Among the three software programs, the Bland-Altman plot showed a substantial bias, the ICC showed a broad range of reliability (0.004-0.97), and repeated-measures ANOVA revealed significant mean volume differences in all brain regions. Similarly, the volume differences of the three software programs had large effect sizes in most regions (0.73-5.51). The effect size was largest in the pallidum in both datasets and smallest in the thalamus and cerebral white matter in the SMC and ADNI datasets, respectively. N% of NQ and DB showed an unacceptably broad Bland-Altman limit of agreement in all brain regions and a very wide range of ICC values (-0.142-0.844) in most brain regions. Conclusion: NQ and DB showed significant differences in the measured volume and N%, with limited agreement and reliability for most brain regions. Therefore, users should be aware of the lack of interchangeability between these software programs when they are applied in clinical practice.

Synergistic effect of soy isoflavone and swimming exercise on improvement of liver function in ovariectomized mice (대두 이소플라본과 수영운동이 난소절제 쥐의 간 기능 개선에 미치는 시너지 효과)

  • Soy isoflavones are attracting attention from postmenopausal women because of their beneficial effects on menopausal symptoms. This study was investigated whether a combination of soy isoflavone genistein and swimming exercise (Gen+SE) would have a beneficial synergistic effect on obesity and improvement of liver function compared to the genistein only (Gen) and swimming exercise only (SE) in ovariectomized mice. Ovariectomized mice were randomly divided into control group (Con), Gen, SE, and Gen+SE, and were fed a high-fat diet for 8 weeks. As a result of examining the body weight, weight of white adipose tissue, lipid accumulation of liver, and serum ALT and AST levels, both Gen and SE decreased compared to Con, and Gen+SE decreased more than compared to Gen and SE. The expression of inflammatory cytokines MCP-1, IL-6 and TNF-𝛼 genes in liver decreased in both Gen and SE compared to Con, and were further decreased in Gen+SE compared to Gen and SE. But The expression of adiponectin showed opposite results. The expression of fatty acid oxidation related genes in liver increased in both Gen and SE compared to Con, and were more effectively than increased in Gen+SE compared to Gen and SE. Therefore this study suggests that the interaction between soy isoflavone and swimming exercise is very effective controlling obesity and recovering decreased liver function, and this is caused by promoting fatty acid oxidation in the liver in ovariectomized mice.

Characteristic analysis of mortar using desulfurization gypsum and carbon dioxide conversion capture materials as a cement admixture (탈황석고와 탄산화물을 혼합재로 사용한 모르타르의 특성 분석)

  • In this study, the characteristics of mortar using carbondioxide conversion capture materials (CCMs), fabricated by reacting CO2 with desulfurization gypsum (DG) by-produced from a oil refinery, as a cement mixture. Based on the chemical component and particle size analysis results, it estimated that desulfurized gypsum reacted with carbon dioxide to produce carbonate crystals such as CaCO3. Using CCMs as a cement mixture, physical property and durability analysis were conducted by measuring such as workability, compressive strength, compressive strength ratio after freezing-thawing and accelerated carbonation depth. The experimental results showed that as the content of the admixture increased, workability and compressive strength characteristics decreased. Compressive strength after freezing-thawing and accelerated carbonation depth also showed similar characteristics to the physical property measurement results. In addition, compared to desulfurized gypsum, using CCMs showed better physical properties and durability. This was assumed to be due to differences in the crystal phases of the mixed materials such as free-CaO and CaCO3.

A Study on the Vulnerability Assessment of Solar Power Generation Facilities Considering Disaster Information (재해정보를 고려한 태양광발전시설의 취약성 평가에 관한 연구)

  • This study aims to develop an evaluation method for solar power facilities considering disaster impacts and to analyse the vulnerabilities of existing facilities. Haenam-gun in Jeollanam-do, where the reassessment of existing facilities is urgent, was selected as the study area. To evaluate the vulnerability from a more objective perspective, principal component analysis and entropy methods were utilised. Seven vulnerability assessment indicators were selected: maximum hourly rainfall, maximum wind speed, number of typhoon occurrence days, number of rainfall days lasting more than five days, maximum daily rainfall, impermeable area ratio, and population density. Among these, maximum hourly rainfall, maximum wind speed, maximum daily rainfall, and number of rainfall days lasting more than five days were found to have the highest weights. The overlay of the derived weights showed that the southeastern regions of Haenam-eup and Bukil-myeon were classified as Grade 1 and 2, whereas the northern regions of Hwawon-myeon, Sani-myeon, and Munnae-myeon were classified as Grade 4 and 5, indicating differences in vulnerability. Of the 2,133 facilities evaluated, 91.1% were classified as Grade 3 or higher, indicating a generally favourable condition. However, there were more Grade 1 facilities than Grade 2, highlighting the need for countermeasures. This study is significant in that it evaluates solar power facilities considering urban disaster resilience and is expected to be used as a basic resource for the installation of new facilities or the management and operation of existing ones.

Clinical and Radiological Features of Diffuse Lacrimal Gland Enlargement: Comparisons among Various Etiologies in 91 Biopsy-Confirmed Patients

  • Objective: To compare the clinical and radiological features of various etiologies of chronic diffuse lacrimal gland enlargement. Materials and Methods: We retrospectively reviewed 91 consecutive patients who underwent surgical biopsy for chronic diffuse lacrimal gland enlargement and were diagnosed with non-specific dacryoadenitis (DA) (n = 42), immunoglobulin G4-related dacryoadenitis (IgG4-RD) (n = 33), and lymphoma (n = 16). Data on patient demographics, clinical presentation, and CT imaging findings (n = 73) and MRI (n = 43) were collected. The following radiologic features of lacrimal gland enlargement were evaluated: size, unilaterality, wedge sign, angle with the orbital wall, heterogeneity, signal intensity, degree of enhancement, patterns of dynamic contrast-enhanced, and apparent diffusion coefficient value. Radiological features outside the lacrimal glands, such as extra-lacrimal orbital involvement and extra-orbital head and neck involvement, were also evaluated. The clinical and radiological findings were compared among the three diseases. Results: Compared to the DA and IgG4-RD groups, the lymphoma group was significantly older (mean 59.9 vs. 46.0 and 49.4 years, respectively; p = 0.001) and had a higher frequency of unilateral involvement (62.5% vs. 31.0% and 15.2%, respectively; p = 0.004). Compared to the IgG4-RD and lymphoma groups, the DA group had significantly smaller lacrimal glands (2.3 vs. 2.8 and 3.3 cm, respectively; p < 0.001) and a lower proportion of cases with a wedge sign (54.8% vs. 84.8% and 87.5%, respectively; p = 0.005). The IgG4-RD group showed more frequent involvement of the extra-orbital head and neck structures, including the infraorbital nerve (36.4%), paranasal sinus (72.7%), and salivary gland (58.6%) compared to the DA and lymphoma groups (4.8%-28.6%) (all p < 0.005). Conclusion: Patient age, unilaterality, lacrimal gland size, wedge sign, and extra-orbital head and neck involvement differed significantly different between lymphoma, DA, and IgG4-RD. Our results will be useful for the differential diagnosis and proper management of chronic lacrimal gland enlargement.

Effect of Knee Joint Injury on Biomechanical Factors during the Uchi-mata (허벅다리걸기 시 무릎 관절 부상이 운동역학적 요인들에 미치는 영향)

  • The purpose of this study was to analyze the effects of knee joint injury experiences of judo players on kinematic factors and center of pressure factors during uchimata. Among right-handed male college judo players specializing in uchimata, 13 people who had a knee joint injury experience(age, 20.69.1±0.75 years; height, 172.85±4.81 cm; body mass, 74.92±5.51 kg; and career, 8.92±0.95 years) and 13 people who did not have a knee joint injury experience(age, 21.08.1±0.76 years; height, 172.54±6.32 cm; body mass, 76.62±9.09 kg; and career, 9.46±0.94 years) within the last 2 years were divided into two groups and participated as subjects. The two groups were evaluated for differences in ankle, knee, and hip joint angle variables, COP range, and velocity components during uchimata. As a result of the study, the EIG group showed smaller values in the knee joint flexion angle at E3 and the hip joint extension angle at E4 during uchimata than the NIG group. In addition, the EIG group showed lower values in the range of motion of the COP and forward movement velocity of the COP in the one-leg support phase than the NIG group.
