Depression Health Literacy and Help-seeking Intention of the Aged Receiving Customized Home Visiting Health Care Services (맞춤형 방문건강관리 대상 노인의 우울 건강정보이해능력과 도움요청의도)

  • Purpose: The purpose of this study was to understand levels of both mental health literacy of depression and intention of help-seeking, and then to identify the relationship of them in Korean older adults. Methods: Participants in this cross-sectional survey were 395 persons over 65 years old receiving customized home visiting health care services at Chungnam province. Data were collected through interviews by visiting nurses in 2011. Results: The proportion of participants with depression was 61.8% ($6.7{\pm}3.6$). 78.2% of subjects appeared to have help-seeking intention for solving depression. The proportions of participants with ability to recognize depression was 69.1%. Although the level about knowledge and belief in self-help interventions were varied according to questionnaires, subjects understood self-help intervention of smoking accurately (86.3%) and physical activity (85.5%). Ability to recognize depression, knowledge and belief about self-help managements, and opinion of medication for treatment among health literacy variables measured in this study were related to help-seeking intention. In addition, women, visual impairment, and lower depression scores were related to help-seeking intention. Conclusion: Results demonstrate that it is necessary to improve depression health literacy to manage effectively depression of vulnerable elderly in communities. These results could be used in developing mental health literacy programs.

The power monitoring terminal which uses a variety of wired and wireless networks and protocol is the target additionally to be considered in addition to the security vulnerabilities that was occurred in the existing terminal. In this research paper, the author analyzes the cryptographic techniques corresponding to the smart meter occurred by the problems that are exposed on the outside which are vulnerable to physical attacks, and intends to propose the design of the security systems for the Smart meter terminal being able to maximize the efficiency of the terminal.

A Survey on the Barrier-Free Environment of Public Office Building in Jeju Area (제주지역 공공업무시설의 장애물 없는 생활환경 실태조사 연구)

  • Purpose: Considering the proportion of the population such as the elderly, pregnant women, infants companion and constantly increasing foreign tourists, there is a high need for implementing an environment without barriers. In terms of policy and institutional aspects, Jeju Special Self-Governing Province is striving to create an environment without barriers and to promote the safety and convenience of Vulnerable Pedestrians. But it is not enough to create a viable barrier-free environment. Methods: Method of this research is to analyze the general population status and installed facilities status for the disabled in Jeju. And analysis of policy research needs and level of barrier-free. By applying the barrier-free environment certification standards should conduct an assessment of the level of the current public office buildings in Jeju. It presents the environmental improvement of public office buildings for barrier-free environment. Result: Barrier-free environment status of the five public office buildings research surveyed all buildings have shown low levels of installed facilities for the disabled. In particular, it was analyzed that represents a very low level in the intermediate facilities and other facilities. The development of a specific installation manual for the facilities for the disabled is required to improve public office buildings. And barrier-free criteria need to reflect on the implementation of regulations. Implications: Through continuous research to analyze the barrier-free level and problems of public office buildings, it will be made to improve the comprehensive and reasonable alternative.

The Adult Guardianship and Medical Issue According to the Amendments of Civil Code (성년후견과 의료 -개정 민법 제947조의 2를 중심으로-)

  • The adult guardianship system has been introduced through amendments of Korean Civil Code for the first time in the March 2011(Act No. 10429, 7. 1. 2013. enforcement). The adult guardianship system has the main purposes to provide a lot of help vulnerable adults and elderly, and protect them on the welfare related with property act, treatment, care, etc. There could be a controversy about whether the protection Legal Guardian's consent(formerly known as the Mental Health Act) or permission of the Family Court(revised Civil Code) are required to, or the Mental Health Act should be revised, when mental patient will be hospitalized forcibly. The author proposes that mental patient with Adult guardians should be determined by Legal Guardian's consent and approval of the Family Court, but mental patient without Adult guardians could be determined by Legal Guardian's consent. The issue of Withdrawing of life-sustaining treatment could be occurred due to the aging society and the development of modern medicine, and this has provided difficult, various problems to mankind in Legal, ethical, and social welfare aspects. The need of Death with dignity law or Natural death law has been reduced for a revision of the Civil Code. Therefore, on the issue of Withdrawing of life-sustaining treatment, in the future, intervention of the court is necessary in accordance with the revised Civil Code Section, and Organ Transplantation Act and the brain death criteria may serve as an important criterion.

Effects of the Satisfaction Level of Living Environment in Rural Area on the Migration Intension (농촌지역의 생활환경 만족수준이 이주의사에 미치는 효과)

  • This study analyzes the effect of the level of satisfaction with the living environment of rural people on migration. The analysis data were used by the Rural Development Administration for the survey of welfare systems for farmers and fishermen in 2017, and samples of 3,776 were used for the analysis. The analysis variables were divided into a group of migrants, a group of permanent residents, and a group of decision-keeping, and the level of satisfaction with the living environment of each of the 10 rural areas was used as an independent variable. According to the analysis results, the basic living base and safety of rural residents were having a positive effect. The results show that the projects for developing rural areas, which increase the basic living base, are making substantial contributions to preventing the breakaway of the rural population. Safety has been neglected in the area of rural planning, but if the level of discontent is not enough, it has had the greatest impact on the livelihoods of rural residents. Welfare and cultural leisure set the main demand level for welfare and cultural leisure to the elderly and vulnerable classes, and interpret it as a result of weakened defense against stress from relocation of residence, difficulty of migration due to low income levels, and migration. Therefore, the paradoxical analysis results could be interpreted as showing that measures to increase the satisfaction of the people on welfare and cultural leisure should be taken.

Factors Affecting the Purchase of Indemnity Private Health Insurance and Related Factors of Outpatient Healthcare Utilization among Chronic Disease Patients (만성질환자에서 실손형 민간의료보험의 가입 관련 요인과 외래 의료이용의 관련 요인)

  • Purpose: The purpose of this study was to investigate determinants of purchasing indemnity private health insurance and its impact on the healthcare utilization among outpatients with chronic disease. Methods: The study analyzed 4,997 chronic ill patients using 2015 Korean Health Panel data. Logistic regression analysis was conducted to analyze the factors affecting the purchase of indemnity private health insurance and multiple regression analysis was conducted to analyze the effect of private health insurance on the number of outpatient visits and outpatient expenditures. Findings: The age, education level, and number of chronic diseases were significant factors affecting the purchasing of indemnity private health insurance among chronic patients. As a result of analyzing the impact of indemnity private health insurance on healthcare utilization, the number of outpatient visits for those who enrolled in the indemnity private health insurance was higher than the number of outpatient visits for those who did not. But there was no statistically significant difference in outpatient medical expenses. Practical Implications: The results of this study shows that indemnity private health insurance may cause inequality in the healthcare utilization of the socially vulnerable. It is necessary to seek ways to strengthen the health security of chronic disease patients and high-risk elderly people who need more healthcare.

Resident Evaluation on Housing and Demand on Residential Service according to Type of Public Rental Housing (공공임대주택의 유형에 따른 거주자 평가와 주거서비스 조사)

  • There is a growing demand for public rental housing since housing price increased rapidly and socially vulnerable people need more stable housing. The purpose of this study is to investigate housing evaluation (satisfaction), moving plan, and residential service according to the type of the public rental housing and the characteristics of the resident, and to suggest the development direction for comprehensive housing services including the community level. Data used for analysis in this study are from 2016 Seoul panel survey of public rental housing residents. A total of 3009 questionnaire from public rental households were collected in 2016 Seoul panel survey. In order to investigate housing requirements, we analyzed household characteristics, housing satisfaction, moving plans, welfare services, residential activities and community facilities, social environment. In this study, the difference of residential satisfaction, moving plan, and residential service according to the type of the public rental housing and the characteristics of the resident is analyzed to identify their housing demands. As a result, there were differences in residential satisfaction, moving plan, and residential service depending on type of public rental housing and characteristics of residents. Residents in public rental housing modified from the multi-family housing after purchase showed lower residential satisfaction, and higher intention for moving plan compared to residents living in other type of public rental housing. In the residential service, residents in permanent lease type have higher experience of housing service as well as demand on housing service. Especially single household, elderly household, female household, or household with handicapped person indicated higher demand for housing services.

Investigating Volumetric changes of Brain Structure in Women Aged 65 to 85 Years Old (65세부터 85세 여성의 뇌 구조 부피 변화 조사)

  • The human body becomes vulnerable to various diseases due to deterioration in structure and function as it ages. In particular, changes in brain structure weaken the immune system against diseases such as vascular and metabolic neuropsychiatric diseases. In this study, we used a magnetic resonance imaging technique that allows non-invasive observation of brain structures and measurement of how the volumes of the brain, gray matter, white matter, and subcortical regions changes with aging in women aged 65 to 85 years. As a result of our investigation, we observed a significant linear decrease in subcortical regions with age. These results suggest that the changes due to aging in the brain structure area are closely related to neuropsychiatric diseases in old age, and can provide information in understanding the vulnerability of the brain in old age.

Home Healthcare Service Awareness Survey for Korean Medicine Doctors: a survey study

  • Objectives: Discussions regarding "medical blind spots" in Korea's "aging society" are continuously rising. In addition, the demand for medical attention and care for the elderly and vulnerable populations continues to increase. Given this, the government is promoting the "home healthcare service" project. This study aims to lay the foundation for promoting this project by investigating the perception of clinical Korean Medicine (KM) doctors in the "community health care" project. Methods: With the cooperation of the Association of Korean Medicine, we sent a questionnaire to all KM doctors through e-mail. The survey included personal information, awareness, appropriate disease and intervention, proper visit location, and pros and cons. Results: A total of 602 responses were collected and analyzed. Approximately 20% of the doctors answered that they were well aware of the service, while 55% responded that they did not know about it. For a visit, a KM doctor selected the appropriate diseases in the order of stroke, dementia and Parkinson's disease, osteoarthritis, and chronic diseases. Among treatments, acupuncture, moxibustion, and herbal medicine exhibited similar results. The most common opinion was that KM doctors should schedule their visits once a week for 6-12 months, which was the most prolonged period among the given options. More than 80% (84.1%) of the doctors replied that care projects were highly essential, and about 63.8% expressed their willingness to participate in these projects. Conclusion: To provide appropriate home health care, we must raise awareness among Korean medicine doctors. In addition, the healthcare budget must be increased to provide the required support.

Trends in incidences of newly notified tuberculosis in Jeju Province, Korea, 2017-2021

  • In 2021,the incidence of newly notified tuberculosis in Jeju Province decreased by 23.0% compared to that in the previous year. This was the largest decline among the 18 municipalities studied. This study aimed to examine trends in the incidence of tuberculosis among different age and sex groups in Jeju Province between 2017 and 2021. This study observed the number of new cases in the annual report from 2017 to 2021, published by the Jeju Center for Infectious Diseases Control and Prevention. The average annual percentage change (AAPC; %) was calculated to determine the trend in crude incidence rates over the past 5 years by sex and age. While a P-value of <0.1 was considered marginally significant, aP-value of <0.05 was considered statistically significant. The AAPC of all age groups in Jeju Province had negative values. In men,there was a statistically significant decrease in ages 30-34, 40-44, 45-49, and 50-54 years and a marginally significant decrease in ages 35-59, 55-59, 60-64, 65-69, 75-79, and ≥80 years. In women, a statistically significant decrease was observed among patients aged 30-34, 35-39, and ≥80 years, and a marginally significant decrease was observed among patients aged 45-49, 50-54, 60-64, 65-69, and 75-79 years. A statistically significant decrease in tuberculosis was observed among patients in their 30s and 40s, and elderly patients aged ≥80 years comprised the group vulnerable to tuberculosis. Based on these findings, tuberculosis prevention and screening programs conducted in Jeju Province were effective.
