양성 성대 질환 환자의 후두 미세 수술 전후 음성 장애 지수 및 음성 분석의 유용성 (Validity of Voice Handicap Index and Voice Analysis following Laryngeal Microsurgery for Benign Vocal Cord Lesions)

  • Background and Objectives : Voice disorders can cause problems in patients with benign vocal cord lesions emotionally, physically, economically and functionally. Neither subjective nor objective voice examinations can evaluate such factors adequately. The Voice Handicap Index (VHI) subjectively evaluates voice disorders in terms of physical, functional, emotional factors and measures the patient's perception of the impact of voice disorder. The purpose of this study is to evaluate the usefulness of VHI in the patients with benign vocal cord lesions. Materials and Method : The authors evaluated 37 patients who experienced laryngeal microsurgery for benign vocal cord lesions from september 2003 to August 2004. The VHI was used to measure the postoperative changes of the patient's perception and acoustic analysis and aerodynamic tests were also done. Statistical analysis was done using paired t-test and Pearson's correlation. Results : The VHI scores showed statistically significant reductions postoperatively. In acoustic analysis, jitter and shimmer had statistically significant reductions after surgery but noise-to-harmonics ratio did not. A statistically significant change in the average MFR and MPT perioperatively was found. The relationship between VHI and acoustic, aerodynamic analysis attained statistical significance. Conclusion : The VHI is a useful assessment tool to monitor the patient's self-perception of voice change after the surgery of benign vocal cord lesions. The VHI measurement, when combined with acoustic and aerodynamic analyses, will be helpful in comparing functional outcomes after voice surgery.

  • This study investigated the differences between emotional parameters BDI, VHI, STAI-X-I and STAI-X-II according to the menstrual cycles of the female and the relation between changes of the depression index and voice characteristics (jitter, shimmer, CPP, HNR, $pF0{\cdot}F1{\cdot}F2{\cdot}F3$, sF0, sF4, sB1, $H1_{c/u}$, $A1_u$, $A3_c$, $H1A3_{c/u}$, $H1A1_u$). Twenty three females ($30{\pm}4.4$ years old) living in Seoul and Gyeonggi Province were participated in this study to answer the questionnaires and record their voice. The participants prolonged /a/ vowel for 5 seconds in a natural condition for their voice recording. Voice data were analyzed using the Matlab and Praat program. A t-test and a correlation analysis were conducted by using SPSS for the statistical analysis. The results are as follows. First, the BDI is significantly higher in group I (lurear phase contrast the menstrual period) and group II (follicular phase against the menstrual period) than group III (luteal phase for follicular phase) (p<.05). Second, shimmer, CPP, pF0 showed a statistically high correlation regarding the BDI in group I (lurear phase contrast the menstrual period). Voice parameters may be useful as supplement in evaluating the emotional change in the phase of menstrual cycle.

성대결절이 있는 성악도에서의 음성분석학적 특징 (Acoustic Characteristics of the Vocal Major Students with Vocal Nodule)

  • Objectives : The purpose of this study was to systematically analyze and compare the acoustic characteristics of vocal major students with vocal nodule when singing. Materials and Methods : Ten sopranos with vocal nodule, who have never been treated due to voice problems, were the subject group. Twenty healthy sopranos major students were the control group for this study. The vocal nodule was confirmed by stroboscopy and the acoustic and electroglottographic analysis was conducted on be groups. Additionally, an inquiry on usual voice problems during speaking and singing were performed on each of the groups. Results : The vowel /a/ was spoken and sung by sopranos of each group. There were no significant differences in Fo, jitter, shimmer, NHR at speaking and singing in either group. We did not register any significant differences of closed quotient and speed quotient in electroglottography for both groups at singing, but speed quotient was increased in subject group at speaking(p<0.05). And the sopranos with nodule demonstrated voice fatigue(70%), strained and tired voice after long period of performance and conversation(60%) on inquiry. Conclusions : Although they had a vocal nodule, this group, who had a long period of voice training, did not present any differences in acoustic and electroglottographic problems during short performance, but, the vocalists noticed delicate voice problems after a long performance. We conclude that the voice training teacher and laryngologist must initiate voice management and speech therapy for vocal nodule, even if there is no specific abnormality in acoustic analysis.

Why Mobile Operators Introduced Data Plans: An Analysis of Voice and Data Usage Patterns

  • With the introduction of the data-oriented plan for LTE service, one may concerned with the background of the ISP's policy in charging for LTE services. In this work we investigate the latest usage patterns of voice and data applications for customers over the current mobile network, via which we investigate why mobile operators introduced data-oriented plans. To be specific, we collected the real-field data for the volume of voice and data traffic from the LTE network before the data-oriented plans were introduced. From the collected data we compute the absolute volume as well as the proportion of voice and data applications. From these observations we infer mobile operators' reasoning behind the decision to introduce data-oriented plans with unlimited voice calls over the mobile network.

청각장애 성인 남성의 음성 특성 (Acoustic Qualities of Phonation in Hearing-impaired Male Adults)

  • The purposes of this experiment were to compare and analyze some voice parameters of the hearing impaired male adults and to suggest a basic data on the speech intervention for the hearing impaired. Voice analysis of four sustained vowels(/a/, /i/, /${\partial}$/, /u/, fundamental Sequency(F0), jitter percent, shimmer percent, and Noise to Harmonic Ratio(NHR) was conducted for the deaf young male adults using a sign laguage(N=5, aged 16-20) and the normal hearing young male adults(N=10, aged 18-20) by using MDVP(Multi-Dimensional Voice Program) in CSL. F0, jitter, and shimmer in the deaf group were significantly higher than those in the normal hearing group. The average of F0 was 151 Hz, which was lower than the results of the previous studies, and there were no significant differences among the sustained vowels. In both groups, the values of the voice parameters were stable on the /a/ or /${\partial}$/, those closed to the standard scores.

  • The persistent and recurrent dysphonia after microlaryngeal surgery was noted in tweleve patients. We reviewed the results of laryngostroboscopy, psychoacoustic evaluation, aerodynamic study and acoustic analysis according to the treatment modality. The causes of persistent dysphonia were attributed to vocal cord scarring, recurrent mass lesion, residual mass lesion, persistent inflammation, and hyperfunctional voice disorder. We noticed the better vocal function in the group treated with voice therapy or surgical therapy than the group treated with voice rest and medication. Therefore, we concluded that vocal function can be improved with the use of active, multidisciplinary approach which includes voice therapy, medical treatment and selected surgical resection according to the laryngeal lesions.

  • Diagnostic evaluation for voice disorders requires a multidimensional approach : physiological examination of the larynx and the vocal tract, aerodynamic studies, examinations of vocal fold vibrations, psychoacoustic evaluations, acoustical analyses, and so on. Previous studies with the use of a computer system suggested that the acoustic evaluation is quite promising for differentiating some causative diseases of voice disorders(Hiki et al., 1975a, b, 1976 ; Kakita et al., 1980). However, a well-equipped computer system for acoustic analysis is very expensive and not available in most voice clinics. A sound spectrograph is available in many voice clinics. (omitted)

  • This paper delves into the comparative analysis of human interactions with AI agents based on the presence or absence of a facial representation, combined with the presence or absence of paralanguage voice elements. The "CASA (Computer-Are-Social-Actors)" paradigm posits that people perceive computers as social actors, not tools, unconsciously applying human norms and behaviors to computers. Paralanguages are speech voice elements such as pitch, tone, stress, pause, duration, speed that help to convey what a speaker is trying to communicate. The focus is on understanding how these elements collectively contribute to the generation of flow, intimacy, trust, and interactional enjoyment within the user experience. Subsequently, this study uses PLS analysis to explore the connections among all variables within the research framework. This paper has academic and practical implications.

  • In voice pathology, various measurements calculated from pitch values are proposed to show voice quality. However, those measurements frequently seem to be inaccurate and unreliable because they are based on some wrong pitch values determined from pathological voice data. In order to solve the problem, we compared several pitch estimation methods to propose a better one in pathological voices. From the database of 99 pathological voice and 30 normal voice data, errors derived from pitch estimation were analyzed and compared between pathological and normal voice data or among the vowels produced by patients with benign vocal fold lesions. Results showed that gross pitch errors were observed in the cases of pathological voice data. From the types of pathological voices classified by the degree of aperiodicity in the speech signals, we found that pitch errors were closely related to the number of aperiodic segments. Also, the autocorrelation approach was found to be the most robust pitch estimation in the pathological voice data. It is desirable to conduct further research on the more severely pathological voice data in order to reduce pitch estimation errors.

음성장애 주관적 평가와 객관적 평가 간의 상관성 연구 (A Study of the Correlation between Subjective and Objective Evaluation of Voice Disorders)

  • The purpose of this study was to examine the relationship between subjective and objective evaluation in speakers with voice disorders. Subjective evaluation indicates the self-reports of voice problems by dysphonic speakers. The relating protocol is the Voice Handicap Index (VHI) and the self-awareness index of voice problems (SAIVP-14). A total of 48 individuals with voice disorders replied to the questionnaire and participated in a voice assessment. Objective evaluations included the perceptual judgement of G grade in GRBAS, acoustic measurements (jitter, shimmer, NHR) by MDVP (CSL 4400), and aerodynamic measurements (MPT, MFR, psub) by PAS (Phonatory Aerodynamic System, KayPentax, USA). Pearson and Spearman correlations were used for the analysis. In the correlation with perceptual judgement (G grade) and VHI-Total, VHI-Physical, and SAIVP-14, there was a significant correlation, but the overall correlation was poor. NHR, jitter, and shimmer were significantly correlated with overall VHI and SAIVP-14. Specifically, the correlation with shimmer was stronger compared to the other measurements. In aerodynamic measures, MFR and MPT showed a significant correlation with VHI-Total, VHI-Emotional, and SAIVP-14, but their correlation was poor. The results of this study suggested that subjective evaluation of self voice problems is meaningfully correlated with objective evaluations, but more data in the multidimensional voice assessment should be collected and analyzed for the reliability and validity of the voice handicap questionnaire.
