  • 성대의 bowing은 성대근의 위축, 노화, 상후두신경마비, 성대수술후, 성대구 등의 원인에 의하여 발생할 수 있으며, 대부분의 예에서 경도의 애성을 호소할 뿐만 아니라 아직 효과적인 치료법이 없기 때문에 적극적인 치료를 받지 못하고 장기간 애성을 호소하는 경우가 많다. 저자들은 성대의 bowing의 진단 및 치료에 도움을 얻고자 최근 2 년간 애성을 주소로 내원한 환자중 성대의 bowing을 보였던 25례(성대구 3례, 성대위축증 2례)을 대상으로 치료전ㆍ후의 음성기능을 분석하여 문헌고찰과 함께 보고하고자 한다.

  • Age-related changes in larynx can have a direct impact on voice quality and general comfort level. Observations of vocal aging have spanned perceptual, acoustic, aerodynamic, physical, electromyographic (EMG) and histological levels. Evidence of differential vocal aging in relation to gender and physical condition has been reported. Perceptual, acoustic, aerodynamic, kinematic, EMG and histological data document age-related changes in laryngeal structure and function with advancing age. These changes contribute to a functional age-related impact of vocal hypofunction or compensatory hyperfunction. This review will focus on the current understanding of the clinical and cellular changes in the larynx that lead to presbyphonia.

정상인과 식도발성 음성에서의 공기역학적 비교 연구 (The Aerodynamic Analysis between Normal Voice and Esophageal Voice)

  • Voice rehabilitation is very important concerning in laryngectomees. Esophageal speech is a common and widely used method of voice restoration. But, until now there is no reliable data which shows the aerodynamic characteristics of esophageal speech. In order to evaluate the vocal quality of normal laryngeal and esophageal speech, several aerodynamic parameters were measured in 13 adults with normal laryngeal voice and 2 excellent esophageal speakers using Aerophone II voice function analyzer. The examined parameters were maximal flow rate, mean airflow rate, subglottic pressure, vocal efficiency, glottic resistance, maximal phonation time and mean sound pressure level. In vocal efficiency, there is no difference between two groups, but in other parameters, marked differences were showed in esophageal speakers, especially mean resistance. Results indicates that esophageal speakers make the efficient voices with poor aerodynamic condition, comparing with normal laryngeal speakers.

On a Reading Aloud to Relieve the Decrease in Blood Oxygen Saturation when Jogging

  • Recently, the problem of hypoxia caused by jogging is attracting attention. To solve this problem, this paper proposed a new solution. This paper proved that as a vocalization method of reading aloud, it is possible to increase air intake and activate lung function to exchange more air and obtain more oxygen. Then, blood oxygen saturation was used as an evaluation index for the body's oxygen content level to prove its effectiveness. A photoelectric pulse oximeter developed on the basis of different light absorption principles in blood was used to test blood oxygen saturation. Experimental results show that a certain degree of hypoxia is induced when a lot of oxygen is required due to jogging. Therefore, it was proved that the new vocal breathing method by reading books can increase the blood oxygen saturation of the body and improve the hypoxia of the body. Reading vocal breathing is a simple and efficient oxygen saturation recovery breathing method.

성악 훈련을 받은 성악인에서의 Voice Range Profile (Voice Range Profiles of Trained Classical Singers)

  • Background and Objectives : The Voice Range Profile(VRP) is a two-dimensional graphic dysplay of an individual's amplitude range as a function of total fundamental frequency range. It is designed as a maximum performance test which can be used as a general indicator of voice problems in the non-professional voice and as a sensitive indicator of problems with the professional voice. The purpose of the study is to obtain a baseline VRT for the classical professional singers and compare it with the normal nonsinger's profile. We also compared the difference of VRP between the classical professional singers who have normal vocal fold and who have vocal folds lesions without dysphonia. Materials and Methods : The VRPs were elicited. from 42 trained classical singers(Soprano 26, Mesosoprano 5, Tenor 9, Bariton 2) and 20 untrained nonsingers(female 10, male 10) using Voice Range Profile Model 4326(Kay Elemetrics USA). The mean values for phonational range with highest and lowest pitch level and range of voice intensity with maximum and minimum intensity level were compared between classical singers and nonsingers. Results and Conclusions : The frequency range and dynamic range were significantly increased for the classical singers in comparison to the nonsingers. But there was no significant difference were found for the VRP between the parts in the classical singers. The classical singers who have vocal fold lesions showed slightly decreased VRP compared to those with healthy vocal folds.

Countertenor 1인의 Modal Register와 Falsetto Register에서의 공기역학적 변화 및 전기성문파형의 변화 연구 (Analysis of Phonatory Aerodynamic & Electroglottography of a Countertenor)

  • Background and Objectives: Countertenors who can produce higher vocal pitch like female classical singer's voice and use both modal and falsetto register. This study was conducted to study phonatory characteristics between modal and falsetto register of the countertenor. Materials and Methods: A male countertenor who had 8 years of experience was examined using a videostroboscopy and his voice was analyzed using aerodynamic measures; fundamental frequency(F0), Mean air flow rate(MFR), intensity(SLP), subglottal air pressure(Psub) with phonatory function analyzer(Nagashima) and acoustic measures; jitter, shimmer, HNR, closed quotient(CQ) using a Electro-glottography(EGG) of Lx. Speech Studio(Laryngoscope, Ltd, UK) and voice range profile of CSL(Kay elemetrics). Results: In the stroboscopy finding, the longitudinal length of vocal folds was increased at the falsetto register and the upper margin of vocal folds vibrated with incomplete closure of true vocal folds. In aerodynamic analysis, intensity was same at the modal and falsetto register. However, MFR, Psub, MPT were higher at the falsetto register. In the electroglottographic analysis, closed quotient(CQ) at the modal register was high and also much higher at the high-pitch falsetto than at the loud falsetto. In the VRP, intensity was similar though F0 was different between modal and falsetto register. Conclusion: It implied that countertenor could produce powerful voice quality by increasing of respiratory pressure and respiratory volume though glottal closure was incomplete. In addition, no change of EGG waveform, similar voice range with alto was observed.

Spectral Characteristics and Nasalance Scores of Hypernasality in Patient with Cleft Palate

  • Differential instrumentation for the diagnoses of individuals with Cleft palate has been used to objectively measure speech problems. The Cepstrum Method was used to study the vocal tract transfer function. The vocal tract transfer function and the source spectrum should be considered in the evaluation of nasal resonance. The aim of this study was to collect quantitative data on the acoustic Instrumentation used for evaluating hypernasality. Normal subjects (9 male, 21 female; 37 male children, 20 female children) and individuals with VPI (13 male, 8 female; 16 male children, 9 female) participated in this study. The vowel /i/ was selected to gauge the severances of hypernasality Spectral and Cepstral studies using CSL was used to identify the acoustic characteristics. Cepstrum analysis shows significant differences in quefrency and amplitude. The quefrency of normal groups was shorter than that of the VPI groups, while the amplitude of normal groups was lower than that of the VPI groups. This may have significance in the evaluation 'of nasal resonance.

방사선 요법이 초기 성대암 및 정상 후두의 음성 지표에 미치는 영향 (Effect of Radiation Therapy on Voice Parameters in Early Glottic Cancer and Normal Larynx)

  • The preservation of the voice-producing mechanism is an important feature in the management of laryngeal cancer by radiotherapy. But, radiation therapy has certain side effects such as mucositis, tissue edema, necrosis and fibrosis which could effect on normal voice production. Several subjective studies that used questionnaires and auditory perceptual judgements of voice have been interpreted to mean that radiation results in a normal or near-normal voice. Objective evidence of the status of vocal function after radiation treatment, however, is still lacking. We analyzed the changes that occur in voice parameters in a group of patients undergoing radiation therapy, in order to determine the effect of radiation on voice quality. In this study acoustic, aerodynamic measures of vocal function were used to determine the characteristics of voice production. We found that voice parameters in early glottic cancer changed meaningfully comparing to normal larynx with or without radiation and radiation therapy has an little effect on normal larynx.

후두 스트로보스코피의 임상적 응용 (Clinical Application of the Laryngostroboscopy in the Laryngeal Disorders)

  • Laryngostroboscopy is one of the most practical techniques for clinical examination of the larynx. The videostroboscopy provides valuable information concerning the nature of vocal folds' vibration, an immediate image of the presence or absence of pathology, and a permanent record. Additionally, when used by trained observers in conjunction with other instrumentation, it can provide both qualitative and quantitative data on vocal function of both the normal and disordered larynx. The authors examined the 388 patients with voice disorders by videostroboscope. This paper describes the clinical procedure of laryngostroboscopy based on some introductory remarks on laryngeal anatomy and function. And the findings of parameters observed by the stroboscopy is noted for the laryngeal disorders.

일측 성대마비 환자에서 성대내전술 후 성대접촉율의 증가가 음질 개선에 미치는 영향 (The Effect of An Increase of Closed Quotient on Improvement of Voice Quality after Type I Thyroplasty in Patients with Unilateral Vocal Cord Paralysis)

  • Purpose : To assess perceptual, acoustic and aerodynamic measure of voice quality in patients with unilateral vocal cord paralysis before and after type I thyroplasty. Methods : The clinical records of patients operated type I thyroplasty in the Departement of otorhinoalryngolgy, Yongdong Severance hospital from November 2001 to November 2003 were reviewed. All patients uderwent a vocal function evaluation including perceptual, acoustic and aerodynamic measures of voice preoperative and on $60^{th}$ postoperative day. The perceptual and acoustic measures were obtained from recording of patients' reading a 'Sanchak' passage. The perceptual evaluation was performed by 2 speech pathologist using a 4-point rating scale. Acoustic parameters(voice range profile low(RAL), voice range profile high(RAH), average fundamental frequency(AFX), closed quotient, harmonic to noise ratio, jitter and shimmer) were investigated by Lx speech studio. Mean flow rate(MFR), subglottic pressure(Psub) and intensity were measured using the Phonatory function analyzer. The maximum phonation time was also measured. The data were statistically analyzed. A paired t-test (p<0.1) was used to compare preoperative and postoperative results. And multiple regression test was used to find which parameter was most correlated to improvement of postoperative voice quality. Results : Among aerodynamic parameters, Psub $(88.11mmH_2O{\rightarrow}58.7mmH_2O)$, MPT(7.87sec${\rightarrow}$12.53sec), MFR (359.8ml/sec${\rightarrow}$161.06ml/sec) were statistically improved. AFx(205.5Hz${\rightarrow}$163.27Hz), AQx(23.9%${\rightarrow}$48.3%), RAL, RAH. Jotter and shimmer were improved. In multiple regression test, AFx and AQx was noted as the two meost correlated parameters to improvement of postoperative breathiness. But general grade of voice quality was more correlated to Psub and shimmer. Conclusion : Vocal fold medialization procedures effectively reduce glottic gap. Increasing of contact area of both vocal folds induced improvement in aerodynamic parameters and leaded stabilizing of vocal fold vibration. That effect results in improvement in acoustic parameters (shimmer, jitter, signal-to-noise ratio, voice range profile) and voice quality.
