Purpose: This study was conducted to determine the influence of inspiratory muscle exercise using visual biofeedback and inspiratory muscle exercise with diaphragm breathing retraining in stroke patients in regard to inspiratory muscle activity and respiratory function and to provide fundamental information on intervention for improvement of pulmonary function in stroke patients. Methods: The current study measured and analyzed inspiratory muscle activity and pulmonary function of 15 randomly selected subjects in a Biofeedback inspiratory muscle exercise (BIE) group that uses visual feedback and 15 subjects in the Diaphragm breathing exercise (DBE) group that uses breathing retraining before and after intervention. Intervention was performed for 30 minutes, 5 times a week, for 8 weeks. Subjects were measured for muscle activity of upper trapezius muscle and lattisimus dorsi muscle using a surface electromyography system and maximum inspiratory pressure was measured using a respiratory measurement device. For homogeneity test of subjects, independent t-test was performed and ANCOVA was performed for comparison of inspiratory muscle activity and pulmonary function between groups. Results: In the study results, the BIE group showed more significant muscle activity than the DBE group in upper trapezius muscle and lattisimus dorsi muscle (p<0.001). In addition, the BIE group showed more pressure than the DBE group in maximum inspiratory pressure (p<0.001). Conclusion: Based on the current study, performing biofeedback respiration exercise simultaneously with breathing retraining in stroke patients can provide more efficient respiratory physical therapy. In addition, it is considered that consistent study on the effectiveness is necessary to further improve clinical availability.
This study is to determine the effects of BUDDEUMI(moxibustion and cupping a boli) therapy on the relief of shoulder pain among middle aged women. methods : A total of 39 middle aged women with shoulder pain are selected : 20 in the experimental group and 19 in the control group. The data was collected by using questionnaires. The BUDDEUMI(moxibustion and cupping a boli) therapy on the should and back was administered to the experimental group, and no treatment was given to the control group. All of the subjects were examined on a subjective symptom scale and on the grade of discomfort and pain in everyday life as developed by Japan's Industrial Fatigue Research Committee of the Industrial Hygienics Society(1988), Sohn, Mong Ho(1999), Visual Analogue Scale as developed by Cline (1992). SPSS/win 12.0 was used for Date analysis, General features demonstrated by frequency number and percentage. The effects of the BUDDEUMI(moxibustion and cupping a boli) therapy was analyzed by a t-test. The homogeneity testing revealed that all study variables are equally distributed between experimental and control groups. The hypothesis, 'the experimental group who received BUDDEUMI(moxibustion and cupping a boli) therapy will decrease in the grade of discomfort of shoulder', is supported. The hypothesis, 'the experimental group who received BUDDEUMI(moxibustion and cupping a boli) therapy will decrease in the Visual Analogue Scale is supported. Conclusion : As a result of this study, BUDDEUMI(moxibustion and cupping a boli) therapy will be able to be used as the self care therapy to improve the symptoms of middle aged women who has the shoulder pain.
이 연구는 척추안정화운동과 도수치료가 급성기 또는 아급성기 허리통증 환자의 통증지수와 장애지수에 미치는 영향을 알아보기 위해 시행되었다. 23명의 허리통증 환자가 자발적으로 참여하였고 무작위로 분류되었다. 도수치료(10명)와 척추안정화운동(13명)은 총 4주간 주 3회, 15분/1회 적용되었다. 각각의 치료 전에 전통적인 치료를 30분간 적용하였다. 치료 전과 후에 시각사상척도와 한국어판 오스웨스트리를 측정하였다. 치료 전에 비해 치료 후 두 치료 모두에서 통중지수와 장애지수가 유의하게 감소하였다(p<.05). 통증지수의 변화율은 도수치료보다 척추안정화운동 시 유의하게 높았지만(p<.05), 장애지수의 변화율은 유의한 차이가 없었다(p>.05). 이상으로 볼 때, 척추안정화운동은 아급성기 또는 급성기 허리통증 환자에서도 통증을 안전하게 줄이는데 도움이 됨을 확인하였다. 따라서 아급성기 이하의 허리통증 환자에서도 통증과 장애를 줄이는데 척추안정화운동이 유용하리라 사료된다.
Muscle misuse dysphonia (MMD) is defined as a behavioral voice disorder resulting from inappropriate contractions of intrinsic and/or extrinsic laryngeal muscles. The purpose of this study was to investigate the effect of motor learning guided laryngeal motor control therapy (MLG-LMCT) which is designed to improve an existing LMT and further the effective voice treatment on people with muscle misuse dysphonia. Forty-six people with MMD (M:F=16:30) participated in this study. The voice samples of the participants were recorded to investigate the effect of MLG-LMCT before and after the voice therapy. Voice samples were analyzed via electro-glotto-graph (EGG). Contact quotient (CQ), speed quotient (SQ), and waveform were reported. In addition, perceptual and acoustical evaluation were conducted to determine the change of voice improvement after treatment. The experimenter massaged the tensioned muscles around the neck. In order to find more proper phonation the experimenter showed the subjects their EGG wave forms as to whether or not they are moving the vocal folds to the appropriate position. Therefore, the EGG wave forms were used as a type of visual feedback. With the wave form, the experimenter helped subjects move the vocal folds and laryngeal muscles to find more proper voice production. The sensory stimuli from the experimenter gradually faded out. A paired dependent t- test revealed that there was significant differences in CQ between pre- and post-therapy. Perceptually, overall, rough, breathy, strain, and transition were significantly reduced. Acoustically, there were significant differences in Fo, jitter, shimmer, and NHR. After using MLG-LMCT, most of the subjects showed improvements in voice quality. The results from this study led us to the following conclusions: Motor learning guided laryngeal motor control therapy (MLG-LMCT) has reduces muscle misuse dysphonia. These results may occur because a visual feedback from EGG wave form can maintain the effect of the muscle tension reduction from laryngeal manual therapy. In case of people with MMD who reduced muscle tension from the therapy (LMT) but, not appropriately manipulating the location of larynx or adducting the vocal folds, MLG-LMCT might be an alternative therapy approach.
The purposes of this study were to ascertain differences of the sensory system, central processing system, effector system, and balance ability according to general characteristics (sex, fall experience, and age), and to identify the contributions of the sensory, central processing, and effector system to balance ability in 83 healthy elderly subjects. The subjects were elderly (over 60 years) who live in Wonju City. All subjects participated in six tests (position sense test, visual acuity, vestibular stepping test, Mini-Mental Status Exam-Korea, strength of dorsiflexors, and Berg Balance Test). Collected data were analyzed by the independent t-test, one-way ANOVA, and stepwise multiple regression. The results were as follows: 1. The results of the six tests according to sex showed that vestibular function was significantly better in women than in men. But visual acuity, cognitive function, strength of dorsiflexors, and balance ability were significantly better in men than in women (p<0.05). 2. There were no significant differences in position sense, visual acuity, vestibular function, cognitive function, strength of dorsiflexors, and balance ability between fallers and non-fallers (p>0.05). 3. However, the results of the six tests according to age (group A: 60-69 years, group B: 70-79 years, group C: 80-89 years) showed significant differences in five tests. In the Scheff$\acute{e}$ test, position sense, visual acuity, and cognitive function showed significantly different results between in group A and group C: the strength of the dorsiflexors was significantly different between in group A and group B, and balance ability was significantly different between in group A and group C, and between in group B and group C (p<0.05). 4. Cognitive function and position sense were positively associated with balance ability. Their power of explanation regarding balance ability was 38% (p<0.05). These results suggest that cognitive function and position sense play an important role in balance ability. This would seem to suggest that programs for evaluation or improvement of elderly's balance ability in the elderly should consider cognitive function and position sense.
목적 : 본 연구는 학교기반 작업치료 중재 및 평가에 관한 근거마련을 위하여 체계적 고찰을 실시하였다. 연구 방법 : 2000년 이후부터 2014년 6월까지 PubMed와 Ovid의 데이터베이스에서 school or children and occupational therapy의 검색용어를 사용하여 검색하여 총 14개의 연구를 선정하였다. 본문을 통해 대상연구의 근거수준, 대상자, 평가도구, 중재영역 및 방법과 효과를 분석하였다. 결과 : 대상자는 정상아동(64.3%)이 가장 많았고, 다음은 자폐(14.4%)이었다. 연령은 6~8세(35.7%)로 가장 많았고, 다음은 5~7세와 5~12세(14.4%)이었다. 중재영역은 글쓰기 및 소근육과 시운동 통합(68.8%)이 가장 많았고, 중재방법은 직접치료(71.4%)가 가장 많았다. 평가도구 분석결과, Beery-Buktenica Test of Visual-Motor Integration (14.9%)가 가장 많았고, 다음은 Bruininks-Oseretsky Test of Motor Proficiency (11.1%)이었다. 학교기반 작업치료의 중재효과에 대한 분석 결과, 시운동 통합, 학습 기술, 참여 수준, 소근육 운동, 놀이, 행동적 문제에서 효과가 있었다. 결론 : 본 연구를 통해 학교기반 작업치료의 중재, 대상자, 평가도구 및 효과에 대한 근거를 마련하였다. 이는 학교 기반 작업치료 임상과 연구의 기초자료로 활용할 수 있을 것이다.
Purpose: This study analyzed the immediate effects of intra-abdominal pressure with visual feedback on the muscle activation of the upper trapezius and sternomastoid during natural inspiration and forced inspiration in individuals with costal respiration. Methods: The eighteen individuals with upper costal breathing pattern participated in this study. Surface electromyography was used to analyze the muscle activity of the upper trapezius and sternomastoid during natural inspiration and forced inspiration before and after intra-abdominal pressure. Results: A significant difference in muscle activation was observed with the muscle type, inspiration type, and test session (p<0.05). The muscle activities of the sternomastoid and upper trapezius decreased significantly during forced inspiration after intra-abdominal pressure training (p<0.05). On the other hand, there was no significant difference during natural inspiration in both muscles (p>0.05). A comparison of the difference between the pre-test and post-test during forced inspiration revealed the upper trapezius to be significantly larger than the sternomastoid (p<0.05). No significant difference was noted during natural inspiration (p>0.05). Conclusion: The intra-abdominal pressure has positive effects on correcting the breathing patterns in individuals with costal respiration.
Although many children with cerebral palsy have problems with their eye movements available data on its intervention is minimal. The purpose of the study was to determine the effectiveness of the postural movement normalization and eye movement program on the oculomotor ability of children with cerebral palsy. Twenty-four children with cerebral palsy (12 male and 12 female), aged between 10 and 12, were invited to partake in this study. The subjects were randomly allocated to two groups: an experimental group received the postural movement normalization and eye movement program and a control group which received conventional therapy without the eye movement program. Each subject received intervention three times a week for twelve weeks. The final measurement was the ocular motor computerized test before and after treatment sessions through an independent assessor. Differences between the experimental group and control group were determined by assessing changes in oculomotor ability using analysis of covariance (ANCOVA). The changes of visual fixation (p<.01), saccadic eye movement (p<.01) and pursuit eye movement (p<.01) were significantly higher in the experimental group than in the control group. These results show that the postural movement normalization and eye movement program may be helpful to treat children with cerebral palsy who lose normal physical and eye movement.
Objective : The purpose of this study is to report the effect of Bee Venom Acua-Acupuncture Therapy to the patient of Pes Anserine Bursitis by using DITI. Patient & Methods : The patient was 60-year-old woman who complained severe knee pain. She was treated by bee venom acuaacupuncture therapy. To estimate the efficacy of tratment, we used DITI, visual analog scale, knee joint check(ROM). Results : In this case, we treated patient of Pes Anserine Bursitis for 28 days. bee venom acua-acupuncture therapy efficiently relieved patient's pain and improved ROM. DITI and Visual analog scale also showed significantly valuable changes.
Objective : The purpose of this study is to report the efficacy of using oriental medical treatment(herb-medicine, acupuncture) with Melonix Calyx Vomiting Therapy for atypical facial pain. Methods : We experienced two cases of atypical facial pain treated with oriental medical treatment(herb-medicine, acupuncture) and Melonix Calyx Vomiting Therapy(MCVT). MCVT is a traditional treatment, which induces vomiting by eating Melonix Calyx powder with water. To evaluate the results of this treatment, we used the Visual Analogue Scale(VAS) and Pain Rating Scale(PRS). Result : After using MCVT with oriental medical treatment, VAS, PRS and clinical symptoms of 2 patients were improved remarkably. Conclusion : This study suggests that using MCVT with oriental medical treatment is effective on atypical facial pain.
