The Effect of Korean Medicine Treatment on Suspected Meniere's Disease with Headache in a Patient Hospitalized at a Korean Medicine Hospital: A Case Report (한방병원에 입원한 두통을 동반한 메니에르 병 의증 환자에 대한 한방 치료 1례)

  • Objectives: This study reports on the efficacy of using Korean medical treatment with suspected Meniere's Disease. Methods: We treated a patient with suspected Meniere's disease using Korean medical treatments (herbal medicines, acupuncture, herbal acupuncture therapy). To evaluate the results, we used the visual analogue scale (VAS) to measure dizziness, tinnitus, ear fullness, and headache. The degree of tinnitus was also evaluated using the Korean Tinnitus Handicap Inventory (K-THI). The generic health status was evaluated using the European Quality of Life-5 Dimensions (EQ-5D) scale. Results: Improvements in the VAS and EQ-5D scale scores were observed after the treatments. The change in the K-THI scores showed that the patient's tinnitus handicap was relieved. Conclusions: The results of this case study suggest that Korean medicine therapies may be effective in the treatment of suspected Meniere's Disease.

A Case Study of Taeumin Patient with Gastro-esophageal reflux disease(GERD) who Treated Successfully with Yeoldahanso-tang(Reduohanshao-tang) (열다한소탕(熱多寒少湯)으로 호전된 역류성 식도염 환자 1례)

  • 1. Objectives: This case study reports a Taeumin patient with Gastro-esophageal reflux disease who had suffered from chest pain, heartburn, acid regurgitation and dyspepsia who improved after Yeoldahanso-tang(Reduohanshao-tang) medication. 2. Methods: This patient had started treatment with Yeoldahanso-tang(Reduohanshao-tang), three times per day for three months and assessed the changes of the main symptoms such as chest pain, heartburn and acid regurgitation from baseline to post-treatment using a questionnaire with visual analogue scale(VAS). Electrogastrography(EGG) was also performed for assessment of gastric function from baseline to post-treatment. 3. Results: After the treatment, symptoms of GERD such as chest pain, heartburn, and acid regurgitation are almost disappeared. The patient don't need to take the proton pump inhibitor therapy. There was significant increase of % bradygastria parameters in fed EEG after treatment with Yeoldahansotang(Reduohanshao-tang). And significant increase of the power ratio after treatment was shown (20.5), compared with initial EGG(0.0). 4. Conclusions: This results show Yeoldahanso-tang(Reduohanshao-tang) can be used to treat GERD with Dry-heat symptom of Taeumin. This results suggest that Sasang Constitutional treatment for GERD patients who has a chronic condition with highly recurrence rate, might be a candidate for a therapeutic agent. Also effects of Yeoldahanso-tang(Reduohanshaotang) on GERD need further studies such as clinical trials.

All-arthroscopic, Guideless Single Suture-button Fixation of Acute Acromioclavicular Joint Dislocation: A Description of the Technique and Early Treatment Results

  • Background: The purpose of this study was to examine the clinical and radiological results of the all-arthroscopic, suture-button fixation technique to treat acute acromioclavicular (AC) joint separations. Methods: All patients with acute AC joint separations received all-arthroscopic, single suture-button (TightRope) procedure without a special guide. Postoperative Constant score (CS), pain level according to visual analogue scale, and range of motion (ROM) were evaluated. For radiological evaluation, coracoclavicular distances were measured bilaterally. Results: Between December 2010 and June 2012, 18 consecutive patients (4 women and 14 men; mean age, 29.3 years) with acute AC joint separations underwent surgical treatment after 6.4 days (range, 2-20 days) following the initial trauma. The average postoperative follow-up was 16.9 months. The mean CS was 92.4 (range, 84-96). The mean external rotation, forward flexion, and abduction were $75.8^{\circ}$ (range, $50^{\circ}-90^{\circ}$), $170^{\circ}$ (range, $150^{\circ}-180^{\circ}$), and $163.8^{\circ}$ (range, $140^{\circ}-180^{\circ}$), respectively. Five patients exhibited coracoclavicular ossifications. In two patients, superficial wound infections were successfully treated with antibiotic therapy. In one patient, a coracoid fracture was observed. No significant differences were found regarding pain, ROM, or strength parameters between both sides. The coracoclavicular distance was discovered to be approximately 2.8 mm greater on the affected side; however, this minimal reduction loss did not affect the functional results. Conclusions: The findings of this study suggests that all-arthroscopic treatment of AC joint separations using the single suture-button technique without a drill guide is safe, yielding good to excellent clinical results.

Effects of Korean Medicine Therapies on Oral Pain in Patients with Burning Mouth Syndrome: a Before and After Study (구강작열감 증후군 환자의 구강내 통증에 대한 한방치료의 효과: 전.후 비교 연구)

  • Objectives : Burning mouth syndrome (BMS) is characterized by chronic pain or a burning sensation in the mouth. There is limited evidence available to provide clear guidelines for treating BMS patients and a variety of different treatments have been used. This study was designed to investigate the Effects of Korean medicine therapies on oral pain in patients with BMS. Methods : We surveyed 30 BMS patients who newly visited the Oral Diseases Clinic in the Kyung Hee University Korean Medicine Hospital from February 2012 to March 2013. When the patients visited the clinic for the first time, they were evaluated on sociodemographic characteristics, BMS questionnaire, severity of pain using visual analogue scale (VAS) and pressure pain threshold (PPT) of the acupuncture point CV17. After 3 weeks of Korean medicine therapies (acupuncture, electroacupuncture, pharmacopuncture and herbal medicine), they were re-evaluated with the VAS and the PPT. Results : After 3 weeks of Korean medicine therapies, 30 patients' oral pain improved and the PPT score on CV17 rose, which means decrease of qi-stagnation score. Conclusions : Korean medicine therapies were effective on oral pain in patients with BMS. To confirm the additional curative effect and evaluate the efficacy of each treatment, well-designed randomized controlled trials will be needed in the future.

Therapeutic Effect of Tetrax based on Visual Feedback Training on Balance Dysfunction due to Ataxia in Subjects with Cerebellar Stroke: A Retrospective Study (소뇌 뇌졸중 환자에서 실조로 인한 균형장애에 대한 시각적 피드백 훈련 기반 테트락스의 치료적 효과: 후향적 연구)

  • PURPOSE: This study is to investigate the therapeutic effect of Tetrax on balance dysfunction caused by ataxia in cerebellar stroke. METHODS: A total of thirty subjects with cerebellar stroke were recruited. The participants was divided into two groups, the experimental (n=15) and the control group (n=15). Tetrax training and conventional physical therapy (CPT) were performed in experimental group, whereas the patients in the control group were treated with CPT twice a day. Each session of the Tetrax and CPT was carried out for 30 minutes, 5 times per week for 4 weeks. Korean version of the Scale for the Assessment and Rating of Ataxia (K-SARA) was the primary outcome measure, and the secondary outcomes covered Berg balance scale (BBS), falling index (FI), Timed up and go (TUG), and modified Barthel index of Korean version (K-MBI). All outcome measures were evaluated before and after 4 weeks. RESULTS: K-SARA was decreased significantly after 4 weeks intervention in both the experimental (p<.05) and the control group (p<.05). Furthermore, the experimental group produced significantly better outcomes in K-SARA, BBS, FI, and TUG compared with the control group (p=.012, p=.027, p=.008, and p=.048). There were significant correlations between K-SARA and BBS, FI, TUG, and K-MBI (p<.001, p<.001, p=.004, and p<.001). CONCLUSION: The restoration of ataxia was related with the improvement of the balance, falling risk, mobility, and activity of daily living. Tetrax training was effectively aided recovery of ataxia after cerebellar stroke.

  • Background: Turbinate hypertrophy is one of the common causes of chronic nasal obstruction. In principle, therapeutic guidelines recommend medical treatment. Failure to treat turbinate thickening despite drug therapy may indicate the need for surgery. The main aim of this study was to determine the effect of radiofrequency surgery, among various other surgical procedures, on people with both nasal septal deviation and turbinate hypertrophy. Methods: Among people with nasal deviation who visited the subject hospital between July 2008 to July 2014, 21 people with nasal septal deviation and severe turbinate hypertrophy before their surgery had undergone septoplasty with turbinoplasty using radiofrequency combined with septoplasty. The degree of the turbinate's hypertrophy was appraised in all the patients before and after the surgery using the rhinoscopy, and acoustic rhinometry was objectively carried out. The subjective effect of the turbinoplasty using radiofrequency was explored through the visual analog scale (VAS) score. Results: The degree of contraction of the nasal mucosa after the rhinoscopy changed from Grades 3 and 4 (100%) to Grades 1 and 2 (95.2%) and Grades 3 (4.8%). The minimal cross-sectional area significantly increased from $0.44{\pm}0.07$ to $0.70{\pm}0.07cm^2$ (p<0.05). The nasal cavity volume increased from $4.79{\pm}0.49$ to $6.76{\pm}0.55cm^2$ (p<0.05). The subjective symptoms evaluated with VAS score a year after the surgery significantly improved (p<0.05). Conclusion: Turbinoplasty using Coblator with septoplasty is an effective treatment method because it expands nasal cavity, has a low incidence of complications, subjectively improves symptoms, and has short treatment duration.

Clinical Study on the Eye Changes in the Goiter Patients (갑상선종환자(甲狀腺腫患者)의 안병변(眼病變)에 관(關)한 임상적(臨床的) 관찰(觀察))

  • 742 cases of thyroid diseases treated at Radio-isotope Clinic, Seoul National University Hospital since October 1965 through July 1968 were analyzed. The patients were classified according to eye findings, as to Infiltrative, Non-infiltrative and No ophthalmopathy. Correlations between ophthalmological findings and symptoms, BMR values and $^{131}I$ uptake rate as well as response to medical treatment were carefully reviewed. Results obtained were as follows: 1. Among goitrous patients 44.5% with infiltrative ophthalmopathy, 24.1% with non-infiltrative ophthalmopathy totaling 68.6% of patients with one or more eye findings were found. Exophthalmos (44%) and visual disturbances (44%) comprised the highest incidence among eye signs. Most frequent eye symptoms were lid swelling (17.1%). 2. Female patients were predominant(91%). 3. Of cases with hyperthyroidism, after treatment with antithyroid drugs and radio-iodine therapy, general symptoms were improved in over 74%, whereas ophthalmopathy was not changed and in some cases the eye signs were aggravated. This tendency was more apparent in infiltrative ophthalmopathy. 4. Significant correlations between exophthalmos, BMR values and $^{131}I$ uptake rate were found. Among the patients of same degree of exophthalmos, BMR values and $^{131}I$ uptake rate showed the highest in non-infiltrative ophthalmopathy, moderate in infiltrative ophthalmopathy and the lowest in no ophthalmopathy.

The Effect of Korean Medical Treatment on Systemic Sclerosis : A Case Report (경피증에 대한 한방치료 효과 : 증례보고)

  • Objectives : The purpose of this study is to report the clinical effect of Korean Medical Treatment and acupuncture based on M-test theory & herbal medicine for treating systemic sclerosis. Methods : We treated a patient diagnosed with systemic sclerosis. We used general acupuncture & acupuncture based on M-test theory, herbal medicine, moxibustion, and physical therapy. The outcomes were evaluated by using peripheral body temperature measurement, numeric rating scale(NRS), visual analog scale(VAS), and an activities of daily living(ADL) questionnaire for systemic sclerosis. Results : The average peripheral body temperature increased from $36.16^{\circ}C$ to $36.38^{\circ}C$. The NRS showed no difference, but VAS decreased from 7.43 to 5.43. The ADL questionnaire score decreased from 17 to 12. Conclusion : This case study suggests that Korean Medical Treatment and acupuncture based on M-test theory & herbal medicine could be effective for treating systemic sclerosis. However, further studies are needed to confirm this result.

Management of Chiasmatic-Hypothalamic Gliomas in Children (소아 시교차-시상하부 신경교종의 치료)

  • Objective : Management strategies for pediatric chiasmatic-hypothalamic gliomas(CHG) include surgery, irradiation, chemotherapy and a combination of these modalities. This study was performed in order to compare the efficacy of various methods of treatment and to describe its optimal management. Material and Method : We have reviewed the results of management of 6 children with a diagnosis of CHG, who were observed closely during the last 8 years. The patients were aged 7 months to 15 years. Our patients presented with diencephalic syndrome, endocrine dysfunction, and progressive visual loss. None of these had evidence of neurofibromatosis-1. Treatment consisted of surgery alone(2), surgry and irradiation(2), surgery, irradiation and chemotherapy(1), and surgery and chemotherapy(1). Results : Four children had large exophytic suprasellar tumors and two showed diffuse midline lesions. Obstructive hydrocephalus was present in all patients. Pathologic examination revealed anaplastic astrocytoma in 1 and lowgrade astrocytoma in 5. Two patients, recently treated with radiation therapy following radical subtotal resection, showed significant tumor reductions and good clinical status. Four patients had partial tumor resection. Of these patients, two developed disease stabilization during follow-up period of 7 and 8 years, respectively. Life-threatening complications were noted in remaining two patients. Conclusion : CHG may follow an unpredicatable course and show a various reponse to each treatment modality. Further studies are indicated to define the optimal method of treatment of CHG in childhood.

The Clinical Study on the Effect of the Sulguanjul-bang No.1 According to Formulation (퇴행성 슬관절염 환자에 대한 슬관절방(膝關節方) 1호(號)의 제형변화 유효성 임상연구)

  • Objective: This study is designed to compare the effects of Sulguanjul-Bang No.1 on osteoarthritis in knee joint according to formulation. Methods: 47 patients with symptomatic osteoarthritis of the knee joint were enrolled in a open clinical trial. The 47 patients were devided into two groups at random and taken Sulguanjul-bang No.1 with decoction type, suspension type respectively for8 weeks. After 8-week treatment period, the following parameters were analyzed ; Visual analogue Scale(VAS), Lysholm index score, patients' global assessment. Results: Sulguanjul-bang No.1 treatment led to significant improvement in the pain and symptoms of osteoarthritis as determined by all efficacy measures. After 8 weeks of therapy, there was significant improvement in VAS, Lysholm index and patients' global assessment. But there is no significant difference between two groups. Conclusions: Sulguanjul-bang No.1 was very beneficial pain relief effect and functional improvement irrespect of formulation, decoction type or suspension type.
