시각정보에 따른 하지운동이 만성 뇌졸중 환자의 하지근육의 근활성도, 균형, 보행에 미치는 영향 (The Effects of Exercise for Lower Extremity Using Visual Information on Lower Extremity Muscle Activity, Balance and Gait in Stroke Patients)

  • Background: This study was to examine the effect of applying leg exercise using visual informations on improving stroke patient's muscle power, balance and gait. Methods: The subjects of this study were hemiplegic patients by stroke, 30 patients were picked up, who were agreed with this research and were having hospital care for 8 weeks at RS, RB and HH medical centers. The study group was 15 and comparison group was 15, totally 30 patients were confirmed. leg exercise using visual informations was applied to study group, leg exercise without visual informations was applied to comparison group. These were proceeded for 8 weeks (5 days a week, 20 minutes a day), change of muscle power, balance and gait was checked to investigate the effect. Electromyography apparatus was used for checking muscle power, K-BBS (Korean version of Berg Balance Scale) and COP (center of pressure) were used for balance, then Functional Gait Assessment (FGA), timed up and go test (TUG) were used for checking the capacity of gait. Results: Vastus lateralis (p=.002), biceps femoris (p=.000), K-BBS (p=.000), COP (p=.007), FGA (p=.002), TUG (p=.009) are significant difference between the two groups. Conclusion: This study showed that visual informations impact change of muscle activity, balance and locomotor ability in stroke patients.

Muscle Activity Based on Real-time Visual Feedback Training Methods by Rehabilitative Ultrasound Image in Elderly and Relationship between Heckmatt Scale, Muscle Thickness and Tone : A Pilot Study

  • Purpose: This study is to investigate the muscle activity based on real-time visual feedback training methods by rehabilitative ultrasound image in elderly and correlation between Heckmatt scale grade, muscle tone and thickness. Design: Cross-sectional study: Pilot study Methods: 6 elderly participated in the study with 2 conditions. Under the condition of rehabilitation ultrasound imaging equipment, all subjects performed voluntary maximal muscle contraction of the quadriceps 3 times using visual feedback based on Rehabilitative Ultrasound Imaging 1.0 (RUSI 1.0). Under the condition of only ultrasound images, all subjects performed voluntary maximal muscle contraction of the quadriceps 3 times using ultrasound image-based visual feedback. The muscle thickness and tone of the quadriceps were measured and the grades were classified by Heckmatt scale and all variables were comparative analyzed. Results: Heckmatt scale grade showed a negative correlation with muscle thickness at relaxation (p<0.05), and a negative correlation with the difference value obtained by subtracting muscle thickness at relaxation from muscle thickness at contraction in ultrasound image condition (p<0.05). The muscle tone during relaxation showed a negative correlation with the muscle thickness during relaxation (p<0.05). Conclusion: In the case of voluntary maximum muscle contraction of the quadriceps muscle in the elderly, it can be seen that the muscle thickness is getting larger when the RUSI 1.0-based visual feedback is provided than with only ultrasound image provided. And the lower Heckmatt scale grade is, the thicker the muscle is, and the lower the muscle tone is.

만성뇌졸중 환자에 시각적 되먹임 트레드밀 보행 훈련이 보행과 균형 능력에 미치는 효과 (The Effect of Treadmill Gait Training Accompanied by Visual Feedback on the Gait and Balance of Chronic Stroke Patients)

  • Purpose: This study compares the effects of treadmill gait training accompanied by visual feedback and general treadmill gait training on the gait and balance ability of patients with chronic stroke. Methods: A total of 11 patients with chronic stroke were randomly divided into either the treadmill gait training accompanied by visual feedback group (six patients) or the general treadmill gait training group (five patients). The gait and balance ability of the two groups were measured before and after the interventions using the functional reach test, the Timed Up and Go (TUG) test, Berg's balance test, and the Biodex balance test. The treadmill gait training accompanied by visual feedback group performed the exercise under the supervision of a therapist after first being provided with a hat and a goal that was devised for the purpose of providing visual feedback information. The interventions were applied to the respective groups for four weeks. For the statistical analysis, we conducted a Mann-Whitney test to compare the results between the two groups. Additionally, the Wilcoxon test was used to compare the results from before and after the intervention in each group. Results: The treadmill gait training accompanied by visual feedback group showed a significant difference in terms of the functional reach test after the intervention when compared to the general treadmill gait training group (p<0.05). Although there was no significant difference, the treadmill gait training accompanied by visual feedback group showed a larger improvement in the TUG test, Berg's balance test, and the Biodex balance test than the general treadmill gait training group (p>0.05). Conclusion: The results of this study suggest that treadmill gait training accompanied by visual feedback can be used as a beneficial intervention scheme for the recovery of the gait and balance ability of patients with chronic stroke.

The Effects of High Intensity Laser Therapy on Pain and Function of Patients with Frozen Shoulder

  • Purpose: This study was to identify the effectiveness of high-intensity laser therapy on pain and function of a frozen shoulder. Methods: Thirty patients were assigned to two groups: the experimental group (n=15) and the control group (n=15). Both groups received traditional therapy for 4 weeks, 3 days a week. The experimental group, however, received an additional high intensity laser therapy. Pain was measured using the visual analogue scale (VAS). The functional ability was measured using the patient specific functional scale (PSFS). A paired t-test was used to determine any differences before and after the treatment, and an independent t-test was used to determine any differences between treatment groups. Results: Both groups showed a statistically significant difference for VAS and PSFS score (p<0.05). In comparison between two groups, more experimental group than control group statistically significant difference (p<0.05). Conclusion: There seems to be a positive effect on pain and function of frozen shoulder from using high intensity laser therapy.

Effect of Fatigue on Force-Matching in the Quadriceps Muscle

  • This study examined the ability of human subjects to match a force in their quadriceps muscle during fatigue. Twenty subjects (mean age: 23.4 yrs, mean height: 167.8 cm, mean weight, 62.6 kg) were enrolled in the experiment. In the force-matching task, the quadriceps muscle generated 50% of the MVIC (maximum voluntary isometric contraction) torque under visual control and then without visual feedback. After inducing fatigue in the quadriceps muscle, the subjects were required to match 50% of the MVIC torque without visual feedback. The perceived magnitude of the force and force-matching errors were measured. 50% of the MVIC torque was perceived from 39.96 Nm in the pre-fatigue condition to 44.95 Nm in the post-fatigue condition. 50% of the MVIC torque-matching errors increased significantly from .55% in the pre-fatigue condition to 9.6% in the post-fatigue condition (p<.001). in addition, there were significantly more force-matching errors in women than in men (p<.01). In conclusion muscle fatigue can interfere with a subject's ability to match a force. This suggests that muscle fatigue may contributes to the sensitization of the proprioception.

가쪽위관절융기 통증 환자에서 복합 부위와 단일 부위에 기능적 마사지와 스트레칭을 적용할 때 통증, 압통 역치와 악력에 미치는 영향 (Effects of Multi-site and Single-site Functional Massage and Stretching on Pain, Tenderness Threshold and Grip Strength in Patients with Lateral Epicondylalgia)

  • Background: The purpose of this study was to investigate the effects of functional massage and stretching, applied to the elbow and shoulder joints, on pain, tenderness threshold, and grip strength. Methods: A total of 29 individuals were assigned to a single site (n=15) or multiple sites (n=14). Pain measured through the visual analogue scale (VAS), tenderness threshold (TTH), and grip strength (GI) were measured before and four weeks after the intervention. Results: After four weeks of treatment, visual analogue scale significantly decreased in both groups (p<.05), and the tenderness threshold and grip strength significantly increased in both groups (p<.05). There was also a significant difference between the two groups (p<.05). Conclusion: The reduction of visual analogue scale and the increase in the tenderness threshold and grip strength were more significant in the multi-site treatment group than in the single-site treatment group.

Does the Addition of Visual Feedback Improve Postural Vertical Training in the Patients with Pusher Syndrome After Stroke?

  • PURPOSE: To compare postural vertical training with and without visual feedback for improving functional recovery in post-stroke hemiparesis patients with pusher syndrome. METHODS: This study used a single-subject research with alternating design with multiple baselines. Three patients with hemiparetic post-stroke diagnosed with pusher syndrome were selected from the inpatients at the department of physical therapy of a local rehabilitation hospital. For subjective postural vertical (SPV) training with and without visual feedback, an alternating treatment was used. The subjects were randomly selected using the sequence of the two training methods upon starting the intervention, and then the training was alternated. SPV training was performed twice a day, once in the morning and again in the afternoon. Scale for contraversive pushing (SCP), postural assessment scale for stroke, and Barthel index score were used to determine the intervention-related changes. RESULTS: Compared to the average score at baseline, the average SCP score for the SPV training without visual feedback decreased from 5.3 to 2.8, from 4.6 to 3, and from 3.5 to 2.7 for subjects 1, 2, and 3, respectively. However, the average score for the SPV training with visual feedback decreased from 5.3 to 3.1, from 4.6 to 3.5, and from 3.5 to 3.3 for subjects 1, 2, and 3, respectively. CONCLUSION: Postural vertical training without visual feedback may be more beneficial than postural vertical training with visual feedback for improving pushing behavior and functional activity in stroke patients with pusher syndrome.

스마트폰 사용시간과 스트레칭이 근경직도 및 집중력과 시력에 미치는 영향 (The Effects of Long-Term Smartphone Usage Time and of Stretching on Stiffness, Concentration, and Visual Acuity)

  • Purpose: This study is an investigation of the effects of long-term smart phone usage and of stretching on muscle stiffness, concentration, and visual acuity. Methods: Forty healthy young adults voluntarily participated in the study and were measured for muscle stiffness, concentration, and visual acuity before smartphone usage and after 30 minutes, 60 minutes, and 90 minutes of smartphone usage. The participants were randomly allocated to the intervention group (n=20) and the control group (n=20). Five minutes after the stretching exercise and eye exercise intervention, the participants were remeasured. Results: After long-term smartphone usage, increases in muscle stiffness and concentration were statistically significant (p<0.05), whereas decreases in visual acuity were statistically significant (p<0.05). After the stretching exercise and eye exercise intervention, muscle stiffness significantly decreased (p<0.05) and visual acuity significantly increased (p<0.05). Concentration, according to the stretching exercise and eye exercise intervention, showed no significant pre- or post-test difference. Conclusion: The results from this study indicate that long-term smartphone usage effectively improved concentration but increased muscle stiffness and decreased visual acuity. Appropriate stretching and eye movement during long-term smartphones usage should mitigate the effects on muscle stiffness and visual acuity. Furthermore, smartphones should be considered an effective tool for concentration training when coupled with the development of appropriate applications.

한국형 전산화 인지재활프로그램(CoTras)이 뇌졸중 환자의 시지각 기능 및 일상생활동작에 미치는 효과 (The Effects of Computer-Based Cognitive Rehabilitation Program(CoTras) for Visual Perception and ADL in Stroke)

  • 목적 : 본 연구는 한국형 전산화 인지재활 프로그램인 코드라스(CoTras)가 뇌졸중 환자의 시지각 기능 및 일상생활동작 수행능력에 미치는 효과를 알아보고자 실시되었다. 연구방법 : 재활전문 C 요양병원에 거주하고 있는 뇌졸중 환자 14명을 대상으로 실험 전과 후를 측정하였다. 처치 전 후 시지각 기능의 변화를 알아보기 위하여 사용한 시지각 평가도구는 비운동성 시지각 평가를 사용하였고 일상생활동작 수행능력을 알아보기 위하여 Fisher(1999)에 의해 개발된 운동 및 처리기술 평가도구를 이용하여 일상생활수행능력을 평가하였다. 전산화 인지재활 프로그램 전과 후를 측정하였으며, 수집된 자료는 SPSS 15.0 통계프로그램을 사용하여 분석하였다. 연구대상자의 치료 전과 후의 시지각 기능 및 일상생활동작 수행능력의 효과를 알아보기 위하여 짝비교(Paired t-test)를 이용하여 분석하였다. 연구결과 : 첫째, 코트라스(CoTras)치료 전과 후의 시지각 기능의 차이를 짝비교(Paired t-test)를 이용해 분석한 결과, 통계학적으로 유의한 차이가 있었다(0.000, p <.05). 둘째, AMPS 운동기술 평균점수는 0.90에서 1.11으로 증가되었으나 통계학적으로 유의한 차이는 없었다. 처리기술 평균점수는 0.08에서 0.46점으로 증가되었으나 통계학적으로 유의한 차이가 없었고, 임상적으로 유의하게 일상생활동작 수행능력이 향상되었다(logit >0.3). 결론 : 한국형 전산화 인지재활 프로그램인 코트라스는 뇌졸중 환자의 시지각 기능을 향상시키는 효과가 있었고, 일상생활 동작 수행능력에 임상적으로 긍정적인 영향을 주었다. 시지각에 장애를 가지고 있는 뇌졸중 환자에게 코트라스(CoTras)가 시지각 기능 증진에 기여할 수 있는 기초자료로 제시할 수 있다.

노르딕 워킹이 앞정강근 피로도에 미치는 영향 (EFFECTS OF NORDIC WALKING ON TIBIALIS ANTERIOR MUSCLE FATIGUE)

  • Purpose: The purpose of this study was to investigate tibialis anterior muscle fatigue through a Nordic walking when using pole. Methods: This study subject was 38 people who were twenties and randomly assigned to a control group with standard walking and Nordic walking group with Nordic walking. All subjects examined muscle fatigue with surface EMG and visual analogue scale for fatigue. The data were analyzed with SPSS window 18.0 program using ANCOVA. Results: The results were of a significant changes to the both tibialis anterior fatigue(p<0.01). The visual fatigue scales weren't significant difference. Conclusion: It was found that Dool-re-gil (Mt. Ji-Ri) was effective for muscle fatigue during walking, and it appears that it could be used clinically. So it is anticipated that improvement in clinical utilization for the people who's through a Nordic walking when using pole.
