Clinical Effectiveness of Traditional Herbal Medicine in the Treatment of Central Post-Stroke Pain and Thalamic Syndrome: a Systemic Review and Meta-analysis of Randomized Clinical Trials Based on Traditional Chinese Medicine Journals (뇌졸중 후 중추성 통증 및 시상증후군에 대한 한약치료의 효과 : 체계적 고찰 및 메타분석 - 중의학 저널을 중심으로)

  • Objectives: This research aimed to investigate Chinese clinical studies on the treatment of central post-stroke pain (CPSP) and thalamic syndrome after stroke with traditional herbal medicine (THM). Methods: Randomized controlled trials verifying the effects of herbal medicine on treating CPSP and thalamic syndrome after stroke were included in the study. Electrical and hand search were conducted in the China National Knowledge Infrastructure (CNKI), National Discovery for Science Leaders (NDSL), Research Information Sharing Service (RISS), Oriental Medicine Advanced Searching Integrated System (Oasis) for CPSP and thalamic syndrome after stroke. A literature search was performed in the Chinese and Korean databases for papers published from January 1, 2010 to October 1, 2018. The selected literature was assessed by Cochrane's risk of bias. Results: Twelve reports on randomized controlled trials met the inclusion criteria from the 227 identified reports. Effective rate, comparison of visual analogy scale, present pain intensity, pain grading index, recurrence rate, follow-up, and a 36-item short form survey instrument were used to evaluate the treatments. The effective rate of the treatment group was significantly higher than that of the control group in all papers. Side effects occurred less frequently in the treatment group than in the western medicine control group. Conclusions: The treatment of CPSP and thalamic syndrome after stroke with THM was shown to be highly effective. Additional well-designed clinical trials are needed. This study can be used as a basis for further research on the treatment of CPSP and thalamic syndrome after stroke.

Changes in the Laterality of Functional Connectivity Associated with Tinnitus: Resting-State fMRI Study

  • Purpose: One of the suggested potential mechanisms of tinnitus is an alteration in perception in the neural auditory pathway. The aim of this study was to investigate the difference in laterality in functional connectivity between tinnitus patients and healthy controls using resting state functional MRI (rs-fMRI). Materials and Methods: Thirty-eight chronic tinnitus subjects and 45 age-matched healthy controls were enrolled in this study. Connectivity was investigated using independent component analysis, and the laterality index map was calculated based on auditory (AN) and dorsal attention (DAN), default mode (DMN), sensorimotor, salience (SalN), and visual networks (VNs). The laterality index (LI) of tinnitus subjects was compared with that of normal controls using region-of-interest (ROI) and voxel-based methods and a two-sample unpaired t-test. Pearson correlation was conducted to assess the associations between the LI in each network and clinical variables. Results: The AN and VN showed significant differences in LI between the two groups in ROI analysis (P < 0.05), and the tinnitus group had clusters with significantly decreased laterality of AN, SalN, and VN in voxel-based comparisons. The AN was positively correlated with tinnitus distress (tinnitus handicap inventory), and the SalN was negatively correlated with symptom duration (P < 0.05). Conclusion: The results of this study suggest that various functional networks related to psychological distress can be modified by tinnitus, and that this interrelation can present differently on the right and left sides, according to the dominance of the network.

Development of Day Fog Detection Algorithm Based on the Optical and Textural Characteristics Using Himawari-8 Data

  • In this study, a hybrid-type of day fog detection algorithm (DFDA) was developed based on the optical and textural characteristics of fog top, using the Himawari-8 /Advanced Himawari Imager data. Supplementary data, such as temperatures of numerical weather prediction model and sea surface temperatures of operational sea surface temperature and sea ice analysis, were used for fog detection. And 10 minutes data from visibility meter from the Korea Meteorological Administration were used for a quantitative verification of the fog detection results. Normalized albedo of fog top was utilized to distinguish between fog and other objects such as clouds, land, and oceans. The normalized local standard deviation of the fog surface and temperature difference between fog top and air temperature were also assessed to separate the fog from low cloud. Initial threshold values (ITVs) for the fog detection elements were selected using hat-shaped threshold values through frequency distribution analysis of fog cases.And the ITVs were optimized through the iteration method in terms of maximization of POD and minimization of FAR. The visual inspection and a quantitative verification using a visibility meter showed that the DFDA successfully detected a wide range of fog. The quantitative verification in both training and verification cases, the average POD (FAR) was 0.75 (0.41) and 0.74 (0.46), respectively. However, sophistication of the threshold values of the detection elements, as well as utilization of other channel data are necessary as the fog detection levels vary for different fog cases(POD: 0.65-0.87, FAR: 0.30-0.53).

Prospective elementary teachers' preconceptions and experiences of diagrams in solving math word problems (초등예비교사의 수학 문장제 해결 도구로서 다이어그램에 대한 초기 관념과 수행)

  • This study involved an investigation of prospective elementary teachers' preconceptions and experiences of diagrams and their ability to draw diagrams in solving math word problems. A questionnaire and two math word problems were administered to prospective elementary teachers who began to taking an introductory mathematics education course. The results from the analysis of their responses to the questionnaire items indicate that prospective elementary teachers appreciate the value of diagrams as tools for problem solving and communication. In addition, prospective elementary teachers have the will not only to teach their future students how to use diagrams but also to encourage them to draw diagrams in solving math word problems. However, the results also indicates that prospective elementary teachers neither use diagrams spontaneously in their math problem solving activities nor have confidence in drawing useful diagrams. Prospective elementary teachers also manifested low scores on the questionnaire items asking whether they were taught how to draw useful diagrams or encouraged by their teachers to use diagrams in their previous learning experiences. The results from the analysis of the diagrams that prospective elementary teachers drew in solving math word problems showed that most of them had difficulty drawing diagrams that represent their reasoning and solving process.

Growth Pattern and Prognostic Factors of Untreated Nonfunctioning Pituitary Adenomas

  • Objective : Pituitary adenomas (PAs) are often detected as incidental findings. However, the natural history remains unclear. The objective of this study was to evaluate the natural history and growth pattern of untreated PAs. Methods : Between 2003 and 2014, 59 PAs were managed with clinico-radiological follow up for longer than 12 months without any kind of therapeutic intervention. Tumor volumes were calculated at initial and last follow-up visit, and tumor growth during the observation period was determined. Data were analyzed according to clinical and imaging characteristics. Results : The mean initial and last tumor volume and diameter were $1.83{\pm}2.97mL$ and $13.77{\pm}6.45mm$, $2.85{\pm}4.47mL$ and $15.75{\pm}8.08mm$, respectively. The mean annual tumor growth rate was $0.33{\pm}0.68mL/year$ during a mean observation period of $46.8{\pm}32.1months$. Sixteen (27%) PAs showed tumor growth. The initial tumor size (HR, 1.140; 95% confidence interval, 1.003-1.295; p=0.045) was the independent predictive factor that determined the tumor growth. Six patients (11%) of 56 conservatively managed non-symptomatic PAs underwent resection for aggravating visual symptoms with mean interval of 34.5 months from diagnosis. By Cox regression analysis, PAs of last longest diameter over 21.75 mm were a significant prognostic factor for eventual treatment. Conclusion : The initial tumor size of PAs was independently associated with the tumor growth. Six patients (11%) of conservatively managed PAs were likely to be treated eventually. PAs of last follow-up longest diameter over 21.75 mm were a significant prognostic factor for treatment. Further studies with a large series are required to determine treatment strategy.

Study of Integrated Brand Communication in Clean Beauty Cosmetics (클린뷰티 화장품에 나타난 통합 브랜드 커뮤니케이션 연구)

  • 'Clean beauty' attracts attention with an increasing interest in cosmetics without harmful ingredients as people wear masks in the age of COVID-19. Thus, this study selected and analyzed clean beauty cosmetic brands circulated in Korea on/offline in 2020. This study extracted 36 clean beauty brands and selected 20 suitable brands through an experts' analysis. For an analysis of clean beauty cosmetic brand communication, components: naming, logo, color, package, and website were drawn to conduct a survey. Preferred were the words they come up with when they think of nature or health for naming; wordmarks in a simple form for logo; greenish or yellowish for color; the simple form aligned center on the container body for package; and the images of plants, animals, and humans for website. To sum up the components, utilizing natural, clean, and light images harmoniously, acted as a factor for preferring the clean beauty cosmetic brands.

A Systematic Review and Meta-analysis of Acupotomy for Scoliosis

  • This review investigated the effectiveness and safety of acupotomy treatment for scoliosis. There were 7 online databases used in the search from inception to March 17, 2021, for randomized controlled trials of the use of acupotomy in patients with scoliosis. The Cochrane risk-of-bias tool was used to assess the methodological quality of the studies included (n = 12). A quantitative synthesis of the randomized controlled trials was performed using RevMan Version 5.3. The effect sizes of studies were presented as mean differences for continuous outcomes and risk ratios for dichotomous outcomes, with a 95% confidence interval. As part of combined therapy, acupotomy was reported to significantly improve Cobb's angle compared with other treatments. Likewise, the Visual Analog Scale score, the Oswestry Disability Index score, and pulmonary function were also reported to be improved following acupotomy combination therapy. Although 5 studies mentioned the criteria for reporting adverse events, only 1 study reported adverse events. In conclusion, acupotomy may be an effective treatment for scoliosis. However, the small number, and heterogeneity of the included studies, as well as the poor methodological quality, indicate that higher-quality studies should be conducted to verify the effectiveness and safety of acupotomy treatment for scoliosis.

Banhabaeckchulchunma-tang for Benign Paroxysmal Positional Vertigo: A Systematic Review and Meta-analysis (양성 발작성 자세변환성 현훈에 대한 반하백출천마탕(半夏白朮天麻湯)의 효과: 체계적 문헌고찰과 메타분석)

  • Objectives : The purpose of this study is to evaluate the effect of Banhabaeckchulchunm a-tang(BBCT) for Benign Paroxysmal Positional Vertigo(BPPV) Methods : We searched randomized controlled trials(RCTs) which assess the effect of BB CT for BPPV through 8 electronic databases from their inception to July 2021. RevMan 5.4 was used to evaluate the risk of bias. Results : 12 RCTs with 901 subjects were included. The BBCT treatment group had significantly higher total effetive rate(TER) than the western medicine treatment group(P=0.0001), and the Dizziness Handicap Inventory(DHI) score(P=0.003), traditional chinese medicine syndrome(TCM syn.) score(P<0.00001), and Visual analog scale(VAS)(P=0.0006) were significantly lower than the western medicine treatment group. The combined treatment of BBCT and canalith repositioning procedure(CRP) group had significantly higher TER than only CRP treatment group(P=0.02), and there was no significant difference in DHI score(P=0.12). TG(P=0.006) and TC(P=0.04) were significantly lower, and ApoA1 was significantly higher(P=0.0001). There was no significant difference in LDL(P=0.24). Conclusions : These results demonstrate that BBCT could be effective for BPPV especially after CRP. However, because of limits of included studies such as high heterogeneity between the literatures, unclear risk of bias, insufficient reports of adverse events(AEs), a well-designed RCTs with a low risk of bias in more diverse countries are needed in the future.

Short-term Benefits of Mobilization for Patients with Non-Specific Neck Pains: Executive Function and Neck Pain Intensity

  • Background: Cervical mobilization has been applied mainly for the improvement of arm and neck movements and pain reduction, and little research has been done to improve the executive function. Since this kind of so-called mechanical neck pain is one of most common symptoms, there are controversial issues about this with spine alignment. Posteroanterior (PA) mobilization from the Maitland concept is a process of examination, assessment, and treatment of neuromusculoskeletal disorder by manipulative physical therapy. Objective: To examine the short-term benefits of mobilization for patients with non-specific neck pain. Design: Dual-group Pretest-Posttest Design from the Quasi-Experimental research Methods: Fourteen participants (male 8, female 6; 20's of their age) with non-specific neck pains which are distributed all the unilateral or bilateral body side were recruited. Participants were categorized to Neck Pain with Movement Coordination Impairments (NPMCI) and Neck Pain with Mobility Deficits (NPMD) groups according to the results of physical examination. Professional physical therapist who has over 15-years-of clinical experience applicated manipulative therapy for the neck pain, an occupational therapist only conducted evaluations; K-NDI (Korean version of the Neck Disability Index), VAS (Visual Analog Scale), BDS-K (Korean version of Behavioral Dyscontrol Scale) for decreasing possible adverse effects; there were no person who reported other symptoms followed 4 weeks from the trial. Results: In the NPMCI group, data analysis indicated statistical differences between the PA mobilization interventions in NDI and BDS-K; even though, pain was reduced in VAS, this is not a significantly differ. In the NPMD group, data analysis represented statistical differences between the PA mobilization interventions in NDI, VAS and BDS-K; the scores were represented to be increased or the pain got relief. Conclusions: PA mobilization techniques according to Maitland concept have beneficial effects in patients with neck pain and other clinical positive effects which included neck disability, pain itself and motor function of upper extremity.

  • Clogging in drainage pipe is one of the important problems, so it needs a remedy urgently. Recently, scale in drainage pipe is removed by water jet cleaning and other treatment. But these treatments need much cost and regular management. The principal component of scale in drainage pipe is $CaCO_{3}$. It was observed the setting of $CaCO_{3}$ by some kind of analysis such as SEM, XRD, visual and weight measuring analysis to solve these problems and prevent setting formation in drainage pipe. As a result, in case of magnetic, particle shape of $CaCO_{3}$ is changed Aragonite from Calcite. Also in case of Quantum Stick, the amount of scale is reduced by expert inspector's result. As a whole, Magnetic treatment and Quantum Stick have the effect for protecting of scale precipitation.
