The relationship between the TMJ internal derangement state including rotational displacement and perforation and the clinical characteristics (회전변위와 천공을 포함한 측두하악관절 내장증 상태와 임상적 특징간의 상호관계)

  • This study was designed to reveal the correlationship between the internal derangement state of TMJ and clinical characteristics including pain and mandibular dysfunction. One hundred and twenty five subjects with TMJ signs and symptoms were chosen for two years. The level of pain and madibular dysfunction were evaluated by Visual Analog Scale(VAS) and Craniomandibular Index(CMI). The diagnostic categories of TMJ internal derangement were determined by arthrography and they included normal disc position, anterior disc displacement with reduction(ADDR), rotational disc displacement with reduction (RDDR), anterior disc displacement without reduction(ADDNR), and rotational disc displacement without reduction(RDDNR). Also disc perforation was used as a criteria to divide the diagnostic subgroups. The obtained results were as follows; 1. The patient distribution of each group was 5 in normal disc position(4%), 40 in ADDR(32%), 30 in RDDR(24%), 34 in ADDNR(27%), and 16 in RDDNR(13%). 2. Perforation was observed in 8% of ADDR, 10% of RDDR, 32% of ADDNR, and 19% of RDDNR. 3. CMI of non-reduction group was higher than that of reduction or normal group(p<0.05), but V AS showed no significant difference.4. CMI of perforation group was higher than that of non-perforation group in reduction group (p<0.05). 5. There were no significant differences of CMI and VAS between anterior disc displacement group and rotational disc displacement group in both reduction and non-reduction group. 6. CMI of RDDNR group was higher than that of RDDR group(p<0.05). 7. There were no significant difference of CMI and VAS between bilateral involvement group and unilateral involvement group(p<0.05).

5 Cases of Climacteric Syndrome Patients with Hwa-Byung (火病을 동반한 갱년기 환자 치험 5례)

  • Objectives: The purpose of this study is to evaluate the effects of Korean traditional treatments for Climacteric Syndrome Patients with Hwa-Byung. Methods: From February 2015 to May 2015, We treated 5 Cases Patients with Korean traditional treatments for a month. We measured treatment effects by Menopause Rating Scale (MRS), Beck's Depression Inventory (BDI), Visual Analogue Scale (VAS) and Digital Infrared Thermal Imaging (DITI). Results: After treatment, First, the scores of MRS, BDI were decreased. Second, the symptoms such as hot flush, pantalgia, palpitation, insomnia, fatigue were significantly improved. Also, the difference of 膻中 (CV17, Danjung) and 關元 (CV4, Guanyuan) temperatures were significantly decreased after treatment. Conclusions: This study suggests that Korean traditional treatments such as Gamicheonglijagam-hwan, acupuncture, moxibustion are effective on Climacteric Syndrome Patients with Hwa-Byung.

Clinical trial of oral administration of Bifidobacterium longum in dogs with atopic dermatitis

  • This study assessed the effects of probiotics in canine atopic dermatitis (AD). We enrolled 11 client-owned dogs with AD and randomly allocated them to two groups. The probiotics group (n = 7) was prescribed with Bifidobacterium longum while the control group (n = 4) received a placebo powder once a day for 12 weeks. In both groups, the degree of skin lesions was evaluated based on the Canine Atopic Dermatitis Extent and Severity Index (CADESI)-4. We also measured the transepidermal water loss (TEWL). We assessed pruritus severity using the pruritus visual analog scale (PVAS). Alteration of consumed drug doses was converted into medication scores. All the evaluation indices were surveyed every 4 weeks. In the probiotics group, there was a significant decrease in the CADESI-4 score at 4, 8, and 12 weeks compared to that of the baseline score (p < 0.05). There was no significant difference in TEWL, PVAS, and medication score at each time point and between groups. Although these results showed that Bifidobacterium longum did not reduce pruritus, TEWL, and the dosage of drugs for canine AD, it was effective in improving skin lesions, therefore, probiotics could be considered in canine AD with severe skin symptoms.

Posterior Cerebral Artery Aneurysm : Surgical Result of 11 Patients

  • Objective : Eleven patients treated with posterior cerebral artery[PCA] aneurysm during 6.3-years period are retrospectively reviewed to determine treatment outcome. Methods : Eleven patients with PCA aneurysm were treated from January 1998 to May 2004. Their medical records and radiologic studies were reviewed retrospectively. The records of these patients were analysed with particular reference to their demographic details, mode of presentation, and treatment outcome. Results : Of the 11 patients, 8 patients presented with symptoms related aneurysmal bleeding. Three patients had unruptured PCA aneurysms. Open or endovascular surgery was performed in 9 patients; None of these patients exhibited a third nerve palsy, visual field deficit, or hemiparesis at the time of presentation. Postoperatively, 2 made a good recovery, 2 had a moderate disability because of cerebral infarction after surgery, and 5 had a severe disability because of cerebral infarction after surgery. Of 2 conservatively treated patients, 1 was doing well but the other died as a result of brain swelling. Conclusion : The treatment of the PCA aneurysms is difficult regardless of the aneurysmal size, site, and treatment modality. All reasonable treatment to reduce the risk of associated morbidity should be considered.

A Survey of Patient Satisfaction after Treating Zygomatic Complex Fractures Using a Coronal Approach (관상절개술을 통한 관골 복합골절 치료에 대한 환자의 만족도 조사)

  • Purpose: It is difficult to objectively evaluate the outcomes of plastic surgical procedures. The combination of aesthetic and medical factors makes outcome quantification difficult. In this study, fracture reduction accuracy was objectively evaluated in patients with zygomatic complex fractures. Patients satisfaction with the accuracy was also examined. In addition, the patients' overall satisfaction and discomfort due to complications were analyzed. Methods: Eighty-five patients who had surgeries via bicoronal incision for zygomatic complex fracture from March 2006 to December 2009 were included in this study. Two plastic surgeons evaluated the accuracy of the fracture reduction with postoperative computed tomography. A survey questionnaire was administered to evaluate the patients' overall satisfaction and the impact of symptoms associated with the procedure on the patients' daily lives. Results: The overall patient satisfaction rate was $82.1{\pm}10.9%$ (range, 45~100%). The level of deformation was $6.7{\pm}10.9%$, the levels of discomfort in daily life due to pain, paresthesia, scar, and facial palsy were $8.5{\pm}13.2%$, $5.8{\pm}8.9%$, $4.4{\pm}9.9%$, and $1.9{\pm}9.2%$, respectively. According to the visual analogue scale, paresthesia was found to be the most frequent symptom (43.5%), and pain was the most troublesome symptom. Conclusion: The use of bicoronal incision for treating zygomatic complex fractures can cause various complications due to wide incision and dissection. However, this technique can provide optimized reduction and rigid fixation. Most of these postoperative complications can cause significant discomfort in the patient. It is thought that the use of correct surgical technique and the accurate knowledge of craniofacial anatomy will result in a reduction of complications and significantly increase patient satisfaction.

Schwannoma Originating from Infraorbital Nerve (안와하신경에서 기원한 신경초종의 치험례)

  • Schwannomas are well-differentiated solitary benign tumors that originate from the schwann cells of the nerve sheath. They can readily occur in the head and neck regions, but the schwannoma originating from the infraorbital nerve is extremely rare and usually painless, slow-growing, and without specific symptoms. The author experienced a rare case of infraorbital schwannoma, which was completely removed through the intraoral approach. A 20-year-old woman was admitted to our hospital for a painless, solid and circular mass located on the right infraorbital region. The eyeball movement and visual field were normal. There was no globe displacement or proptosis. Preoperative computed tomography demonstrated $13{\times}10{\times}5mm$-sized soft tissue mass. On March 2011, the mass was removed through an intraoral approach. On histopathological examination, the gross specimen consisted of a smooth, well-encapsulated and light yellowish solid mass, measuring $12{\times}7{\times}5mm$. Microscopically, it presented a typical manifestation of schwannoma with Antoni A area with Verocay body, and Antoni B area on H&E stain. The result of the immunohistochemical staining was positive for the S-100 protein. The patient had hypoesthesia of the nasal septum and vestibule in the postoperative period, and this finding confirmed that the internal nasal branch of infraorbital nerve was the nerve in which the schwannoma originated. Infraorbital schwannomas are very rare and must be included in the differential diagnosis of the orbital masses inferior to the eyeball. In the case of early diagnosis, the small-sized infraorbital schwannomas can be completely removed without any scar through an intraoral approach.

Single and mixed chelants-assisted phytoextraction of heavy metals in municipal waste dump soil by castor

  • The phytoextraction of some toxic heavy metals from municipal waste dump soil by castor plant (Ricinus communis) was tested under natural and single or mixed chelant-assisted scenarios in pot microcosms. A sandy loam with total metal contents (mg/kg): Cd (84.5), Cu (114.5), Ni (70.3), Pb (57.8), and Zn (117.5), was sampled from an active dumpsite in Calabar, Nigeria and used for the study. Castor (small seed variety) was grown under natural phytoextraction or single/binary chelant (citric acid, oxalic acid, and EDTA) applications (5-20 mmol/kg soil) for 63 days. Castor exhibited no visual phytotoxic symptoms with typically sigmoid growth profiles at the applied chelant doses. Growth rates, however, decelerated with increase in chelant dose. Post-harvest biomass yields were higher under chelant application than for natural phytoextraction. Both root and shoot metal concentrations (mg/kg) increased quasilinearly and significantly ($p{\leq}0.05$) with increase in chelant dose, furnishing maximum levels as: Cd (55.6 and 20.9), Cu (89.5 and 58.4), Ni (49.8 and 19.6), Pb (32.1 and 12.1), and Zn (99.5 and 46.6). Ranges of translocation factors, root and shoot bioaccumulation factors were 0.21-3.49, 0.01-0.89 and 0.01-0.51, respectively. Overall, the binary chelant treatments were less toxic for R. communis growth and enhanced metal accumulation in shoots to a greater extent than the single chelant scenarios, but more so when EDTA was present in the binary combination. This suggests that the mixed chelants could be considered as alternative treatments for enhanced phytoextraction and revegetation of degraded waste dump soils.

A Case Report of Postabortal Syndrome Patient (유산 후 산후풍 환자 1례에 대한 임상보고)

  • Objectives: This study is to report the clinical effect of oriental medicine on a patient suffering from postabortal syndrome with somatic pain disorder, Qi deficiency and depression. Methods: The patient had a miscarriage by cervical incompetence in 24 weeks' gestation even though she had operated cervical cerclage in 20 weeks' gestation. The patient received herbal medication, acupuncture, moxibustion during 20 days of outpatient treatment. The clinical effects were evaluated through VAS (Visual Analogue Scale) and EPDS (Edinburgh Postnatal Depression Scale Test). Results: The clinical symptoms of somatic pain and Qi deficiency were reduced during the treatment. The EPDS score also decreased from 19 points (high risk level) to 8 points (low risk level). Conclusions: This case study showed that herbal medicine, acupuncture and moxibustion treatment appeared to effectively reduce postabortal syndrome. Since patients previously experienced cervical incompetence tend to have habitual abortion for next pregnancy, further long term observation and preventive treatment are needed in this case for next safe pregnancy and childbirth.

A Case of Contact Dermatitis by Pain Relieving Patch Deteriorated after Taking Lotus Root (파스 접촉 알레르기 치료 중 연근 복용 후 악화된 중등도 알레르기 피부 1례)

  • Objectives : The purpose of this study is to report that Korean medical treatment is good for deteriorated moderate allergic skin after taking lotus root while getting treatment for contact dermatitis by pain relieving patch.Methods : A 54-year-old woman patient with allergic contact dermatitis received treatment with acupuncture, herbal medicine, etc. We used observation and Visual Anologue Scale(VAS) to evaluate recovery from erythema, pruritus and skin damages.Results : A visible symptoms to the naked eye and VAS scale were effectively decreased after Korean medicine treatments.Conclusions : Korean medical treatments were effective for allergic contact dermatitis deteriorated by lotus root while getting treatment contact dermatitis by pain relieving patch.

A case of Pustulosis Palmaris et Plantarsis with Extra-Palmoplantar lesions (수장족저외 병변을 겸한 수장족저농포증 치험 1례)

  • Objective : The purpose of this study is to report the Korean medicine treatment on Pustulosis Palmaris et Plataris with Extra-Palmoplantar lesions. Method : We adopted herbal medicine(Seungchung-tang-1, Eunjin-1), acupuncture and vapor treatment to treat Pustulosis Palmaris et Plantaris with Extra-Palmoplantar lesions. Steroids were administered for preventing aggravation. We observed patient's hot sensation, itching and pain of both hands and patient's itching and pain of both feet with the Visual Analogue scale(VAS). Also photographs were taken for judging inflammatory skin. Result : After the treatment, the grade of VAS was decreased from 10 to 0 and clinical symptoms were resolved. By judging photographs patient's inflammatory skin was cured. Administered steroid were also reduced from 15 mg per day to 0. Conclusion : This study shows that the Korean medicine treatment was effective to treat Pustulosis Palmaris et Plataris with Extra-Palmoplantar lesions and enable to stop steroid therapy.
