청리자감탕(淸離滋坎湯)으로 호전된 갱년기 여성의 상열감(上熱感)과 복부냉증(腹部冷症)에 대한 DITI를 이용한 평가 (A Clinical Case Study to Evaluate the Hot flush and Abdominal Cold Hypersensitivity Relief Efficacy of Cheonglijagamtang in Climacteric Women by DITI)

  • Purpose : To quantitative analyse the hot flush and abdominal cold hypersensitivity relief efficacy of Cheonglijagamtang in climacteric women by DITI. Methods : The patients was a 59-year-old climacteric woman who was suffered by hot flush and abdominal cold hypersensitivity. The patient was treated by herb medicine, acupuncture treatment, moxa treatment and physical treatment. The progress of symptoms was evaluated by checking the change of VAS, and inspecting the DITI. Results : Visual analog scale of hot flush and abdominal cold hypersensitivity in this climacteric women have decreased after treatment. The temperature of two abdominal site(RN12.RN4) after treatment get higher than those before treatment. Difference of acupoint-Indang and other abdominal site(RN12.RN4)'s temperature have decreased after treatment. Conclusion : The result suggest that DITI can be used for the diagnosis of hot flush and coldness relief efficacy in climacteric women.

구토(嘔吐)를 동반한 두위변화(頭位變化)에 따른 현훈증(眩暈症) 치험(治驗) 1예(例) (A Clinical Study about Treatment of Vertiginous Patient)

  • 1. Objectives Benign Paroxysmal Positional Vertigo (BPPV) is thought to be due to debris which has collected within a part of the inner ear. It is best characterized by true vertigo and geotropic nystagmus that occur a few seconds after specific head movements. The symptoms of BPPV include dizziness or vertigo, lightheadedness, imbalance, and nausea. The vertigo usually lasts no more than a minute. This study is reported to evaluate the effect of Geopung-san(祛風散) to patient with vertigo. 2. Methods This patient was treated by Geopung-san(祛風散) and Acupuncture, who was diagnosed as Soeumin interior disease. We used Visual analogue scale(VAS) for the assessment of vertigo. 3. Results This patient's main symptom is vertigo according to head movement with nausea and vomiting. So she was treated by Geopung-san(祛風散) in "Dongyisasangsinpyun(東醫四象新編)" and showed a positive response about her problems. 4. Conclusions We suggest that Geopung-san(祛風散) is effected significantly to patient with vertigo, but then we consider that studies about the case like this are more needed.

족저근막 이완요법과 침치료를 이용한 족저근막염 치험 2례 (Two Clinical Cases on Plantar Fasciitis Using Myofacial Releasing Therapy and Acupuncture Therapy)

  • Objectives : Plantar fasciitis is most common cause of heel pain which starts from anterior tubercle of calcaneus. It is chronic inflamation of plantar fascia, reduces collagen and water content of heel pad which incur the degenerative changes with elastic fiber weakness. We have evaluated the effect of myofacial releasing therapy and acupuncture therapy by experimenting two patients suffering from plantar fasciitis. Methods : Two patients were diagnosed as plantar fasciitis through their symptoms. We used myofacial releasing therapy and acupuncture therapy to the patients and measured VAS(Visual analogue scale). Results and Conclusions : After treating myofacial releasing therapy and acupuncture therapy in two cases, We figured out that the patients were on the mend. These results suggest that myofacial releasing therapy and acupuncture therapy were effective to heel pain.

외상성 경부 증후군의 치료를 위한 전척수신경차단의 효과 (Total Spinal Block for Treatment of Post-traumatic Cervical Syndrome)

  • In March of l992, We performed two intentional total spinal blocks for the relief of pain. This was for 2 cases of post-traumatic cervical syndrome whose various symptoms were chronically unresponsive to the usual conservative treatments. We regularly checked the blood pressure, pulse rate, oxygen saturation and observed clinically the changes of respiration, consciousness, lid and light reflexes during the total spinal block. Pain relief was evaluated by using the Visual Analog Scale which is designed to measure the subjective intensity of pain. The results were as follows; 1) The effectiveness of total spinal block was 60% in case 1, 40% in case 2. 2) We observed two complications from the procedure. Firstly during the block, these were transient periods of hypotension. Following the block, accidental procedures related direct neural trauma resulted in anterior chest wall pain. In conclusion we believe that total spinal block is a satisfactory and reliable method for the treatment of post-traumatic cervical syndrome.

경부 경막외 블록에 의한 편두통의 완화 경험 -증례 보고- (Improvement of Migraine by Cervical Epidural Block -A case report-)

  • Migraine is a disabling headache that can occur with or without aura. We present here a case of migraine that was effectively managed by a series of cervical epidural blocks. A 41-year-old woman who had suffered from severe headache on her left temporal area for 12 years visited our pain clinic. Her 11-point numeric pain rating scale was 10 out of 10 at the first visit and the symptoms were associated with homonymous visual disturbances, paresthesia on the left face, shoulder and arm, and general weakness. For the first 5 years after the headaches began, her headache was relatively well controlled by acetaminophen; after then, the acetaminophen wasn't effective. After wandering from this hospital to the next one in search of relief, she managed to visit our pain clinic. We tried several blocks including cervical epidural block, and she was continuously medicated with sumatriptan. Her headache was gradually relieved. Now, her 11-point numeric rating scale is 1-2 out of 10 at the most during her headache attacks.

수근관 증후군의 한의학적 치료에 대한 논문 고찰 -국내 학술지를 중심으로- (Korean Medicine for Carpal Tunnel Syndrome: a Systematic Review of Literature Published in Korea)

  • The aim of this review is to investigate studies of Korean medical treatment of carpal tunnel syndrome published in Korea and the evidence of it's effects in order to suggest a better research method in the future. Four Korean web databases were searched by using key words such as 'carpal', and checked relevant Korean journals. The papers were classified into the field of study, publish date, diagnostic methods, evaluation methods and analysed the study tendency. There are 9 case studies, 4 controlled studies. The paper was published almost every year more than once. Acupucture, pharmacopuncture, acupotomy, warm needle acupuncture, taping, etc. were used as treatment tools. The two most commonly used diagnostic method for carpal tunnel syndrome are Phalen's test and Tinel's sign. The two most commonly used evaluation method are VAS(Visual Analog Scale) and subjective symptoms. Korean medical treatment of carpal tunnel syndrome was effective in all studies. Korean medical treatment of carpal tunnel syndrome was effective in all studies. So it is possible that Korean medical treatment of carpal tunnel syndrome is useful as therapeutic medicine, but there are not sufficient evidence based papers, so there should be further studies.

오공 약침을 병행한 경추증성 척수증 환자 1례에 대한 임상적 고찰 (Clinical Study on a Case of Cervical Myelopathy with Additional Scolopendrae Corpus Pharmacopuncture)

  • Objective : The purpose of this study was to report a case of a treatment with Scolopendrae Corpus Herbal-Acupuncture for one patient suffering from the Cervical Myelopathy. Scolopendrae Corpus Herbal-Acupuncture has been used as a analgesics and antispasmodics in the oriental medicine and reported to have antiinflammatory effect, sedative action and immunity enhancing activity, on one patient suffering from the Cervical Myelopathy. Methods : TThe patient was treated with Scolopendrae Corpus Herbal-Acupuncture, acupuncture, herbal medication. Scolopendrae Corpus Pharmacopuncture was administered two or three times per week and we injected 0.2~1.0 ml of Scolopendrae Corpus Pharmacopuncture to the patient's neck and both arms. The improvement of the symptoms was evaluated by JOA score(Japanese Orthopedic Association score), Nurick grading, VAS(Visual Analog Scale), reflex testing, Finger escape sign, Grip and Release test. Result : After 5 weeks of treatment, the patient's JOA score, Nurick grading and VAS were improved from 12 to 16, from grade III to grade II and from 10 to 3 respectively. The movement and power of upper and lower limbs were restored to nearly normal range.

월비탕(越婢湯)으로 치료된 베체트병의 결절홍반양 병변 치험 1례 (A Case of Erythema Nodosum-like Lesions of a Behcet's Disease Patient treated with Wolbi-Tang)

  • Background and Objective : Erythema nodosum-like lesions in patients with Behcet's disease is a reactive dermatosis that occurs in response to various stimuli, such as bacterial, fungal and viral infections, drugs and malignancies. We observed and treated a case of erythema nodosum-like lesions with Behcet's disease. We present this case because there were no former reports of erythema nodosum-like lesions with Behcet's disease treated by herbal medicine. Methods : The patient was a 28-year-old. The chief complains were erythema, nodose, edema, burning sensation, tenderness on both legs, forearms and feet, arthralgia and genital ulcers. She was treated with Wolbi-Tang for eleven days. We diagnosed the medication according to the results of manual abdominal examination of the patient. We used the Visual Analogue Scale(VAS). Results : After the treatment the grade of VAS was decreased and clinical symptoms were improved. Conclusions : Wolbi-Tang can be used on the treatment of erythema nodosum-like lesions with Behcet's disease.

Comparison of the Short-Term Effectiveness of Hard and Soft Stabilizing Appliances in Treating Patients with Acute Temporomandibular Disc Displacement without Reduction

  • Purpose: The aim of this study is to compare the short-term effectiveness of hard and soft stabilizing appliances (H-A and S-A) in treating patients with acute temporomandibular joint (TMJ) disc displacement without reduction (DDwoR). Methods: Eighty one patients diagnosed as acute TMJ DDwoR at oral medicine of Chosun University Dental Hospital from November 2016 to September 2017 were included in this study. The subjects were divided into three groups: The control group (34 patients treated without occlusal appliance), the H-A group (31 patients treated with H-A) and the S-A group (16 patients treated with S-A). The pain intensity using Visual Analog Scale and maximum mouth opening (MMO) were evaluated every week for 8 weeks. Results: The improvement of the pain intensity and MMO in the H-A group and the S-A group compared with the control group were noted (p<0.05). Conclusions: This study suggests that hard and S-As may be equally useful in treating patients with TMJ DDwoR. S-A could be recommended to reduce the symptoms of TMJ DDwoR in short period.

  • Disuse atrophy involves gradual muscle weakening due to inadequate usage and can cause temporomandibular disorder (TMD). A 45-year old man with TMD symptoms on the left side, who had disuse atrophy of the masticatory muscles on the right side following surgical removal of a trigeminal schwannoma on the right side, first visited the Department of Orofacial Pain and Oral Medicine at Kyung Hee University Dental Hospital with left jaw pain and difficulty in opening mouth and chewing. He had been experiencing difficulties in cognitive function, decrease in visual acuity, impaired speech, and writing deficits after brain surgery. Furthermore, he complained of abnormal occlusion on the right side, which interfered with his ability to chew comfortably and open his mouth effectively. Herein, we describe a contralateral TMD case due to ipsilateral disuse atrophy after brain surgery for a trigeminal schwannoma and our successful treatment with medication, physical therapy, and stabilization splint.
