Clinical study on the effects of a composition containing oriental medicine for eye symptoms of VDT syndrome (한방조성물이 VDT 증후군의 안 증상 개선에 미치는 임상효능연구)

  • Objectives: This study was conducted to evaluate the clinical efficacy of EC102, a composition containing oriental medicine, for relieving VDT syndrome, including eye symptoms and musculo-skeletal symptoms. Methods: The study was conducted by using a double blind randomized controlled trial design. 40 subjects were randomly assigned to the placebo group or the EC102 group. The visual analogous scale (VAS) was used to evaluate various VDT symptoms, including eye pain, eye fatigue, back pain, shoulder pain and so on. Refractory indexes were measured using refractometer, and tear film break-up time (BUT) were measured using fluorescein strip. Blood circulation was measured using photo-plethysmography. Blood pressure and blood biochemistry were also measured. Statistical analysis was conducted by a Mann-Whitney test using SPSS 9.0 software. Results: The EC102 group showed a statistically significant reduction of eye symptoms including eye fatigue, eyelid fatigue, itching, pain, and photophobia when compared with the placebo group (P<0.05). The EC102 group also showed a statistically significant reduction of back pain compared with the placebo group (P<0.05). No side effects on liver function and blood biochemistry were observed. Conclusions: EC102 has significant clinical efficacy for relieving symptoms of VDT syndrome.

  • Objective : This study was designed to report the clinical effects of Cheonggan-soyosan-gamibang on patients who have suffered from UL-symptom(鬱症). Method : Patients complained of a headache, dizziness, chest discomfort, burning feeling, indigestion. Anorexia, insomnia, and fatigue, etc. We concluded that these symptoms are UL-symptoms(鬱症). Therefore, we treated patients with oriental medicine, especially Cheonggan-soyosan-gamibang, and found that those symptoms and signs were improved by VAS, which is visual analogue scale. Result : After patients were treated with Cheonggan-soyosan-gamibang, symptoms and signs were improved by VAS. Conclusion : Through this study, Cheonggan-soyosan-gamibang on patients who have suffered from UL-symptoms(鬱症), proved to be effective.

  • Objective : In order to investigate symptoms prevalence related to visual display terminal(VDT) syndrome, and to evaluate the variables affecting the development of the VDT syndrome. Methods : a questionnaire survey was conducted on 127(81 men and 46 women) VDT operators who had been studied in Soongsil University graduated school in Seoul. Result : As a result of analysis with data collected by questionnaire, l. Variables affecting the level of general symptoms were sex, the total time of VDT operation per day, type of typewriting, location of keyboard, illumination, VDT working duration, the time of VDT operation without rest. 2. Variables affecting the level of eye symptoms were sex, the total time of VDT operation per day, type of typewriting, VDT working duration, illumination, method of eye-resting. 3. Variables affecting the level of musculoskeletal symptoms were sex, the total time of VDT operation per day, the time of VDT operation without rest, illumination. Conculsions : In order to protect workers from VDT syndrome, it is urgently required to implement standard management recommendations including restriction of VDT working hours and allowance of more sufficient resting time for VDT workers.

4 Cases of Chronic Abdominal Pain Children Treated by Sihogyeji-tanggagam with Acupuncture and Moxibustion Treatment (시호계지탕가감방과 침구치료로 호전된 만성 복통 환아 4례)

  • This study aim to report 4 cases with chronic abdominal pain whose symptoms are alleviated by Sihogyeji-tanggagam with Korean medicine treatments. The patients were decided to administer Sihogyeji-tanggagam through abdominal examination. The patients were treated by Korean medicine, including acupuncture, moxibustion, and herbal medicine (Sihogyeji-tanggagam). We used Gastrointestinal Symptom Rating Scale (GSRS), Abdominal examination, Visual Analog Scale (VAS), and Subjective Unit of Clinical Symptoms to evaluate the progress of treatments. The improvement of clinical symptoms appeared to be effective with out any remarkable side effects. The abdominal pain of all patients stared to improve as soon as treatment begin, and disappeared in 1-2 weeks. We also confirmed that the abdominal symptoms improved together with the improvement of clinical symptoms. This study suggests that Sihogyeji-tanggagam is effective on reducing symptoms of chronic abdominal pain, but further studies should be followed.

Clinical Study on Predisposing Factors of Visual Acuity Decrease in the View of Oriental Medicine (편시력저하요인에 관한 한의학적 임상고찰)

  • Visual acuity Decrease is one of the most common symptoms of adolescents. Acupuncture method has been used for improvement of visual acuity from ancient period. But it is not sufficent to recognize underlying causes and predisposing factors of visual acuity decrease. So we investigate the common causes and factors on myopia outpatients group. We researched 992 outpatients who were treated at Department of Acupuncture & Moxibustion, National Medical Center from June 2001 to May 2002. The outpatients were classified into several groups according to visual acuity and age. Common predisposing factors of visual acuity decrease were assessed through questionnaires based on the Classics of the traditional oriental medical bibliography. The results are as follows; age distribution is 52.9$\%$ for female, 47.1$\%$ for male. The predisposing factors were ranked as poor posture(66.9$\%$), unbalanced diet(57.2$\%$), heredity(53.8$\%$), irregular diet habit(51.3$\%$) and so on. These results playa role as preliminary data in recongnition of myopia in the view of oriental medicine.

Psychotic Symptoms of Hashimoto's Encephalopathy: A Diagnostic Challenge

  • Hashimoto's encephalopathy (HE) is a rare and underdiagnosed neuropsychiatric illness. We present the case of a 17-year-old girl who was admitted to a tertiary-care psychiatric center with acute onset psychosis and fever. Her psychotic symptoms were characterized by persecutory and referential delusions, as well as tactile and visual hallucinations. Her acute behavioral disturbance warranted admission and treatment in a psychiatric setting (risperidone tablets, 3 mg/day). She had experienced an episode of fever with a unilateral visual acuity defect approximately 3 years before admission, which was resolved with treatment. Focused clinical examination revealed an enlarged thyroid, and baseline blood investigations, including thyroid function test results were normal. Abnormal laboratory investigations revealed elevated anti-thyroid peroxidase (anti-TPO) and anti-thyroglobulin (anti-TG) levels (anti-TPO of 480 IU/mL; anti-TG of 287 IU/mL). Results of other investigations for infection, including cerebrospinal fluid examination, electroencephalography, and brain magnetic resonance imaging were normal. She was diagnosed with HE and was treated with intravenous corticosteroids (methylprednisolone up to 1 g/day; tapered and discontinued after a month). The patient achieved complete remission of psychotic symptoms and normalization of the anti-thyroid antibody titers. Currently, at the seventh month of follow-up, the patient is doing well. This case highlights the fact that in the absence of well-defined clinical diagnostic criteria, a high index of suspicion is required for early diagnosis of HE. Psychiatrists need to explore for organic etiologies when dealing with acute psychiatric symptoms in a younger age group.

Changes in Plasma Prolactin and Growth Hormone Level and Visual Problem after Radiation Therapy (RT) of Pituitary Adenoma (뇌하수체선종: 방사선치료에 따른 PRL, GH 및 시야변화)

  • Twenty-four cases of pituitary adenoma, 13 males and 11 females with the age ranging from 11 to 65 years, received radiation therapy(RT) on the pituitary area with 6MV linear accelerator during past 25 months at the Division of Radiation Therapy, Kangnam St. Mary's Hospital, Catholic Medical College. Of 24 cases of RT, 20 were postoperative and 4 primary. To evaluate the effect of RT, we analysed the alteration of the endocrinologic tests, neurologic abnormalities, major clinical symptoms, endocrinologic changes and improvement in visual problems after RT. The results were as follows ; 1. Major clinical symptoms were headache, visual defects, diabetes insipidus, hypogonadisms and general weakness in decreasing order of frequency. 2. All but the one with Nelson's syndrome showed abnormal neuroradiologic changes in the sella turcica with an invasive tumor mass around supra· and para-sellar area. 3. Endocrinological classifications of the patient were 11 prolactinoma, 4 growth hormone -secreting tumors, 3 ACTH-secreting tumors consisting of one Cushing's disease and two Nelson's syndrome, and 6 nonfunctioning tumors. 4. Eleven of 14 patients, visual problems were improved after treatment but remaining 3 were unchanged. 5. Seven of 11 prolactinomas returned to normal hormonal level after postoperative and primary RT and 3 patients are being treated with bromocriptine (BMCP) but one lost case. 6. Two of 4 growth hormone·secreting tumor returned to normal level after RT but the remaining 2 are being treated with BMCP, as well.

A Case-Report of Soyangin with Acute Urinary Retention and Lower Urinary Tract Symptoms Treated by Yukmijihwang-tanggagambang (급성 요폐 및 하부요로증상의 소양인(少陽人) 육미지황탕가감방(六味地黃湯加減方) 치험 1례)

  • We report 1 case about Yukmijihwang-tanggagambang that was considered by Sasang constitution that was used for an elderly Soyangin patient with acute urinary retention, lower urinary tract symptoms and other symptoms. We treated an elderly Soyangin patient who suffered from acute urinary retention, lower urinary tract symptoms and other symptoms, who had history of benign prostatic hyperplasia. We considered his Sasang constitution and prescribed Yukmijihwnag-tanggagambang mainly. The improvement of his symptoms were evaluated by presence of symptoms, times of urination and visual analogue scale. After treatment that was been Yukmijihwang-tanggagambang mainly, patient's symptoms were disappeared or improved. And there was not any adverse effect. This study shows that using Yukmijihwang-tanggagambang that was considered by Sasang constitution can be effective treatment for an elderly Soyangin patient's acute urinary retention, lower urinary tract symptoms and other symptoms.

Visual fatigue in Watching 3 Dimension Television (3D TV 시청에 있어서 시청 피로)

  • Purpose: This study was to evaluate visual fatigue with passing of watching 3D TV in short term and with experience of watching 3DTV in long term. Methods: 98 adult subjects aged $33.5{\pm}5.5$ years (22 to 51 years; 12 females and 86 males) agreed to participate in this study. Subjects were asked to watch 52 inch LED 2D and 3D television (Shutter glasses method) at 2.7 meters for 65minutes with wearing their habitual glasses or contact lenses. For evaluating visual fatigue, subjects were verbally responded to 11 questions : eye straining, eye paining, dry eye, sore eye, watery eye, photophobia, blur vision, diplopia, eye fatigue, headache, and dizziness with scale 0 to 3 at each measurement while watching 3D and 2D TV. Results: The mean scores of visual fatigue were $2.08{\pm}2.14$, $3.19{\pm}3.02$, $3.40{\pm}3.37$, $3.53{\pm}3.07 $for after 5 minutes, 25 minutes, 45 minutes, and 65 minutes respectively for 3D TV, and $0.40{\pm}1.03$, $0.22{\pm}0.70$, $0.22{\pm}0.58$, and $0.17{\pm}0.52$ after 25, 45, and 65 minutes respectively for 2D TV. Visual fatigue for watching 3D TV was significantly higher than for watching 2D TV at all measurements sessions (paired t-test, p < 0.001). The visual fatigue significantly increased during watching 3D TV for 65 minutes (p < 0.001, RM-ANOVA). The visual fatigue during watching 3D TV was significantly increased until 25 minutes (paired t-test, p < 0.001), stable after that. For correlation between visual fatigue and 3D watching experience, the more 3D watching experiences were significantly the less visual fatigues in photophobia, blur vision, diplopic and dizzy symptoms (ANOVA, all F(1, 96) = 4.500, all p < 0.05), but there was not significantly different in the other symptoms (ANOVA, F (1, 96) = 2.123, p = 0.148). Conclusions: Visual fatigue for watching 3D TV was higher than for watching 2D TV, increase by 25 minutes. It was different by symptoms for correlation between visual fatigue and 3D watching experience.

Relationship Between the Prevalence of Allergic Rhinitis and Ocular Symptoms for Children in New Built School (신축된 초등학교 학생에서 눈 자각 증상과 알레르기 증상과의 관계)

  • Purpose: This study was performed to investigate the relationship between ocular and allergy symptoms in children by environmental pollutants emitted from new constructed building. Methods: Two different types of survey (6-Item Dry Eye Questionnaire and Ocular Surface Disease Index) were used to study dry eye syndromes and ocular subjective symptoms for a six-grade of elementary school students (54 males and 61 females) in newly built buildings in the past three month, located near capital city, Seoul. Atopy and medical history of allergic rhinitis were investigated with using questionnaire for allergy, and also nose itchiness and ocular symptoms were checked as visual analog scale(10-score) with using questionnaire of allergy rhinitis and conjunctivitis under the guideline of ARIA(Allergic Rhinitis Impacts on Asthma). Results: Fifty seven students (50%) from DEQ and 19 students (16.52%) from OSDI were diagnosed as dry eye, and 18 students (15.79%) were diagnosed as dry eye from both questionnaires. It was surveyed that 45 students (39.5%) and 29 students (25.4%) had atopy and allergy rhinitis, respectively. Among 45 students having atopy, 30 students and 8 students were diagnosed as dry eye with using DEQ and OSDI, respectively. And also, 22 students were diagnosed as dry eye by DEQ and 5 students were diagnosed as dry eye by OSDI from 29 students having allergy rhinitis. Among 29 students having both medical history atopy and allergy rhinitis, 15 students detected observable ocular symptom. The complicating allergy symptoms with ocular syndrome were arranged with sneeze, nose itchiness, nose stuffiness, and runny nose in order of frequency. As students recognized eye symptoms, the number of allergy found and student's number were increased proportionally. Conclusions: It was found that children's subjective ocular symptoms were confirmed in the environment as newly built building. The number of student for dry eye was proportionally increased to students having atopy and allergy symptoms.
