A Case Report on Post Spinal Surgery Syndrome in Restless Leg Syndrome (하지불안 증후군을 동반한 척추 수술 후 증후군 환자 치험 1례)

  • Objective: To report the effect of oriental medical treatment on post spinal surgery syndrome (PSSS) in restless leg syndrome (RLS). Methods: We treated this patient with oriental medicine and measured his progress using a Visual Analogue Scale (VAS), the Oswestry Disability Index (ODI), and the International Restless Leg Scale (IRLS). Results: After treatment, most symptoms had decreased: the VAS score dropped from 6 to 3, ODI from 22 to 14, and IRLS from 34 to 20. Conclusions: Oriental medicine may be effective in alleviating PSSS and RLS. However, more rigorous studies are required to identify exactly what treatment is more efficient for PSSS and RLS.

Case Report of the Patient with the Atopic Dermatitis, Psoriasis Treated with Mahaenggamseog-tang (마행감석탕(麻杏甘石湯)으로 호전을 보인 아토피 피부염, 건선 증례보고)

  • This report describes the effectiveness of mahaenggamseog-tang(麻杏甘石湯) on the patients who suffered from atopic dermatitis and psoriasis. We administered mahaenggamseog-tang with heat-clearing and lung-dispersing(淸熱宣肺) effect to two dermatitis patients who showed sweating with panting(汗出而喘), vexation and thirst(煩渴), aversion to heat(惡熱), agitation(煩燥), swelling of the face and eyes(面目浮腫). The severity of dermatitis was evaluated by visual analogue scale(VAS), pictures, SCORAD index(case 1), PASI(case 2). After the treatment, VAS/SCORAD index of case 1 changed from 9/71.5 to 0/6.1; VAS/PASI of case 2 from 8/36.8 to 1/6.8. Mahaenggamseog-tang could improved the symptoms of atopic dermatitis and psoriasis in this study. This study offers an approach to managing chronic dermatitis more effectively using Mahaenggamseog-tang which used mainly for bronchial diseases and obesity.

  • Adrenoleukodystrophy (ALD) is a rare X-linked genetic disorder associated with various central nervous system problems and adrenal insufficiency. It is common in childhood and symptoms include loss of previously acquired neurological abilities, seizures, ataxia, Addison's disease as well as degeneration of visual and auditory function. These children frequently require anesthesia during imagining procedure such as MRI or during some surgical procedures like gastrostomy. There is special need for careful management because of numerous anesthetic challenges like difficulty in cooperation, seizure disorders, life-threatening airway obstruction, copious oral secretion and possibility of aspiration. In addition, adrenal involvement and hypofunction must be considered for safe anesthesia management. We report a successful anesthetic management in a patient with ALD for dental procedures.

Investigation on the Meridian-Muscle Therapy for Myogenic Nerve Entrapment Syndrome (경근요법(經筋療法)을 통한 근원성 신경 포착 증후군의 치료)

  • Objectives : Scalenus anticus syndrome and Piriformis syndrome is representative of myogenic Nerve entrapment syndrome, and their clinical symptoms are similar to HIVD(herniated Intervertebral disc) of cervical or lumbar spine. But, distinguished by muscle test, these syndrome apply to Meridian-muscle therapy. Methods : Meridian-muscle therapy consists of Ashi(阿是)-point therapy, taping therapy, myofascial release technique, manipulation, their based on the traditional meridian-muscle theory. This theory is similar to myofascial pain syndrome in western medicine. The study population consisted of 9 patients who were already diagnosed as Nerve entrapment syndrome with radiological examination & physical examination and muscle test. The evaluation of clinical outcome was done by Visual Analogue Scale (VAS) and Pain Assesment Questionnaire(PAQ). Results and Conclusions : After treatment, All patient's VAS is decreased as $2.11{\pm}1.59$ and the evaluation of clinical effect was excellent(6 cases) or good(3 cases) according to PAQ. Conclusively, Meridian muscle therapy is efficacious against Nerve entrapment syndrome.

A Case Report on a Patient of Pain after Core Decompression Surgery on Avascular Necrosis of Femur Head Treated with Korean Medicine Treatments (대퇴골두 무혈성 괴사 감압술 시술 후 발생한 고관절 통증 환자의 치험 1례)

  • Objectives : The aim of this study was to report the improvements in pain that a woman experienced after receiving Korean medicine treatments for the pain associated with core decompression surgery due to avascular necrosis (AVN) of both femur heads. Methods : The patient was diagnosed as having AVN in both femur heads and underwent core decompression surgery. After operation, she had pain on her right inguinal region and her range of motion (ROM) was restricted. She was treated with Korean medicine including, herbal medicine, acupuncture, Chuna Manual Therapy and bee venom acupuncture (BV). This study was measured by using the Visual Analogue Scale (VAS) and the ROM scale. Results : After conservative treatment, the patient's pain was reduced and ROM was increased. Daily living quality had improved. Conclusions : In this case, Korean conservative medicine therapy with Chuna Manual Therapy had a positive effect on the symptoms after core decompression surgery due to avascular necrosis of the femur head.

A Clinical Study on the Factors that Influence Functional Evaluation of Stroke (중풍의 기능평가에 영향을 미치는 요인에 관한 임상적 고찰)

  • Objectives: This study investigated significant factors that influence functional evaluation of stroke so as to be a fundamental data for estimating prognosis of stroke patients. Methods: 204 patients were studied within 7 days of admission, after the diagnosis of stroke through brain CT scan, brain MRI scan and clinical observations. They were hospitalized in the oriental medical hospital of Dongeui University from February to July in 2001. They were examined at the early stage of onset, after 2 weeks, 4 weeks and 6 weeks, and measured for average mark and the degree of improvement by using the Activity Index. Results: Ischemic stroke, past history of stroke, hypertension, diabetes mellitus, risk factor of obesity, non-professional emergency treatment and hospitalizing time after 1 day from onset, high blood pressure, tachycardia pulse and high blood sugar in abnormal vital sign in acute stage, conscious, cognitive or communication disorder, motor aphasia, dysphagia, constipation for more than 3 days, urinary incontinence, visual field defect, insomnia, and chest discomfort in early stage of onset had a negative influence on functional evaluation. Conclusions: Type of stroke, past history, risk factors, emergency treatment and hospitalizing time after onset, abnormal vital sign and intercurrent symptoms in Acute stage were relevant factors in predicting functional evaluation of stroke.

Microvascular autologous submandibular gland transplantation in severe cases of keratoconjunctivitis sicca

  • Dry eye syndrome is a relatively common disease of the tears and ocular surfaces that results in discomfort, visual disturbance, and tear film instability with possible damage to the ocular surfaces. Microvascular submandibular gland (SMG) transfer offers a surgical alternative for a permanent autologous substitution of tears using the basal secretion of a transplanted SMG. Long-term follow-up reveals that this technique is a lasting and effective solution for patients with severe dry eye syndrome. The uncomfortable symptoms were relieved, and the frequency of use of pharmaceutical tear substitutes was reduced. Objective examination showed significant improvement in tear film and some features of ocular surface such as breakup time of tear film and corneal staining. Patients may suffer from obstruction of Wharton's duct or epiphora after surgery. Activation of secretion-related receptors could improve the early hypofunction of the denervated SMG and prevent the duct obstruction. Reduction surgery, partial SMG transplantation, uses of atropine gel or Botulinum toxin A could be the choices of treatment for epiphora.

A Case Report of Chronic Fatigue Syndrome treated by Jisilchija-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 지실치자탕(枳實梔子湯) 투여 후 호전된 만성피로증후군 1례)

  • Objective : The purpose of this paper is to report the improvement of patient with Chronic Fatigue Syndrome(CFS) treated by herb medication based on Shanghanlun disease pattern identification diagnostic system. Methods : According to 'Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions', the patient was diagnosed with Eumyangyeokchahunobok-byung, number 393 provision, and took Jisilchija-tang herb medication for 75days. The changing symptom of chronic fatigue syndrome was estimated by QOL-CFS(Chalder Fatigue Scale) and VAS(Visual Analogue Scale). Results : The QOL-CFS changed 21 to 8 and VAS changed 10 to 1. Conclusions : It is existing theory that Eumyangyeokchahunobok-byung is caused by 'having too much sex relation'. But the author diagnosed Eumyangyeokchahunobok-byung as 'Symptoms worsen when awaken at night, and sleep at day caused by poor sleep pattern' according to the palaeography about shanghanlun, and got a good results.

The Efficacy of Scalene Injection in Thoracic Outlet Syndrome

  • Objective : To evaluate the efficacy of scalene injection in patients with thoracic outlet syndrome. Methods : We selected 142 patients diagnosed with thoracic outlet syndrome between January 2005 and October 2009. We performed a series of scalene injection with conservative treatment in all cases. Patients rated their pain degrees using a visual analogue scale. We also evaluated the time to return to everyday life and work, and patients' functional capacity. Results : There were no complications or instances of inadvertent somatic or sympathetic ganglionic blockade after scalene injection. Overall, 111 patients (76.5%) experienced improved symptoms after the first set of scalene injection and 128 patients (88.2%) improved after scalene injection followed by conservative treatment. Of the 68 patients who returned to work during the study period, 54 returned within 1 week, and 62 within 2 weeks. Of those who returned to work, 61 reported nearly full functional capacity. We found that scalene injection was more effective in cases of thoracic outlet syndrome related to trauma than in those related to work-related repetitive stress. Conclusion : In patients with thoracic outlet syndrome, scalene injection effectively reduces pain. We recommend scalene injection as an adjunct to conservative treatment.

A Case Report of a Patient who was suspected Osteitis Pubis treated with Korean Medicine (한방치료로 호전된 치골염이 의심되는 보행장애 환자 치험 1례)

  • Objectives : The purpose of this case is to investigate the effect of Korean Medicine on a patient who was suspected osteitis pubis. Methods : The patient was treated with herbal medicine, acupuncture, cupping therapy and physical therapy. We evaluated the results of the treatment by Hughes Scale and Visual Analogue Scale(VAS). Results : After treatment, the patient's symptoms were improved. Conclusions : These results show us that Korean medicine has effect on a patient who was suspected osteitis pubis.
