A Case Report of Herbal medicine Byeong-geo-tang(竝祛湯 : bing qu tang) on rheumatoid arthritis (병거탕을 사용한 류마토이드 관절염 치험 1례)

  • Objectives: The purpose of this study is to find clinical approachment of rheumatoid arthritis with herbal medicine Byeong-geo-tang(竝祛湯 ; bing qu tang) Methods: We provided acupuncture, bee venom aqua - acupuncture, Byeong-geo-tang(竝祛湯 ; bing qu tang) on a patient of rheumatoid arthritis. Symptoms were estimated with the visual analog scale. Results: Patients chief complaints were pain and ankylosis of her right fingers and left toes and pain of her both wrists. Symptoms were relieved from 7 to 0 VAS after treatment. Conclusions: We observed that complex therapy of acupuncture, bee venom aqua - acupuncture, Byeong-geo-tang(竝祛湯 ; bing qu tang) redused symptoms of the patient. We expect this clinical report may be helpful to rheumatoid arthritis treatment.

A Comparative Study on the Characteristics of Performance-related Musculoskeletal Symptoms between Orchestra Performers and University Student Performers (관현악단 전문 연주자와 대학생 연주자의 연주 관련 근골격계 증상의 발생 특성에 관한 비교)

  • Background: Instrument players are exposed to occupational musculoskeletal symptoms due to constant movement and strain to meet the needs of their audience. This can cause dissonance between the player and the instrument and can cause medical problems in the musculoskeletal system such as pain, tendinitis, muscle spasms, and joint hyperextension from overuse of body parts. Objective: The purpose of this study is to comparatively analyze the characteristics and occurrence of musculoskeletal symptoms in professional and student players, and to find out the awareness of the need for health care professionals and special exercise programs for orchestra players. Methods: The subjects were professional orchestral musicians 191 from the three symphony orchestras and music students 209 from the four universities volunteered to participate in this study; 393 subjects (98.25%) completed the questionnaire. symptom prevalence and related factors of playing-related musculoskeletal disorders (PRMDs) were surveyed with a self-administered questionnaire. Standardized Nordic Questionnaire was complemented by adding a visual-among scale and used to diagnose PRMD. Results: High prevalence of musculoskeletal complaints in both professional musicians (85.30%) and student musicians (82.30%). Professional musicians and student musicians also suffered the highest problem in shoulder and neck complaints. Significant differences were found between groups that recognized of necessity for specialized health professionals, professional organizations, and special programs for musicians. Conclusions: Professional musicians and student musicians, like orchestra players, are already exposed to overuse syndrome. However, student musicians lack awareness of the risk of injury compared to orchestra players. We suggest that an appropriate treatment management program for the prevention and early treatment of musculoskeletal system damage should be provided to performers at an early stage.

EFFICACY OF HAMῡL OF MARHAME DAKHILYUN AND ROGHANE SAUSAN IN CERVICITIS (ILTIHAB-E-UNUQ-UL- REHAM)- AN OPEN OBSERVATIONAL CLINICAL STUDY

  • Background and objective: Cervicitis are an inflammatory condition of the cervix. This may be acute, chronic, active & specific or non-specific, which may cause various sign & symptoms. Abnormal vaginal discharge, lower abdominal pain, lower backache, post coital bleeding. If not treated timely it causes various complications viz PID, infertility, endometritis, ectopic pregnancy etc. Hence it has been decided to conduct a clinical trial for its management. Method: This study was an open observational study. The entire patients were allocated by considering the criteria of inclusion & exclusion. Marhame dakhilyun with roghane sausan is given 10 mg as ḥamūl at bed time, after menses for 21 days. All the patients were assessed by primary outcome of abnormal vaginal discharge, lower abdominal pain, lower backache, post coital bleeding & secondary outcomes of vaginal symptoms scale score (VSS) which score the vaginal discharge with QOL in cervicitis patients and vaginal analogue scale (VAS) for pain. Result: In this present study, marked improvement is observed in cervicitis. The mean ± SD of vaginal discharge before & after treatment is 2.57±0.050, 0.33±0.48 respectively which is highly significant with p value of<0.0001**. VSS score before & after treatment is 21.27±6.12, 7.47±2.48 respectively with p<0.0001** which is highly significant. VAS score used for LAB & LPA before & after treatment is 6.63±1.09, 1.90±1.29 respectively with p<0.0001** which is highly significant. Interpretation & Conclusion: The study revealed that the formulation has been found effective in healing congestion, hypertrophied of the cervix and discharge and relieving the others associated symptoms of cervicitis. It is useful and provided immediate and effective treatment for cervicitis. Hence, the trial drug can be recommended for its management.

A Case Report of Korean Medicine Treatment for an Intractable Hiccup Patient with Unknown Cause after a Traffic Accident (교통사고 이후 발생한 원인불명의 난치성 딸꾹질 환자의 한방 치료 1례)

  • Objectives: The purpose of this study was to report a case of a woman with intractable hiccups after a traffic accident through Korean medicine treatment. Methods: A combination of treatments, including herbal medicine (Soshiho-tang), pharmacopuncture, acupuncture, Chuna, cupping, and moxibustion, was administered to the patient for 21 days. The frequency and duration of hiccups, the degree of body shaking, and total sleep time were measured daily. The accompanying symptoms were measured daily using the Visual Analogue Scale. Results: After Korean medicine treatment, Hiccup symptoms disappeared. Further, the degree of clinical symptoms and QOL improved significantly after Korean medicine treatment. No adverse events were observed. Conclusions: This study suggests that Korean medicine treatment can be an effective treatment for intractable hiccups after traffic accidents.

A Case Report of Korean Medicine Treatment Including Samchulgunbi-tang-gagambang in a Patient with Vestibular Neuritis (비위허약으로 변증된 전정신경염 환자의 삼출건비탕가감방을 포함한 한의 치료 치험 1례)

  • Background: Vestibular neuritis is a common cause of acute unilateral peripheral vestibulopathy. Vestibular neuritis is the second most common disease among patients with dizziness. Clinical symptoms of vestibular neuritis include the sudden onset of vertigo with spontaneous nystagmus, unsteady gait, nausea, and vomiting that last from days to weeks. However, even after the vertigo disappears, difficulty maintaining balance while walking may persist for weeks to months. Antihistamines, serotonin receptor blockers, and benzodiazepine vestibular suppressants are widely used as symptomatic treatments to reduce the severity of symptoms that occur in the acute phase. Case Summary: A patient diagnosed with acute vestibular neuritis was treated with acupuncture, moxibustion, and herbal medicine. We used the visual analog scale (VAS) to assess each symptom and the vertigo score to observe the effect of treatment. After treatment, the VAS scores for each symptom and the vertigo score decreased, and the severity of nystagmus was reduced. Conclusion: This study suggests that Korean medicine treatments, including Samchulgunbi-tang-gagam, could be effective in improving the clinical symptoms of vestibular neuritis.

Surgical Outcomes of Pituitary Apoplexy

  • Objective : Pituitary apoplexy is a rare clinical syndrome caused by pituitary hemorrhage, hemorrhagic infarction, or ischemic infarction within a pituitary tumor or surrounding structure. We analyzed surgical outcomes of pituitary apoplexy. Methods : From 1995 to 2004, we reviewed our experience of 29 cases with pituitary apoplexy. In all patients, pre- and postoperative clinical presentation were checked and endocrine study were performed. Results : The most frequent symptoms were visual disturbance [24 cases, 82.8%] and headache [22 cases, 75.9%]. After surgery, headache improved in 86.4%, 88.9% among 18cases who had preoperative reduction in visual acuity and 75.0% among 12 cases who had preoperative reduction in visual field improved. In endocrine study, long-term steroid and thyroid hormone replacement therapy was necessary in 42.9% of 14 cases presenting preoperative hypopituitarism. Postoperative transient hypopituitarism developed in 5 cases [33.3%] and they all recovered in follow up study. Postoperative endocrinological recovery were in 77.9% of 9 cases with preoperative prolactinoma, 1 case in 2 cases with acromegaly and one case with Cushing disease. Postoperative complications were diabetes inspidus[DI] in 1 case [3.4%]. cerebrospinal fluid[CSF] leakage in 2 cases [6.8%] and death in 1 case [3.4%] due to sepsis, Conclusion : We report good results through surgery of pituitary apoplexy in a clinical and endocrine outcomes. The surgery should be performed as soon as possible to be a suitable method for treating pituitary apoplexy.

Encephalo-duro-arterio-synangiosis(EDAS) using Occipital Artery in Children with Moyamoya Disease

  • Objective : Although an encephaloduroarteriosynangiosis procedure using the superficial temporal artery [STA-EDAS] is an effective indirect bypass method in children with moyamoya disease[MMD], there is still a need for an additional bypass operation that can cover the area of the posterior circulation. The goal of this study is to evaluate the efficacy of the EDAS procedure using the occipital arteries [OA-EDAS]. Methods : From August 2003 to April 2004, We performed OA-EDAS in sixteen patients with MMD who have a circulatory insufficiency in the territory of the posterior cerebral artery[PCA]. The medical records were reviewed retrospectively. The surgical outcomes, including the changes in neurological status and imaging studies, with the degree of neovascularization on the cerebral angiogram, and the hemodynamic changes on single-photon emission computed tomography[SPECT], were analyzed. Results : These 16 children consisted of 5 boys and 11 girls aged 2 to 9 years. The clinical outcome of their PCA symptoms, such as visual transient ischemic attacks[TIAs] or visual field defect, was favorable in 14 patients of 16. Nine patients of 11 who underwent follow up magnetic resonance imaging[MRI] showed favorable MRI changes. On angiogram most of the patients exhibited good or fair revascularization of the PCA territory [7 of 8]. The hemodynamic changes on SPECT in the PCA territory after surgery showed improved vascular reserve in 13 of the 16 territories. Conclusion : OA-EDAS is a safe and efficacious revascularization procedure in patients with MMD who have compromised cerebral perfusion in PCA territory, or with visual TIAs.

Binocular Vision Corrective Spectacle Lenses Reduce Visual Fatigue in 3-D Television Viewing (양안시 교정안경의 3차원 텔레비전 시청 중 발생한 안정피로 감소)

  • Purpose: Three-dimensional (3D) displays are very useful in many fields, but induce physical discomforts in some people. This study is to assess symptom type and severity of asthenopia with their habitual distance corrective spectacle (HDCS) and their binocular vision corrective spectacle lenses (BVCSL) in people who feel physical discomforts. Methods: 35 adult subjects (ages $32.2{\pm}4.4$ yrs) were pre-screened out of 98 individuals to have the highest symptom/asthenopia scores following 65 minutes of 3D television viewing with HDCS. These 35 individuals were then retested symptom/asthenopia scores during they watched 3D television for 65 minutes at a distance of 2.7 m with wearing BVCSL of horizontal, vertical or base down yoked prisms. A 4-point symptom-rating scale questionnaire (0=no symptom and 3=severe) was used to assess 11 symptoms (e.g., blur, diplopia, etc.) related to visual fatigue/visual discomfort. Distance and near lateral phoria were measured using Howell phoria card and vertical phoria were measured using Maddox rod. Symptoms induced by watching 3D TV were compared between wearing HDCS and BVCSL. Results: Asthenopia in watching 3D TV with wearing BVCS was significantly lower than wearing HDCS at 5, 25, 45, and 65 minutes (all p < 0.001, paired t-tests). In only refractive error power correction power group, all asthenopia was not significantly different between HDCS and BVCSL (all $p{\geq}0.05$, paired t-tests). In prism correction groups for binocular imbalance, symptoms of asthenopia, however, was significantly lower for when wearing BVCSL than when wearing HDCS (all p < 0.05). Conclusions: Correction of phoria/vergence-based binocular vision imbalance can reduce asthenopia during 3D television viewing. An individual with binocular vision imbalance need corrected/compensated glasses with appropriate prisms prior to prolonged viewing of 3D television displays to reduce asthenopia/visual fatigue.

Related Factors to Musculoskeletal Discomfort Symptoms on Some Middle·High school Teachers (일부 중·고등학교 교사의 근골격계 불편증상 관련요인)

  • This cross-sectional study was investigated musculoskeletal discomfort symptoms and related factors on some middle high school teachers. Self-questionnaire of KOSHA CODE H-30-2003 was done with 250 teachers from 1st to 15th October, 2010, the data from 231 teachers (68 male, 163 female) was statistically analyzed to search the factors related to musculoskeletal discomfort symptoms. According to NIOSH rate of musculoskeletal discomfort symptoms by body parts was 36.8%. Musculoskeletal discomfort symptoms related to age, school types, subjective health status, housekeeping time, VDT work time and regular rest. After adjusting for related variables, odds ratio (OR) of musculoskeletal discomfort symptoms was correlation significantly to subjective health status unhealthy (OR 11.75, 95% Confidence Interval, CI, 3.56-378.78). In addition, ORs (95% CI) of age (40-49) and housekeeping time (${\geq}3$) were 4.63 (1.82-26.18) and 4.33 (1.97-19.34). Analysis of the factors influencing the musculoskeletal discomfort symptoms vary in different parts of the body. The most discomfort symptoms by parts was neck (26.0%) and shoulder (30.0%). In the neck region was related to subjective health status and regular rest. In the shoulder and waist region was subjective health status and sex. Age was wrist/finger, leg/foot was related to subjective health status, sex and VDT work time. Age, school types, subjective health status, housekeeping time, VDT work time and regular rest related to musculoskeletal discomfort symptoms and the most discomfort symptoms by parts was neck and shoulder.

Factors Related to Depressive Symptoms Among the Elderly in Urban and Rural Areas (일부 지역사회 노인들의 우울수준과 관련요인)

  • Objectives: This study was performed to determine the levels of depression symptoms among the community elderlies and to reveal its related factors, specifically aimed at revealing psycho-social factors such as social support net-work, sense of self-esteem, state anxiety, interpersonal behavior trait and locus of control. Methods: The interviews were performed during the period from April 1st to June 30th, 2007 to the 790 elderlies in urban and rural areas. Results: The mean scores of depression (CES-D) among all subjects were 16.65±10.97(urban: 18.81±10.31, rural: 14.49±11.20) and they were higher in urban than rural. The level of depression symptoms was influenced by the variables of educational level, with or without spouse, bear for living expenses, sense of satisfaction in daily life. As for health related behaviors, they were influenced by the variables of with or without regular exercise. As for health status, they were influenced by the subjective health status, with or without disability of body, visual acuity, hearing ability, ability of food mastication, urinary incontinence, with or without help in activity of daily living (ADL) and instrumental activity of daily living (IADL). As for social support net-work and social activities, they were influenced by number of offspring, frequency of offspring contents, frequency of going out and activity of hobbies. As for psycho-social factors, they were influenced by sense of self-esteem, state anxiety and social support, especially, the level of depression symptoms was higher influenced by the variables of psycho-social factors than other variables. Conclusions: Lower of the level of depression symptoms among the community elderlies would require development and application of programs to keep under management psycho-social factors as well as looking for ways to promoting the present health status.
