A Clinical Case Study of Idiopathic Bilateral Facial Nerve Palsy Treated with Korean and Western Medicine (특발성 양측성 안면신경마비의 한양방 복합치료 치험 1례)

  • Objectives : The purpose of this study is to report on the good results achieved through Korean and western medicine treatment on patients with idiopathic bilateral facial nerve palsy. Methods : We treated a female patient with idiopathic bilateral facial nerve palsy by Korean medicine treatment such as herbal medicine, acupuncture, pharmacopuncture, cupping therapy, and taping therapy. The patient also received western medicine treatment. The treatment effect was evaluated by visual observation through photography, House-Brackmann scale, Yanagihara's scale, standardization of muscular paralysis, and clinical symptoms. Results : After the treatment, muscle movements and clinical symptoms improved, and all facial palsy evaluation indicators also showed improvement. In particular, the House-Brackmann-scale showed improvement from (3/4) to (1/2). Conclusions : This case study shows that Korean-western medicine combination treatment for idiopathic bilateral facial nerve palsy can be effective. We hope that case reports and clinical research will continue to be actively conducted in the future.

A Case of Facial Flushing and Xerotic Eczema that Improved with Korean Medicine Treatment Focused on Paljin-tang Gamibang (팔진탕가미방(八珍湯加味方) 중심의 한의 치료로 호전된 안면홍조와 건성습진 치험 1례)

  • Objectives : The purpose of this study is to report the effect of paljin-tang gamibang on facial flushing and xerotic eczema. Methods : We treated a 72-year-old female elderly patient who has facial flushing and xerotic eczema by korean medicine. She was treated from December 13th, 2023 to February 28th, 2024. The treatment applied was acupuncture, pharmacopuncture, herbal ointment and herbal medicine including 'paljin-tang gamibang'. The severity of symptoms was evaluated by photo comparison, visual analogue scale(VAS) and dermatology life quality index(DLQI). Results : After 11 weeks of treatment, the patient's symptoms improved. VAS of pruritus changed 10 to 1, VAS of dryness changed 10 to 1, DLQI changed 25 to 3. Conclusions : This case study showed an effectiveness of Korean medicine treatment including paljin-tang gamibang treated on facial flushing and xerotic eczema.

Clinical Outcomes of Pulsed Radiofrequency Neuromodulation for the Treatment of Occipital Neuralgia

  • Objective : Occipital neuralgia is characterized by paroxysmal jabbing pain in the dermatomes of the greater or lesser occipital nerves caused by irritation of these nerves. Although several therapies have been reported, they have only temporary therapeutic effects. We report the results of pulsed radiofrequency treatment of the occipital nerve, which was used to treat occipital neuralgia. Methods : Patients were diagnosed with occipital neuralgia according to the International Classification of Headache Disorders classification criteria. We performed pulsed radiofrequency neuromodulation when patients presented with clinical findings suggestive occipital neuralgia with positive diagnostic block of the occipital nerves with local anesthetics. Patients were analyzed according to age, duration of symptoms, surgical results, complications and recurrence. Pain was measured every month after the procedure using the visual analog and total pain indexes. Results : From 2010, ten patients were included in the study. The mean age was 52 years (34-70 years). The mean follow-up period was 7.5 months (6-10 months). Mean Visual Analog Scale and mean total pain index scores declined by 6.1 units and 192.1 units, respectively, during the follow-up period. No complications were reported. Conclusion : Pulsed radiofrequency neuromodulation of the occipital nerve is an effective treatment for occipital neuralgia. Further controlled prospective studies are necessary to evaluate the exact effects and long-term outcomes of this treatment method.

Comparative Analysis of the Training Fidelity of Vision Training Trainee (시기능 훈련 대상자의 훈련 충실도 비교 분석)

  • Purpose: The purpose of this case study is to analyze of the training fidelity of vision training trainee. Mehtods: The vision training observation group included 75 clients (male=40, female=35) who had binocular vision disorders without opthalmic diseases or vertical phoria. Results: A high degree of visiting rate and training fidelity showed improvement of binocular vision function, sensation and symptoms. It had a high degree of success rate and preference rate. Conclusions: We suggest that high training fidelity is an effective and alternative method for binocular vision disorder therapy.

A Case Report of Sequela of Operation of Talus Osteonecrosis (Osteonecrosis 수술 후 족과부동통(足踝部疼痛) 후유증 치험 1례)

  • Objective : The clinic study or report about Oriental Medical treatment about osteonecrosis is very insufficient. Therefore, we report a case about a sequela of operation of talus osteonecrosis treated by Oriental Medical treatments. Methods : The patient was managed by bee venom and Carthami Flos Herbal-Acupuncture, Sa-am and body acupucture, moxibustion, physical theraphy and herbal medicine. We evaluated the patient through Visual Analogue Scale(VAS). Results : After 25 days of treatment, the patient showed that clinical symptoms was decreased and VAS changed from 10 to 2. Conclusion : In this case, Oriental Medical treatments for a sequela of operation of talus osteonecrosis was effective. But further studies are required to confirm the effect of these methods.

Two Cases on Anti-inflammatory Effect of Scolopendrid Herbal-Acupuncture (오공약침(蜈蚣藥鍼)의 항염증 효과 치험 2례)

  • Objective : This study was designed to investigate the anti-inflammatory effect of Scolopendrid Herbal-Acupuncture. Methods : Scolopendrid Herbal-Acupuncture was administered one time on everyday and A-shi points were selected. Except Scolopendrid Herbal-Acupuncture administration, other therapies(conventional body acupuncture, physical therapy and so on) were not performed at all. We evaluated the patient through Visual Analogue Scale(VAS) and Digital Infrared Thermal Imaging(DITI). Results : After 2 or 3 times of treatment, the patient showed that clinical symptoms and VAS were lessened and there was a change of temperature in DITI. Conclusions : According to the results, Scolopendrid Herbal-Acupuncture may have anti-inflammatory effects. But further studies are required to prove the exact effect.

Characteristics of Visuo-Spatial Information Processing in Children with Autism Spectrum Disorder

  • Although atypical sensory processing is a core feature of autism spectrum disorder (ASD), there is considerable heterogeneity among ASD individuals in the modality and symptoms of atypical sensory processing. The present study examined visual processing of children with ASD, focusing on the complexity and orientation of visual information. Age- and -IQ-matched Korean children (14 ASD and 14 typically-developing (TD) children) received an orientation discrimination task involving static spatial gratings varied in complexity (simple versus complex) and orientation (horizontal versus vertical). The results revealed that ASD children had difficulty perceiving complex information regardless of orientation, whereas TD children had more difficulty with vertical gratings than horizontal gratings. Thus, group-level differences between ASD and TD children appeared greater when gratings were presented horizontally. Unlike ASD adult literature, however, ASD children did not show superior performance on simple gratings. Our findings on typical and atypical processing of ASD children have implications for both understanding the characteristics of ASD children and developing diagnostic tools for ASD.

Comparison of safety and analgesic efficacy of diclofenac sodium with etodolac after surgical extraction of third molars: a randomized, double-blind, double-dummy, parallel-group study

  • Background: Surgical extraction of third molars is associated with postoperative pain and swelling at the extraction site. Pain is commonly managed using non-steroidal anti-inflammatory drugs (NSAIDs). Postoperative pain is usually moderate to severe in the first 12 h postoperatively and lasts for 3-5 days. However, with NSAIDs, these symptoms usually subside within 24 h. Diclofenac sodium and etodolac are NSAIDs, more selectively cyclooxygenase-2 inhibitors, with good analgesic efficacies. Methods: We compared the safety and analgesic efficacy of diclofenac sodium with etodolac peroral after surgical extraction of third molars in a double-blind, double-dummy, parallel-group study. The subjective pain improvement and pain relief after 2, 6, 24, 48, and 72 h using the visual analogue scale were measured as the study outcome. Results: Etodolac was equivalent to diclofenac sodium in pain alleviation at all postoperative time periods. No significant differences were found between diclofenac sodium and etodolac groups (P > 0.05). Both study medications were well tolerated and safe with mild adverse effects in only a few participants. Conclusion: Diclofenac sodium and etodolac are comparable in terms of analgesic efficacy and safety after surgical removal of third molars.

Investigation on the correlation Improvement Rate of Symptoms with Moire Topography Analytic Improvement Rate (추나요법을 시행한 요통환자의 호전도와 Moire 영상 개선도와의 상관관계)

  • The present study retrospectively investigated clinical outcome at patients with low back pain or sciatica during Chuna treatment (flexion-distraction technique). The study population consisted of 29 patients. Discogenic group consisted of 21 patients who were already diagnosed as HNP of lumbar spine with serial MRIs(magnetic resonance imaging) or CTs(computerized tomography). Simple LBP group consisted of 8 patients with low back pain & sciatica who were non-specific disorder on radiologic examination. All patients were treated with flexion-distraction technique, one of Chuna technique, under analysis of Moire Topography. And the evaluation of clinical outcome was done twice during this study by Moire Topography Analytic Point and Low Back Pain Assesment, Visual Analogue Scale. The results were summarized as follows; Total improvement rate of Moire Topography was $25.8{\pm}17.8%$, and the rate of Low Back Pain Assesment was $56.5{\pm}23.0%$, Visual Analogue Scale of post-treatment was $32.6{\pm}22.5$ Between Improvement rate of Moire Topography and improvement rate of Low Back Pain Assesment, significant correlation was proved(Person's coefficient was 0.381, p<0.05). After all, it is certain improvement of Moire Topography represents symptom's improvement.

Effect of Bee Venom Ointment Treatment for Chemotherapy-induced Peripheral Neuropathy : A Case Series (항암화학요법에 의한 말초신경병증의 봉독크림 치료 : 연속증례)

  • Objective : This is a case series reporting degree of response to bee venom ointment as a symptom-control therapy for chemotherapy-induced peripheral neuropathy (CIPN). Methods : Four consecutive patients with CIPN were referred to the East-West Cancer Center from October 7th, 2013, to November 9th, 2013. Patients with CIPN were treated with bee venom ointment 1-2 times per day for 7-25 days. Visual Analog Score (VAS) pain scale was compared before and after treatment. All patients were closely examined for any allergenic responses following each treatment session. Results : Using each patient as their own comparator, marked improvements of VAS were observed in 4 patients. Most important, there were no related adverse side effects found. Conclusion : Bee venom Ointment may be considered as an complementary therapy for managing CIPN symptoms.
