본 연구는 등받이 유무에 따른 하지 운동프로그램이 허리통증이 있는 대학생에게 미치는 영향을 알아 보기 위해 균형능력, KODI, VAS 검사를 이용하였다. 광주광역시 소재 K대학교 학생 32명을 대상으로 4주간 탄력밴드를 이용한 하지 운동프로그램을 진행하였으며 등받이의 유무에 따른 집단 별 16명씩 무작위로 선정하여 진행하였다. 균형능력과 KODI 점수에서는 집단과 시간에 의한 상호작용이 없는 것으로 나타난 반면 VAS 점수에서는 집단과 시간에 의한 상호작용이 나타났다. 또한 두 집단 모두 중재 후에 균형능력, KODI, VAS 점수에서 유의하게 증가하였다. 따라서 등받이 유무와 상관없이 의자에 앉아서 시행하는 하지운동 프로그램이 오랜시간 앉아서 생활하며 허리통증이 있는 학생과 직장인에게 일상에서 쉽게 할 수 있는 운동 프로그램으로 적용하는 것이 필요할 것으로 생각된다.
Background: Pain is often measured using psychophysical scaling techniques. However, all of these methods found their limits, since they were based on the subjective sensations reported by the subjects. It is, therefore, desirable to validate psychophysical pain measures by simultaneously measuring some physiological correlate of nociception. We studied an objective method for measuring pain in human volunteer using R(III) nociceptive flexion reflex. Methods: Four different intensity of electrical stimuli between perception and 1.4 times the R(III) nociceptive flexion threshold were delivered to the sole of the feet in 8 normal volunteers. We measured the flexion reflex activity in the skin over the ipsilateral tibialis anterior muscle and subjects rated each stimulus on a visual analog scale (VAS) Results: Both R(III) nociceptive flexion reflex activity and VAS ratings showed a linear relationship with stimulus intensity and with each other in all volunteers. Conclusions: R(III) nociceptive flexion reflex elicited through electrical stimulation may used as an objective pain measurement, previsionary based on our study paradigm.
Background: Dental fear is usually associated with hemodynamic changes. Fear of pain during the surgical removal of a lower impacted third molar might cause patients anxiety, thereby leading to avoidance of any future dental therapy. This study aimed to determine the effect of experiencing a surgical impacted-tooth removal on the pain perception-related anxiety and hemodynamic status. Method: Twenty-seven healthy patients aged 15-30 years (mean age, 24 years), for whom surgical removal of bilateral lower third molars was advised, were included. This prospective, randomized, controlled, split-mouth study involved operations on both sides of the mandibular arch, with a 1-month washout period in between. Blood pressure and heart rate were measured before the surgical procedure, during and after the injection, preoperatively, and postoperatively. Pain perception was evaluated using a 100-mm visual analog scale during the injection, preoperatively, and postoperatively after the numbness disappeared. Differences in the blood pressure, heart rate, and pain perception between the two appointments were analyzed using the paired t-test. For all statistical analyses, SPSS version 11.5 was used. Results: The mean pain perception values during the injection and preoperatively showed no significant differences between the two appointments (P > 0.05); however, significant differences in the blood pressure and heart rate were noted before the surgical procedure; preoperatively, the blood pressure alone showed a significant difference (P < 0.05). Conclusion: There was a significant decrease in the blood pressure and heart rate preoperatively; hence, experiencing a surgical impacted-tooth removal can reduce the subsequent preoperative anxiety in healthy patients.
본 연구는 8방향 경사와 회전운동이 만성요통환자의 통증과 동적균형에 미치는 영향을 알아보고자 시행되었다. 20명의 만성요통환자가 실험에 참여하였다. 10명(실험군)은 물리치료기구[온열팩{Hot Pack(H/P), 경피신경자극{Transcutaneous Electric Nerve Stimulation(TENS)}, 초음파{Ultra Sound(U/S)}]를 이용한 치료를 받은 후에 8방향 경사와 회전운동을 실시하였고 나머지 10명(대조군)은 물리치료기구를 이용한 치료만 실시되었다. 치료 중재는 주 3회, 6주 동안 적용되었다. 동적균형과 통증은 Star Excursion Balance Test(SEBT)과 Visual Analog Scale(VAS)를 이용하여 측정하였다. VAS 점수는 두 그룹 모두 감소하였으나 실험군에서 보다 유의하게 감소하였다. 동적균형 능력 또한 실험군에서 보다 유의한 증가를 나타내었다. 따라서 8방향 경사와 회전운동은 만성요통환자의 통증과 동적균형에 효과가 있었다고 사료된다.

Objective : Some patients with disc herniation who underwent discectomy complain of back pain after surgery and are unsatisfied with the surgical results. This study aimed to evaluate the relationship between preoperative disc height (DH), postoperative DH, and pain score 12 months after surgery in patients who underwent microdiscectomy for herniated lumbar disc. Methods : This study enrolled patients who underwent microdiscectomy at a medical center between January 2012 and December 2020. Patients with X-ray or computed tomography and pain score assessment (visual analog scale score) prior to surgery, immediately post-op, and at 1, 6, and 12 months after surgery were included. The DH index was defined as DH/overlying vertebral width. The DH ratio was defined as the postoperative DH/preoperative DH. Simple linear regression and multivariate linear regression analyses were applied to assess the correlation between DHs and leg pain scores 12 months after surgery. Results : A total of 118 patients who underwent microdiscectomy were included. DH decreased up to 12 months after surgery. The DH ratio at 1, 6, and 12 months after discectomy showed a significant positive correlation with the pain scores at 12 months after discectomy (1 month : p=0.045, B=0.52; 6 months : p=0.008, B=0.78; 12 months : p=0.005, B=0.69). Multivariate linear regression analysis revealed that the level of surgery, sex, age, and body mass index had no significant relationship with back pain scores after 12 months. Conclusion : In patients who underwent microdiscectomy, the DH ratios at 1, 6, and 12 months after surgery were prognostic factors for back pain scores at 12 months after surgery. Aggressive discectomy is recommended for lower postoperative DH ratios and Visual analog scale scores, leading to improved patient satisfaction.

Background: Surgical extraction of third molars is associated with postoperative pain and swelling at the extraction site. Pain is commonly managed using non-steroidal anti-inflammatory drugs (NSAIDs). Postoperative pain is usually moderate to severe in the first 12 h postoperatively and lasts for 3-5 days. However, with NSAIDs, these symptoms usually subside within 24 h. Diclofenac sodium and etodolac are NSAIDs, more selectively cyclooxygenase-2 inhibitors, with good analgesic efficacies. Methods: We compared the safety and analgesic efficacy of diclofenac sodium with etodolac peroral after surgical extraction of third molars in a double-blind, double-dummy, parallel-group study. The subjective pain improvement and pain relief after 2, 6, 24, 48, and 72 h using the visual analogue scale were measured as the study outcome. Results: Etodolac was equivalent to diclofenac sodium in pain alleviation at all postoperative time periods. No significant differences were found between diclofenac sodium and etodolac groups (P > 0.05). Both study medications were well tolerated and safe with mild adverse effects in only a few participants. Conclusion: Diclofenac sodium and etodolac are comparable in terms of analgesic efficacy and safety after surgical removal of third molars.
Objectives : Herniation of Nucleus Pulpous (HNP) of Lumbar is one of the most common causes of low back pain. The aim of this study is to investigate the effectiveness of acupuncture, bee venom acupuncture (BVA), and bee venom Pharmacopuncture (BVP) therapy for HNP. Methods : We separated 35 patients with HNP of L-spine into three groups; Acupuncture treated group, BVA treated group, and BVP treated group, and monitored for 30 days. 4 grades of recovery degree, score of Visual Analog Scale (VAS), and Oswestry Disability Index (ODI) were examined for comparison. Results : 1. Visual Analogue Scale of low back pain and radiating pain showed significant decrease in BVA and BVP groups compared to the Acupuncture group at VAS0_2, VAS0_3 evaluations. 2. ODI of disability of daily activities showed significant decrease in BVA and BVP groups compared to the Acupuncture group at VAS0_1, VAS0_2, and VAS0_3 evaluations. 3. Compared to the Acupuncture group, BVA and BVP groups were improved in S.L.R.T and R.O.M of L-spine as confirmed by the physical examination. 4. Treatment efficacy was more apparent in acute cases as ODI was significantly lower than chronic cases. Conclusion : BVA and BVP therapies can be used in addition to the acupuncture therapy for highly effective treatment of HNP. Further clinical studies are required to verify these findings.
Objective : The purpose of this study was to report the effect of Bogijetong-tang on diabetic peripheral neuropathy. Methods : Nine patients who were diagnosed or suspected with diabetic peripheral neuropathy enrolled in this study. They took Bogijetong-tang 2 or 3 times a day and were given acupuncture therapy. To evaluate the therapeutic effect, Visual Analog Scale (VAS) or Total Symptom Score (TSS) were examined at intervals of about 7 days. Result : TSS score decreased an average of 2.74 points and VAS scores decreased an average of 3.67 points. As the pain decreased, accompanied gait disturbance also improved in some patients. Conclusion : Bogijetong-tang is effective in treating diabetic peripheral neuropathy, but study of the concrete mechanism and efficacy is needed.
Objective : The study was performed to evaluate the effect of Kinesio taping on pain of Left hip joint. Methods : A 35-year-old, female patient who suffers from pain of left hip joint by brain tumor was treated by Kinesio taping therapy. The improvement of the patient's pain of left hip joint was evaluated by Visual Analog Scale(VAS), Faces Pain Rating Scale, McGill Pain Questionnaire(MPQ), and Pain Rating Score(PRS). Result : During the 9 days with Kinesio taping, VAS and Pain Rating Scale were decreased slightly. But Faces Pain Rating Scale and McGill Pain Questionnaire showed no change. Conclusions : The result suggests that Kinesio Taping was effective treatment to pain but not intensive. Further study is needed to evaluate the significancy of this report.
Objectives : The purpose of this report is to examine the effect of Jiejing-Quxue in the Shoulder pain patient. Methods : Clinical studies were done 40 patients who were treated with shoulder pain to Dept. of Acupuncture & Moxibusition, of Oriental Medicine Se-Myung University from August 1, 2005 to October 28, 2005. Subjects were divided into two groups Shunjing-Quxue needling group(Group A) and Jiejing-Quxue needling group(Group B). In Shunjing-Quxue needling group, we treated patients with one acupunture point on one meridian pathway. In Jiejing-Quxue needling group, we treated patients with two each acupunture points on two each meridian pathways. For evaluating satisfaction of patients, we use the method for Martin. A.N. to present. For evaluating change of pain, Visual Analog Scale(VAS) was examined after each treatment. Results : In satisfaction of patients, the patients of Group B were more satisfied than them of Group A. The VAS score of Group B were decreased significantly compared to that of Group A(p<0.05) Conclusion : The Jiejing-Quxue acupunture treatment may be good effects on the Soulder pain. So we suggest the possibility to use this treatment for Shoulder pain.
