부착형 치주 창상 피복재가 치주수술 후 동통 및 치유에 미치는 효과에 관한 임상 연구 (A clinical study on the effect of attachable periodontal wound dressing on postoperative pain and healing)

  • 목적: 치주포대는 감염 위험을 줄이고 치유를 증대 시킨다는 연구들이 있다. 본 연구는 치주 수술 후 부착형 창상 피복재의 사용이 치유 및 수술 만족도에 미치는 영향을 평가하고자 하였다. 연구 재료 및 방법: 치주 수술이 필요한 환자 28명을 대상으로 상악 또는 하악의 양측 사분면에 치주 수술을 시행하였다. Visual Analogue Scale (VAS)를 이용해 술 후 동통, 출혈, 식이 불편감, 지각 과민을 평가하고 추가적으로 작열감과 수술 만족도에 관해 평가하였다. 결과: 수술 후 동통, 출혈, 식이 불편감에 관한 VAS 평균 값은 창상 피복재 유무에 따라 각각; 동통: 2.82, 3.96 (P = 0.002), 출혈: 1.61, 2.54 (P = 0.008), 식이 불편감: 2.82, 4.18 (P < 0.001)로 창상 피복재를 적용한 그룹에서 불편감이 적었으며, 통계적으로 유의하였다. 작열감, 지각 과민과 창상 피복재 적용 유무 사이에 유의한 차이는 없었다. 수술 만족도는 75%에서 창상 피복재를 사용한 경우 높았다. 결론: 본 연구의 결과에 따르면, 창상 피복재의 사용유무와 작열감, 지각 과민과의 통계적 유의성은 없었으나 창상 피복재를 사용한 경우 술 후 동통, 출혈, 식이 불편감이 적었다.

근막이완술이 경추성 두통환자의 뇌혈류와 통증수준에 미치는 영향 (The Effects of Myofascial Relaxation on Blood Flow Velocity of the Cranial Artery and Pain Level in Cervicogenic Headache Patients)

  • Purpose: The aim of this study was to evaluate a possible role for cranial artery velocity in cervicogenic headache. Methods: We studied 13 patients with cervicogenic headaches (M=9, F=4, age=$24.50{\pm}3.50\;y$) these were compared to 20 normal subjects (M=10, F=10, age=$23.30{\pm}2.90\;y$). Cervicogenic headache patients were divided into two groups (a myofascial relaxation group, a placebo group). Pain levels were measured using a visual analog scale (VAS). Transcranial Doppler (TCD) ultrasound recordings were used to measure blood flow velocity in the cranial artery. Results: Cervicogenic headache patients exhibited a significantly lower blood flow velocity in the middle cerebral artery. The patients who received an intervention had a significantly higher blood flow velocity in the left middle cerebral artery, right vertebral artery and basilar artery. Compared to the placebo group, the myofascial relaxation group exhibited a significantly decreased level of pain. Conclusion: Cervicogenic headache is pain referred to the head from a source in the cervical spine. Manual therapy affect to cervical spine and soft tissue as well as the passage of these vessels and nerves to recover because it is thought to be able to help.

요통환자의 침치료와 족침 병행치료에 대한 비교연구 (Comparison of Effectiveness between Acupuncture and its Cotreatment with Foot Acupuncture on Low Back Pain)

  • Objectives : The purpose of this study is to investigate the effectiveness of treating low back pain patients with Foot acupuncture. Methods : Clinical study was conducted to 40 patients who were treated in Dept. of Acupuncture and Moxibusion, Woosuk University Hospital from March 1 to December 31, 2010. Subject were randomly divided into two groups. In one group(test group), patients were treated with both acupuncture and Foot acupuncture. And in the other group(control group), patients were treated with acupuncture. To estimate the efficacy of treatments, Visual Analog Scale(VAS) and Oswestry disability index(ODI) were checked. 12 patients were excluded and we investigated 14 patients of control group and 14 patients of test group entirely. Results : 1. In both two groups, VAS and ODI of patients were decreased significantly. 2. ODI of patients in test group were decreased more significantly than ODI of patients in control group. Conclusions : We suggest that cotreatment of acupuncture and foot acupuncture on low back pain patients can be recommended as a useful therapy.

만성요통 환자에 대한 미세전류자극 치료가 통증 및 기능 회복에 미치는 영향 (The Effects of Microcurrent Stimulation on Recovery of Function and Pain in Chronic Low Back Pain)

  • Purpose : This study observed the recovery of function and pain on the eletrostimulation therapy(transcutaneous electrical nerve stimulation(TENS) and micro current therapy(MT)). Methods : Subjects were assigned randomly either experimental group(n=17) who were taken TENS or the control group(n=20) who were taken MT. Also subjects were assessed for pain(Visual Analog Scale; VAS) and function(Oswestry LBP disability index). Results : The VAS has a statistically significant difference with each groups(p<.05), but there was no statistically significant difference with intergroup during treatment period(p>.05). The Oswestry LBP disability index has a statistically significant difference with each groups(p<.05), also there was a statistically significant difference with intergroup during treatment period(p<.05). Conclusion : The efficancy of microcurrent therapy used in this study should be futher investigated in a long period study and objective study outcomes.

Klippel-Feil 증후군 환자의 경추 도수치료 후 발생한 부작용 치험1례 (A Case Report of Neck Pain Patient with Klippel-Feil Syndrome by Cervical Manipulation Treatment)

  • The Klippel-Feil Syndrome (KFS) is characterized by congenital fusion of two or more vertebrae with hypermobile normal segment. According to this, a patient with KFS can be at risk of severe neurological symptoms after manipulation treatment. We had a KFS patient who suffered from neck pain and limited range of motion at cervical after manipulation treatment. The patient, 49-year-old woman was diagnosed as KFS through Cervical X-ray and MRI. The patient was treated by acupuncture therapy and SCENAR therapy. We measured neck pain by visual analog scale (VAS) and neck disability index (NDI) and checked range of motion at cervical before and after the treatments. After 8 times treatments, the patient's pain decreased and the range of motion increased. From this case, we can recognize the risk of Chuna manipulation treatment for KFS patient. So, we suggest that radiological examination is needed before cervical Chuna manipulation treatment to avoid adverse reactions.

  • Objectives: The purpose of this study is to report the improvement after oriental medical treatment of partial tear of supraspinatus tendon. Method: We treated 2 patients having shoulder pain due to partial tear of supraspinatus tendon with Oriental medical treatment, including Sa-am acupuncture DaeJang-Jeonggyeok, A-shi point(阿是穴) and herbal medicine. We checked visual analog scale (VAS) score and range of movement (ROM). Results: We treated shoulder pain. Thereafter ROM improved and V AS score dropped to the level of 2 to 4 points from 10 points at onset. Conclusion: This report shows Oriental medical treatment has effectiveness on partial tear of supraspinatus tendon. Follow-up study is needed.

Comparison of the Short-Term Effectiveness of Hard and Soft Stabilizing Appliances in Treating Patients with Acute Temporomandibular Disc Displacement without Reduction

  • Purpose: The aim of this study is to compare the short-term effectiveness of hard and soft stabilizing appliances (H-A and S-A) in treating patients with acute temporomandibular joint (TMJ) disc displacement without reduction (DDwoR). Methods: Eighty one patients diagnosed as acute TMJ DDwoR at oral medicine of Chosun University Dental Hospital from November 2016 to September 2017 were included in this study. The subjects were divided into three groups: The control group (34 patients treated without occlusal appliance), the H-A group (31 patients treated with H-A) and the S-A group (16 patients treated with S-A). The pain intensity using Visual Analog Scale and maximum mouth opening (MMO) were evaluated every week for 8 weeks. Results: The improvement of the pain intensity and MMO in the H-A group and the S-A group compared with the control group were noted (p<0.05). Conclusions: This study suggests that hard and S-As may be equally useful in treating patients with TMJ DDwoR. S-A could be recommended to reduce the symptoms of TMJ DDwoR in short period.

Effect of Menstrugole on primary dysmenorrhea: a randomized clinical trial

  • Objective Primary dysmenorrhea occurs in more than 50% of women of reproductive age. This survey evaluated the effects of Menstrugole on the alleviation of primary dysmenorrhea. Methods This study was performed in western Iran between August 15 and December 15, 2017, in female students with primary dysmenorrhea. The students were randomly divided into 2 groups and received placebo or a Menstrugole capsule for 3 days in each of 2 menstrual cycles. The visual analog scale was used to assess pain severity. The ${\chi}^2$-test, one-way analysis of variance, and the paired t-test were used for statistical evaluation. Results We compared the severity of pain between the 2 groups. There was a significant change in the mean pain score during the first (P<0.001) and second months (P<0.001) after the use of Menstrugole, compared with that in the placebo group. Conclusion Menstrugole decreased pain severity in female students, and can be considered by health care providers for treatment of primary dysmenorrhea.

Comparison of the effects of articaine and bupivacaine in impacted mandibular third molar tooth surgery: a randomized, controlled trial

  • Background: The aim of this randomized, triple-blind trial was to determine the anesthetic, analgesic, and hemodynamic effects of articaine and bupivacaine in the extraction of impacted mandibular third molar teeth. Methods: Twenty-six patients who underwent removal of bilaterally symmetric mandibular third molars were randomly assigned to articaine and bupivacaine groups in a split-mouth design. The onset of anesthetic action, intraoperative comfort, total amount of solution used, duration of postoperative anesthesia and analgesia, rescue analgesic use, postoperative pain, intraoperative bleeding, and hemodynamic parameters were evaluated. Results: In the articaine group, the onset of anesthetic activity was faster, intraoperative comfort was greater, and effective anesthesia required less local anesthetic solution. The bupivacaine group showed a significantly longer duration of postoperative anesthesia and analgesia, in addition to lower visual analog scale values at 6 and 48 hours postoperatively. There were no significant differences between the two solutions regarding rescue analgesic medication use, intraoperative bleeding, or hemodynamics. Conclusion: Articaine showed greater clinical efficacy than bupivacaine in intraoperative anesthesia, achieving faster onset of anesthetic action and greater patient comfort while also requiring less reinforcement during surgery. However, bupivacaine was superior in terms of postoperative anesthesia, reducing postoperative pain due to its residual anesthetic and analgesic effects. Both anesthetic solutions led to similar hemodynamics at low doses in mandibular third molar surgery

전남 일부 지역 임플란트 수술용 가이드 이용 환자 만족도 조사 (Patient satisfaction survey for implant surgical guides in some areas of Jeollanam-do)

  • Objectives: This study, investigated the satisfaction of patients who received the procedure using an implant guide to identify factors affecting satisfaction in dental hospitals and clinics in Jeollanam-do. Methods: From December 2021 to February 2022, questionnaires randomly were collected from 128 patients aged 20 to 90 years visiting dental hospitals and clinics using surgical guides in some areas of Jeollanam-do. Results: There was a negative correlation between the overall satisfaction average and Visual Analog Scale (VAS) pain level (r=-0.414, p<0.001), and a negative correlation between VAS pain level and VAS satisfaction (r=-0.273, p<0.05), As subjective pain decreased, subjective satisfaction increased, and as subjective pain increased, the overall satisfaction average also decreased, showing an inverse correlation. Factors affecting satisfaction with the guide for implant surgery were whether you would like to recommend it to people around you (β=0.556), whether you would like to explain the guide (β=0.194), and whether you thought it was good to receive the guide (β=0.199). Conclusions: Therefore, in order to increase the satisfaction of the implant surgical guide procedure, it is considered important to perform a sufficient explanation of the implant surgical guide before the procedure.
