Study on Pain, Efficacy, and Comfort during First Stage of Laboring Women (UNICEF추천병원에서 분만한 산부의 분만통증, 분만자신감, 분만편안함에 관한 연구)

  • Purpose: This study was purposed to survey pain, efficacy, and comfort during the first stage of labor and to identify correlations among them. Method: The subjects were 116 laboring women who were admitted to a baby-friendly hospital recommended by UNICEF for breast feeding. Data were collected using a structured questionnaire composed of labor pain, labor efficacy, and labor comfort with 10 cm visual analog scale respectively. The data were analyzed by frequency, Pearson's correlation, and ANOVA. Result: Labor pain, labor efficacy, and labor comfort in latent phase were positive correlation with them in active phase. The more pain, the less labor efficacy and the less comfort in latent phase. and the more pain, the less comfort in active phase. Labor efficacy was positively correlated with labor comfort in both phase. Lastly, women supported by their husbands had significantly higher labor pain than women supported by their mothers. Conclusion: The results suggest that nurses develop a labor efficacy program for pregnant women and their mothers or spouses.

Comparison of the Effects of Joint Mobilization, Gym Ball Exercises, and Breathing Exercises on Flexion Relaxation Phenomenon and Pain in Patients with Chronic Low Back Pain

  • Background: Although various exercises have been performed for patients with chronic low back pain (CLBP), the effects of these exercises including joint mobilization, gym ball exercises, and breathing exercises on flexion relaxation ratio (FRR) have not been compared. Objective: To compare the effects of joint mobilization, gym ball exercises, and breathing exercises on the flexion relaxation phenomenon (FRP) and pain in patients with chronic low back pain. Design: Randomized pretest-posttest control group design. Methods: Thirty-six patients with chronic low back pain who were undergoing rehabilitation at a rehabilitation center were included. The patients were randomly divided into three groups: joint mobilization group (JMG; n=12), gym ball exercise group (GBG; n=12), and breathing exercise group (BEG; n=12). The exercises were performed for 40 minutes a day, twice a week, for a total of 12 weeks. Results: There were no significant differences in FRR between the three groups (P>.05). Significant decreases in the modified visual analog scale (MVAS) scores after intervention between the groups were found (P<.05). The GBG was significantly decreases from the JMG in the MVAS (P<.05). However, there were significant improvements between the pre- and post-interventional findings on FRR and MVAS in the three groups (P<.05). Conclusion: We demonstrated that intervention using joint mobilization, gym ball exercises, and breathing exercises improve FRP and pain in patients with CLBP.

Pregabalin versus Gabapentin Efficacy in the Management of Neuropathic Pain Associated with Failed Back Surgery Syndrome

  • Objective : Failed back surgery syndrome (FBSS) is a common long-term complication following spine surgeries characterized by chronic persistent pain; different strategies of management were employed to deal with it. This clinical trial aims to compare the efficacy of Pregabalin and Gabapentin in the management of this condition. Methods : A double-blind, randomized, comparative study (clinical trial registry NCT05324761 on 11th April 2022) with two parallel arms with Pregabalin and Gabapentin were used in arms one and two, respectively. Visual analog scale was used for basal and endpoint assessment of pain. T-test and analysis of covariance were used to deal with different variables. A pairwise test was used to compare pairs of means. Results : Of 66 patients referred to the trial, 64 were eligible, with 60 patients completing the 30 days trial. Both pregabalin and gabapentin effectively reduce pain, with significant p-values of 0.001 for each group. However, the pregabalin group was superior to gabapentin in pain reduction (p=0.001). Gender was an insignificant factor (p=0.574 and p=0.445 for the pregabalin and gabapentin groups, respectively, with a non-significant reduction (p=0.393) for both groups in total. Location of stenosis before surgery and type of surgery performed show non-significant effect on pain reduction for both groups. Conclusion : Both pregabalin and gabapentin effectively and safely relieve neuropathic pain associated with FBSS; pregabalin was significantly more effective irrespective of the patients' gender.

  • Objectives : This study is designed in order to evaluate the therapeutic effect of Scalp Acupuncture and Do-ma Acupuncture related with Governer Vessel(Dok) and Bladder Meridian(Chok-taeyang) on acute low back pain Methods : We investigated 30 patients suffering from acute low back pain were admitted to Dunsan O.M.hospital from March. 1st 2005 to December. 30th 2005. We divided into two groups: one group was treated with Scalp acupunture, another group was treated with common acupuncture. 30 patients, were diagnosed only straightened curvature on lateral view of lumbar spine X -ray. Scalp acupunture related with Governer Vessel(Dok) and Bladder Meridian(Chok-taeyang) and Do-ma acupuncture. To estimate the efficacy of treatment that applied for two groups, we compared the visual analog scale(VAS), Oswestry disabiliby index(ODI) and Straight Leg Raising Test(SLRT) score of two groups statistically. Results : Both Scalp acupuncture related with Governer Vessel(Dok) and Bladder Meridian (Chok-taeyang) and Do-ma acupuncture therapy showed good effect on acute low back pain. It was proved by the difference between VAS, ODI and SLRT score checked before treatment and what checked after treatment on each groups. Conclusion: The therapeutic effect of Scalp Acupuncture and Do-ma Acupuncture on acute low back pain can be recommended as a useful therapy to treat acute low back pain.

  • Objective : Herniation of Nucleus Pulpous(HNP) of Lumbar is the most important reason that causes low back pain. The aim of this study is to investigate the effectiveness of Bee-venom acua-acupuncture therapy for protrusion disc patients. Methods : To evaluate the effectiveness of Bee-Venom Acupuncture Therapy, 20 patients were treated by bee-venom acua-acupuncture therapy. To estimate the efficacy of treatment, we used Quardruple Visual Analog Scale(QVAS). Results : 1. As a objectivity treatment record, they test treatment record good 60%, fair 25%, excellent 15%. 2. After bee-venom therapy, pain rate changed from 8.25 to 2.15. 3. By the results which puts out the statistics in sex, a pain rate of male changed from 8.75 to 2.50, a pain rate of female 7.92 to 1.92. Intentional difference is none as a therapy. By the results which puts out the statistics in age, after forties changed from 7.78 to 2.22 and before forties changed from 7.90 to 1.92. By the results which puts out the statistics in disc herniation, pain rate of central type changed from 8.29 to 2.29, pain rate of left type changed from 8.20 to 1.40, pain rate of Right type changed from 8.00 to 4.00.

The comparison of Effectiveness between Bee Venom and Sweet Bee Venom Therapy on Chronic Lower Back Pain. (만성요통환자에 대한 Bee Venom과 Sweet Bee Venom의 치료효능 비교 연구)

  • Objective The aim of this study is to investigate if Sweet Bee Venom(SBV) Therapy has the equal effect in comparison with Bee Venom (BV) Therapy on Chronic Lower Back Pain. Methods Clinical studies were conducted to 39 patients who were treated Chronic Lower Back Pain in Dept. of Acupuncture and Moxibustion, Dongeui University from March 1 to June 30, 2008. Subjects were randomly devided into 2 groups : BV treated group(Group A, n=19), SBV treated group(Group B, n=20) In BV treated group(Group A), we treated patients with dry needle acupuncture and BV Therapy. In SBV treated group(Group B), we treated patients with dry needle acupuncture and SBV Therapy. All process of treatment were performed by double blinding method. 1. To estimate the efficacy of venom in controlling pain, we have checked Visual Analog Scale(VAS). 2. For evaluating functional changes of patients, we have checked Oswestry Lower Back Disability Questionnaire(ODI). 3. To estimate Itching which is the most prominent symptom of allergic reaction, we have checked Visual Analog Scale(VAS). Results 1. In controlling pain, the results of BV treated group(Group A) is more effective than that of SBV treated group(Group B). 2. In promoting function, the results of BV treated group (Group A) is more effective than that of SBV treated group(Group B). 3. In controlling itching, the results of SBV treated group(Group B) is more effective than that of BV treated group(Group A). Conclusions According to the study, SBV Therapy shows more effective result than BV Therapy in controlling itching. But BV Therapy is more effective than SBV Therapy in controlling pain and promoting function.

Comparative evaluation of virtual reality distraction and counter-stimulation on dental anxiety and pain perception in children

  • Background: This study evaluated the efficacy of virtual reality (VR) distraction and counter-stimulation (CS) on dental anxiety and pain perception to local anesthesia in children. Methods: A prospective, randomized, single-blinded interventional clinical trial with a parallel design was used. Seventy children 7-11 years old who required local anesthesia (LA) for pulp therapy or tooth extraction were recruited and allocated to two groups with equal distribution based on the intervention. Group CS (n = 35) received CS and Group VR (n = 35) received VR distraction with ANTVR glasses. Anxiety levels (using pulse rate) were evaluated before, during, and after administration of local anesthesia, while pain perception was assessed immediately after the injection. Wong-Baker faces pain-rating scale (WBFPS), visual analog scale (VAS), and Venham's clinical anxiety rating scale (VCARS) were used for pain evaluation. Student's t-test was used to test the mean difference between groups, and repeated measures ANOVA was used to test the mean difference of pulse rates. Results: Significant differences in mean pulse rates were observed in both groups, while children in the VR group had a higher reduction (P < 0.05), and the mean VCARS scores were significant in the VR group (P < 0.05). Mean WBFPS scores showed less pain perception to LA needle prick in the CS group while the same change was observed in the VR group with VAS scores. Conclusions: VR distraction is better than CS for reducing anxiety to injection in children undergoing extraction and pulpectomy.

Impaired Health-Related Quality of Life in Brazilian Children with Chronic Abdominal Pain: A Cross-Sectional Study

  • Purpose: We compared the health-related quality of life (HRQOL) of children and adolescents with functional abdominal pain disorders (FAPDs) and organic abdominal pain disorders (ORGDs). Methods: This was a single-center, cross-sectional, observational study. The PedsQL 4.0 generic cores scales parent proxy-report was administered to parents/caregivers of 130 and 56 pediatric patients with FAPDs and ORGDs respectively on their first visit. The self-reported pain intensity in the patients was assessed using a visual analog scale (VAS) and facial affective scale (FAS). Results: Irritable bowel syndrome was the most prevalent FAPDs, and the most prevalent ORGDs were reflux esophagitis (41.1%) and gastritis associated with Helicobacter pylori (21.4%). There was no difference in HRQOL among patients diagnosed with ORGDs and FAPDs (p>0.05). Patients with ORGDs and FAPDs had lower HRQOL Scale scores than healthy Brazilian and American children's references, with a high proportion of children at risk for impaired HRQOL (p<0.0001). There was no difference in the VAS and the FAS scores between the ORGDs and the FAPDs. FAPDs had a higher prevalence of girls' and couples' disagreement (p<0.02), although poor school performance (p<0.0007) and bullying (p<0.01) were higher in patients with ORGD. Conclusion: This study revealed that there was a difference in impaired HRQOL between patients with ORGDs and FAPDs. Thus, considering the high prevalence of chronic abdominal pain in children, a well-founded treatment plan is necessary for a multidisciplinary cognitive-behavioral Pain management program.

A Case Report of Warming Acupunture Therapy at Buyang, Golyun and Its Cotreatment with Traditional Korean Medicine in a Traffic Accident Patient with Neck Pain (교통사고 후 발생한 경항통 환자에 대한 부양, 곤륜혈의 온침 병행치료 증례보고)

  • Objectives : The aim of this study was to observe the clinical effect of warming acupunture therapy at Buyang(BL59), Golyun(BL60) in a traffic accident patient with neck pain. Methods : A 32-years-old, female patient with neck pain was treated by warming acupunture therapy at Buyang(BL59), Golyun(BL60) from April 28th to May 5th. The improvement of the patient's neck pain was evaluated by Visual Analog Scale(VAS), Pain Rating Score(PRS) and Neck Disability Index(NDI). Result : After treatment, flexion & extension VAS was decreased from 7.0 to 3.0 and lateral bending VAS was decreased from 7.5 to 4.0. PRS and NDI was improved 70 to 54, 41 to 35, respectively. Conclusions : The result suggests that warming acupunture therapy at Buyang(BL59), Golyun(BL60) may have therapeutic effect on neck pain patients.

  • Background: The purpose of this study was to compare the availability of propofol and fentanyl (P + F) with diazepam and morphine (D + M) for intravenous conscious sedation during third molar surgery. Methods: Forty patients without systemic disease were operated under IV conscious sedation administered by either of the two techniques. Monitoring consisted of continuous observation of pulse rate, blood pressure, oxygen saturation, and the respiratory rate and were recorded every 15 minutes. Cooperation score was measured 5 and 15 minutes after induction of IV sedation. Following the operation, the surgeon and patients completed questionnaires including pain visual analog scale, amnesia, and side effects. Results: The P + F group was significantly more cooperative than the D + M group. The side effects of D + M group included pain on injection, nausea/vomiting and abdominal pain. The side effects of P + F group included talkativeness, nausea/vomiting, temporary apnea, pain on injection and vertigo. Conclusions: In this study, there were following benefits in the P + F group; more cooperative status and less pain perception. But respiratory depression developed in some patients.
