A Retrospective Review about Korean Medicine Treatment for Obesity and Change of Body Composition on Climacteric Women in a Korean Medicine Hospital (일개 한방병원에 내원한 갱년기 여성의 한의 비만 치료와 체성분 변화에 대한 후향적 분석)

  • Objectives: This study aimed to identify the current use of Korean medicine for obesity and its effect for women in climacteric period. Methods: We studied women aged 45-55 who visited Daejeon Korean medicine Hospital of Daejeon University to lose body weight from January 1, 2021 to December 31, 2022 via an analysis of the medical records. The treatment duration was continuous for more than 2 weeks, and a body composition was measured by Inbody 770 at 2 to 4 weeks after the first visit. Results: 28 patients were finally selected and their average age was 49.32±3.38 years. Based on the body mass index (BMI) classification, 19 were in the 1st obesity group, 5 in the 2nd obesity group, 3 in the overweight group and 1 in the normal group. Patients usually complained comcomitant symptoms, and the symptoms of menopausal disorder was the most frequent. The average treatment duration was 3.68±0.67 weeks and the average treatment frequency was 3.93±0.98 times. All patients took herbal medicines Gambi-tang and 23 took modified fasting therapy including Gamro-su. 14 were treated by whole body far-infrared therapy and 6 were gotten partial obesity treatment. Among patients treated for accompanying symptoms, menopausal disorders were the most common (35.71%), and herbal medicine such as Gamisoyou-san, Hominis Placenta Pharmacopuncture, moxibustion, and cupping were used. After treatment, on average, body composition changed significantly; body weight 3.28±1.82 kg, BMI 1.36±1.04 kg/m2, body fat 1.70±1.67 kg, skeletal muscle mass 0.81±0.91 kg, abdominal circumference 2.04±2.6 cm, and visceral fat area 8.91±12.83 cm2. Conclusions: We analyzed general characteristics, BMI distribution, types of Korean medicine treatment and change of body composition. This study could be used as reference to provide practical data of treatment for obese women in climacteric period.

Effect of Ginseng and Herbal Plant Mixtures on Anti-Obesity in Obese SD Rat Induced by High Fat Diet (홍삼 및 천연물 혼합소재의 비만 유도 흰쥐에 대한 항비만 효과에 관한 연구)

  • Anti-obesity effects of ginseng and herbal plant mixtures were investigated to develop natural materials for anti-obesity. After inducing obesity with high fat diet for 8 weeks in male SD rats, ginseng and herbal plant mixtures DM1 (ginseng, puer tea, opuntia) and DM2 (ginseng, puer tea) were administrated orally to rats for another 8 weeks. During administration, food efficacy ratio and body weight of rat were measured twice weekly. After administration, body weight, body fat contents, and serum lipid level were estimated for anti-obesity effect and hematological analysis blood level of ALP and ASP was checked for safety. Body weight in rats fed high fat diet was significantly increased. Body weight in obese rats induced by high fat diet was significantly decreased by DM1 and DM2 feedings. The amount of body fat (epididymal, perirenal and visceral fat, brown adipose tissue) was significantly reduced by DM1 and DM2 treatments. The amount of TG, the concentration of leptin in blood plasma, and the concentration of insulin in blood plasma were significantly diminished by DM1 and DM2. Lipid accumulation on liver was reduced in DM2. There were no side effects among all groups according to blood analysis, hematological findings, and body weight. The findings of this study suggest that DM1 and DM2 may be effective materials for anti-obesity through reducing plasma triglyceride and body fats, and also decreasing body weight without side effects.

Effect of Garlic on Serum Lipids Profiles and Leptin in Rats Fed High Fat Diet

  • Although garlic has been reported to have impressive effects in lowering serum lipids, there have been controversial evaluations on these effects. To find the potential fator causing the inconsistency in the previous studies, we examined the effects of two types of garlic according to the producing-area (hangihyung garlic, nangihyung garlic) on serum lipid profiles and leptin level. Thirthy six of 4 wk old Sprague Dawley male rats fed high fat diet (40% of calories as fat) for 6 wks to induce obesity, and subsequently fed 5% garlic powder supplemented (HF+H: hangihyung garlic powder, HF+N: nangihyung garlic powder) high fat diets (w/w) for further 5 wk. For the comparison, normal control group fed AIN-76A diet (11.7% of calories as fat). Supplementation with hangihyung and nangihyung garlic resulted in a significant reduction of high fat induced body weight gain, white fat (i.e., epididymal, visceral and peritoneal fat) development, adipocyte hypertrophy and the development of hyperinsulinemia and hyperliptinemia. Serum triglyceride and total cholesterol level was greatly reduced by hangihyung garlic supplementation (p<0.05). The HDL-cholesterol level was increased by dietary hangihyung and nangihyung garlic. There were slight non-significant decreases in triglyceride and total cholesterol of HF+N group as compared to those of HF group. Leptin level of HF+H group was found to be significantly lower than HF group (p<0.05). There was no significant difference among N group and HF+N group. These results suggest that hangihyung garlic may lead to the higher activity in improving lipid profiles than nangihyung garlic. Whether the hypolipidemic effect of garlic increases in a species-dependent has yet to be determined and awaits further research.

Effects of Low Level of Levan Feeding on Serum Lipids, Adiposity and UCP Expression in Rats (저농도 레반 공급이 혈중 지질 및 체지방 형성과 UCP 발현에 미치는 영향)

  • This study described the effect of levan (9-2,6-linked fructose polymer) feeding on serum lipids, adiposity and uncoupling protein (UCP) expression in growing rats. Levan was synthesized from sucrose using bacterial levansucrase. UCP is a mitochondrial protein that uncouples the respiratory chain from oxidative Phosphorylation and generates heat instead of ATP, thereby increase energy expenditure. We observed that 3% or 5% levan containing diet reduced serum triglyceride levels, visceral and peritoneal fat mass and induced the UCP expression in rats fed high fat diet in previous study. To determine whether the intake of low level of levan may have the hypolipidemic and anti-obesity effect, 4 wk old Sprague Dawley male rats were fed AIN-76A diet for 6 wk, and sub-sequently fed 1% or 2% levan solution for further 5 wk. Intake of 1% levan in liquid form reduced serum triglyceride and serum total cholesterol levels to 50% and 66% of control group, respectively. Although epididymal and peritoneal fat masses were not affected by levan feeding, visceral fat mass was lower in 1% levan group compared to control group. The expression of UCP2 mRNA in brown adipose tissue, skeletal muscle and hypothalamus and UCP3 mRNA in skeletal muscle were not changed by levan feeding, while the UCP2 mRNA in white adipose tissue was up-regulated by levan feeding. In conclusions, intake of low level of levan solution reduced serum triglyceride and total cholesterol, restrained the visceral fat accumulation and increased UCP expression in white adipose tissue in rats. This study suggests that hypolipidemic and anti-obesity effect of levan attributed to anti-lipogenesis and inefficeint energy utilization by up-regulation of UCPs.

Massa Medicata Fermentata improves fatty liver in high fat diet-fed nonalcoholic fatty liver disease's mouse model (고지방식이 유도 비알콜성지방간 마우스 모델에서 육신국(六神麯)의 지방간(脂肪肝) 개선효과)

  • Objectives : This study was undertaken to verify the effects of Massa Medicata Fermentata (MMF) on nonalcoholic fatty liver disease (NAFLD) using high fat diet-fed male mice. Methods : Fifty four male C57BL/6N mice (age matched) were used for all experiments. Nine standard chow diet-fed mice were used as normal group and forty five high fat diet-fed obese mice were randomly divided into 5 groups: control, atorvastatin-10mg/kg, MMF(1)-62.5mg/kg, MMF(2)-125mg/kg and MMF(3)-250mg/kg. After all groups were treated with several kinds of diets for 8 weeks, we measured body weight gain, adipose tissue weights, plasma lipid and glucose metabolism, visceral organ weights, histological analysis for liver on the mice. Results : MMF-treated mice had lower body weight gain compared with controls. Among MMF-treated mice, the effect was magnified in MMF(2). MMF(3)-treated mice had lower blood plasma total cholesterol (TC) and glucose level compared with controls. MMF decreased hepatic lipid accumulation, liver fibrosis and liver inflammation of mice compared with controls. The effects was maximized in MMF(2) and atorvastatin. Blood plasma aspartate aminotransferase (AST), alanine aminotransferase (ALT), ${\gamma}$-glutamyltransferase (${\gamma}$-GT) concentrations tends to be decreased by MMF compared with controls. Blood plasma AST, ALT, ${\gamma}$-GT concentrations and organ weights were not changed by MMF, indicating that all three kinds of MMF do not show any hepatotoxicity. Conclusions : These results suggest that MMF improves NAFLD by reducing body weight gain, hepatic lipid accumulation, liver fibrosis, liver inflammation.

Anti-obesity effect of 3,5-dicaffeoylquinic acid on high-fat diet mouse (고지방식이 마우스에서 3,5-dicaffeoylquinic acid의 항비만 효과)

  • This study was performed to confirm the influence of chlorogenic acid (CGA) and 3,5-dicaffeyolquinic acid (3,5-diCQA) intake on problems caused by high-fat diet. CGA was more effective in suppressing weight gain than 3,5-diCQA. In contrast, 3,5-diCQA was more effective in improving glucose tolerance than CGA. In the biopsy, it was confirmed that CGA inhibited visceral fat and liver fat accumulation. 3,5-diCQA also inhibited visceral fat accumulation, but 3,5-diCQA increased liver fat accumulation. The liver fat accumulation induced oxidative stress, but 3,5-diCQA reduced oxidative damage through its antioxidant activity. The increased liver fat accumulation was because a 3,5-diCQA greatly increased Akt phosphorylation and decreased AMPK phosphorylation in the liver. Consequently, CGA was effective in alleviating the problems caused by high-fat diets, while maintaining normal balance. 3,5-diCQA also showed a positive effect on problems caused by high-fat diets, but it increased liver fat accumulation and thereby had negative consequences.

Inhibition of Azoxymethane-induced Colorectal Aberrant Crypt Foci in Mice Fed a High-fat Diet by Pleurotus eryngii (Eringi) and Hypsizygus marmoreus (Bunashimeji)

  • Obesity markedly increases the risk of colorectal cancer. Recently, the preventive effects of edible mushrooms on triglyceride elevation and visceral fat accumulation have been reported. Here, the effects of Pleurotus eryngii (Eringi) and Hypsizygus marmoreus (Bunashimeji) on azoxymethane (AOM)-induced aberrant crypt foci (ACF; precancerous lesions) in the colorectums of mice fed a high-fat diet were examined. Eringi (ER) and Bunashimeji (BU) mushroom powder samples were used. Six-week-old male C57BL/6J mice received an intraperitoneal injection of AOM (10 mg/kg) once a week for two weeks, and were sacrificed and dissected at 6 weeks after the start of the experiment. After the initiation of the experiment, they received a normal diet (ND), high-fat diet (HFD), HFD + ER (1 or 5% of diet), or HFD + BU (1 or 5% of diet). As a result, body and fat weights were significantly lower in the 5% ER and BU groups than in the HFD group. Liver triglyceride levels were also significantly lower in the 5% ER and BU groups. Total liver cholesterol levels were significantly lower in the 5% ER group. The numbers of ACF (especially large ACF) showed strong inhibitory effects in both ER and BU groups. Measurement of the cell proliferation marker Ki-67 labeling index in the colonic mucosa demonstrated more significant suppression in both ER and BU groups than in the HFD group. These results suggest that the simultaneous intake of ER and BU may inhibit colorectal tumorigenesis in HFD-fed mice.

Sour cherry ameliorates hepatic lipid synthesis in high-fat diet-induced obese mice via activation of adenosine monophosphate-activated protein kinase signaling

  • Purpose: Sour cherry (Prunus cerasus L.) contains abounding phytochemicals, such as polyphenols and anthocyanins, and has antioxidative effects. Adenosine monophosphate-activated protein kinase (AMPK) is a crucial regulator in enhancing the lipid metabolism. This study hypothesized that the intake of sour cherry affects AMPK signaling. Therefore, this study examined whether sour cherry regulates AMPK to balance the hepatic lipid metabolism and exert ameliorating effects. Methods: Male C57BL/6J mice had obesity induced with a 45% fat diet. The mice were divided into four groups: control (CON), high-fat diet (HFD), low percentage sour cherry powder (LSC), and high percentage sour cherry powder (HSC). The mice in the sour cherry groups were fed 1% sour cherry or 5% sour cherry in their respective diets for 12 weeks. Results: The body weight, visceral fat weight, and lipid droplet size significantly decreased in the treatment groups. The serum and hepatic triglyceride and total cholesterol levels improved significantly in the HSC group. The low-density lipoprotein cholesterol levels were also reduced significantly, whereas the high-density lipoprotein cholesterol levels were increased significantly in both treatment groups. The sterol regulator binding protein-1c and fatty acid synthase expression levels as fatty acid synthesis-related enzymes were significantly lower in the treatment groups than in the high-fat diet group. Furthermore, the adipose triglyceride lipase and hormone-sensitive lipase expression levels as lipolytic enzyme activity and AMPK/acetyl-CoA carboxylase/carnitine palmitoyltransferase-1 as fatty acid β-oxidation-related pathway were upregulated significantly in both sour cherry groups. Conclusions: These results show that sour cherry intake improves hepatic lipid synthesis and chronic diseases by activating AMPK signaling. Therefore, this study suggests that phytochemical-rich sour cherry can be developed as a healthy functional food.

Association of visceral fat obesity and other lifestyle factors with prostate cancer (비만으로 인한 전립선암 발생의 영향연구 : 복부비만도 측정과 그 외 생활 요인)

  • 최근 우리나라의 암 발생률은 꾸준히 증가추세에 있고 그 중에서도 전립선암은 갑상선암을 제외했을 때 남성에서 가장 높은 증가 추세를 보이고 있는 암이다. 전립선암은 그 동안 선진국에서 주로 발병하는 것으로 알려진 암 종이지만 서구화된 식습관과 생활 행태 변화로 인하여 국내에서도 발생률이 증가하는 것으로 추정하고 있다. 따라서 국내 환경에 맞는 내장 지방 분포와 같은 정확한 비만도 측정을 통해 우리나라에서 증가율 1위를 보이고 있는 전립선암 발생의 원인 관계를 찾고자 한다. 본 연구는 환자와 정상군 비교를 하는 환자-대조군 연구이고 본 연구를 위하여 임상시험 윤리위원회 (IRB) 승인을 받았다 (NCC2014-0124). 환자군은 2014년 8월 1일부터 2015년 1월 6일 까지 국립암센터를 방문하여 전립선암을 진단받고 수술과 항암치료 그리고 방사선 치료를 시작하지 않은 초진을 대상으로 하였고, 정상군은 2009년 11월부터 2014년 9월 30일까지 공단검진을 목적으로 국립암센터를 방문한 정상인을 대상으로 하였다. 전립선암 연구는 환자 총 52명에 평균 66세 (51 - 82세)이고 정상군은 총 50명에 평균 64세 (59 - 75세) 이다. 모든 연구 대상자들의 생활 요인 평가를 위하여 암 가족력, 흡연, 음주 상태, 운동, 등을 추가적으로 설문 조사하였다. MRI 영상의 배꼽 주위에서의 내장 지방과 피하지방을 전용 분석 컴퓨터를 활용하여 측정 하였다. 복부비만율은 환자군과 정상군에서뿐만 아니라 (p = 0.03), 전립선암 악성도와 (Gleasonscore; p = 0.001)도 통계적 차이를 나타냈다. 하지만 BMI 결과와는 전립선암 발생과 악성도에서 무관함을 보인 것에 반해 허리둘레는 전립선암의 발생에 영향을 미치는 결과를 나타났다. 한편 전립선암의 또 다른 악성도 지표인 PSA는 비만 측정치와의 상관성이 Gleason score와 보다 대체적으로 낮게 나타났다. 학력, 운동량, 흡연, 음주 상태와 같은 생활 특성에 따른 전립선암 발생의 영향 관계는 뚜렷하지 않았다. 결론적으로 본 연구를 통해서 전립선암의 발생 위험도와 악성도 지표로 복부 비만도가 유용함을 나타냈고 간단한 신체 계측 지표 활용으로는 BMI보다 허리둘레 측정치가 더 암 발생의 연관성이 높음을 보였다.

Fumigaclavine C attenuates adipogenesis in 3T3-L1 adipocytes and ameliorates lipid accumulation in high-fat diet-induced obese mice

  • Fumigaclavine C (FC), an active indole alkaloid, is obtained from endophytic Aspergillus terreus (strain No. FC118) by the root of Rhizophora stylosa (Rhizophoraceae). This study is designed to evaluate whether FC has anti-adipogenic effects in 3T3-L1 adipocytes and whether it ameliorates lipid accumulation in high-fat diet (HFD)-induced obese mice. FC notably increased the levels of glycerol in the culture supernatants and markedly reduced lipid accumulation in 3T3-L1 adipocytes. FC differentially inhibited the expressions of adipogenesis-related genes, including the peroxisome proliferator-activated receptor proteins, CCAAT/enhancer-binding proteins, and sterol regulatory element-binding proteins. FC markedly reduced the expressions of lipid synthesis-related genes, such as the fatty acid binding protein, lipoprotein lipase, and fatty acid synthase. Furthermore, FC significantly increased the expressions of lipolysis-related genes, such as the hormone-sensitive lipase, Aquaporin-7, and adipose triglyceride lipase. In HFD-induced obese mice, intraperitoneal injections of FC decreased both the body weight and visceral adipose tissue weight. FC administration significantly reduced lipid accumulation. Moreover, FC could dose-dependently and differentially regulate the expressions of lipid metabolism-related transcription factors. All these data indicated that FC exhibited anti-obesity effects through modulating adipogenesis and lipolysis.
