본 논문은 가상 자전거 시스템을 이용한 자세균형 훈련의 정량적 평가에 대한 연구이다. 본 연구의 실험은 20명의 정상 성인을 대상으로 하여 자세균형에 미치는 요인들을 분석하였다 측정 파라메터로서 주행시간. 주행속력. 주행경로 이탈의 횟수. 압력중심, 그리고 체중이동에 대한 변수들을 추출하여 정량적으로 평가하였다 또한, 균형훈련의 효과를 향상시키기 위해, 체중이동에 따른 시각적 피드백 정보의 유용성을 분석하였다 실험결과. 반복훈련결과. 체중이동 분포의 visual feedback 정보를 제시했을때 COP의 총길이가 줄어들어 동요도가 줄고 체중이동값이 50 %에 가까워져 자세제어 및 조절능력이 향상됨을 알 수 있었다. 따라서 가상 자전거 시스템은 효율적인 자세균형 재활 훈련 장비로서 효과가 있음을 확인하였으며 분석방법은 재활훈련 분야에서 널리 활용될 수 있는 가능성을 보여 주었다.
본 연구는 가상현실(Virtual Reality, VR) 콘텐츠를 활용한 인지재활 프로그램이 노인의 인지기능, 우울감, 상지기능 및 일상생활활동능력에 미치는 영향을 알아보고자 하였다. 연구 집단은 가상현실 인지재활 적용군인 실험군과 보편적 인지자극프로그램 적용군인 대조군으로 나누어 효과성을 분석하였다. 연구결과 MMSE-K 점수가 실험군 13.0%, 대조군 2.3% 향상되었다. 실험군의 영역별 향상도는 MBI 3.1%, MFT(Rt.) 7.1%, MFT(Lt.) 3.5%, K-GDS -25.4%를 나타냈다. 각 집단 간 사전-사후 점수 변화를 비교해 본 결과 일상생활활동능력(p<.001)과 MFT(Rt.)(p<.01)에서 집단 간 유의미한 차이가 있었다. 또한 뇌파를 통해 우울감의 정도를 확인하기 위해 절대 알파파의 변화량을 비교해 본 결과 통계적으로 유의한 차이는 없었지만, 실험군에서 평균값이 양의 값으로 증가하는 결과를 확인할 수 있었다. 이번 연구는 가상현실 콘텐츠를 활용한 인지재활프로그램의 효과성을 검증한 실험으로 노인의 일상생활활동능력, 인지기능, 우울감 및 상지기능을 유지 및 개선하기 위한 새로운 중재방법을 제시한 것에 그 의미가 있겠다.
가상현실 훈련은 신체 기능을 개선하기 위한 중재로 제안되었다. 그러나 급성 뇌졸중 환자의 ADL 개선에 초점을 둔 가상현실 훈련의 효과는 명확하지 않다. 본 연구의 목적은 급성 뇌졸중 환자의 손 기능 및 일상생활활동(ADL)에 대한 가상 현실 훈련의 효과를 조사하는 것이다. 급성 뇌졸중 환자 16명이 본 연구에 포함되었다. 실험 군(VRA군)은 각 세션마다 ADL에 중점을 둔 30분의 가상현실 훈련을 받았으며, 통제군은 30분의 보편적인 가상현실 훈련을 받았다. 피험자의 손기능과 ADL을 조사하기 위해 JTHFT(Jebsen-Tylor Hand Function Test)와 K-MBI (Korean Modified Barthel Index)를 각각 사용했다. 두 그룹 모두 중재 후 마비측과 비마비측의 손기능 및 K-MBI 총점에서 유의미한 개선을 보였다. 실험군은 통제군보다 중재 후 K-MBI의 자가관리 영역에서 유의하게 큰 향상을 보였다. 이러한 결과는 급성 뇌졸중 환자에서 ADL에 중점을 둔 가상 현실 훈련이 기존의 가상 현실 훈련보다 자기관리에 더 나은 영향을 줄 수 있음을 시사한다.
PURPOSE: This study investigated the effect of a head-mounted display (HMD) based virtual reality (VR) cognitive rehabilitation program on the cognitive ability and activities of daily living in patients with mild cognitive impairment. METHODS: This study enrolled 5 mild cognitive impairment patients at the G welfare center in Gwangju metropolitan city. Patients were examined using the MMSEDS (Mini mental state examination-dementia screening). The VR cognitive rehabilitation program was applied twice a week for four weeks. The functional independence measure (FIM) and Loewenstein occupational therapy cognitive assessment-geriatric population (LOTCA-G) were compared before and after applying the VR cognitive rehabilitation program. RESULTS: Subsequent of application of the VR cognitive rehabilitation program, significant increase was observed in the LOTCA-G score. The FIM score was also perceived to increase, after the intervention, but was not statistically significant. CONCLUSION: Application of the VR cognitive rehabilitation program as a therapy is effective in improving the visual perception, and upper extremity executive ability, especially orientation, thinking organization, attention, and concentration.
The purpose of this study is developing a virtual bicycle system for improving the ability of postural balance control for adults in various age groups. The system consists of an exercise bicycle that allows tilt in accordance with the postural balance of the subject in the system, a visual display that shows virtual road, and a visual feedback system. The rider of the system tries to maintain balance on the bicycle with a visual feedback of a virtual road while the pedaling speed, the heading direction, and various weight distribution information are updated to the subject as visual feedbacks in the display. A series of experiments were performed with various subjects to find the factors related to postural balance control in the system. The related parameters obtained were weight shift, magnitude of the deviation from the center of the virtual road, and variables related to the movement of the center of pressure. The results found that the ability to control postural balance in the system improved with the presentation of visual feedback information of the distribution of weight. It was also found that the general performance of the subject on balance in the system improved after ten days long training. The results show that the newly developed system can be used for the diagnosis of postural balance as well as for the stimulation of various senses such as vision and somatic sense in the field of rehabilitation training.
본 연구의 목적은 산학연 공동기술개발사업으로 개발한 가상현실재활시스템이 뇌졸중 환자에게 긍정적인 영향을 주는지 임상 실험으로 알아보고 이를 통해 개발제품의 임상적 사용 가능성을 검증하기 위함이다. 임상실험을 통해 검증하고자 가상현실재활시스템을 재활병원에 입원하고 있는 뇌졸중 24명(뇌출혈 13명, 뇌경색 11명)에게 제공하였다. 실험군(13명)에는 가상현실재활시스템 위주의 훈련을 실시하였고, 대조군(11명)에는 재활치료의 유형인 보편적 기능훈련으로 일상생활활동 훈련, 지필물과 테이블 탑 위주의 인지재활 훈련, 운동조절 훈련을 실시하였다. 실험군과 대조군 모두 주 3회 하루 2회씩 총 8주간 실시하였다. 두 집단 모두 사전사후 기능적 독립 측정(Functional Independent Measure; FIM), 한국형 간이정신상태 검사(Mini-Mental State Examination for Korean; MMSE-K), 자아존중감척도 평가를 실시하였다. 실험결과 가상현실재활시스템을 제공한 집단과 보편적 재활치료를 받은 집단이 일상생활활동, 기초인지, 자아존중감척도에서 유사한 결과를 보였다. 중재 전후를 비교해 본 결과 가상현실재활시스템을 제공한 집단이 중재 전보다 중재 후에 일상생활활동, 기초인지, 자아존중감에서 모두 의미 있는 향상이 있었다(P<.05). 산학공동으로 개발한 가상현실재활시스템이 뇌졸중 환자의 일상생활 기능과 기초 인지능력을 회복시키고, 삶의 만족과 인간행동의 원인이 되는 자아존중감에 긍정적인 영향을 보여 주었기 때문에 가상현실재활시스템의 임상 적용 가능성을 긍정적으로 확인하였다. 추후 연구에서는 가상현실재활시스템을 뇌손상 환자와 치매 환자의 다양한 기능 회복에 대한 임상적 유용성을 검증할 필요가 있다.
Objectives : This study was to investigate the effects on using Virtual reality exercise program($Wii-Fit^{TM}$) for dynamic balance and walking ability in patients with stroke. Methods : The 22 subjects were randomly selected from the patients of the S hospital who met the study conditions. They were divided into a $Wii-Fit^{TM}$ balance game group of 12 patients and a conventional physical therapy group of 10 patients. The $Wii-Fit^{TM}$ balance game group received $Wii-Fit^{TM}$ balance game group general physiotherapy for 5 days a weeks, 30 minutes a day, for a 4 weeks and the conventional physical therapy group received general physiotherapy for the same period. The subjects were measured and compared for Brunel balance assessment, functional gait assessment, 6 minute walk test, GAITRite system before and after the program. Results : The experimental group tend to improve more than control group in shifting the weight to the affected side(p=0.040) and tap test(p<0.001). The experimental group tend to improve more than control group in FGA(p=0.016). The experimental group improved significantly more than control group in 6MWT(p=0.008). The experimental group improved significantly more than control group in gait speed, cadence, stride length. Conclusions : Virtual Reality program($Wii-Fit^{TM}$) with conventional physical therapy shows the benefits on dynamic balance and gait parameters in patients with stroke.
Objective: The objective of this study is to investigate the effect of treadmill training with real optic flow scene on functional recovery of balance and balance self-efficacy in stroke patients. Design: Single blind, Randomized controlled trial. Methods: Nine patients following stroke were divided randomly into the treadmill with optic flow group (n=3), treadmill with virtual reality group (n=3), and control group (n=3). Subjects in the treadmill with optic flow group wore a head-mounted display in order to receive a speed modulated real optic flow scene during treadmill training for 30 minutes, while those in the treadmill with virtual reality group and control group received treadmill training with virtual reality and regular therapy for the same amount of time, five times per week for a period of three weeks. Timed up and go test (TUG) and activities-specific balance confidence scale (ABC scale) were evaluated before and after the intervention. Results: TUG in the treadmill training with optic flow group showed significantly greater improvement, compared with the treadmill training with virtual reality group and control group (p<0.05). Significantly greater improvement in the ABC scale was observed in the treadmill training with optic flow group and the tread mill training with virtual reality group, compared with the control group (p<0.05). Conclusions: Findings of this study demonstrate that treadmill training with real optic flow scene can be helpful in improving balance and balance self-efficacy of patients with chronic stroke and may be used as a practical adjunct to routine rehabilitation therapy.
In this paper, a new rehabilitation training system was developed to improve equilibrium sense of the elderly by combining virtual reality technology with a fixed exercise bicycle. In order to evaluate the effectiveness of the training system, the elderly participated as test subject in the investigation of the influence of different the parameters on postural balance control. We measured three different running modes of virtual bicycle system with two successive sets. The parameters measured were running time, velocity, the weight movement, the degree of the deviation from the road, and the variables about the center of pressure. The repeated training, our results showed that the running capability of the elderly improve compared. In addition, it was found out that the ability of postural control and the equilibrium sense was improved with the presentation of the visual feedback information of the distribution of weight. From the results of this experiment, we showed that our newly developed system might be useful in the diagnosis of equilibrium sense or in the improvement of the sense of sight and, somatic, and vestibular sense of the elderly in the field of rehabilitation training.
본 연구는 장애인 재활을 위해 최신 공학기술과 재활프로그램을 융합한 가상현실 기술의 효과에 관련된 연구동향을 분석한 문헌연구이다. 2000~2014년에 발표된 국내 논문 60편을 대상으로 P.I.C.O방법을 준용하여 분석한 결과, 주로 성인 노인 뇌졸중 환자를 대상으로 가상현실을 적용한 장애인 재활 프로그램 관련 연구가 많았으며, 가상현실 구현을 위해 투사형 시스템이 자주 사용되었다. 중재효과 확인을 위한 종속변수로는 균형, 상지기능, 보행이 주로 사용되었고 이를 ICF의 구성요소로 재해석하여 도출한 연구주제로는 이동이 가장 많았다. 반면 교육, 직업, 여가 등 참여영역을 다룬 연구는 매우 적었다. 향후 장애인의 실생활에 유용한 가상현실 중재를 위해서는 사회적 관점에 초점을 두어야하며, 동시에 가상현실 기술의 개발 시에도 적용방법과 비용 측면에서 용이한 접근을 염두에 두고 공학기술과 재활서비스가 융합되어야 할 것이다.
